Understanding social interpersonal interaction via synchronization templates of facial events
Automatic facial expression analysis in inter-personal communication is challenging. Not only because conversation partners' facial expressions mutually influence each other, but also because no correct interpretation of facial expressions is possible without taking social context into account. In this paper, we propose a probabilistic framework to model interactional synchronization between conversation partners based on their facial expressions. Interactional synchronization manifests temporal dynamics of conversation partners' mutual influence. In particular, the model allows us to discover a set of common and unique facial synchronization templates directly from natural interpersonal interaction without recourse to any predefined labeling schemes. The facial synchronization templates represent periodical facial event coordinations shared by multiple conversation pairs in a specific social context. We test our model on two different dyadic conversations of negotiation and job-interview. Based on the discovered facial event coordination, we are able to predict their conversation outcomes with higher accuracy than HMMs and GMMs
Report of the Executive Council's Working Group on the Establishment of a CGIAR Science Council
This paper was prepared for the Executive Council by the Working Goup on the Establishment of a CGIAR Science Council. It is a detailed proposal on how the CGIAR Technical Advisory Committee could be transformed into a Science Council. The report calls for the creation of a small independent Science Council that would enjoy a wider mandate in scope than TAC's current mandate. Case-mix adjusted hospital mortality is a poor proxy for preventable mortality: a modelling study
Risk-adjustment schemes are used to monitor hospital performance, on the assumption that excess mortality not explained by case mix is largely attributable to suboptimal care. We have developed a model to estimate the proportion of the variation in standardised mortality ratios (SMRs) that can be accounted for by variation in preventable mortality. The model was populated with values from the literature to estimate a predictive value of the SMR in this context—specifically the proportion of those hospitals with SMRs among the highest 2.5% that fall among the worst 2.5% for preventable mortality. The extent to which SMRs reflect preventable mortality rates is highly sensitive to the proportion of deaths that are preventable. If 6% of hospital deaths are preventable (as suggested by the literature), the predictive value of the SMR can be no greater than 9%. This value could rise to 30%, if 15% of deaths are preventable. The model offers a ‘reality check’ for case mix adjustment schemes designed to isolate the preventable component of any outcome rate
Adult and paediatric mortality patterns in a referral hospital in Liberia 1 year after the end of the war
The aim of this study was to describe and analyse hospital mortality patterns after the Liberian war. Data were collected retrospectively from January to July 2005 in a referral hospital in Monrovia, Liberia. The overall fatality rate was 17.2% (438/2543) of medical admissions. One-third of deaths occurred in the first 24h. The adult fatality rate was 23.3% (241/1034). Non-infectious diseases accounted for 56% of the adult deaths. The main causes of death were meningitis (16%), stroke (14%) and heart failure (10%). Associated fatality rates were 48%, 54% and 31% respectively. The paediatric fatality rate was 13.1% (197/1509). Infectious diseases caused 66% of paediatric deaths. In infants <1 month old, the fatality rate was 18% and main causes of death were neonatal sepsis (47%), respiratory distress (24%) and prematurity (18%). The main causes of death in infants > or =1 month old were respiratory infections (27%), malaria (23%) and severe malnutrition (16%). Associated fatality rates were 12%, 10% and 19%. Fatality rates were similar to those found in other sub-Saharan countries without a previous conflict. Early deaths could decrease through recognition and early referral of severe cases from health centres to the hospital and through assessment and priority treatment of these patients at arrival
Immunohistochemical evaluation of human epidermal growth factor receptor 2 and estrogen and progesterone receptors in breast carcinoma in Jordan
INTRODUCTION: Although breast carcinoma (BC) is the most common malignancy affecting Jordanian females and the affected population in Jordan is younger than that in the West, no information is available on its biological characteristics. Our aims in this study are to evaluate the expression of estrogen receptor (ER) and progesterone receptor (PR) and Her-2/neu overexpression in BC in Jordan, and to compare the expression of these with other prognostic parameters for BC such as histological type, histological grade, tumor size, patients' age, and number of lymph node metastases. METHOD: This is a retrospective study conducted in the Department of Pathology at Jordan University of Science and Technology. A confirmed 91 cases of BC diagnosed in the period 1995 to 1998 were reviewed and graded. We used immunohistochemistry to evaluate the expression of ER, PR, and Her-2. Immunohistochemical findings were correlated with age, tumor size, grade and axillary lymph node status. RESULTS: Her-2 was overexpressed in 24% of the cases. The mean age of Her-2 positive cases was 42 years as opposed to 53 years among Her-2 negative cases (p = 0.0001). Her-2 expression was inversely related to ER and PR expression. Her-2 positive tumors tended to be larger than Her-2 negative tumors with 35% overexpression among T3 tumors as opposed to 22% among T2 tumors (p = 0.13). Her-2 positive cases tended to have higher rates of axillary metastases, but this did not reach statistical significance. ER and PR positive cases were seen in older patients with smaller tumor sizes. CONCLUSION: Her-2 overexpression was seen in 24% of BC affecting Jordanian females. Her-2 overexpression was associated with young age at presentation, larger tumor size, and was inversely related to ER and PR expression. One-fifth of the carcinomas were Her-2 positive and ER negative. This group appears to represent an aggressive form of BC presenting at a young age with large primary tumors and a high rate of four or more axillary lymph node metastases
Postoperative pain management in children: Guidance from the pain committee of the European Society for Paediatric Anaesthesiology (ESPA Pain Management Ladder Initiative)
The main remit of the European Society for Paediatric Anaesthesiology (ESPA) Pain Committee is to improve the quality of pain management in children. The ESPA Pain Management Ladder is a clinical practice advisory based upon expert consensus to help to ensure a basic standard of perioperative pain management for all children. Further steps are suggested to improve pain management once a basic standard has been achieved. The guidance is grouped by the type of surgical procedure and layered to suggest basic, intermediate, and advanced pain management methods. The committee members are aware that there are marked differences in financial and personal resources in different institutions and countries and also considerable variations in the availability of analgesic drugs across Europe. We recommend that the guidance should be used as a framework to guide best practice
The age-specific quantitative effects of metabolic risk factors on cardiovascular diseases and diabetes: a pooled analysis.
BACKGROUND: The effects of systolic blood pressure (SBP), serum total cholesterol (TC), fasting plasma glucose (FPG), and body mass index (BMI) on the risk of cardiovascular diseases (CVD) have been established in epidemiological studies, but consistent estimates of effect sizes by age and sex are not available. METHODS: We reviewed large cohort pooling projects, evaluating effects of baseline or usual exposure to metabolic risks on ischemic heart disease (IHD), hypertensive heart disease (HHD), stroke, diabetes, and, as relevant selected other CVDs, after adjusting for important confounders. We pooled all data to estimate relative risks (RRs) for each risk factor and examined effect modification by age or other factors, using random effects models. RESULTS: Across all risk factors, an average of 123 cohorts provided data on 1.4 million individuals and 52,000 CVD events. Each metabolic risk factor was robustly related to CVD. At the baseline age of 55-64 years, the RR for 10 mmHg higher SBP was largest for HHD (2.16; 95% CI 2.09-2.24), followed by effects on both stroke subtypes (1.66; 1.39-1.98 for hemorrhagic stroke and 1.63; 1.57-1.69 for ischemic stroke). In the same age group, RRs for 1 mmol/L higher TC were 1.44 (1.29-1.61) for IHD and 1.20 (1.15-1.25) for ischemic stroke. The RRs for 5 kg/m(2) higher BMI for ages 55-64 ranged from 2.32 (2.04-2.63) for diabetes, to 1.44 (1.40-1.48) for IHD. For 1 mmol/L higher FPG, RRs in this age group were 1.18 (1.08-1.29) for IHD and 1.14 (1.01-1.29) for total stroke. For all risk factors, proportional effects declined with age, were generally consistent by sex, and differed by region in only a few age groups for certain risk factor-disease pairs. CONCLUSION: Our results provide robust, comparable and precise estimates of the effects of major metabolic risk factors on CVD and diabetes by age group
Tuning the optical properties of silicon quantum dots via surface functionalization with conjugated aromatic fluorophores

This work was financially supported by NSERC Canada Discovery (Charpentier).Silicon Quantum Dots (SQDs) have recently attracted great interest due to their excellent optical properties, low cytotoxicity, and ease of surface modification. The size of SQDs and type of ligand on their surface has a great influence on their optical properties which is still poorly understood. Here we report the synthesis and spectroscopic studies of three families of unreported SQDs functionalized by covalently linking to the aromatic fluorophores, 9-vinylphenanthrene, 1-vinylpyrene, and 3-vinylperylene. The results showed that the prepared functionalized SQDs had a highly-controlled diameter by HR-TEM, ranging from 1.7–2.1 nm. The photophysical measurements of the assemblies provided clear evidence for efficient energy transfer from the fluorophore to the SQD core. Fӧrster energy transfer is the likely mechanism in these assemblies. As a result of the photogenerated energy transfer process, the emission color of the SQD core could be efficiently tuned and its emission quantum efficiency enhanced. To demonstrate the potential application of the synthesized SQDs for bioimaging of cancer cells, the water-soluble perylene- and pyrene-capped SQDs were examined for fluorescent imaging of HeLa cells. The impact of local authorities’ interventions on household waste collection: a case study approach using time series modelling
At a local Government level there have been many interventions and changes made to household waste collection services to meet new regulatory requirements. These changes include separate collection of recyclable and organic materials. This paper has used a time series model to quantify the success of interventions introduced by a LA. The case study was a medium sized UK LA, Charnwood Borough Council (CBC), the research analyses monthly data of quantities of recyclates, garden waste for composting and residual waste for landfill disposal. The time series model was validated with a five year data set and used to measure the impacts of the various changes to identify which intervention was the most successful, while controlling for season and number of working days. The results show the interventions analysed both had abrupt and permanent positive impacts on the yield of recyclable materials, and a corresponding negative impact on the residual waste. The model could be added to the National data base to help LAs to compare interventions and to understand which schemes encourage householder participation and improve recycling performance
