    Tomografia de corpo todo no trauma e seus desfechos na mortalidade: uma revisão sistemática: Whole body tomography in trauma and its outcomes in mortality: a systematic review

    A tomografia computadorizada de corpo inteiro é altamente sensível e representa o padrão-ouro no cenário de diagnóstico da sala de trauma. WBCT fornece uma ferramenta de diagnóstico rápido, que reduz a mortalidade em pacientes gravemente feridos. A lesão traumática é a terceira principal causa de morte em geral. Para otimizar os resultados nesses pacientes, os hospitais empregam imagens de tomografia computadorizada de corpo inteiro (WBCT) devido ao alto rendimento diagnóstico e potencial para identificar lesões perdidas. No entanto, isso atrasa intervenções de tempo crítico. Atualmente, há uma ausência de qualquer evidência de alto nível para apoiar ou refutar qualquer visão. Uma busca sistemática da literatura foi realizada nas bases de dados MEDLINE, Embase, Web of Science, Cochrane Library e demais bases dedados eletrônicas. As publicações eram elegíveis se contivessem dados originais comparando TC de corpo total imediata em pacientes com trauma e associação com a mortalidade. A análise mostra que a TC está associada a melhores resultados, incluindo uma menor taxa de mortalidade geral, entretanto estudos randomizados e controlados merecem ser realizados para que se possa estabelecer de forma fidedigna essa relação

    Manejo cirúrgico de pacientes traumatizados por lesões elétricas: uma abordagem da fase aguda do trauma elétrico

    The aim of this study is to disassemble, through scientific articles and government exposure, the main concepts related to electric shock injury. And as secondary objectives, to address a perspective of adequate treatment in this scenario and surgical steps in the management of patients traumatized by electric shock. An in-depth search of articles was carried out in the Scientific Electronic Library Online (SciELO) and PubMed databases, with the following Health Sciences Descriptors (DeCS):Electric burns; Shock; Debridement; Hydration; Amputation. Articles were selected between the periods of 2000 to 2023, in Portuguese, to increase the level of relevance and quality of the review, given the scarcity of scientific studies on the subject. It is concluded with the analysis of these studies that the early medical intervention of an electrical trauma is essential. The correct management, before 15 days, can positively corroborate the survival rates of these patients, avoiding possible complications. However, the proportion of the accident can affect medical management, representing a problem for patients, attributing greater symptoms.El objetivo de este estudio es desmontar, a través de artículos científicos y exposición gubernamental, los principales conceptos relacionados con la lesión por descarga eléctrica. Y como objetivos secundarios, abordar una perspectiva de tratamiento adecuado en este escenario y pasos quirúrgicos en el manejo de pacientes traumatizados por descarga eléctrica. Se realizó una búsqueda exhaustiva de artículos en las bases de datos Scientific Electronic Library Online (SciELO) y PubMed, con los siguientes Descriptores en Ciencias de la Salud (DeCS): Quemaduras eléctricas; Choque; desbridamiento; Hidratación; Amputación. Los artículos fueron seleccionados entre los períodos de 2000 a 2023, en portugués, para aumentar el nivel de relevancia y calidad de la revisión, dada la escasez de estudios científicos sobre el tema. Se concluye con el análisis de estos estudios que la intervención médica temprana de un trauma eléctrico es fundamental. El correcto manejo, antes de los 15 días, puede corroborar positivamente las tasas de supervivencia de estos pacientes, evitando posibles complicaciones. Sin embargo, la proporción del accidente puede afectar el manejo médico, representando un problema para los pacientes, atribuyéndoles mayor sintomatología.Objetivo desse estudo é desmontar, por meio de artigos científicos e exposição governamentais, os principais conceitos que tangem a lesão por choque elétrico. E como objetivos secundários, abordar uma perspectiva de tratamento adequado nesse cenário e etapas cirúrgicas do manejo de pacientes traumatizados por choque elétrico. Foi realizada uma profunda pesquisa de artigos nas bases de dados Scientific Electronic Library Online (SciELO) e PubMed, com os seguintes Descritores em Ciências daSaúde(DeCS):Queimaduraselétricas; Choque; Desbridamento; Hidratação; Amputação. Foram selecionados artigos entre os períodos de 2000 a 2023, no idioma Português, para ampliar o nível de relevância e a qualidade da revisão, visto a escassez de estudos científicos sobre a temática. Conclui-se com a análise desses estudos que, a intervenção médica precoce de um trauma elétrico é essencial. O manejo correto, antes dos 15 dias, pode corroborar positivamente com as taxas de sobrevida desses pacientes, evitando possíveis complicações. Contudo, a proporção do acidente pode afetar a conduta médica, representando um problema para os pacientes, atribuindo maiores sintomas

    The objective of this study is to disassemble, through scientific articles, the aspects related to aesthetic plastic surgery and the body satisfaction resulting from these procedures by the individuals who undergo them. A deep search of articles was carried out in the databases Medical Literature Analysis and Retrieval System Online (MEDLINE), Scientific Electronic Library Online (SciELO) and PubMed, with the following Health Sciences Descriptors (DeCS): Body; Body image; Surgical procedures. Articles were selected between the periods of 1998 to 2023, in French, Spanish, English and Portuguese, to increase the level of relevance and quality of the review, given the scarcity of scientific studies on the subject. This review study made it possible to expose the current moment of aesthetic plastic surgeries, in addition to understanding the profile of the individuals who undergo it. Body dissatisfaction becomes an image disorder, correlated with feelings and emotions, are reasons that lead to the search for “correction” through aesthetic procedures. However, plastic surgery can alleviate dissatisfaction and is a tool against different body contexts.El objetivo de este estudio es desmontar, a través de artículos científicos, los aspectos relacionados con la cirugía plástica estética y la satisfacción corporal resultante de estos procedimientos por parte de los individuos que se someten a ellos. Se realizó una búsqueda profunda de artículos en las bases de datos Medical Literature Analysis and Retrieval System Online (MEDLINE), Scientific Electronic Library Online (SciELO) y PubMed, con los siguientes Health Sciences Descriptors (DeCS): Body; Imagen corporal; Procedimientos quirúrgicos. Se seleccionaron artículos entre los períodos de 1998 a 2023, en francés, español, inglés y portugués, para aumentar el nivel de relevancia y calidad de la revisión, dada la escasez de estudios científicos sobre el tema. Este estudio de revisión permitió exponer el momento actual de las cirugías plásticas estéticas, además de comprender el perfil de los individuos que se someten a ella. La insatisfacción corporal se convierte en un trastorno de la imagen, correlacionado con los sentimientos y emociones, son motivos que llevan a la búsqueda de “corrección” a través de procedimientos estéticos. Sin embargo, la cirugía plástica puede aliviar la insatisfacción y es una herramienta frente a diferentes contextos corporales.Objetivo desse estudo é desmontar, por meio de artigos científicos, os aspectos que tange a cirurgia plástica estética e a satisfação corporal advinda desses procedimentos pelos indivíduos submetidos. Foi realizada uma profunda pesquisa de artigos nas bases de dados Medical Literature Analysis and Retrievel System Online (MEDLINE), Scientific Electronic Library Online (SciELO) e PubMed, com os seguintes Descritores em Ciências da Saúde (DeCS): Body; Body image; Surgical procedures. Foram selecionados artigos entre os períodos de 1998 a 2023, nos idiomas Frances, Espanhol, Inglês e Português, para ampliar o nível de relevância e a qualidade da revisão, visto a escassez de estudos científicos sobre a temática. Este estudo de revisão possibilitou expor o momento atual das cirurgias plásticas estéticas, além da compreensão do perfil dos indivíduos que são submetidos. A insatisfação do corpo se torna um distúrbio de imagem, correlacionados a sentimentos e emoções, são motivos que levam a busca de “correção” através de procedimentos estéticos.     Risk portfolio on modern finance has become increasingly technical, requiring the use of sophisticated mathematical tools in both research and practice. Since companies cannot insure themselves completely against risk, as human incompetence in predicting the future precisely that written in Al-Quran surah Luqman verse 34, they have to manage it to yield an optimal portfolio. The objective here is to minimize the variance among all portfolios, or alternatively, to maximize expected return among all portfolios that has at least a certain expected return. Furthermore, this study focuses on optimizing risk portfolio so called Markowitz MVO (Mean-Variance Optimization). Some theoretical frameworks for analysis are arithmetic mean, geometric mean, variance, covariance, linear programming, and quadratic programming. Moreover, finding a minimum variance portfolio produces a convex quadratic programming, that is minimizing the objective function ðð¥with constraintsð ð 𥠥 ðandð´ð¥ = ð. The outcome of this research is the solution of optimal risk portofolio in some investments that could be finished smoothly using MATLAB R2007b software together with its graphic analysis

    Pervasive gaps in Amazonian ecological research

    Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050.     Protective Effect of Metformin Against Walker 256 Tumor Growth is Not Dependent on Metabolism Improvement

    Background/Aims: The objective of the current work was to test the effect of metformin on the tumor growth in rats with metabolic syndrome. Methods: We obtained pre-diabetic hyperinsulinemic rats by neonatal treatment with monosodium L-glutamate (MSG), which were chronically treated every day, from weaning to 100 day old, with dose of metformin (250 mg/kg body weight). After the end of metformin treatment, the control and MSG rats, treated or untreated with metformin, were grafted with Walker 256 carcinoma cells. Tumor weight was evaluated 14 days after cancer cell inoculation. The blood insulin, glucose levels and glucose-induced insulin secretion were evaluated. Results: Chronic metformin treatment improved the glycemic homeostasis in pre-diabetic MSG-rats, glucose intolerance, tissue insulin resistance, hyperinsulinemia and decreased the fat tissue accretion. Meanwhile, the metformin treatment did not interfere with the glucose insulinotropic effect on isolated pancreatic islets. Chronic treatment with metformin was able to decrease the Walker 256 tumor weight by 37% in control and MSG rats. The data demonstrated that the anticancer effect of metformin is not related to its role in correcting metabolism imbalances, such as hyperinsulinemia. However, in morphological assay to apoptosis, metformin treatment increased programmed cell death. Conclusion: Metformin may have a direct effect on cancer growth, and it may programs the rat organism to attenuate the growth of Walker 256 carcinoma
