    Stevens-Johnson syndrome and toxic epidermal necrolysis: a review

    Stevens Johnson Syndrome (SJS) and Toxic Epidermal Necrolysis (TEN) are considered a single entity with variability in the extent of the lesions, characterized by erythema multiforme that may involve mucosa. Severe cutaneous reactions secondary to medications are classified according to the area of epidermal detachment. The activation of cytotoxic T cells and macrophages is mediated mainly by IL-2 and interferon gamma secreted by Th1 lymphocytes, and the activation of eosinophils and B lymphocytes in IgE is mediated by secreted IL-4, IL-5, IL-10 and IL13 by B lymphocytes. The topography of SJS is predominantly central, affecting the trunk and sometimes a generalized dissemination is shown that affects a body surface area of less than 10%, characterized by irregular violaceous erythematous macules of target shooting, which can form confluent blisters. TEN is characterized by a skin detachment greater than 30% of the body surface, whose predominant lesion is diffuse erythema with individual macules, which give rise to detachment surfaces greater than 5 cm. The treatment is symptomatic, nonspecific, and aimed at avoiding complications, carried out in specialized intensive care units, due to ignorance of the pathogenesis.     DD04107-Derived neuronal exocytosis inhibitor peptides: Evidences for synaptotagmin-1 as a putative target

    The analgesic peptide DD04107 (Pal-EEMQRR-NH2) and its acetylated analogue inhibit a-calcitonin gene-related peptide (a-CGRP) exocytotic release from primary sensory neurons. Examining the crystal structure of the SNARE-Synaptotagmin-1(Syt1) complex, we hypothesized that these peptides could inhibit neuronal exocytosis by binding to Syt1, hampering at least partially its interaction with the SNARE complex. To address this hypothesis, we first interrogate the role of individual side-chains on the inhibition of a-CGRP release, finding that E1, M3, Q4 and R6 residues were crucial for activity. CD and NMR conformational analysis showed that linear peptides have tendency to adopt a-helical conformations, but the results with cyclic analogues indicated that this secondary structure is not needed for activity. Isothermal titration calorimetry (ITC) measurements demonstrate a direct interaction of some of these peptides with Syt1-C2B domain, but not with Syt7-C2B region, indicating selectivity. As expected for a compound able to inhibit a-CGRP release, cyclic peptide derivative Pal-E-cyclo[EMQK]R-NH2 showed potent in vivo analgesic activity, in a model of inflammatory pain. Molecular dynamics simulations provided a model consistent with KD values for the interaction of peptides with Syt1-C2B domain, and with their biological activity. Altogether, these results identify Syt1 as a potential new analgesic target. © 202

    Antebrachial microvascular flap for reconstruction of the pelvic limb: case report

    Complex defects in the soft tissues of the foot and ankle caused by high-energy trauma, infection, cancer or diabetes require rapid and effective treatment which decreases the risk of infection, bone healing time, hospital stay and number total of surgical approaches. Authors report a clinical case of a 23-year-old female patient who suffers high-energy trauma due to a motorcycle accident which causes exposure of the distal third of the tibia, loss of the tibial joint face, and loss of tissue throughout the region of the malleolus medial of the right pelvic extremity. Tissue transfer to the pelvic limb is performed using left contralateral radial antebrachial free flap as donor area. This case report aims to emphasize the key aspects of the high energy trauma approach by using microvascular flaps, as well as the satisfactory evolution of the patient that directly affects the prognosis for life and function

    HTLV-1 infection in solid organ transplant donors and recipients in Spain

    HTLV-1 infection is a neglected disease, despite infecting 10-15 million people worldwide and severe illnesses develop in 10% of carriers lifelong. Acknowledging a greater risk for developing HTLV-1 associated illnesses due to immunosuppression, screening is being widely considered in the transplantation setting. Herein, we report the experience with universal HTLV testing of donors and recipients of solid organ transplants in a survey conducted in Spain. All hospitals belonging to the Spanish HTLV network were invited to participate in the study. Briefly, HTLV antibody screening was performed retrospectively in all specimens collected from solid organ donors and recipients attended since the year 2008. A total of 5751 individuals were tested for HTLV antibodies at 8 sites. Donors represented 2312 (42.2%), of whom 17 (0.3%) were living kidney donors. The remaining 3439 (59.8%) were recipients. Spaniards represented nearly 80%. Overall, 9 individuals (0.16%) were initially reactive for HTLV antibodies. Six were donors and 3 were recipients. Using confirmatory tests, HTLV-1 could be confirmed in only two donors, one Spaniard and another from Colombia. Both kidneys of the Spaniard were inadvertently transplanted. Subacute myelopathy developed within 1 year in one recipient. The second recipient seroconverted for HTLV-1 but the kidney had to be removed soon due to rejection. Immunosuppression was stopped and 3 years later the patient remains in dialysis but otherwise asymptomatic. The rate of HTLV-1 is low but not negligible in donors/recipients of solid organ transplants in Spain. Universal HTLV screening should be recommended in all donor and recipients of solid organ transplantation in Spain. Evidence is overwhelming for very high virus transmission and increased risk along with the rapid development of subacute myelopathy

    La presente aportación recoge la visión de 46 especialistas de quince países iberoamericanos sobre las formas de entender y promover la convivencia escolar en los centros educativos de los distintos países. Sus aportaciones son un conjunto de descripciones, experiencias y valoraciones significativas y en relación al contexto considerado. Las aportaciones no buscan tanto radiografiar la temática a nivel teórico como presentar lo más significativo de cada realidad y las propuestas que, al respecto, se realizan. La orientación es claramente organizativa, si consideramos que una parte común de todas las aportaciones tiene que ver con las políticas de convivencia escolar, programas aplicados, aspectos organizativos a nivel de institución, experiencias significativas y retos para la mejora. Se cubre así y de nuevo un propósito fundamental de la Red AGE, como es el de fomentar el intercambio de experiencias, la promoción del conocimiento sobre administración y gestión educativa y la reflexión sobre la práctica de la gestión. La finalidad última es la de mejorar el funcionamiento de los centros educativos (y, a través de ellos, de los sistemas educativos), procurando sean de calidad y un instrumento para el cambio profesional y social

    Dinámica del crecimiento de un bosque templado bajo manejo en el noroeste de México

    The dynamics of the tree component of second-growth natural forests of Pinus in northwestern Mexico was analyzed. For this, the behavior of different dasometric parameters related to the density and dimensions of trees was evaluated, from data obtained previous to silvicultural interventions. Records of all individuals with diameter at breast height ≥ 7.6 cm were used, from three periodic inventories of permanent plots (1986, 1996 and 2006). Mean test comparisons were carried out for the dasometric parameters among assessments. Differences in dimensions of the parameters in successive evaluations denote the dynamics of growth and development belonging to a regular forest. The average trees and plots growth showed higher individual increment in the first period. This behavior was constant at the forest level in the last period, suggesting a continuous yield, which is produced by a sustainable forest management system. In conclusion, the studied forest is an even-aged forest which records an increment in the size of trees and the volume of the stands, and allows sustained timber production with silvicultural interventions applied for its development.Se analizó la dinámica del elemento arbóreo de bosques naturales de segundo crecimiento de Pinus en el noreste de México. Para ello, a partir de datos previos a las intervenciones silvícolas, se evaluó el comportamiento de diferentes parámetros dasométricos relacionados con la densidad y las dimensiones del arbolado. Se utilizaron registros de todos los individuos con diámetro a la altura del pecho mayor o igual a 7.6 cm, provenientes de tres monitoreos periódicos (años 1986, 1996 y 2006) de parcelas permanentes. A las variables dasométricas se les realizaron pruebas de comparación de medias entre evaluaciones. Las diferencias en dimensiones de los parámetros, en evaluaciones sucesivas, resaltan la dinámica de crecimiento y desarrollo propia de masas regulares. El crecimiento de árboles promedio y parcelas mostró un incremento individual mayor en el primer periodo, que se mantuvo a nivel bosque en el último periodo, lo que sugiere un rendimiento continuo, producto de un sistema de manejo forestal sustentable. En conclusión, el bosque es regular, registra un incremento en las dimensiones del arbolado y el volumen de la masa, y permite sostener la producción maderable con las intervenciones silvícolas aplicadas para su desarrollo

    Evolutionary view through the starless cores in Taurus

    International audienceContext. The chemical and physical evolution of starless and pre-stellar cores are of paramount importance to understanding the process of star formation. The Taurus Molecular Cloud cores TMC 1-C and TMC 1-CP share similar initial conditions and provide an excellent opportunity to understand the evolution of the pre-stellar core phase. Aims. We investigated the evolutionary stage of starless cores based on observations towards the prototypical dark cores TMC 1-C and TMC 1-CP. Methods. We mapped the prototypical dark cores TMC 1-C and TMC 1-CP in the CS 3 → 2, C 34 S 3 → 2, 13 CS 2 → 1, DCN 1 → 0, DCN 2 → 1, DNC 1 → 0, DNC 2 → 1, DN 13 C 1 → 0, DN 13 C 2 → 1, N 2 H + 1 → 0, and N 2 D + 1 → 0 transitions. We performed a multi-transitional study of CS and its isotopologs, DCN, and DNC lines to characterize the physical and chemical properties of these cores. We studied their chemistry using the state-of-the-art gas-grain chemical code N AUTILUS and pseudo time-dependent models to determine their evolutionary stage. Results. The central n H volume density, the N 2 H + column density, and the abundances of deuterated species are higher in TMC 1-C than in TMC 1-CP, yielding a higher N 2 H + deuterium fraction in TMC 1-C, thus indicating a later evolutionary stage for TMC 1-C. The chemical modeling with pseudo time-dependent models and their radiative transfer are in agreement with this statement, allowing us to estimate a collapse timescale of ~1 Myr for TMC 1-C. Models with a younger collapse scenario or a collapse slowed down by a magnetic support are found to more closely reproduce the observations towards TMC 1-CP. Conclusions. Observational diagnostics seem to indicate that TMC 1-C is in a later evolutionary stage than TMC 1-CP, with a chemical age ~1 Myr. TMC 1-C shows signs of being an evolved core at the onset of star formation, while TMC 1-CP appears to be in an earlier evolutionary stage due to a more recent formation or, alternatively, a collapse slowed down by a magnetic support
