Safety of intravenous thrombolysis for acute ischemic stroke in patients receiving antiplatelet therapy at stroke onset
<p><b>Background and Purpose:</b> Antiplatelets (APs) may increase the risk of symptomatic intracerebral hemorrhage (ICH) following intravenous thrombolysis after ischemic stroke.</p>
<p><b>Methods:</b> We assessed the safety of thrombolysis under APs in 11 865 patients compliant with the European license criteria and recorded between 2002 and 2007 in the Safe Implementation of Treatments in Stroke (SITS) International Stroke Thrombolysis Register (SITS-ISTR). Outcome measures of univariable and multivariable analyses included symptomatic ICH (SICH) per SITS Monitoring Study (SITS-MOST [deterioration in National Institutes of Health Stroke Scale >= 4 plus ICH type 2 within 24 hours]), per European Cooperative Acute Stroke Study II (ECASS II [deterioration in National Institutes of Health Stroke Scale >= 4 plus any ICH]), functional outcome at 3 months and mortality.</p>
<p><b>Results:</b> A total of 3782 (31.9%) patients had received 1 or 2 AP drugs at baseline: 3016 (25.4%) acetylsalicylic acid (ASA), 243 (2.0%) clopidogrel, 175 (1.5%) ASA and dipyridamole, 151 (1.3%) ASA and clopidogrel, and 197 (1.7%) others. Patients receiving APs were 5 years older and had more risk factors than AP nave patients. Incidences of SICH per SITS-MOST (ECASS II respectively) were as follows: 1.1% (4.1%) AP naive, 2.5% (6.2%) any AP, 2.5% (5.9%) ASA, 1.7% (4.2%) clopidogrel, 2.3% (5.9%) ASA and dipyridamole, and 4.1% (13.4%) ASA and clopidogrel. In multivariable analyses, the combination of ASA and clopidogrel was associated with increased risk for SICH per ECASS II (odds ratio, 2.11; 95% CI, 1.29 to 3.45; P = 0.003). However, we found no significant increase in the risk for mortality or poor functional outcome, irrespective of the AP subgroup or SICH definition.</p>
<p><b>Conclusion:</b> The absolute excess of SICH of 1.4% (2.1%) in the pooled AP group is small compared with the benefit of thrombolysis seen in randomized trials. Although caution is warranted in patients receiving the combination of ASA and clopidogrel, AP treatment should not be considered a contraindication to thrombolysis.</p>
The management of iron deficiency in inflammatory bowel disease
__Background__ Iron deficiency is a common and undertreated problem in inflammatory bowel disease (IBD).
__Aim__ To develop an online tool to support treatment choice at the patient-specific level.
__Methods__ Using the RAND/UCLA Appropriateness Method (RUAM), a European expert panel assessed the appropriateness of treatment regimens for a variety of clinical scenarios in patients with non-anaemic iron deficiency (NAID) and iron deficiency anaemia (IDA). Treatment options included adjustment of IBD medication only, oral iron supplementation, high-/low-dose intravenous (IV) regimens, IV iron plus erythropoietin-stimulating agent (ESA), and blood transfusion. The panel process consisted of two individual rating rounds and three plenary discussion meetings.
__Results__ The panel reached agreement on 71% of treatment indications. 'No treatment' was never considered appropriate, and repeat treatment after previous failure was generally discouraged. For 98% of scenarios, at least one treatment was appropriate. Adjustment of IBD medication was deemed appropriate in all patients with active disease. Use of oral iron was mainly considered an option in NAID and mildly anaemic patients without disease activity. IV regimens were often judged appropriate, with high-dose IV iron being the preferred option in 77% of IDA scenarios. Blood transfusion and IV+ESA were indicated in exceptional cases only.
__Conclusions__ The RUAM revealed high agreement amongst experts on the management of iron deficiency in patients with IBD. High-dose IV iron was more often considered appropriate than other options. To facilitate dissemination of the recommendations, panel outcomes were embedded in an online tool, accessible via http://ferroscope.com/
Measurement of W Polarisation at LEP
The three different helicity states of W bosons produced in the reaction e+
e- -> W+ W- -> l nu q q~ at LEP are studied using leptonic and hadronic W
decays. Data at centre-of-mass energies \sqrt s = 183-209 GeV are used to
measure the polarisation of W bosons, and its dependence on the W boson
production angle. The fraction of longitudinally polarised W bosons is measured
to be 0.218 \pm 0.027 \pm 0.016 where the first uncertainty is statistical and
the second systematic, in agreement with the Standard Model expectation
Time-integrated luminosity recorded by the BABAR detector at the PEP-II e+e- collider
This article is the Preprint version of the final published artcile which can be accessed at the link below.We describe a measurement of the time-integrated luminosity of the data collected by the BABAR experiment at the PEP-II asymmetric-energy e+e- collider at the ϒ(4S), ϒ(3S), and ϒ(2S) resonances and in a continuum region below each resonance. We measure the time-integrated luminosity by counting e+e-→e+e- and (for the ϒ(4S) only) e+e-→μ+μ- candidate events, allowing additional photons in the final state. We use data-corrected simulation to determine the cross-sections and reconstruction efficiencies for these processes, as well as the major backgrounds. Due to the large cross-sections of e+e-→e+e- and e+e-→μ+μ-, the statistical uncertainties of the measurement are substantially smaller than the systematic uncertainties. The dominant systematic uncertainties are due to observed differences between data and simulation, as well as uncertainties on the cross-sections. For data collected on the ϒ(3S) and ϒ(2S) resonances, an additional uncertainty arises due to ϒ→e+e-X background. For data collected off the ϒ resonances, we estimate an additional uncertainty due to time dependent efficiency variations, which can affect the short off-resonance runs. The relative uncertainties on the luminosities of the on-resonance (off-resonance) samples are 0.43% (0.43%) for the ϒ(4S), 0.58% (0.72%) for the ϒ(3S), and 0.68% (0.88%) for the ϒ(2S).This work is supported by the US Department of Energy and National Science Foundation, the Natural Sciences and Engineering Research Council (Canada), the Commissariat à l’Energie Atomique and Institut National de Physique Nucléaire et de Physiquedes Particules (France), the Bundesministerium für Bildung und Forschung and Deutsche Forschungsgemeinschaft (Germany), the Istituto Nazionale di Fisica Nucleare (Italy), the Foundation for Fundamental Research on Matter (The Netherlands), the Research Council of Norway, the Ministry of Education and Science of the Russian Federation, Ministerio de Ciencia e Innovación (Spain), and the Science and Technology Facilities Council (United Kingdom). Search for Anomalous Couplings in the Higgs Sector at LEP
Anomalous couplings of the Higgs boson are searched for through the processes
e^+ e^- -> H gamma, e^+ e^- -> e^+ e^- H and e^+ e^- -> HZ. The mass range 70
GeV < m_H < 190 GeV is explored using 602 pb^-1 of integrated luminosity
collected with the L3 detector at LEP at centre-of-mass energies
sqrt(s)=189-209 GeV. The Higgs decay channels H -> ffbar, H -> gamma gamma, H
-> Z\gamma and H -> WW^(*) are considered and no evidence is found for
anomalous Higgs production or decay. Limits on the anomalous couplings d, db,
Delta(g1z), Delta(kappa_gamma) and xi^2 are derived as well as limits on the H
-> gamma gamma and H -> Z gamma decay rates

Bose-Einstein Correlations of Neutral and Charged Pions in Hadronic Z Decays
Bose-Einstein correlations of both neutral and like-sign charged pion pairs
are measured in a sample of 2 million hadronic Z decays collected with the L3
detector at LEP. The analysis is performed in the four-momentum difference
range 300 MeV < Q < 2 GeV. The radius of the neutral pion source is found to be
smaller than that of charged pions. This result is in qualitative agreement
with the string fragmentation model
Z Boson Pair-Production at LEP
Events stemming from the pair-production of Z bosons in e^+e^- collisions are
studied using 217.4 pb^-1 of data collected with the L3 detector at
centre-of-mass energies from 200 GeV up to 209 GeV. The special case of events
with b quarks is also investigated.
Combining these events with those collected at lower centre-of-mass energies,
the Standard Model predictions for the production mechanism are verified. In
addition, limits are set on anomalous couplings of neutral gauge bosons and on
effects of extra space dimensions
Neutral-Current Four-Fermion Production in e+e- Interactions at LEP
Neutral-current four-fermion production, e+e- -> ffff is studied in 0.7/fb of
data collected with the L3 detector at LEP at centre-of-mass energies
root(s)=183-209GeV. Four final states are considered: qqvv, qqll, llll and
llvv, where l denotes either an electron or a muon. Their cross sections are
measured and found to agree with the Standard Model predictions. In addition,
the e+e- -> Zgamma* -> ffff process is studied and its total cross section at
the average centre-of-mass energy 196.6GeV is found to be 0.29 +/- 0.05 +/-
0.03 pb, where the first uncertainty is statistical and the second systematic,
in agreement with the Standard Model prediction of 0.22 pb. Finally, the mass
spectra of the qqll final states are analysed to search for the possible
production of a new neutral heavy particle, for which no evidence is found
Measurement of Exclusive rho+rho- Production in Mid-Virtuality Two-Photon Interactions and Study of the gamma gamma* -> rho rho Process at LEP
Exclusive rho+rho- production in two-photon collisions between a quasi-real
photon, gamma, and a mid-virtuality photon, gamma*, is studied with data
collected at LEP at centre-of-mass energies root(s)=183-209GeV with a total
integrated luminosity of 684.8pb^-1. The cross section of the gamma gamma* ->
rho+ rho- process is determined as a function of the photon virtuality, Q^2,
and the two-photon centre-of-mass energy, W_gg, in the kinematic region:
0.2GeV^2 < Q^2 <0.85GeV^2 and 1.1GeV < W_gg < 3GeV. These results, together
with previous L3 measurements of rho0 rho0 and rho+ rho- production, allow a
study of the gamma gamma* -> rho rho process over the Q^2-region 0.2GeV^2 < Q^2
< 30 GeV^2
