    Hepatic lipoma: a case report

    Karaciğerde lipom nadir görülen benign tümörlerdir. Bu lezyonlar genellikle boyutlarına bağlı olmaksızın bulgu vermezler ve rastlantısal olarak saptanırlar. Histolojik olarak bu lezyonlar matür adipoz dokudan oluşur ve malign dejenerasyon riski yoktur. Lipomlar karakteristik görüntüleme bulgularına sahiptir. Kırkiki yaşında kadın hasta dispeptik yakınmalar ile hastanemize başvurdu. Hastaya abdominal ultrasonografi (US) yapıldı ve karaciğerde sağ lobda iyi sınırlı, solid, homojen, hiperekoik kitle lezyonu saptandı. Magnetik rezonans (MR) görüntülemede aynı lezyon T1 ve T2 ağırlıklı görüntülerde hiperintens kitle olarak izlendi. Kitle lezyonu postkontrastlı yağ baskılı T1 ağırlıklı görüntülerde homojen hipointens olup kontrast tutulumu göstermedi. US ve MR görüntüleme bulguları değerlendirildiğinde son radyolojik tanı karaciğerde lipom olarak kondu.Hepatic lipomas are rare benign tumours. They are usually asymptomatic regardless of size and are discovered incidentally. Histologically these lesions consist of mature adipose tissue and have no risk of malignant degeneration. The imaging appearance of lipomas is characteristic. A 42-year-old woman was admitted to the hospital because of dyspeptic symptoms. Abdominal ultrasound (US) examination was performed and solid, homogenous, hyperechoic, well delineated tumor mass in right liver lobe was revealed. Magnetic resonance (MR) imaging demonstrated the same lesion on T1 and T2 weighted images as hyperintense mass. The lesion was homogeneously hypointense and did not show enhancement on postcontrast fat-suppressed T1-weighted images. The final radiological diagnosis was hepatic lipoma according to US and MR image findings

    Psikiyatri kliniğinde çalışan hemşirelerin psikiyatri hemşiresinin rollerine ilişkin görüşleri Amaç: Bu çalışma, psikiyatri kliniklerinde çalışan hemşirelerin psikiyatri hemşiresinin rollerine ilişkin görüşlerinin belirlenmesi amacıyla gerçekleştirilmiştir. Yöntem: Tanımlayıcı tipte olan bu çalışma, Ocak 2011 ile Mayıs 2011 tarihleri arasında yapılmıştır. Çalışmada örneklem seçimine gidilmemiş, çalışmayı kabul eden 195 hemşire örneklemi oluşturmuştur. Veriler araştırmacılar tarafından oluşturulan 4 açık uçlu, toplam 27 sorudan oluşan anket formu ile toplanmıştır. Çalışma verileri tanımlayıcı istatistiksel analizlerle değerlendirilmiştir. Bulgular: Araştırmaya katılan hemşirelerin %80.4’ü kadındır ve yaş ortalaması 34.27±1.19’dur. Psikiyatri kliniklerinde çalışan hemşireler, klinikte yaptıkları temel girişimlerin, %76.8’inin psikofarmakolojik tedavi, %52.6’sının gözlem, %41.8’inin hastaların öz bakımını sağlamak olduğunu belirtmişler ve terapötik iletişim, atölye/uğraş terapisi ve bireysel/grup terapileri gibi girişimleri daha az yaptıklarını çoğunlukla evrak işleri ile uğraştıklarını ifade etmişlerdir. Sonuç: Çalışma kapsamındaki hemşirelerin tamamına yakını (%96.4) rol ve sorumluluklarının net olmadığını, yasalarda da yer almadığını düşündüklerini ifade etmişlerdir. Çalışma bulguları sonucunda psikiyatri hemşirelerinin rollerinin ve yetkilerinin net olarak tanımlanması gerektiği söylenebilir

    Infected pancreatic necrosis: outcomes and clinical predictors of mortality. A post hoc analysis of the MANCTRA-1 international study

    : The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135-15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359-5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138-5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184-5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598-9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090-6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286-5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p < 0.001; 95% CI 1.912-7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138-0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143-0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible.     Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.     An embedding technique to determine ττ backgrounds in proton-proton collision data

    An embedding technique is presented to estimate standard model tau tau backgrounds from data with minimal simulation input. In the data, the muons are removed from reconstructed mu mu events and replaced with simulated tau leptons with the same kinematic properties. In this way, a set of hybrid events is obtained that does not rely on simulation except for the decay of the tau leptons. The challenges in describing the underlying event or the production of associated jets in the simulation are avoided. The technique described in this paper was developed for CMS. Its validation and the inherent uncertainties are also discussed. The demonstration of the performance of the technique is based on a sample of proton-proton collisions collected by CMS in 2017 at root s = 13 TeV corresponding to an integrated luminosity of 41.5 fb(-1).Peer reviewe

    Measurement of the Splitting Function in &ITpp &ITand Pb-Pb Collisions at root&ITsNN&IT=5.02 TeV

    Data from heavy ion collisions suggest that the evolution of a parton shower is modified by interactions with the color charges in the dense partonic medium created in these collisions, but it is not known where in the shower evolution the modifications occur. The momentum ratio of the two leading partons, resolved as subjets, provides information about the parton shower evolution. This substructure observable, known as the splitting function, reflects the process of a parton splitting into two other partons and has been measured for jets with transverse momentum between 140 and 500 GeV, in pp and PbPb collisions at a center-of-mass energy of 5.02 TeV per nucleon pair. In central PbPb collisions, the splitting function indicates a more unbalanced momentum ratio, compared to peripheral PbPb and pp collisions.. The measurements are compared to various predictions from event generators and analytical calculations.Peer reviewe

    Measurement of nuclear modification factors of gamma(1S)), gamma(2S), and gamma(3S) mesons in PbPb collisions at root s(NN)=5.02 TeV

    The cross sections for ϒ(1S), ϒ(2S), and ϒ(3S) production in lead-lead (PbPb) and proton-proton (pp) collisions at √sNN = 5.02 TeV have been measured using the CMS detector at the LHC. The nuclear modification factors, RAA, derived from the PbPb-to-pp ratio of yields for each state, are studied as functions of meson rapidity and transverse momentum, as well as PbPb collision centrality. The yields of all three states are found to be significantly suppressed, and compatible with a sequential ordering of the suppression, RAA(ϒ(1S)) > RAA(ϒ(2S)) > RAA(ϒ(3S)). The suppression of ϒ(1S) is larger than that seen at √sNN = 2.76 TeV, although the two are compatible within uncertainties. The upper limit on the RAA of ϒ(3S) integrated over pT, rapidity and centrality is 0.096 at 95% confidence level, which is the strongest suppression observed for a quarkonium state in heavy ion collisions to date. © 2019 The Author(s).     Electroweak production of two jets in association with a Z boson in proton-proton collisions root s =13 TeV

    A measurement of the electroweak (EW) production of two jets in association with a Z boson in proton-proton collisions at root s = 13 TeV is presented, based on data recorded in 2016 by the CMS experiment at the LHC corresponding to an integrated luminosity of 35.9 fb(-1). The measurement is performed in the lljj final state with l including electrons and muons, and the jets j corresponding to the quarks produced in the hard interaction. The measured cross section in a kinematic region defined by invariant masses m(ll) > 50 GeV, m(jj) > 120 GeV, and transverse momenta P-Tj > 25 GeV is sigma(EW) (lljj) = 534 +/- 20 (stat) fb (syst) fb, in agreement with leading-order standard model predictions. The final state is also used to perform a search for anomalous trilinear gauge couplings. No evidence is found and limits on anomalous trilinear gauge couplings associated with dimension-six operators are given in the framework of an effective field theory. The corresponding 95% confidence level intervals are -2.6 <cwww/Lambda(2) <2.6 TeV-2 and -8.4 <cw/Lambda(2) <10.1 TeV-2. The additional jet activity of events in a signal-enriched region is also studied, and the measurements are in agreement with predictions.Peer reviewe

    Bose-Einstein correlations of charged hadrons in proton-proton collisions at s\sqrt s = 13 TeV

    Bose-Einstein correlations of charged hadrons are measured over a broad multiplicity range, from a few particles up to about 250 reconstructed charged hadrons in proton-proton collisions at s \sqrt{s} = 13 TeV. The results are based on data collected using the CMS detector at the LHC during runs with a special low-pileup configuration. Three analysis techniques with different degrees of dependence on simulations are used to remove the non-Bose-Einstein background from the correlation functions. All three methods give consistent results. The measured lengths of homogeneity are studied as functions of particle multiplicity as well as average pair transverse momentum and mass.
