    À partir d’une enquête exploratoire auprès de douze primipares francophones de la région d’Ottawa, au Canada, rencontrées trois fois (à leur huitième mois de grossesse, à deux et à six mois après l’accouchement), les auteures décrivent la situation générale des répondantes. Puis elles se penchent sur le récit de ces femmes à propos de leurs attentes et de leur réalité à l’égard de la maternité et elles analysent en particulier le discours des répondantes sur l’allaitement (lors de la deuxième rencontre, sans que cela ait été particulièrement prévu, l’allaitement occupait en effet la place centrale qui reflétait, la plupart du temps, un profond malaise). Puis les auteures présentent les politiques de santé sur l’allaitement maternel, pour ensuite illustrer le différentiel constaté entre la perspective actuellement promue en la matière et l’expérience des femmes rencontrées. Les auteures veulent illustrer comment l’expérience de l’allaitement tend à être encadrée par les professionnelles de la santé de manière telle que le bien-être des parturientes et des nouvelles mères devient un aspect secondaire dans leur expérience. En conclusion, les auteures tentent d’étayer la perspective selon laquelle l’expérience de la maternité subit d’autant plus idéologiquement l’influence du milieu hospitalier que les services offerts aux femmes qui accouchent sont congrus.From an exploratory investigation done with twelve French-speaking first-time mothers living around Ottawa, Canada, we will describe the three visits we did with them : the first visit was during the 8th month of their pregnancy, the second and third visits were at 2 and 6 months after the baby’s delivery. We will then look at their narratives of the expectations, and the reality of giving birth and becoming mothers. We will also report on their breastfeeding experiences. At our second visit, the breastfeeding experience became the main topic, which was described as a profound « malaise » (awkwardness). The second part of this publication will briefly introduce the breastfeeding health policy and describe the current environment of breastfeeding support received, which clearly values the baby’s wellness, often to the detriment of that of the woman giving birth. Lastly, we will put the emphasis on the difference between the promoted official view on breastfeeding and the actual experiences of the women we met. We will conclude by asserting that the experience of motherhood is influenced equally by the ideological perspective offered by the hospital environment, and by the lack of assistance offered to new mothers

    Validation de la grille d'observation du message de santé (GOMS)

    Cette étude méthodologique vise la validation de la grille d'observation du message de santé (GOMS). Les objectifs spécifiques sont de vérifier la consistance interne, l'homogénéité, les concepts opératoires et la fidélité inter et intra groupe du GOMS construit à partir du cadre de référence "Persuasive Health Message" de Witte (l992). Cinq messages de santé tétévisés sur le VIH/SIDA ont été utilisés auprès d'un échantillon de 110 étudiants universitaires de premier cycle en sciences santé, subdivisés en cinq groupes. Le calcul des coefficients de Cronbach et les analyses d'homogénéité selon les procédés de Guttman et de Spearman-Brown ont permis de vérifier la consistance interne de la grille GOMS et de chacune des dimensions. Les analyses factorielles en composante principale par rotation orthogonale ont permis de vérifier la validité des concepts opératoires utilisés dans la construction du GOMS: persuasion, menace et efficacité. La présentation aléatoire des messages, dans un devis plan niché (nested), a permis de vérifier par un test de MANOVA, l'effet de séquence, l'effet de groupe et l'effet du temps sur le GOMS et chacune des dimensions. La consistance interne a fourni (au temps 1) un Ì de Cronbach de .70 au GOMS et de .79, .47 et .67 aux dimensions persuasion, menace et efficacité. Les analyses d'homogénéité ont généré des coefficients de l'ordre de -.45 et -.46 au GOMS démontrant ainsi que les dimensions de persuasion et les dimensions menace et efficacité sont trop différentes pour former un tout. La saturation des facteurs démontre que la dimension persuasion ne compte pas trois mais bien deux facteurs alors qu'aux dimensions menace et efficacité s'ajoute une troisième dimension (non menace). Enfin, les tests de MANOVA ont révélés que le temps et la séquence influencent significativement les réponses obtenues pour tout le GOMS et pour la dimension persuasio

    Altimetry for the future: Building on 25 years of progress

    In 2018 we celebrated 25 years of development of radar altimetry, and the progress achieved by this methodology in the fields of global and coastal oceanography, hydrology, geodesy and cryospheric sciences. Many symbolic major events have celebrated these developments, e.g., in Venice, Italy, the 15th (2006) and 20th (2012) years of progress and more recently, in 2018, in Ponta Delgada, Portugal, 25 Years of Progress in Radar Altimetry. On this latter occasion it was decided to collect contributions of scientists, engineers and managers involved in the worldwide altimetry community to depict the state of altimetry and propose recommendations for the altimetry of the future. This paper summarizes contributions and recommendations that were collected and provides guidance for future mission design, research activities, and sustainable operational radar altimetry data exploitation. Recommendations provided are fundamental for optimizing further scientific and operational advances of oceanographic observations by altimetry, including requirements for spatial and temporal resolution of altimetric measurements, their accuracy and continuity. There are also new challenges and new openings mentioned in the paper that are particularly crucial for observations at higher latitudes, for coastal oceanography, for cryospheric studies and for hydrology. The paper starts with a general introduction followed by a section on Earth System Science including Ocean Dynamics, Sea Level, the Coastal Ocean, Hydrology, the Cryosphere and Polar Oceans and the ‘‘Green” Ocean, extending the frontier from biogeochemistry to marine ecology. Applications are described in a subsequent section, which covers Operational Oceanography, Weather, Hurricane Wave and Wind Forecasting, Climate projection. Instruments’ development and satellite missions’ evolutions are described in a fourth section. A fifth section covers the key observations that altimeters provide and their potential complements, from other Earth observation measurements to in situ data. Section 6 identifies the data and methods and provides some accuracy and resolution requirements for the wet tropospheric correction, the orbit and other geodetic requirements, the Mean Sea Surface, Geoid and Mean Dynamic Topography, Calibration and Validation, data accuracy, data access and handling (including the DUACS system). Section 7 brings a transversal view on scales, integration, artificial intelligence, and capacity building (education and training). Section 8 reviews the programmatic issues followed by a conclusion

    Serelaxin as a potential treatment for renal dysfunction in cirrhosis: Preclinical evaluation and results of a randomized phase 2 trial

    <div><p>Background</p><p>Chronic liver scarring from any cause leads to cirrhosis, portal hypertension, and a progressive decline in renal blood flow and renal function. Extreme renal vasoconstriction characterizes hepatorenal syndrome, a functional and potentially reversible form of acute kidney injury in patients with advanced cirrhosis, but current therapy with systemic vasoconstrictors is ineffective in a substantial proportion of patients and is limited by ischemic adverse events. Serelaxin (recombinant human relaxin-2) is a peptide molecule with anti-fibrotic and vasoprotective properties that binds to relaxin family peptide receptor-1 (RXFP1) and has been shown to increase renal perfusion in healthy human volunteers. We hypothesized that serelaxin could ameliorate renal vasoconstriction and renal dysfunction in patients with cirrhosis and portal hypertension.</p><p>Methods and findings</p><p>To establish preclinical proof of concept, we developed two independent rat models of cirrhosis that were characterized by progressive reduction in renal blood flow and glomerular filtration rate and showed evidence of renal endothelial dysfunction. We then set out to further explore and validate our hypothesis in a phase 2 randomized open-label parallel-group study in male and female patients with alcohol-related cirrhosis and portal hypertension. Forty patients were randomized 1:1 to treatment with serelaxin intravenous (i.v.) infusion (for 60 min at 80 μg/kg/d and then 60 min at 30 μg/kg/d) or terlipressin (single 2-mg i.v. bolus), and the regional hemodynamic effects were quantified by phase contrast magnetic resonance angiography at baseline and after 120 min. The primary endpoint was the change from baseline in total renal artery blood flow.</p><p>Therapeutic targeting of renal vasoconstriction with serelaxin in the rat models increased kidney perfusion, oxygenation, and function through reduction in renal vascular resistance, reversal of endothelial dysfunction, and increased activation of the AKT/eNOS/NO signaling pathway in the kidney. In the randomized clinical study, infusion of serelaxin for 120 min increased total renal arterial blood flow by 65% (95% CI 40%, 95%; <i>p <</i> 0.001) from baseline. Administration of serelaxin was safe and well tolerated, with no detrimental effect on systemic blood pressure or hepatic perfusion. The clinical study’s main limitations were the relatively small sample size and stable, well-compensated population.</p><p>Conclusions</p><p>Our mechanistic findings in rat models and exploratory study in human cirrhosis suggest the therapeutic potential of selective renal vasodilation using serelaxin as a new treatment for renal dysfunction in cirrhosis, although further validation in patients with more advanced cirrhosis and renal dysfunction is required.</p><p>Trial registration</p><p>ClinicalTrials.gov <a href="https://clinicaltrials.gov/ct2/show/NCT01640964" target="_blank">NCT01640964</a></p></div

    SEIS: Insight’s Seismic Experiment for Internal Structure of Mars

    By the end of 2018, 42 years after the landing of the two Viking seismometers on Mars, InSight will deploy onto Mars’ surface the SEIS (Seismic Experiment for Internal Structure) instrument; a six-axes seismometer equipped with both a long-period three-axes Very Broad Band (VBB) instrument and a three-axes short-period (SP) instrument. These six sensors will cover a broad range of the seismic bandwidth, from 0.01 Hz to 50 Hz, with possible extension to longer periods. Data will be transmitted in the form of three continuous VBB components at 2 sample per second (sps), an estimation of the short period energy content from the SP at 1 sps and a continuous compound VBB/SP vertical axis at 10 sps. The continuous streams will be augmented by requested event data with sample rates from 20 to 100 sps. SEIS will improve upon the existing resolution of Viking’s Mars seismic monitoring by a factor of ∼ 2500 at 1 Hz and ∼ 200 000 at 0.1 Hz. An additional major improvement is that, contrary to Viking, the seismometers will be deployed via a robotic arm directly onto Mars’ surface and will be protected against temperature and wind by highly efficient thermal and wind shielding. Based on existing knowledge of Mars, it is reasonable to infer a moment magnitude detection threshold of Mw ∼ 3 at 40◦ epicentral distance and a potential to detect several tens of quakes and about five impacts per year. In this paper, we first describe the science goals of the experiment and the rationale used to define its requirements. We then provide a detailed description of the hardware, from the sensors to the deployment system and associated performance, including transfer functions of the seismic sensors and temperature sensors. We conclude by describing the experiment ground segment, including data processing services, outreach and education networks and provide a description of the format to be used for future data distribution

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
