    An adaptive scheme for wheelchair navigation collaborative control

    In this paper we propose a system where machine and human cooperate at every situation via a reactive emergent behavior, so that the person is always in charge of his/her own motion. Our approach relies on locally evaluating the performance of the human and the wheelchair for each given situation. Then, both their motion commands are weighted according to those efficiencies and combined in a reactive way. This approach benefits from the advantages of typical reactive behaviors to combine different sources of information in a simple, seamless way into an emergent trajectory.Peer ReviewedPostprint (author’s final draft

    Alfalfa genomic selection for different stress-prone growing regions

    Alfalfa (Medicago sativa L.) selection for stress-prone regions has high priority for sustainable crop–livestock systems. This study assessed the genomic selection (GS) ability to predict alfalfa breeding values for drought-prone agricultural sites of Algeria, Morocco, and Argentina; managed-stress (MS) environments of Italy featuring moderate or intense drought; and one Tunisian site irrigated with moderately saline water. Additional aims were to investigate genotype × environment interaction (GEI) patterns and the effect on GS predictions of three single-nucleotide polymorphism (SNP) calling procedures, 12 statistical models that exclude or incorporate GEI, and allele dosage information. Our study included 127 genotypes from a Mediterranean reference population originated from three geographically contrasting populations, genotyped via genotyping-by-sequencing and phenotyped based on multi-year biomass dry matter yield of their dense-planted half-sib progenies. The GEI was very large, as shown by 27-fold greater additive genetic variance × environment interaction relative to the additive genetic variance and low genetic correlation for progeny yield responses across environments. The predictive ability of GS (using at least 37,969 SNP markers) exceeded 0.20 for moderate MS (representing Italian stress-prone sites) and the sites of Algeria and Argentina while being quite low for the Tunisian site and intense MS. Predictions of GS were complicated by rapid linkage disequilibrium decay. The weighted GBLUP model, GEI incorporation into GS models, and SNP calling based on a mock reference genome exhibited a predictive ability advantage for some environments. Our results support the specific breeding for each target region and suggest a positive role for GS in most regions when considering the challenges associated with phenotypic selection.EEA Santiago del EsteroFil: Annicchiarico, Paolo.     A new collaborative shared control strategy for continuous elder/robot assisted navigation

    In nowadays aging society, many people require mobility assistance. Autonomous wheelchairs may provide some help, but they are not supposed to overtake all control on human mobility, as this is reported to lead to loss of residual capabilities and frustration. Instead, persons and wheelchairs are expected to cooperate. Traditionally, shared control hands control from human to robot depending on a triggering event. In this paper, though, we propose a method to allow constant cooperation between humans and robots, so that both have some weight in the emergent navigating behavior. We have tested the proposed method on a robotized Meyra wheelchair at Santa Lucia Hospedale in Rome with several volunteering in-patients presenting different disabilities.     Extracellular serine empowers epidermal proliferation and psoriasis-like symptoms

    The contribution of nutrient availability to control epidermal cell proliferation, inflammation, and hyperproliferative diseases remains unknown. Here, we studied extracellular serine and serine/glycine metabolism using human keratinocytes, human skin biopsies, and a mouse model of psoriasis-like disease. We focused on a metabolic enzyme, serine hydroxymethyltransferase (SHMT), that converts serine into glycine and tetrahydrofolate-bound one‑carbon units to support cell growth. We found that keratinocytes are both serine and glycine auxotrophs. Metabolomic profiling and hypoxanthine supplementation indicated that SHMT silencing/inhibition reduced cell growth through purine depletion, leading to nucleotide loss. In addition, topical application of an SHMT inhibitor suppressed both keratinocyte proliferation and inflammation in the imiquimod model and resulted in a decrease in psoriasis-associated gene expression. In conclusion, our study highlights SHMT2 activity and serine/glycine availability as an important metabolic hub controlling both keratinocyte proliferation and inflammatory cell expansion in psoriasis and holds promise for additional approaches to treat skin diseases

    Variation of morphological descriptors for the evaluation of tomato germplasm and their stability across different growing conditions

    [EN] Germplasm and breeding materials are usually characterized using morphological and agronomic descriptors, which should have a high heritability. Despite the widespread use of tomato (Solanum lycopersicum) standardized descriptors, little information exists on environmental effects on descriptor values and their heritability. We have evaluated 12 tomato accessions from seven cultivar groups in three different environments (open-field conventional, open-field organic, and greenhouse) and characterized them with 36 descriptors. A wide range of variation was found for most descriptors, demonstrating their utility for describing tomato materials and their diversity and relationships. The analysis of descriptors variation reveals that while for some descriptors with a simple genetic control the accession effect accounts for 100% of the variation, for others like yield per plant only 10.83% of the variation observed is due to the accession effect. Although significant differences were found among environments for most descriptors, including a much higher yield in the open-field conventional environment than in the two others, the environmental effect was low for most traits. However, the genotype×environment effect generally had an important contribution to the structure of variation for many descriptors, and for three traits it had the highest contribution to the percentage of the sum of squares. As a result of the variation structure, the heritability values are high (> 0.7) for only 10 descriptors, while for five is low (< 0.3). Principal components analysis (PCA) reveals that projections in the PCA graph of a same accession grown in different environments plot together in the same area of the PCA graph. Although cultivar groups are generally clearly separated in the PCA graph, accessions from the same cultivar group in some cases are intermixed.     Management of acute diverticulitis with pericolic free gas (ADIFAS). an international multicenter observational study

    Background: There are no specific recommendations regarding the optimal management of this group of patients. The World Society of Emergency Surgery suggested a nonoperative strategy with antibiotic therapy, but this was a weak recommendation. This study aims to identify the optimal management of patients with acute diverticulitis (AD) presenting with pericolic free air with or without pericolic fluid. Methods: A multicenter, prospective, international study of patients diagnosed with AD and pericolic-free air with or without pericolic free fluid at a computed tomography (CT) scan between May 2020 and June 2021 was included. Patients were excluded if they had intra-abdominal distant free air, an abscess, generalized peritonitis, or less than a 1-year follow-up. The primary outcome was the rate of failure of nonoperative management within the index admission. Secondary outcomes included the rate of failure of nonoperative management within the first year and risk factors for failure. Results: A total of 810 patients were recruited across 69 European and South American centers; 744 patients (92%) were treated nonoperatively, and 66 (8%) underwent immediate surgery. Baseline characteristics were similar between groups. Hinchey II-IV on diagnostic imaging was the only independent risk factor for surgical intervention during index admission (odds ratios: 12.5, 95% CI: 2.4-64, P =0.003). Among patients treated nonoperatively, at index admission, 697 (94%) patients were discharged without any complications, 35 (4.7%) required emergency surgery, and 12 (1.6%) percutaneous drainage. Free pericolic fluid on CT scan was associated with a higher risk of failure of nonoperative management (odds ratios: 4.9, 95% CI: 1.2-19.9, P =0.023), with 88% of success compared to 96% without free fluid ( P &lt;0.001). The rate of treatment failure with nonoperative management during the first year of follow-up was 16.5%. Conclusion: Patients with AD presenting with pericolic free gas can be successfully managed nonoperatively in the vast majority of cases. Patients with both free pericolic gas and free pericolic fluid on a CT scan are at a higher risk of failing nonoperative management and require closer observation

    How future surgery will benefit from SARS-COV-2-related measures: a SPIGC survey conveying the perspective of Italian surgeons

    COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March-April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients' management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p &lt; 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p &lt; 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p &lt; 0.0001) or urgent (20.4% vs. 38.5%; p &lt; 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons' personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p &lt; 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon-patient/family communication. From the Italian surgeons' perspective, COVID-related measures will continue to influence future surgical clinical practice

    Molecular mechanisms of cell death: recommendations of the Nomenclature Committee on Cell Death 2018.

    Over the past decade, the Nomenclature Committee on Cell Death (NCCD) has formulated guidelines for the definition and interpretation of cell death from morphological, biochemical, and functional perspectives. Since the field continues to expand and novel mechanisms that orchestrate multiple cell death pathways are unveiled, we propose an updated classification of cell death subroutines focusing on mechanistic and essential (as opposed to correlative and dispensable) aspects of the process. As we provide molecularly oriented definitions of terms including intrinsic apoptosis, extrinsic apoptosis, mitochondrial permeability transition (MPT)-driven necrosis, necroptosis, ferroptosis, pyroptosis, parthanatos, entotic cell death, NETotic cell death, lysosome-dependent cell death, autophagy-dependent cell death, immunogenic cell death, cellular senescence, and mitotic catastrophe, we discuss the utility of neologisms that refer to highly specialized instances of these processes. The mission of the NCCD is to provide a widely accepted nomenclature on cell death in support of the continued development of the field

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
