Services just for men? Insights from a national study of the well men services pilots.
Men continue to have a lower life expectancy in most countries compared to women. Explanations of this gendered health inequality tend to focus on male risk taking, unhealthy lifestyle choices and resistance to seeking help from health services. In the period 2005-2008 the Scottish Government funded a nationwide community health promotion programme aimed at improving men's health, called Well Men Service Pilots (henceforth WMS)
Association of Panton Valentine Leukocidin (PVL) genes with methicillin resistant Staphylococcus aureus (MRSA) in Western Nepal: a matter of concern for community infections (a hospital based prospective study)
BACKGROUND: Methicillin resistant Staphylococcus aureus (MRSA) is a major human pathogen associated with nosocomial and community infections. Panton Valentine leukocidin (PVL) is considered one of the important virulence factors of S. aureus responsible for destruction of white blood cells, necrosis and apoptosis and as a marker of community acquired MRSA. This study was aimed to determine the prevalence of PVL genes among MRSA isolates and to check the reliability of PVL as marker of community acquired MRSA isolates from Western Nepal. METHODS: A total of 400 strains of S. aureus were collected from clinical specimens and various units (Operation Theater, Intensive Care Units) of the hospital and 139 of these had been confirmed as MRSA by previous study. Multiplex PCR was used to detect mecA and PVL genes. Clinical data as well as antimicrobial susceptibility data was analyzed and compared among PVL positive and negative MRSA isolates. RESULTS: Out of 139 MRSA isolates, 79 (56.8 %) were PVL positive. The majority of the community acquired MRSA (90.4 %) were PVL positive (Positive predictive value: 94.9 % and negative predictive value: 86.6 %), while PVL was detected only in 4 (7.1 %) hospital associated MRSA strains. None of the MRSA isolates from hospital environment was found positive for the PVL genes. The majority of the PVL positive strains (75.5 %) were isolated from pus samples. Antibiotic resistance among PVL negative MRSA isolates was found higher as compared to PVL positive MRSA. CONCLUSION: Our study showed high prevalence of PVL among community acquired MRSA isolates. Absence of PVL among MRSA isolates from hospital environment indicates its poor association with hospital acquired MRSA and therefore, PVL may be used a marker for community acquired MRSA. This is first study from Nepal, to test PVL among MRSA isolates from hospital environment. Infection by the hepatitis C virus in chronic renal failure patients undergoing hemodialysis in Mato Grosso state, central Brazil: a cohort study
BACKGROUND: Hepatitis C virus (HCV) is a significant problem for patients undergoing hemodialysis therapy. This situation has never been studied in Mato Grosso state, central Brazil. This study was conducted aiming to estimate the prevalence of the anti-HCV and the incidence of seroconversion in the main metropolitan region of the state. METHODS: 433 patients from the six hemodialysis units were interviewed and anti-HCV was tested by a third-generation enzyme immunoassay. An open cohort of patients who tested negative for anti-HCV at the entry of the study was created and seroconversions was assessed monthly. The staff responsible for the units were interviewed to assess whether the infection control measures were being followed. Logistic and Cox regression analysis were performed in order to assess risk factor to HCV. RESULTS: The entry on the study took place between January 2002 and June 2005. 73 out of 433 (16.9%, CI95%: 13.3–20.8) was found to be anti-HCV reactive. The multivariate analysis indicated as risk factors associated to anti-HCV the duration of the hemodialysis treatment, the number of transfusions received, and the unit of treatment. An open cohort of 360 patients who tested negative for anti-HCV was created, with a following average of 24 (± 15) months. Forty seroconversions were recorded corresponding to an incidence density of 4.6/1000 patient-months, ranges 0 to 30 among the units. Cox regression indicated the time of hemodialysis (RR = 2.2; CI95%: 1.1–4.6; p < 0.05) and the unit where treatment was performed (RR = 42.4; CI95%: 9.9–180.5; p < 0.05) as risk factors for seroconversion. The three units with highest anti-HCV prevalence and incidence were identified as those that more frequently failed to apply control measures. CONCLUSION: The study demonstrated high prevalence and incidence of anti-HCV in some of the hemodialysis units. Time on hemodialysis therapy was an independent factor associated to HCV. Blood transfusion was associated with anti-HCV in initial survey but was not important in incident cases. Failure of applying control meaures was more evident in units with the highest HCV prevalence and incidence. The results suggest that nosocomial transmission was the main spread factor of HCV in the studied population
The impact of employee level and work stress on mental health and GP service use: an analysis of a sample of Australian government employees
BACKGROUND: This study sought to identify the extent to which employee level and work stressors were associated with mental health problems experienced by Australian government employees, and with their use of primary care services. METHODS: 806 government employees aged between 40 and 44 years were surveyed as part of an epidemiological study conducted in Australia. Data collected from participants included sociodemographic attributes, physical health, psychological measures and work stressors relating to job control, job demands, job security and skills discretion at work. For 88% of these participants, information on visits made to general practitioners (GPs) for the six months before and after their survey interview was obtained from health insurance records. RESULTS: When work stress and personal factors were taken into account, men at more junior levels reported better mental health, more positive affect and used fewer GP services. Women at middle-management levels obtained less GP care than their more senior counterparts. Both men and women who reported higher levels of work stress were found to have poorer mental health and well-being. The impact of such stressors on GP service use, however, differed for men and women. CONCLUSION: Measures of work stress and not employee level affect the mental health and well-being of government employees. For governments with responsibility for funding health care services, reducing work stress experienced by their own employees offers potential benefits by improving the health of their workforce and reducing outlays for such services
Spin alignment measurements using vector mesons with ALICE detector at the LHC
We present new measurements related to spin alignment of K*(0) vector
mesons at mid-rapidity for Pb-Pb collisions at root s(NN) = 2.76 and
5.02 TeV. The spin alignment measurements are carried out with respect
to production plane and 2nd order event plane. At low p(T) the spin
density matrix element rho(00) for K*(0) is found to have values
slightly below 1/3, while it is consistent with 1/3, i.e. no spin
alignment, at high p(T). Similar values of rho(00) are observed with
respect to both production plane and event plane. Within statistical and
systematic uncertainties, rho(00) values are also found to be
independent of root s(NN). rho(00) also shows centrality dependence with
maximum deviation from 1/3 for mid-central collisions with respect to
both the kinematic planes. The measurements for K*(0) in pp collisions
at root s = 13 TeV and for K-s(0) (a spin 0 hadron) in 20-40\% central
Pb-Pb collisions at root s(NN) = 2.76 TeV are consistent with no spin
alignment
Muon physics at forward rapidity with the ALICE detector upgrade
ALICE is the experiment specifically designed to study the Quark-Gluon
Plasma (QGP) in heavy-ion collisions at the CERN LHC. The ALICE detector
will be upgraded during the Long Shutdown 2, planned for 2019-2020, in
order to cope with the maximum interaction rate of 50 kHz of Pb-Pb
collisions foreseen for Runs 3 and 4. The ambitious programme of
high-precision measurements, expected for muon physics after 2020,
requires an upgrade of the front-end and readout electronics of the
existing Muon Spectrometer. This concerns the Cathode Pad Chambers (CPC)
used for tracking and the Resistive Plate Chambers (RPC) used for
triggering and for muon identification. The Muon Forward Tracker (MFT),
an internal tracker added in front of the front absorber of the existing
Muon Spectrometer, is also part of the ALICE detector upgrade programme.
It is based on an assembly of circular planes made of Monolithic Active
Pixel Sensors (MAPS), covering the pseudorapidity range 2.5 < eta < 3.6.
The MFT will improve present measurements and enable new ones. A
selection of results from physics performance studies will be presented,
together with an overview of the technical aspects of the upgrade
project
Higher moment fluctuations of identified particle distributions from ALICE
Cumulants of conserved charges fluctuations are regarded as a potential tool
to study the criticality in the QCD phase diagram and to determine the
freeze-out parameters in a model-independent way. At LHC energies, the
measurements of the ratio of the net-baryon (net-proton) cumulants can be used
to test the lattice QCD predictions. In this work, we present the first
measurements of cumulants of the net-proton number distributions up to
order in Pb--Pb collisions at = 2.76 and 5.02 TeV as a
function of collision centrality. We compare our cumulant ratios results with
the STAR experiment net-proton results measured in the first phase of the Beam
Energy Scan program at RHIC. The results can be used to obtain the chemical
freeze-out parameters at LHC.Comment: 4 pages, 3 figures, Proceedings of XXVIIth International Conference

Direct photon elliptic flow in Pb-Pb collisions at root s(NN)=2.76 TeV
The elliptic flow of inclusive and direct photons was measured at mid-rapidity in two centrality classes 0-20% and 20-40% in Pb-Pb collisions at root s(NN) = 2.76 TeV by ALICE. Photons were detected with the highly segmented electromagnetic calorimeter PHOS and via conversions in the detector material with the e(broken vertical bar)e pairs reconstructed in the central tracking system. The results of the two methods were combined and the direct-photon elliptic flow was extracted in the transverse momentum range 0.9 < p(T) < 6.2 GeV/c. A comparison to RHIC data shows a similar magnitude of the measured direct-photon elliptic flow. Hydrodynamic and transport model calculations are systematically lower than the data, but are found to be compatible. (C) 2018 The Author. Published by Elsevier B.V.Peer reviewe
