    Amyotrophic lateral sclerosis patients' self-reported satisfaction with assistive technology

    Introduction : Assistive devices are prescribed for amyotrophic lateral sclerosis (ALS) patients with motor deficits, but little is known about their perceived benefit. Therefore, we assessed ALS patients' satisfaction with commonly prescribed devices. Methods : A telephone survey of 63 ALS patients from a single multidisciplinary clinic was conducted to assess the frequency of use, perceived usefulness, and satisfaction with 33 assistive devices. Results : Of those assistive technologies used “often or always” by ≥20% of respondents, arm rails by the toilet, elevated toilet seat, shower seat, shower bars, and slip-on shoes were ranked very highly for both usefulness and satisfaction. The ankle brace for ambulation, transfer board, speaker phone, and electronic seating controls were also ranked highly. The button hook, dressing stick, and long-handled reaching tool received low ratings for both usefulness and satisfaction. Conclusions : ALS patients reported high usefulness and satisfaction levels with all bathroom adaptive devices and certain low-technology devices.     Descent toward the icehouse: Eocene sea surface cooling inferred from GDGT distributions

    The TEX86 proxy, based on the distribution of marine isoprenoidal glycerol dialkyl glycerol tetraether lipids (GDGTs), is increasingly used to reconstruct sea surface temperature (SST) during the Eocene epoch (56.0–33.9 Ma). Here we compile published TEX86 records, critically reevaluate them in light of new understandings in TEX86 palaeothermometry, and supplement them with new data in order to evaluate long-term temperature trends in the Eocene. We investigate the effect of archaea other than marine Thaumarchaeota upon TEX86 values using the branched-to-isoprenoid tetraether index (BIT), the abundance of GDGT-0 relative to crenarchaeol (%GDGT-0), and the Methane Index (MI). We also introduce a new ratio, % GDGTRS, which may help identify Red Sea-type GDGT distributions in the geological record. Using the offset between TEX86H and TEX86L(ΔH-L) and the ratio between GDGT-2 and GDGT-3 ([2]/[3]), we evaluate different TEX86 calibrations and present the first integrated SST compilation for the Eocene (55 to 34 Ma). Although the available data are still sparse some geographic trends can now be resolved. In the high latitudes (>55°), there was substantial cooling during the Eocene (~6°C). Our compiled record also indicates tropical cooling of ~2.5°C during the same interval. Using an ensemble of climate model simulations that span the Eocene, our results indicate that only a small percentage (~10%) of the reconstructed temperature change can be ascribed to ocean gateway reorganization or paleogeographic change. Collectively, this indicates that atmospheric carbon dioxide (pCO2) was the likely driver of surface water cooling during the descent toward the icehouse

    Evaluation of appendicitis risk prediction models in adults with suspected appendicitis

    Background Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis. Methods A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis). Results Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P < 0·001). Of 15 validated risk prediction models, the Adult Appendicitis Score performed best (cut‐off score 8 or less, specificity 63·1 per cent, failure rate 3·7 per cent). The Appendicitis Inflammatory Response Score performed best for men (cut‐off score 2 or less, specificity 24·7 per cent, failure rate 2·4 per cent). Conclusion Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy.     MgSe has been studied using energy dispersive X-ray diffraction to 202 GPa and local density approximation and ultrasoft pseudopotentials to 500 GPa. MgSe undergoes a “continuous” phase transformation from the rocksalt to FeSi (B28) beginning at around 99 &#177; 8 GPa and approaching sevenfold coordination at 202 GPa. Theoretical computation finds the B28 transition beginning at 58 GPa followed by a transition to an orthorhombic distortion of the B2 structure at 429 GPa

    Petrology of igneous clasts in Northwest Africa 7034: Implications for the petrologic diversity of the martian crust

    The martian meteorite Northwest Africa (NWA) 7034 was examined both petrographically and geochemically using several micro-beam techniques including electron probe microanalysis and secondary ion mass spectrometry. We have identified various clast types of igneous, sedimentary, and impact origin that occur within the breccia, and we define a classification scheme for these materials based on our observations, although our primary focus here is on the petrology of the igneous clasts. A number of different igneous clasts are present in this meteorite, and our study revealed the presence of at least four different igneous lithologies (basalt, basaltic andesite, trachyandesite, and an Fe, Ti, and P (FTP) rich lithology). These lithologies do not appear to be related by simple igneous processes such as fractional crystallization, indicating NWA 7034 is a polymict breccia that contains samples from several different igneous sources. The basalt lithologies are a good match for measured rock compositions from the martian surface, however more exotic lithologies (e.g., trachyandesite and FTP lithologies) show this meteorite contains previously unsampled rock types from Mars. These new rock types provide evidence for a much greater variety of igneous rocks within the martian crust than previously revealed by martian meteorites, and supports recent rover observations of lithologic diversity across the martian surface. Furthermore, the ancient ages for the lithologic components in NWA 7034 indicate Mars developed this lithologic diversity in the early stages of crust formation
