    Rentabilidad económica de explotaciones agrarias de secano según tamaño y tipo de laboreo

    [EN] The objective of the present study was to assess, in semiarid conditions of central Spain, the economic feasibility of chisel ploughing and no-tillage compared to mouldboard ploughing for rainfed winter wheat and forage legume production on different farm sizes. A decision support system was used to solve for the least-cost machinery selection for each farm enterprise and tillage system considered. No differences were observed in either wheat or forage vetch crop yields averaged across several years, irrespective of the tillage system used. The economic performance was found to depend on the tillage system adopted and farm size.[ES] El objetivo de este trabajo es evaluar, desde un punto de vista económico, el efecto que supone pasar de un sistema de laboreo tradicional a uno de laboreo reducido en explotaciones cerealistas de la meseta castellana. Se utilizan datos experimentales de campo para estimar las producciones, así como un sistema de decisión experto para seleccionar el parque de maquinaria que conlleva el menor coste de utilización. La rotación de cultivos que se ha considerado ha sido trigo/leguminosa. No se encontraron diferencias significativas entre los rendimientos obtenidos en los tres sistemas de laboreo comparados.     Función sexual antes y después de la reparación quirúrgica del prolapso genital

    Objetivo: Comparar la función sexual (FS) de mujeres con prolapso genital (PG) antes y después de su reparación quirúrgica. Métodos: Investigación de tipo comparativa y aplicada, con diseño cuasi experimental, prospectivo y de campo, donde se evaluó la FS de mujeres con diagnóstico de PG antes y después del tratamiento quirúrgico con técnicas convencionales, mediante el Cuestionario Sexual para Prolapso genital e Incontinencia Urinaria versión corta (PISQ-12). Resultados: Al comparar la FS antes y después de la cirugía reparadora del PG, se determinó que tanto la puntuación total del PISQ-12 (15,90 ± 6,51 vs. 32,17 ± 3,62) como las puntuaciones de las dimensiones respuesta sexual (5,87 ± 2,80 vs. 10,97 ± 2,80) y limitaciones sexuales femeninas (4,88 ± 3,90 vs. 16,77 ± 3,00) fueron significativamente más altas luego de la intervención quirúrgica (p<0,001), a excepción del indicador intensidad del orgasmo (0,80 ± 0,71 vs. 0,87± 0,73; p= 0,722) y la dimensión limitaciones sexuales de la pareja (4,37 ± 2,14 vs. 3,56 ± 2,70; p=0,815) donde sus puntuaciones antes y después del tratamiento quirúrgico no fueron estadísticamente significativas (p&gt;0,05). Conclusiones: Las mujeres con PG presentan una pobre FS, la cirugía reparadora del PG por técnicas convencionales mejoró significativamente la FS de las pacientes con disfunción del piso pélvico, permitiéndoles obtener a estas mujeres una vida sexual más placentera, con mejoría de su calidad de vida

    Long-term effect of 2 intensive statin regimens on treatment and incidence of cardiovascular events in familial hypercholesterolemia : The SAFEHEART study

    Funding: This study was supported by Fundación Hipercolesterolemia Familiar; Grant G03/181 Grant 08-2008 Centro Nacional de Investigaci?n Cardiovascular (CNIC).Background: Maximal doses of potent statins are the basement of treatment of familial hypercholesterolemia (FH). Little is known about the use of different statin regimens in FH. Objectives: The objectives of the study were to describe the treatment changes and low-density lipoprotein cholesterol (LDL-C) goal achievement with atorvastatin (ATV) and rosuvastatin (RV) in the SAFEHEART cohort, as well as to analyze the incidence of atherosclerotic cardiovascular events (ACVEs) and changes in the cardiovascular risk. Methods: SAFEHEART is a prospective follow-up nationwide cohort study in a molecularly defined FH population. The patients were contacted on a yearly basis to obtain relevant changes in life habits, medication, and ACVEs. Results: A total of 1939 patients were analyzed. Median follow-up was 6.6 years (5-10). The estimated 10-year risk according the SAFEHEART risk equation was 1.61 (0.67-3.39) and 1.22 (0.54-2.93) at enrollment for ATV and RV, respectively (P <.001). There were no significant differences at the follow-up: 1.29 (0.54-2.82) and 1.22 (0.54-2.76) in the ATV and RV groups, respectively (P =.51). Sixteen percent of patients in primary prevention with ATV and 18% with RV achieved an LDL-C <100 mg/dL and 4% in secondary prevention with ATV and 5% with RV achieved an LDL-C <70 mg/dL. The use of ezetimibe was marginally greater in the RV group. One hundred sixty ACVEs occurred during follow-up, being its incidence rate 1.1 events/100 patient-years in the ATV group and 1.2 in the RV group (P =.58). Conclusion: ATV and RV are 2 high-potency statins widely used in FH. Although the reduction in LDL-C levels was greater with RV than with ATV, the superiority of RV for reducing ACVEs was not demonstrated
