    Metodologia de construção do itinerårio do paciente candidato ao transplante cardíaco

    The aim was to report the experience regarding an educational activity on the itinerary of patients who are candidates for heart transplantation. This is a descriptive-reflexive study, of a qualitative nature, in the form of an experience report. The study setting was in a University Hospital in the State of Rio de Janeiro, in March 2023, with residents from the cardiovascular nursing program, tutors, and nurses on duty with experience in transplantation. The activity proposed in a playful way using kraft paper, to build a mural of continuities and images of visual representation with the technologies and actors involved in the patient's itinerary, enabled discussion about the transplant recipient's journey from indication for transplant to follow-up at the post-operative period, involving possible entry points according to the patient's clinic and socio-family context, through care mapping. At the end of the activity, it was possible to improve the participants' knowledge, explore teamwork, creatively organize the proposed itineraries, clarify doubts, and contribute to improving assistance in different areas of activity beyond hospital care.Objetivou-se relatar a experiĂȘncia referente a uma atividade educativa sobre o itinerĂĄrio do paciente candidato ao transplante cardĂ­aco. Trata-se de um estudo descritivo-reflexivo, de natureza qualitativa, na modalidade de relato de experiĂȘncia. O cenĂĄrio de estudo foi em um Hospital UniversitĂĄrio do Estado do Rio de Janeiro, no mĂȘs de março de 2023, com a participação de residentes do programa de enfermagem cardiovascular, tutores e enfermeiros plantonistas com experiĂȘncia em transplante. A atividade proposta de forma lĂșdica com uso de papel kraft, para construção de mural de continuidades e imagens de representatividade visual com as tecnologias e atores envolvidos no itinerĂĄrio do paciente possibilitou a discussĂŁo sobre o percurso do transplantado desde a indicação ao transplante atĂ© o acompanhamento no pĂłs-operatĂłrio, envolvendo as possĂ­veis portas de entrada de acordo com a clĂ­nica do paciente e contexto sociofamiliar, atravĂ©s do mapeamento do cuidado.     High anti-SARS-CoV-2 antibody seroconversion rates before the second wave in Manaus, Brazil, and the protective effect of social behaviour measures: results from the prospective DETECTCoV-19 cohort

    Background: The city of Manaus, Brazil, has seen two collapses of the health system due to the COVID-19 pandemic. We report anti-SARS-CoV-2 nucleocapsid IgG antibody seroconversion rates and associated risk factors in Manaus residents before the second wave of the epidemic in Brazil. Methods: A convenience sample of adult (aged ≄18 years) residents of Manaus was recruited through online and university website advertising into the DETECTCoV-19 study cohort. The current analysis of seroconversion included a subgroup of DETECTCoV-19 participants who had at least two serum sample collections separated by at least 4 weeks between Aug 19 and Oct 2, 2020 (visit 1), and Oct 19 and Nov 27, 2020 (visit 2). Those who reported (or had no data on) having a COVID-19 diagnosis before visit 1, and who were positive for anti-SARS-CoV-2 nucleocapsid IgG antibodies at visit 1 were excluded. Using an in-house ELISA, the reactivity index (RI; calculated as the optical density ratio of the sample to the negative control) for serum anti-SARS-CoV-2 nucleocapsid IgG antibodies was measured at both visits. We calculated the incidence of seroconversion (defined as RI values ≀1·5 at visit 1 and ≄1·5 at visit 2, and a ratio >2 between the visit 2 and visit 1 RI values) during the study period, as well as incidence rate ratios (IRRs) through cluster-corrected and adjusted Poisson regression models to analyse associations between seroconversion and variables related to sociodemographic characteristics, health access, comorbidities, COVID-19 exposure, protective behaviours, and symptoms. Findings: 2496 DETECTCoV-19 cohort participants returned for a follow-up visit between Oct 19 and Nov 27, 2020, of whom 204 reported having COVID-19 before the first visit and 24 had no data regarding previous disease status. 559 participants were seropositive for anti-SARS-CoV-2 nucleocapsid IgG antibodies at baseline. Of the remaining 1709 participants who were seronegative at baseline, 71 did not meet the criteria for seroconversion and were excluded from the analyses. Among the remaining 1638 participants who were seronegative at baseline, 214 showed seroconversion at visit 2. The seroconversion incidence was 13·06% (95% CI 11·52–14·79) overall and 6·78% (5·61–8·10) for symptomatic seroconversion, over a median follow-up period of 57 days (IQR 54–61). 48·1% of seroconversion events were estimated to be asymptomatic. The sample had higher proportions of affluent and higher-educated people than those reported for the Manaus city population. In the fully adjusted and corrected model, risk factors for seroconversion before visit 2 were having a COVID-19 case in the household (IRR 1·49 [95% CI 1·21–1·83]), not wearing a mask during contact with a person with COVID-19 (1·25 [1·09–1·45]), relaxation of physical distancing (1·31 [1·05–1·64]), and having flu-like symptoms (1·79 [1·23–2·59]) or a COVID-19 diagnosis (3·57 [2·27–5·63]) between the first and second visits, whereas working remotely was associated with lower incidence (0·74 [0·56–0·97]). Interpretation: An intense infection transmission period preceded the second wave of COVID-19 in Manaus. Several modifiable behaviours increased the risk of seroconversion, including non-compliance with non-pharmaceutical interventions measures such as not wearing a mask during contact, relaxation of protective measures, and non-remote working. Increased testing in high-transmission areas is needed to provide timely information about ongoing transmission and aid appropriate implementation of transmission mitigation measures.     O perfil semiolĂłgico do paciente portador de hemorragia digestiva alta

    OBJETIVO: O seguinte estudo objetivou descrever a semiologia do paciente portador de hemorragia digestiva alta, considerando como determinante na avaliação de potencias focos hemorrĂĄgicos. METODOLOGIA: Foram realizadas buscas nas plataformas do SciELO, LILACS, PubMed, Scopus e Google Scholar,utilizando os descritores gastrointestinal bleeding, peptic ulcerous disease e varicose hemorrhage, sendo identificados 35 estudos, dos quais foram incluĂ­dos 13 artigos completos. Desses estudos, 5 avaliaram as principais etiologias, 2 o surgimento de novos testes diagnĂłsticos, 2 analisaram os aspectos epidemiolĂłgicos e 1 a sintomatologia apresentada pelo acometimento da hemorragia digestiva alta. Observou-se inicialmente a abundĂąncias de informaçÔes conceituais sobre o sangramento, como um transtorno clĂ­nico comum, acompanhada de inĂșmeras manifestaçÔes, considerando que o foco hemorrĂĄgico pode ocorrer em qualquer porção do trato gastrointestinal. Neste estudo, todas as publicaçÔes eleitas apresentaram o quadro semiolĂłgico composto por algia abdominal, indĂ­cios de choque hipovolĂȘmico e taquicardia, alguns exibiram quedas abruptas da pressĂŁo arterial, odinofagia, ĂȘmese, nĂĄuseas e estado ictĂ©rico. Os pacientes implicados, cronicamente, jĂĄ manifestaram ocorrĂȘncias prĂ©vias, devido ao carĂĄter recidivante torna-se essencial investigar a existĂȘncia de varizes, fĂ­stula aorto-entĂ©rica, angiodisplasia e doença ulcerosa. CONCLUSÃO: Elucida-se que a hemorragia digestiva alta representa a principal causa de sangramento do trato gastrointestinal, majoritamente manifesta-se como hematĂȘmese ou melena e cursam com o quadro sintomatolĂłgico que auxilia na avaliação da gravidade deste e o embasamento de potenciais focos de sangramento e que contribuam para disseminação de informaçÔes e intervençÔes futuras

    Particle-flow reconstruction and global event description with the CMS detector

    The CMS apparatus was identified, a few years before the start of the LHC operation at CERN, to feature properties well suited to particle-flow (PF) reconstruction: a highly-segmented tracker, a fine-grained electromagnetic calorimeter, a hermetic hadron calorimeter, a strong magnetic field, and an excellent muon spectrometer. A fully-fledged PF reconstruction algorithm tuned to the CMS detector was therefore developed and has been consistently used in physics analyses for the first time at a hadron collider. For each collision, the comprehensive list of final-state particles identified and reconstructed by the algorithm provides a global event description that leads to unprecedented CMS performance for jet and hadronic tau decay reconstruction, missing transverse momentum determination, and electron and muon identification. This approach also allows particles from pileup interactions to be identified and enables efficient pileup mitigation methods. The data collected by CMS at a centre-of-mass energy of 8 TeV show excellent agreement with the simulation and confirm the superior PF performance at least up to an average of 20 pileup interactions

    Identification of heavy-flavour jets with the CMS detector in pp collisions at 13 TeV

    Many measurements and searches for physics beyond the standard model at the LHC rely on the efficient identification of heavy-flavour jets, i.e. jets originating from bottom or charm quarks. In this paper, the discriminating variables and the algorithms used for heavy-flavour jet identification during the first years of operation of the CMS experiment in proton-proton collisions at a centre-of-mass energy of 13 TeV, are presented. Heavy-flavour jet identification algorithms have been improved compared to those used previously at centre-of-mass energies of 7 and 8 TeV. For jets with transverse momenta in the range expected in simulated tt‟\mathrm{t}\overline{\mathrm{t}} events, these new developments result in an efficiency of 68% for the correct identification of a b jet for a probability of 1% of misidentifying a light-flavour jet. The improvement in relative efficiency at this misidentification probability is about 15%, compared to previous CMS algorithms. In addition, for the first time algorithms have been developed to identify jets containing two b hadrons in Lorentz-boosted event topologies, as well as to tag c jets. The large data sample recorded in 2016 at a centre-of-mass energy of 13 TeV has also allowed the development of new methods to measure the efficiency and misidentification probability of heavy-flavour jet identification algorithms. The heavy-flavour jet identification efficiency is measured with a precision of a few per cent at moderate jet transverse momenta (between 30 and 300 GeV) and about 5% at the highest jet transverse momenta (between 500 and 1000 GeV)

    An embedding technique to determine ττ backgrounds in proton-proton collision data

    An embedding technique is presented to estimate standard model tau tau backgrounds from data with minimal simulation input. In the data, the muons are removed from reconstructed mu mu events and replaced with simulated tau leptons with the same kinematic properties. In this way, a set of hybrid events is obtained that does not rely on simulation except for the decay of the tau leptons. The challenges in describing the underlying event or the production of associated jets in the simulation are avoided. The technique described in this paper was developed for CMS. Its validation and the inherent uncertainties are also discussed. The demonstration of the performance of the technique is based on a sample of proton-proton collisions collected by CMS in 2017 at root s = 13 TeV corresponding to an integrated luminosity of 41.5 fb(-1).Peer reviewe
