Identification of Candida Species Associated with Blood Infection by Multiplex PCR and Phenotypic Characteristics
Background and Aim: The correct identification of Candida species is one of the most critical procedures in prognostic and therapeutic significance, allowing an early and appropriate antifungal therapy. This study aimed to evaluate multiplex PCR as a rapid diagnostic method and traditional phenotypic tests in identifying Candida species isolated from candidemia cases.
Methods: In this study, 38 Candida spp were isolated from culturing of human blood obtained from patients suspected to candidemia. The isolated species were evaluated by phenotypic and molecular methods including carbohydrate assimilation test, colony colour on CHROMagar Candida, chlamydoconidia production, germ tube formation and Multiplex PCR. Multiplex PCR was performed using specific primers of 4 common species. The results of multiplex PCR were compared with those obtained from phenotypic tests.
Results: According to multiplex PCR findings, the isolated Candida species were identified as C. albicans, C. parapsilosis, C. glabrata, and C. tropicalis. Phenotypic tests identified that 23 (60.52%), 8 (21.05%), 5 (13.15%), and 2 (5.26%) isolates belonged to C. albicans, C. parapsilosis, C. glabrata, and C. tropicalis, respectively, that were confirmed by multiplex PCR results. C. albicans and C. parapsilosis had the same carbohydrate assimilation pattern but were differentiated based on their colonies color on CHROMagar and the ability of C. albicans to produce chlamydoconidia and germ tube. C. glabrata (100%) and C. tropicalis (100%) assimilated trehalose and cellobiose, respectively.
Conclusion: Our study showed that the both phenotypic and molecular techniques provide appropriate information for identification of Candida spp from blood samples.

Effect of TiO2 nanoparticle on light fastness and degradation of dyedfabric with direct dye
The photocatalytic degradation of various dye solutions has been studied using nanoTiO2 (NTO) as a catalyst. The effect ofNTO on the light fastness of dyed cotton fabric has been studied using two different dyes, namely Direct Blue 71 and Direct Red 31with high and low light fastness respectively. After dyeing, the fabric is coated with NTO using a dip-pad–dry-cure process. Thecoated fabrics are then exposed to UV light for different periods of time. The results indicate that the coated cotton fabrics show asignificant photo-oxidative activity under UV light. The color change of cotton fabric dyed with Direct Blue 71 with high lightfastness is found less than that of the sample dyed with Direct red 31 with low light fastness
Developing a Psychological-Emotional Empowerment Training Package Based on the Lived Experiences of Single-Parent Male Adolescents and Its Effectiveness on Cognitive Fusion
Introduction: This study aimed to develop a psychological-emotional empowerment educational package based on the lived experiences of single-parent male adolescents and subsequently test its effectiveness on their cognitive fusion.Methods: The research method was qualitative, employing a phenomenological-descriptive approach, while the quantitative part was semi-experimental with a pre -test-post-test design, a control group, and a two-month follow-up period. The statistical population included male teenagers from single-parent families in the first period of secondary school in Abadeh city in 2022-2023. In the qualitative part, 15 single-parent male adolescents were purposefully selected and subjected to in-depth interviews. In the quantitative part, 35 adolescents were selected through purposive sampling and randomly assigned to experimental and control groups (18 in the experimental and 17 in the control group). The adolescents in the experimental group received the psychological-emotional empowerment training package over ten weeks in ten 90-minute sessions. A pre-test was performed before the intervention, a post-test after the intervention, and a follow-up phase two months later. The Cognitive Fusion Questionnaire (Gillanders et al., 2014) was used in this study. The data were analyzed using mixed ANOVA and a post hoc Bonferroni test via SPSS23 software.Results: According to experts, the results showed that the psychological-emotional empowerment training package has sufficient content validity. Additionally, this training package impacted the cognitive fusion (P<0.0001; Eta=0.66; F=63.41) of single-parent male adolescents.Conclusion: Based on the research findings, it can be concluded that a training package of psychological-emotional empowerment, by fostering emotional competencies, gaining competence, autonomy, effectiveness, communication with others, empathy, and kind behavior towards others, can be an effective method to decrease cognitive fusion in single-parent male adolescents
Assessment of Meteorological Drought Hazard Area using GIS in Ghareh Aghaj basin, Iran
In this paper area under hazard of Meteorological Drought was evaluated
using GIS technique. In Ghareh Aghaj watershed, meteorological drought
has the most profound effect on the way of living and regional economy.
Hence knowledge of the meteorological drought hazard area of their
occurrence and their course is an essential aspect for planning. The
purpose of this study was to make a model of the meteorological drought
hazard area using GIS. A set of meteorological drought indices were
studied and reviewed to define areas under hazard. Meteorological
drought indicators used in the present model include: Annual rainfall,
climate change, Coefficient variation (CV) of annual rainfall, climate,
ratio of the number of arid years with SPI <-1 to the number of
total years in each station, ratio of the number of vernal arid seasons
with SPI<-1 to the number of total years in each station, and the
number of sequential arid years using definition of WMO. The data
analyzed have been gathered from the records, reports and maps
published by the governmental offices of Iran. Most of these indicators
were performed using average annual rainfall data and average annual
temperature of minimum 16 years record of 20 stations. Each of the
hazard indicator maps and also final hazard map are classified into 4
hazard classes of drought: mild, moderate, severe and very severe. The
final hazard classes were defined on the basis of hazard scores arrived
at by assigning the appropriate attributes to the indicators and the
final hazard map was prepared by overlaying different hazard indicator
maps in the GIS, deploying the new model. The final Hazard Map shows
that moderate hazard areas (67% of the basin) are much widespread than
areas under severe hazard (37% of the basin) which are observed in the
Southeast of the region
Silymarin Administration Attenuates Cirrhotic-induced Cardiac Abnormality in the Rats: A Possible Role of β1-adrenergic Receptors and L-type Voltage-Dependent Calcium Channels
Background: Cirrhotic cardiomyopathy is a well-recognized cardiac dysfunction in cirrhotic patients. Studies have confirmed the protective effects of silymarin in different types of cardiac injury. This study aimed to examine the effectiveness and molecular mechanism of silymarin against myocardial dysfunction and hypertrophy in a rat model of cirrhosis.
Methods: The experiment was performed at Alborz University of Medical Sciences (Karaj, Iran) during 2020-2021. Thirty-two male Wistar rats were randomly divided into four groups of Sham-operated (control group for surgical procedures), Bile Duct Ligated (BDL), and two Silymarin extract (SE)-treated groups of 300 and 600 mg/Kg/day. After 28 days, serum levels of AST, ALT, GGT, and ALP, liver histopathological status, as well as cardiac mechanical function, were assessed. Cardiac β1-adrenergic receptors (β1-AR), L-type voltage-dependent calcium channels (L-VDCC), and GATA4 mRNA expression were also determined using real-time RT-PCR. Data analysis was performed using the one-way ANOVA followed by Duncan’s multiple range test. Histological data has been analyzed with Kruskal-Wallis nonparametric test. The analysis was performed at P≤0.05.
Results: BDL was associated with a significant elevation in serum AST, ALT, GGT, and ALP, development of necrosis and fibrosis of the liver texture, increased Heart Weight and Heart Weight to Body Weight ratio, enhanced cardiac mechanical function as well as a significant up-regulation of ventricular β1-AR and L-VDCC. Administration of SE600, but not SE300, significantly reduced the serum levels of the enzymes and alleviated signs of liver necrosis and fibrosis. Cirrhotic-induced cardiac dysfunction was also restored by SE600, but not by the lower dose. In addition, cardiac expression of the β1-AR and L-VDCC was down-regulated toward normal values by either higher or lower doses of the SE.
Conclusion: Silymarin treatment in higher dose attenuated cirrhosis-associated cardiac remodeling and reduced cardiac mechanical dysfunctions
Characterization of Phenotypic and Genotypic Diversity of Stenotrophomonas maltophilia Strains Isolated From Selected Hospitals in Iran
Stenotrophomonas maltophilia is an environmental Gram-negative bacterium that has rapidly emerged as an important nosocomial pathogen in hospitalized patients. Treatment of S. maltophilia infections is difficult due to increasing resistance to multiple antibacterial agents. The purpose of this study was to determine the phenotypic and genotypic characterization of S. maltophilia isolates recovered from patients referred to several hospitals. A total of 164 clinical isolates of S. maltophilia were collected from hospitals in various regions in Iran between 2016 and 2017. Antibiotic susceptibility testing was performed by disc diffusion method and E-test assay according to the Clinical and Laboratory Standards Institute (CLSI) guideline. The ability of biofilm formation was assessed with crystal violet staining and then, biofilm-associated genes were investigated by PCR-sequencing method. The presence of L1 (a metallo-β-lactamase), L2 (a clavulanic acid-sensitive cephalosporinase), sul1 and sul2 (resistance to Trimethoprim/Sulfamethoxazole), Smqnr (intrinsic resistance to quinolones), and dfrA genes (dihydrofolate reductase enzyme that contributes to trimethoprim resistance) was also examined by PCR-sequencing. Relative gene expression of smeDEF efflux pump was assessed by real-time PCR. Genotyping was performed using the multi-locus sequencing typing (MLST) and repetitive extragenic palindromic-PCR (Rep-PCR). Isolates were resistant to imipenem (100%), meropenem (96%), doripenem (96%), and ceftazidime (36.58%). Notably, 5 (3.04%) isolates showed resistant to trimethoprim-sulfamethoxazole (TMP-SMX), an alarming trend of decreased susceptibility to TMP-SMX in Iran. Minocycline and levofloxacin exhibited the highest susceptibility of 91.46 and 99.39%, respectively. Using the crystal violet staining, 157 (95.73%) isolates had biofilm phenotype: 49 (29.87%), 63 (38.41%), and 45 (27.43%) isolates were categorized as strong-, moderate- and weak-biofilm producer while 7 isolates (4.26%) were identified a non-biofilm producer. Biofilm genes had an overall prevalence of 145 (88.41%), 137 (83.53%), and 164 (100%) of rmlA, rpfF, and spgM, respectively. L1, L2, Smqnr, sul1, and sul2 resistance genes were detected in 145 (88.41%), 156 (96.12%), 103 (62.80%), 89 (54.26%), and 92 (56.09%) isolates, respectively. None of the S. maltophilia isolates were positive for dfrA12, dfrA17, and dfrA27 genes. Gene expression analysis showed that smeD efflux system was overexpressed in two out of the five clinical isolates (40%) that showed resistance to TMP-SMX. Most of the isolates were genetically unrelated. Two new sequence types (ST139 and ST259) were determined. Our results showed that TMP-SMX was still an effective antibiotic against S. maltophilia. The findings of the current study revealed an increasing prevalence of antibiotic resistance and biofilm genes in clinical S. maltophilia isolates in Iran
Global, regional, and national incidence, prevalence, and years lived with disability for 354 diseases and injuries for 195 countries and territories, 1990-2017: a systematic analysis for the Global Burden of Disease Study 2017.
The Global Burden of Diseases, Injuries and Risk Factors 2017 includes a comprehensive assessment of incidence, prevalence, and years lived with disability (YLDs) for 354 causes in 195 countries and territories from 1990 to 2017. Previous GBD studies have shown how the decline of mortality rates from 1990 to 2016 has led to an increase in life expectancy, an ageing global population, and an expansion of the non-fatal burden of disease and injury. These studies have also shown how a substantial portion of the world's population experiences non-fatal health loss with considerable heterogeneity among different causes, locations, ages, and sexes. Ongoing objectives of the GBD study include increasing the level of estimation detail, improving analytical strategies, and increasing the amount of high-quality data. METHODS: We estimated incidence and prevalence for 354 diseases and injuries and 3484 sequelae. We used an updated and extensive body of literature studies, survey data, surveillance data, inpatient admission records, outpatient visit records, and health insurance claims, and additionally used results from cause of death models to inform estimates using a total of 68 781 data sources. Newly available clinical data from India, Iran, Japan, Jordan, Nepal, China, Brazil, Norway, and Italy were incorporated, as well as updated claims data from the USA and new claims data from Taiwan (province of China) and Singapore. We used DisMod-MR 2.1, a Bayesian meta-regression tool, as the main method of estimation, ensuring consistency between rates of incidence, prevalence, remission, and cause of death for each condition. YLDs were estimated as the product of a prevalence estimate and a disability weight for health states of each mutually exclusive sequela, adjusted for comorbidity. We updated the Socio-demographic Index (SDI), a summary development indicator of income per capita, years of schooling, and total fertility rate. Additionally, we calculated differences between male and female YLDs to identify divergent trends across sexes. GBD 2017 complies with the Guidelines for Accurate and Transparent Health Estimates Reporting

Globally, for females, the causes with the greatest age-standardised prevalence were oral disorders, headache disorders, and haemoglobinopathies and haemolytic anaemias in both 1990 and 2017. For males, the causes with the greatest age-standardised prevalence were oral disorders, headache disorders, and tuberculosis including latent tuberculosis infection in both 1990 and 2017. In terms of YLDs, low back pain, headache disorders, and dietary iron deficiency were the leading Level 3 causes of YLD counts in 1990, whereas low back pain, headache disorders, and depressive disorders were the leading causes in 2017 for both sexes combined. All-cause age-standardised YLD rates decreased by 3·9% (95% uncertainty interval [UI] 3·1-4·6) from 1990 to 2017; however, the all-age YLD rate increased by 7·2% (6·0-8·4) while the total sum of global YLDs increased from 562 million (421-723) to 853 million (642-1100). The increases for males and females were similar, with increases in all-age YLD rates of 7·9% (6·6-9·2) for males and 6·5% (5·4-7·7) for females. We found significant differences between males and females in terms of age-standardised prevalence estimates for multiple causes. The causes with the greatest relative differences between sexes in 2017 included substance use disorders (3018 cases [95% UI 2782-3252] per 100 000 in males vs s1400 [1279-1524] per 100 000 in females), transport injuries (3322 [3082-3583] vs 2336 [2154-2535]), and self-harm and interpersonal violence (3265 [2943-3630] vs 5643 [5057-6302]). Global all-cause age-standardised YLD rates have improved only slightly over a period spanning nearly three decades. However, the magnitude of the non-fatal disease burden has expanded globally, with increasing numbers of people who have a wide spectrum of conditions. A subset of conditions has remained globally pervasive since 1990, whereas other conditions have displayed more dynamic trends, with different ages, sexes, and geographies across the globe experiencing varying burdens and trends of health loss. This study emphasises how global improvements in premature mortality for select conditions have led to older populations with complex and potentially expensive diseases, yet also highlights global achievements in certain domains of disease and injury
