    Emergency medical dispatch recognition, clinical intervention and outcome of patients in traumatic cardiac arrest from major trauma : an observational study

Published by BMJ.OBJECTIVES: The aim of this study is to describe the demographics of reported traumatic cardiac arrest (TCA) victims, prehospital resuscitation and survival to hospital rate. SETTING: Helicopter Emergency Medical Service (HEMS) in south-east England, covering a resident population of 4.5 million and a transient population of up to 8 million people. PARTICIPANTS: Patients reported on the initial 999 call to be in suspected traumatic cardiac arrest between 1 July 2016 and 31 December 2016 within the trust's geographical region were identified. The inclusion criteria were all cases of reported TCA on receipt of the initial emergency call. Patients were subsequently excluded if a medical cause of cardiac arrest was suspected. OUTCOME MEASURES: Patient records were analysed for actual presence of cardiac arrest, prehospital resuscitation procedures undertaken and for survival to hospital rates. RESULTS: 112 patients were reported to be in TCA on receipt of the 999/112 call. 51 (46%) were found not to be in TCA on arrival of emergency medical services. Of the 'not in TCA cohort', 34 (67%) received at least one advanced prehospital medical intervention (defined as emergency anaesthesia, thoracostomy, blood product transfusion or resuscitative thoracotomy). Of the 61 patients in actual TCA, 10 (16%) achieved return-of-spontaneous circulation. In 45 (88%) patients, the HEMS team escorted the patient to hospital. CONCLUSION: A significant proportion of patients reported to be in TCA on receipt of the emergency call are not in actual cardiac arrest but are critically unwell requiring advanced prehospital medical intervention. Early activation of an enhanced care team to a reported TCA call allows appropriate advanced resuscitation. Further research is warranted to determine which interventions contribute to improved TCA survival.Peer reviewedFinal Published versio

In this paper, we address the resource’s relevance and applicability to (second) language pedagogy and the development of sample exercises and other support material made available through the SweCcn website. We also discuss the conditions for integrating SweCcn with other linguistic resources, in interlingual as well as Swedish contexts

    Productive collocation knowledge and advanced CEFR-levels in Swedish as a second language: A conceptual replication of Forsberg Lundell, Lindqvist & Edmonds (2018)

    This study constitutes a conceptual replication of Forsberg Lundell et al. (2018), who investigated whether productive collocation knowledge – a linguistic feature known to be indicative of high-level L2 proficiency – developed between the B2 and C1 levels of the Common European Framework of Reference for Languages scale in second-language (L2) French. The results showed significant development. The present study set out to replicate these findings in L2 Swedish, in order to investigate whether the reported development would stand cross-linguistic validation. To this end, a test of productive collocation knowledge in L2 Swedish was developed based on 22 separate computerized newspaper corpora of Swedish, searchable via the corpus tool 'Korp' at SpråkbankenText (Swedish Language Bank). The method of the item selection was identical to that of the Forsberg Lundell et al., but the replication could only be conceptual since the reference corpora are different, from different languages. The test was conducted comparing participants from B2 and C1 levels in Swedish ('N' = 60). The results replicated the original study, confirming a significant difference in productive collocation knowledge between the B2 and C1 levels. Furthermore, in addition to the replication, the study explored frequency and Mutual Information score as potential factors for collocation item difficulty. The study found no significant effects for frequency or for Mutual Information score. Finally, the impact of cross-linguistic similarity was also investigated, grouping the results for participants with Germanic and non-Germanic first languages. This analysis did not point to any noteworthy effects

    Differing marine animal biomass shifts under 21st century climate change between Canada's three ocean

Outcomes are challenging to project at national scales, despite their importance for management decisions. Using an ensemble of six global marine ecosystem models we analyzed marine ecosystem responses to climate change from 1971 to 2099 in Canada's Exclusive Economic Zone (EEZ) under four standardized emissions scenarios. By 2099, under business-as-usual emissions (RCP8.5) projected marine animal biomass declined by an average of −7.7% (±29.5%) within the Canadian EEZ, dominated by declines in the Pacific (−24% ± 24.5%) and Atlantic (−25.5% ± 9.5%) areas; these were partially compensated by increases in the Canadian Arctic (+26.2% ± 38.4%). Lower emissions scenarios projected successively smaller biomass changes, highlighting the benefits of stronger mitigation targets. Individual model projections were most consistent in the Atlantic and Pacific, but highly variable in the Arctic due to model uncertainties in polar regions. Different trajectories of future marine biomass changes will require regional-specific responses in conservation and management strategies, such as adaptive planning of marine protected areas and species-specific management plans, to enhance resilience and rebuilding of Canada's marine ecosystems and commercial fish stocks

    Imagined Bodies: architects and their constructions of later life

    This article comprises a sociological analysis of how architects imagine the ageing body when designing residential care homes for later life and the extent to which they engage empathetically with users. Drawing on interviews with architectural professionals based in the United Kingdom, we offer insight into the ways in which architects envisage the bodies of those who they anticipate will populate their buildings. Deploying the notions of ‘body work’ and ‘the body multiple’, our analysis reveals how architects imagined a variety of bodies in nuanced ways. These imagined bodies emerge as they talked through the practicalities of the design process. Moreover, their conceptions of bodies were also permeated by prevailing ideologies of caring: although we found that they sought to resist dominant discourses of ageing, they nevertheless reproduced these discourses. Architects’ constructions of bodies are complicated by the collaborative nature of the design process, where we find an incessant juggling between the competing demands of multiple stakeholders, each of whom anticipate other imagined bodies and seek to shape the design of buildings to meet their requirements. Our findings extend a nascent sociological literature on architecture and social care by revealing how architects participate in the shaping of care for later life as ‘body workers’, but also how their empathic aspirations can be muted by other imperatives driving the marketisation of care

    A global database for metacommunity ecology, integrating species, traits, environment and space

    The use of functional information in the form of species traits plays an important role in explaining biodiversity patterns and responses to environmental changes. Although relationships between species composition, their traits, and the environment have been extensively studied on a case-by-case basis, results are variable, and it remains unclear how generalizable these relationships are across ecosystems, taxa and spatial scales. To address this gap, we collated 80 datasets from trait-based studies into a global database for metaCommunity Ecology: Species, Traits, Environment and Space; “CESTES”. Each dataset includes four matrices: species community abundances or presences/absences across multiple sites, species trait information, environmental variables and spatial coordinates of the sampling sites. The CESTES database is a live database: it will be maintained and expanded in the future as new datasets become available. By its harmonized structure, and the diversity of ecosystem types, taxonomic groups, and spatial scales it covers, the CESTES database provides an important opportunity for synthetic trait-based research in community ecology

    Parent-of-origin-specific allelic associations among 106 genomic loci for age at menarche.

    Age at menarche is a marker of timing of puberty in females. It varies widely between individuals, is a heritable trait and is associated with risks for obesity, type 2 diabetes, cardiovascular disease, breast cancer and all-cause mortality. Studies of rare human disorders of puberty and animal models point to a complex hypothalamic-pituitary-hormonal regulation, but the mechanisms that determine pubertal timing and underlie its links to disease risk remain unclear. Here, using genome-wide and custom-genotyping arrays in up to 182,416 women of European descent from 57 studies, we found robust evidence (P < 5 × 10(-8)) for 123 signals at 106 genomic loci associated with age at menarche. Many loci were associated with other pubertal traits in both sexes, and there was substantial overlap with genes implicated in body mass index and various diseases, including rare disorders of puberty. Menarche signals were enriched in imprinted regions, with three loci (DLK1-WDR25, MKRN3-MAGEL2 and KCNK9) demonstrating parent-of-origin-specific associations concordant with known parental expression patterns. Pathway analyses implicated nuclear hormone receptors, particularly retinoic acid and γ-aminobutyric acid-B2 receptor signalling, among novel mechanisms that regulate pubertal timing in humans. Our findings suggest a genetic architecture involving at least hundreds of common variants in the coordinated timing of the pubertal transition
