Evaluación de la calidad de servicio en percepción del enoturista mediante la Lógica Borrosa y las Soluciones Ideales

Este artículo resuelve un problema multidimensional para evaluar la satisfacción de los enoturistas que visitaron una bodega situada en el sur de Brasil utilizando un método basado en la lógica borrosa. De esta forma, se calcula un índice sintético de satisfacción para un grupo de segmentos en los que estamos interesados, a saber, género e imagen previa a la visita. El problema de la ambigüedad lingüística se aborda desde la teoría de los conjuntos borrosos aplicando números borrosos triangulares (NBT). El indicador sintético de la satisfacción se basa en el concepto de grado de optimalidad mediante la técnica de la semejanza a las soluciones ideales. Los resultados obtenidos muestran que los segmentos analizados experimentan una distinta satisfacción. Nuestros resultados pueden usarse por los directivos de la bodega, así como por los planificadores y administradores de la región, para mejorar la competitividad de este nicho de mercado.This article solves a multidimensional problem in order to evaluate the satisfaction of the wine tourist that visited a winery located in the South of Brazil, employing a method based on fuzzy logic. Thus, a synthetic index of satisfaction is calculated for a group of segments in which we are interested, namely, gender and image prior to the visit. The problem of linguistic ambiguity is addressed with the fuzzy sets theory, applying triangular fuzzy numbers (TFN). The synthetic index of satisfaction is based on the concept of the degree of optimality by the technique of similarity of ideal solutions. The results obtained show that the analyzed segments experienced different satisfaction. Our results can be used by the winery directors as well as by the regional planners and managers, in order to improve the competitiveness of this market niche

Tourism and heritage. The case of Valparaiso (Chile) and the profile of the cultural tourist

Comentamos inicialmente las categorías patrimoniales de la UNESCO analizando seguidamente diversos aspectos sobre turismo y patrimonio, así como emblemáticos casos internacionales publicados en revistas indexadas en SCOPUS. Posteriormente reseñamos la situación de Valparaíso, ciudad chilena inmersa en un considerable impulso de desarrollo turístico: su entorno de riqueza paisajística (puerto, bahía, cerros, costa, playas) y su riqueza patrimonial (arquitectura, gastronomía, historia) la han hecho merecedora de ser nombrada WHS1 por la UNESCO. La encuesta realizada a los visitantes nos permite detallar su perfil sociodemográfico, incidiendo en las principales valoraciones como destino patrimonial. La mayoría son nacionales de Chile, ya conocían la ciudad, se encuentran muy satisfechos con la visita y con la relación calidad ‑ precio de los servicios. Nuevos retos y desafíos relacionados con la acertada gestión en sus políticas turísticas y urbanísticas se presentan como aspectos de relevante consideración.We comment initially the heritage categories of UNESCO, analyzing then various aspects on tourism and heritage, as well as emblematic international cases published in journals indexed in SCOPUS. Subsequently we reviewed the situation of Valparaiso, Chilean city immersed in a considerable impulse of tourist development: its landscape richness environment (port, bay, hills, coast, beaches) and its patrimonial richness (architecture, gastronomy, history) they have made it to be named WHS by UNESCO. The survey of visitors allows us to detail their sociodemographic profile, and to highlight the main opinions as hereditary destination. Most of them are nationals of Chile, they already knew the city, they are very satisfied with the visit and with the quality‑price relation of the services. New challenges related to the successful management in its tourism and urban development policies are presented as aspects of relevant consideration

Clinical infections by herpesviruses in patients treated with valproic acid: A nested case-control study in the Spanish Primary Care Database, BIFAP

The objective of this study is to evaluate the risk of clinical infections by herpesviruses in patients exposed to valproic acid (VPA).We performed a case-control study nested in a primary cohort selected from the Spanish primary care population-based research database BIFAP (Base de datos para la Investigación Farmacoepidemiológica en Atención Primaria) over the period 2001–2015. The events of interest were those diseases caused by any herpesviruses known to infect humans. For each case, up to 10 controls per case matched by age, gender, and calendar date were randomly selected. A conditional logistic regression was used to compute adjusted odds ratios (OR) and their 95% confidence intervals (95% CI). Current use of VPA was associated with a trend towards a reduced risk of clinical infections by herpesviruses as compared with non-users (OR 0.84; CI 95% 0.7–1.0; p = 0.057). Among current users, a trend to a decreased risk with treatment durations longer than 90 days was also observed. The results show a trend to a reduced risk of clinical infection by herpesviruses in patients exposed to VPA. These results are consistent with those in vitro studies showing that, in cultured cells, VPA can inhibit the production of the infectious progeny of herpesviruses. Improvement of the teaching and learning process in the department of business administration and economic history

El Espacio Europeo de Educación Superior (EEES) se centra en el proceso de enseñanza/aprendizaje de competencias. Para que dicho proceso pueda llevarse a cabo adecuadamente es necesario que los estudiantes desarrollen estrategias de estudio y aprendizaje y los profesores nuevos métodos de enseñanza. Precisamente la innovación educativa consiste en llevar a cabo cambios que mejoren dicho proceso de enseñanza y aprendizaje. El objetivo de este trabajo es descubrir las percepciones que los estudiantes tienen acerca de dichas estrategias y métodos, para obtener información a partir de la cual llevar a cabo mejoras e innovaciones en el proceso educativo. Se trata de estudiantes de grados a los que el Departamento de dirección de empresas e historia económica imparte docencia. Los resultados permiten concluir que los estudiantes tienen percepciones claras acerca de las estrategias de estudio/aprendizaje y de los métodos de enseñanza que podrían utilizar los profesores para mejorar el logro académico. También se concluye que existen diferencias por género y ciclo académico, y que es posible generar dos modelos causales explicativos de la percepción de logro del estudiante, uno relativo a las estrategias de estudio y aprendizaje, otro respecto a los métodos de enseñanza del profesoradoThe European Higher Education Space (EHES) focuses on the teaching/learning competencies process. So that this process can be carried out properly it is necessary for students to develop study and learning strategies and new teachers teaching methods. Precisely educational innovation is to carry out changes that improve the teaching and learning process. The aim of this work is to discover student´s perceptions about these strategies and methods, to obtain information from which to carry out improvements and innovations in the educational process. They are students from grades in which teachers of the Department of Business Administration and Economic History work. The results allow show that students have clear perceptions of study/learning and strategies of teaching methods that the teachers could use to improve academic achievement. Also it is concluded that differences exist by gender and academic cycle, and that it is possible to generate two explanatory causal models of perception on achievement of student, one relating to learning and study strategies, another on faculty teaching methods

Bronchoscopist's perception of the quality of the single-use bronchoscope (Ambu aScope4™) in selected bronchoscopies: a multicenter study in 21 Spanish pulmonology services

Background: The disposable bronchoscope is an excellent alternative to face the problem of SARS-CoV-2 and other cross infections, but the bronchoscopist’s perception of its quality has not been evaluated. Methods: To evaluate the quality of the Ambu-aScope4 disposable bronchoscope, we carried out a cross-sectional study in 21 Spanish pulmonology services. We use a standardized questionnaire completed by the bronchoscopists at the end of each bronchoscopy. The variables were described with absolute and relative frequencies, measures of cen‑ tral tendency and dispersion depending on their nature. The existence of learning curves was evaluated by CUSUM analysis. Results: The most frequent indications in 300 included bronchoscopies was bronchial aspiration in 69.3% and the median duration of these was 9.1 min. The route of entry was nasal in 47.2% and oral in 34.1%. The average score for ease of use, image, and aspiration quality was 80/100. All the planned techniques were performed in 94.9% and the bronchoscopist was satisfed in 96.6% of the bronchoscopies. They highlighted the portability and immediacy of the aScope4TM to start the procedure in 99.3%, the possibility of taking and storing images in 99.3%. The CUSUM analysis showed average scores>70/100 from the frst procedure and from the 9th procedure more than 80% of the scores exceeded the 80/100 score

Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, individuals with clonal mosaic events (clonal mosaicism for chromosome alterations and/or loss of chromosome Y) showed an increased risk of COVID-19 lethality

First Latin American clinical practice guidelines for the treatment of systemic lupus erythematosus: Latin American Group for the Study of Lupus (GLADEL, Grupo Latino Americano de Estudio del Lupus)-Pan-American League of Associations of Rheumatology (PANLAR)

Systemic lupus erythematosus (SLE), a complex and heterogeneous autoimmune disease, represents a significant challenge for both diagnosis and treatment. Patients with SLE in Latin America face special problems that should be considered when therapeutic guidelines are developed. The objective of the study is to develop clinical practice guidelines for Latin American patients with lupus. Two independent teams (rheumatologists with experience in lupus management and methodologists) had an initial meeting in Panama City, Panama, in April 2016. They selected a list of questions for the clinical problems most commonly seen in Latin American patients with SLE. These were addressed with the best available evidence and summarised in a standardised format following the Grading of Recommendations Assessment, Development and Evaluation approach. All preliminary findings were discussed in a second face-to-face meeting in Washington, DC, in November 2016. As a result, nine organ/system sections are presented with the main findings; an 'overarching' treatment approach was added. Special emphasis was made on regional implementation issues. Best pharmacologic options were examined for musculoskeletal, mucocutaneous, kidney, cardiac, pulmonary, neuropsychiatric, haematological manifestations and the antiphospholipid syndrome. The roles of main therapeutic options (ie, glucocorticoids, antimalarials, immunosuppressant agents, therapeutic plasma exchange, belimumab, rituximab, abatacept, low-dose aspirin and anticoagulants) were summarised in each section. In all cases, benefits and harms, certainty of the evidence, values and preferences, feasibility, acceptability and equity issues were considered to produce a recommendation with special focus on ethnic and socioeconomic aspects.
