    Avaliação da adesão terapêutica anti-hipertensiva em idosos

    Introduction: in the year 2006 there were around 690 million people over 60 years old in the world. In developed countries, more than 20% of the population was elderly, as a result of an increase in life expectancy and declining birth rates. Objective: to characterize the therapeutic antihypertensive adherence in patients older than 60 years. Method: a cross-sectional descriptive study on antihypertensive therapeutic adherence in the elderly was carried out in the "Omar Ranedo Pubillones" polyclinic during 2007. Forty-three patients were chosen from a sample of 344. Results: the prescription of hypotensive drugs was adequate in 86.6% of the patients studied. The main prescription errors are due to lower therapeutic intervals, as well as higher than recommended doses. One third of patients had deficient knowledge about hypertension and 48.3% did not comply with the treatment indicated by the physician. Conclusions: in most of the patients included in the research there is an inadequate adherence to antihypertensive treatment, which will determine in the short, medium and long term an increase in morbidity and mortality due to cardiovascular and cerebrovascular diseases, and its consequent social impact.Introducción: en el año 2006 existían en el mundo alrededor de 690 millones de personas mayores de 60 años. Método: se realizó un estudio descriptivo transversal sobre adherencia terapéutica antihipertensiva en el adulto mayor, en el policlínico “Omar Ranedo Pubillones” durante el año 2007. Objetivo: caracterizar a adesão terapêutica anti-hipertensiva em pacientes com mais de 60 anos. Método: Um estudo transversal foi realizada aderência anti-hipertensivo em idosos, na clínica "Omar Ranedo Pubillones" durante 2007. 43 pacientes de amostra 344. Resultados: a prescrição de medicamentos anti-hipertensivos adequados em 86,6% dos pacientes estudados. Os principais erros de prescrição são decorrentes de menores intervalos terapêuticos, além de maiores que as doses recomendadas. Um terço dos pacientes tinha conhecimento deficiente sobre hipertensão e 48,3% não cumpriam o tratamento indicado pelo médico. Conclusões: A maioria dos pacientes incluídos na investigação não é a adesão inadequada ao tratamento anti-hipertensivo, o que vai determinar a curto, médio e longo prazo o aumento da morbidade e mortalidade por doenças cardiovasculares, cerebrovasculares e consequente impacto social

    Microbiological diagnosis of neonatal sepsis in Guantanamo’s General Teaching Hospital "Agostinho Neto"

    Se realiza una investigación para establecer el diagnóstico microbiológico de sepsis neonatal en el Hospital General Docente “Agostinho Neto” de enero 2008 – marzo 2010. El universo de estudio estuvo constituido por 111 neonatos. Se aplicaron técnicas estadísticas de resumen de datos del nivel descriptivo de investigación. La sepsis neonatal en el periodo estudiado tuvo un comportamiento decreciente. Los principales factores de riesgo dependientes de la madre fueron: parto por cesárea, parto pretérmino, sepsis vaginal, los relacionados con el neonato resultaron el bajo peso al nacer, la utilización de catéteres, tubos endotraqueales, el uso previo de antimicrobianos y la ventilación asistida. Los gérmenes predominantes fueron los Staphylococcus coagulasa negativo, Staphylococcus aureus y Klebsiella spp.An investigation was carried out to establish the microbiological diagnosis of neonatal sepsis in the General Teaching Hospital "Agostinho Neto" from January 2008 to March 2010. The universe of study consisted of 111 neonates. Statistical summary techniques were applied at the descriptive level of research. Neonatal sepsis in the studied period had a decreasing behavior. The main risk factors of the mother were: cesarean delivery, preterm delivery, vaginal sepsis, neonatal-related outcomes were low birth weight, use of catheters, endotracheal tubes, prior antimicrobial use and assisted ventilation. The predominant germs were coagulase negative Staphylococcus, Staphylococcus aureus and Klebsiella spp

    Antimicrobial resistance in sputum of patients with lower respiratory infections

    Se realiza un estudio para identificar los gérmenes presentes en las muestras de esputo y determinar la resistencia antimicrobiana en pacientes con síntomas y signos de infecciones respiratorias bajas, atendidos en el Hospital General Docente “Dr. Agostinho Neto” de Guantánamo en el 2013. El universo de estudio lo constituyen las 949 muestras útiles de esputo para la realización de cultivos. La resistencia antimicrobiana se determina por los métodos estándares del Clínical Laboratory Standard Institute (CLSI). Se encontró predominio en esputos procedentes de salas de ingreso. El 29.2 % de los esputos resultó positivo al cultivo bacteriológico, el 4.2 % en los esputos BAAR y el 55.6 % en el cultivo micológico. Las bacterias gram negativas constituyeron el mayor número de aislamientos en los enfermos con infecciones del tracto respiratorio inferior. El Streptococcus beta- hemolítico fue el germen gram positivo más identificado. Se obtuvo un alto índice de resistencia al ampicillin, cefepime y ceftriaxona en la mayoría de las bacterias aisladas.  A study was performed to identify the germs present in the sputum samples and determine antimicrobial resistance in patients with symptoms and signs of lower respiratory infections, treated at the General Teaching Hospital “Dr. Agostinho Neto" of Guantánamo in 2013. The universe of study was 949 sputum samples for performing culture. Antimicrobial resistance was determined by standard clinical methods at Laboratory Standard Institute (CLSI). It was found predominance in sputum from admission rooms. The 29.2 % of    sputum tested positive at bacteriological culture, 4.2% in AFB or BAR sputum and 55.6 % in mycological culture. Gram-negative bacteria constituted the majority of isolates in patients with lower respiratory tract infections. The hemolytic Streptococcus was the gram-positive germ more identified. A high rate of resistance was obtained to ampicillin, ceftriaxone, cefepime, in most                 isolated bacteria

    Experiência da Brigada Médica Cubana "Henry Reeve" no confronto com COVID-19 na África do Sul

    Introduction: South Africa, the nation most affected by SARS-CoV-2 in Africa, requested medical assistance from Cuba. The Henry Reeve International Contingent of Specialized Doctors in Disaster Situations and Serious Epidemics sent a brigade to strengthen medical collaboration. Objective: review the composition of the group, the assistance activities carried out, experience and impact of the work supported by the "Henry Reeve" Cuban Medical Brigade in South Africa to confront COVID-19 and other health problems. Method: a descriptive cross-sectional study was carried out between April 2020 and May 2021. Some instruments were built to collect daily and weekly information, as well as the Minsap model 241-492 with monthly periodicity.Results: the brigade was made up of 184 collaborators: 114 physicians (62.0%) and 70 health technicians and graduates (38.0%). Sex male was predominant (61.4%) and the average age was 43 years. More than 255,940 patients were attended at the field hospital, of which 116,234 were COVID-19 confirmed patients. A total of 1,294 surgical interventions, 320 deliveries and 44,692 nursing procedures were performed, which contributed to saving the lives of 1,519 patients, 66% of whom were COVID-19 positive. The death of 643 patients was regretted, 97% of them due to disease complications. The work carried out by epidemiologists, medical equipment maintenance specialists, biostatisticians and pharmacists was highlighted. Conclusions: the meritorious work carried out by the "Henry Reeve" Cuban Medical Brigade in the fight against the COVID-19 pandemic and its support to health institutions of the South African National Health System is acknowledged.Introducción: Sudáfrica, nación más afectada por el SARS-CoV-2 en África, solicitó ayuda médica a Cuba. El Contingente Internacional de Médicos Especializados en Situaciones de Desastres y Graves Epidemias “Henry Reeve” envió una brigada para reforzar la colaboración médica. Objetivo: describir la composición del grupo, actividades asistenciales realizadas, experiencia e impacto del trabajo de la Brigada Médica Cubana “Henry Reeve” en Sudáfrica para el enfrentamiento a la COVID-19 y otros problemas de salud. Método: se realizó una investigación descriptiva transversal, entre abril 2020 y mayo 2021. Se confeccionaron instrumentos para la recolección de la información diaria y semanal, así como el modelo 241-492 del Minsap con periodicidad mensual. Resultados: la brigada estuvo conformada por 184 colaboradores: 114 médicos (62,0 %) y 70 licenciados y técnicos de salud (38,0 %). Predominó el sexo masculino (61,4 %) y la edad promedio fue de 43 años. Fueron atendidos más de 255 940 pacientes, de ellos, 116 234 relacionados con la COVID-19. Se realizaron 1 294 intervenciones quirúrgicas, 320 partos y 44 692 procederes de enfermería; lo que contribuyó a salvarle la vida a 1 519 pacientes, entre ellos el 66 % fue positivo a COVID-19. Se lamentó el fallecimiento de 643 pacientes, el 97 % por complicaciones de dicha enfermedad. Se destacó el trabajo realizado por epidemiólogos, electromédicos, bioestadísticos y farmacéuticos.Conclusiones: se reconoce la meritoria labor desempeñada por la Brigada Médica Cubana “Henry Reeve” en el enfrentamiento a la COVID-19 y su contribución en instituciones del Sistema Nacional de Salud de Sudáfrica.Introdução: A África do Sul, a nação mais afetada pelo SARS-CoV-2 na África, solicitou ajuda médica de Cuba. O Contingente Internacional de Médicos Especializados em Situações de Desastre e Epidemias Graves “Henry Reeve” enviou uma brigada para reforçar a colaboração médica. Objetivo: descrever a composição do grupo, as atividades assistenciais realizadas, a experiência e o impacto do trabalho da Brigada Médica Cubana "Henry Reeve" na África do Sul para enfrentar o COVID-19 e outros agravos à saúde.Método: pesquisa descritiva transversal realizada entre abril de 2020 e maio de 2021. Foram confeccionados instrumentos de coleta de informações diárias e semanais, bem como o modelo Minsap 241-492 com periodicidade mensal. Resultados: a brigada era composta por 184 colaboradores: 114 médicos (62,0%) e 70 graduados e técnicos em saúde (38,0%). Predominou o sexo masculino (61,4%) e a média de idade foi de 43 anos. Mais de 255.940 pacientes foram tratados, deles 116.234 relacionados ao COVID-19. Foram realizadas 1.294 intervenções cirúrgicas, 320 partos e 44.692 procedimentos de enfermagem; o que contribuiu para salvar a vida de 1.519 pacientes, entre eles 66% eram positivos para COVID-19. A morte de 643 pacientes foi lamentada, 97% devido a complicações desta doença. O trabalho realizado por epidemiologistas, eletromédicos, bioestatísticos e farmacêuticos foi destaque.     Experience in the implementation of multi-center clinical test in the province of Guantánamo

    El Centro Nacional Coordinador de Ensayos Clínicos y su Red Nacional de Coordinación de Ensayos Clínicos son los responsables de garantizar el cumplimiento de los estándares internacionales establecidos y asegurar la protección de los sujetos en estudio, el rigor científico, ético y la confiabilidad en los resultados de los servicios en toda Cuba. En este trabajo se evaluó la experiencia provincial en la ejecución de más de 19 protocolos de ensayos clínicos multicéntricos para validar 12 productos farmacéuticos biotecnológicos, en más de 17 indicaciones médicas, para lo cual se realizó un estudio descriptivo transversal donde se evaluaron todos los protocolos de investigación que fueron conducidos por el Grupo de Coordinación de Ensayos Clínicos de Guantánamo desde 2003 hasta 2011. Se tienen en cuenta las actividades capacitantes realizadas por este, dirigidas a elevar la calidad de los estudios clínicos ejecutados desde la formación de los investigadores en el tema de Ensayos Clínicos.The National and coordinator of Clinical tests and its National Network and Coordination of clinical  Test  are responsible for ensuring compliance with established international standards and ensure the protection of person in study-object, scientific rigor , ethical rigor and reliability results of the services throughout Cuba. In this study was evaluated the provincial experience in the execution of over 19 protocols for multicenter clinical tests to validate 12 biotechnical and pharmaceuticals products , over 17 medical indications, for which a transversal descriptive  study was done where were  evaluated  all protocols that were conducted by the Coordination Group for Clinical test ,Guantanamo from 2003 to 2011.     Global transpiration data from sap flow measurements : the SAPFLUXNET database

    Plant transpiration links physiological responses of vegetation to water supply and demand with hydrological, energy, and carbon budgets at the land-atmosphere interface. However, despite being the main land evaporative flux at the global scale, transpiration and its response to environmental drivers are currently not well constrained by observations. Here we introduce the first global compilation of whole-plant transpiration data from sap flow measurements (SAPFLUXNET, https://sapfluxnet.creaf.cat/, last access: 8 June 2021). We harmonized and quality-controlled individual datasets supplied by contributors worldwide in a semi-automatic data workflow implemented in the R programming language. Datasets include sub-daily time series of sap flow and hydrometeorological drivers for one or more growing seasons, as well as metadata on the stand characteristics, plant attributes, and technical details of the measurements. SAPFLUXNET contains 202 globally distributed datasets with sap flow time series for 2714 plants, mostly trees, of 174 species. SAPFLUXNET has a broad bioclimatic coverage, with woodland/shrubland and temperate forest biomes especially well represented (80 % of the datasets). The measurements cover a wide variety of stand structural characteristics and plant sizes. The datasets encompass the period between 1995 and 2018, with 50 % of the datasets being at least 3 years long. Accompanying radiation and vapour pressure deficit data are available for most of the datasets, while on-site soil water content is available for 56 % of the datasets. Many datasets contain data for species that make up 90 % or more of the total stand basal area, allowing the estimation of stand transpiration in diverse ecological settings. SAPFLUXNET adds to existing plant trait datasets, ecosystem flux networks, and remote sensing products to help increase our understanding of plant water use, plant responses to drought, and ecohydrological processes.     Taking the pulse of Earth's tropical forests using networks of highly distributed plots

    Tropical forests are the most diverse and productive ecosystems on Earth. While better understanding of these forests is critical for our collective future, until quite recently efforts to measure and monitor them have been largely disconnected. Networking is essential to discover the answers to questions that transcend borders and the horizons of funding agencies. Here we show how a global community is responding to the challenges of tropical ecosystem research with diverse teams measuring forests tree-by-tree in thousands of long-term plots. We review the major scientific discoveries of this work and show how this process is changing tropical forest science. Our core approach involves linking long-term grassroots initiatives with standardized protocols and data management to generate robust scaled-up results. By connecting tropical researchers and elevating their status, our Social Research Network model recognises the key role of the data originator in scientific discovery. Conceived in 1999 with RAINFOR (South America), our permanent plot networks have been adapted to Africa (AfriTRON) and Southeast Asia (T-FORCES) and widely emulated worldwide. Now these multiple initiatives are integrated via ForestPlots.net cyber-infrastructure, linking colleagues from 54 countries across 24 plot networks. Collectively these are transforming understanding of tropical forests and their biospheric role. Together we have discovered how, where and why forest carbon and biodiversity are responding to climate change, and how they feedback on it. This long-term pan-tropical collaboration has revealed a large long-term carbon sink and its trends, as well as making clear which drivers are most important, which forest processes are affected, where they are changing, what the lags are, and the likely future responses of tropical forests as the climate continues to change. By leveraging a remarkably old technology, plot networks are sparking a very modern revolution in tropical forest science.
