    Patella malalignment, pain and patellofemoral progression: the Health ABC Study

    SummaryObjectivePatellofemoral (PF) joint osteoarthritis (OA) is strongly correlated with lower extremity disability and knee pain. Risk factors for pain and structural progression in PF OA are poorly understood. Our objective was to determine the association between patella malalignment and its relation to pain severity, and PF OA disease progression.MethodsWe conducted an analysis of data from the Health ABC knee OA study. Health ABC is a community based, multi-center cohort study of 3075 Caucasian and Black men and women aged 70–79 at enrollment. Weight bearing skyline knee X-rays were obtained in a subset (595) of subjects, with and without knee pain, at year 2 and year 5 (mean follow-up 36 months). Films were read paired, and PF osteophytes (OST) and joint space narrowing (JSN) were scored on a 0–3 scale using the Osteoarthritis Research Society International atlas. We defined progression of PF OA as any increase in JSN score. Three measures of patella malalignment were made: sulcus angle; patella tilt angle; and patella subluxation medially or laterally (bisect offset). Knee symptoms were assessed using a knee specific Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) knee pain subscale. We assessed the relationship between baseline patella malalignment and pain severity (linear regression for WOMAC) and compartment specific PF OA progression (logistic regression for dichotomous outcomes). We classified continuous measures of patella alignment into quartile groups. We performed multivariable adjusted logistic regression models, including age, gender and body mass index (BMI) to assess the relation of baseline patella alignment to the occurrence of PF JSN progression using generalized estimating equations (GEE).ResultsThe subjects had a mean age 73.6 (SD 2.9), BMI 28.8 (SD 4.9), 40.3% male, and 46% were Black. Medial displacement of the patella predisposed to medial JSN progression; odds for each quartile 1, 1.2, 1.2, 2.2 (P for trend=0.03), whilst protecting from lateral JSN progression; odds for each quartile 1, 0.7, 0.6, 0.4 (P for trend=0.0004). Increasing patella tilt protected from medial JSN progression; odds for each quartile 1, 0.8, 0.5, 0.2 (P<0.0001) and trended to increasing pain severity (P=0.09).ConclusionPatella malalignment is associated with PF disease progression. Medial displacement and tilt of the patella predisposes to medial JSN progression, whilst lateral displacement is predictive of lateral JSN progression. The influence of patella malalignment has important implications since it is potentially modifiable through footwear, taping and/or knee bracing

    Gravity-induced Wannier-Stark ladder in an optical lattice

    We discuss the dynamics of ultracold atoms in an optical potential accelerated by gravity. The positions and widths of the Wannier-Stark ladder of resonances are obtained as metastable states. The metastable Wannier-Bloch states oscillate in a single band with the Bloch period.     Hidden dimers and the matrix maps: Fibonacci chains re-visited

    The existence of cycles of the matrix maps in Fibonacci class of lattices is well established. We show that such cycles are intimately connected with the presence of interesting positional correlations among the constituent `atoms' in a one dimensional quasiperiodic lattice. We particularly address the transfer model of the classic golden mean Fibonacci chain where a six cycle of the full matrix map exists at the centre of the spectrum [Kohmoto et al, Phys. Rev. B 35, 1020 (1987)], and for which no simple physical picture has so far been provided, to the best of our knowledge. In addition, we show that our prescription leads to a determination of other energy values for a mixed model of the Fibonacci chain, for which the full matrix map may have similar cyclic behaviour. Apart from the standard transfer-model of a golden mean Fibonacci chain, we address a variant of it and the silver mean lattice, where the existence of four cycles of the matrix map is already known to exist.     Topological quantization and degeneracy in Josephson-junction arrays

    We consider the conductivity quantization in two-dimensional arrays of mesoscopic Josephson junctions, and examine the associated degeneracy in various regimes of the system. The filling factor of the system may be controlled by the gate voltage as well as the magnetic field, and its appropriate values for quantization is obtained by employing the Jain hierarchy scheme both in the charge description and in the vortex description. The duality between the two descriptions then suggests the possibility that the system undergoes a change in degeneracy while the quantized conductivity remains fixed.Comment: To appear in Phys.     Effects of the field modulation on the Hofstadter's spectrum

    We study the effect of spatially modulated magnetic fields on the energy spectrum of a two-dimensional (2D) Bloch electron. Taking into account four kinds of modulated fields and using the method of direct diagonalization of the Hamiltonian matrix, we calculate energy spectra with varying system parameters (i.e., the kind of the modulation, the relative strength of the modulated field to the uniform background field, and the period of the modulation) to elucidate that the energy band structure sensitively depends on such parameters: Inclusion of spatially modulated fields into a uniform field leads occurrence of gap opening, gap closing, band crossing, and band broadening, resulting distinctive energy band structure from the Hofstadter's spectrum.     A long in situ section of the lower ocean crust: results of {ODP} Leg 176 drilling at the Southwest Indian Ridge

    Ocean Drilling Program Leg 176 deepened Hole 735B in gabbroic lower ocean crust by 1 km to 1.5 km. The section has the physical properties of seismic layer 3, and a total magnetization sufficient by itself to account for the overlying lineated sea-surface magnetic anomaly. The rocks from Hole 735B are principally olivine gabbro, with evidence for two principal and many secondary intrusive events. There are innumerable late small ferrogabbro intrusions, often associated with shear zones that cross-cut the olivine gabbros. The ferrogabbros dramatically increase upward in the section. Whereas there are many small patches of ferrogabbro representing late iron- and titanium-rich melt trapped intragranularly in olivine gabbro, most late melt was redistributed prior to complete solidification by compaction and deformation. This, rather than in situ upward differentiation of a large magma body, produced the principal igneous stratigraphy. The computed bulk composition of the hole is too evolved to mass balance mid-ocean ridge basalt back to a primary magma, and there must be a significant mass of missing primitive cumulates. These could lie either below the hole or out of the section. Possibly the gabbros were emplaced by along-axis intrusion of moderately differentiated melts into the near-transform environment. Alteration occurred in three stages. High-temperature granulite- to amphibolite-facies alteration is most important, coinciding with brittle-ductile deformation beneath the ridge. Minor greenschist-facies alteration occurred under largely static conditions, likely during block uplift at the ridge transform intersection. Late post-uplift low-temperature alteration produced locally abundant smectite, often in previously unaltered areas. The most important features of the high- and low-temperature alteration are their respective associations with ductile and cataclastic deformation, and an overall decrease downhole with hydrothermal alteration generally =<5% in the bottom kilometer. Hole 735B provides evidence for a strongly heterogeneous lower ocean crust, and for the inherent interplay of deformation, alteration and igneous processes at slow-spreading ridges. It is strikingly different from gabbros sampled from fast-spreading ridges and at most well-described ophiolite complexes. We attribute this to the remarkable diversity of tectonic environments where crustal accretion occurs in the oceans and to the low probability of a section of old slow-spread crust formed near a major large-offset transform being emplaced on-land compared to sections of young crust from small ocean basins
