Patrones alimenticios y prevalencia de obesidad y sobre peso en escolares del sector público y privado en la zona metropolitana Guadalupe-Zacatecas 2015
Anteriormente, el exceso de peso se consideraba signo de belleza, salud y estatus social; en la
actualidad, se sabe que es un problema de índole multifactorial que atañe a un gran porcentaje
de la población. De acuerdo a cifras oficiales en México, en 2016 la prevalencia combinada de
sobrepeso y obesidad en adultos (IMC ≥25 kg/m2) es mayor en las mujeres (75.6 por ciento)
que en los hombres (69.4 por ciento), y la prevalencia de obesidad (IMC ≥30 kg/m2) es más
alta en el sexo femenino que en el masculino.

Mostrando un aumento de 1.3 puntos
porcentuales para obesidad y sobrepeso según lo registrado entre 2012 y 2016.
Para la población en edad escolar (de cinco a once años de edad), la prevalencia nacional
combinada de sobrepeso y obesidad en 2016, utilizando los criterios de la OMS, fue de 33.2
por ciento (17.9 y 15.3 por ciento respectivamente). Estas prevalencias en niños en edad
escolar representan alrededor de 5’246,908 niños con sobrepeso y obesidad en el ámbito
nacional. En el 2012, esta prevalencia fue de 34.4 por ciento, es decir, 1.2 puntos porcentuales
más que en los recientes resultados, sin embargo, a pesar de esta tendencia de disminución, los
intervalos de confianza de 2016 son relativamente amplios, por lo que no es posible concluir
que la prevalencia combinada de sobrepeso y obesidad en escolares haya disminuido.

Los patrones alimenticios han sufrido una serie de cambios radicales con repercusiones
directas a la salud, donde anteriormente la dieta estaba constituida por el consumo de cereales,
tubérculos, vegetales y frutas; posterior a ello, se caracterizó por un aumento en el consumo de
azúcar, grasas y alimentos procesados finalizando en un mayor consumo de alimentos con alto
contenido de grasa y azúcar y bajo contenido de fibra dietética, provocando obesidad y
sobrepeso.

En México, existe una media de consumo de 3.7 grupos de alimentos
recomendables (frutas, verduras, leguminosas, carnes no procesadas, agua, huevo y lácteos) y de 2.8 para los grupos no recomendables (carnes procesadas, comida rápida, botanas y dulces,
cereales dulces, bebidas no lácteas endulzadas y bebidas lácteas endulzadas) en niños escolares.

El notable aumento en las investigaciones realizadas en la última década en cuanto a
alimentación y estado nutricional se debe precisamente a que las tendencias de los índices de
prevalencia de obesidad y sobrepeso se mantienen, así como la gran cantidad de problemas de
salud que se encuentran asociados a este cuadro clínico como la hipertensión arterial, la
diabetes mellitus tipo dos, complicaciones respiratorias, alteraciones del crecimiento, etcétera.

La importancia de la detección y el control de estas enfermedades a tiempo radica en
modificar los estilos de vida y los hábitos alimenticios de los escolares, con la finalidad de
disminuir los riesgos a la salud, ya que, los niños obesos tienen mayor predisposición a seguir
desarrollando obesidad en etapas posteriores de la vida, así como la aparición de patologías a
temprana edad. Otras consecuencias de la obesidad son problemas físicos, psicológicos y
sociales. Por lo tanto, para poder intervenir de manera certera en esta enfermedad crónica, es
necesaria una mayor comprensión de los determinantes sociales que desencadenan esta
epidemia. Se sabe que hay factores sociodemográficos, culturales y educacionales que han
hecho que la población adopte nuevos patrones alimenticios y diferentes hábitos y costumbres
de alimentación. Todos estos cambios han producido un aumento en la prevalencia de la
obesidad y el sobrepeso en niños de edad escolar.
La siguiente investigación formó parte de un proyecto doctoral titulado «Sobrepeso y
obesidad escolar, impacto del sistema alimentario. El caso de la zona metropolitana
Guadalupe- Zacatecas, 2000-2015»

determinantes para la construcción de patrones alimenticios de los escolares del sector público

Genetic Evidence for Multiple Sources of the Non-Native Fish Cichlasoma urophthalmus (Günther; Mayan Cichlids) in Southern Florida
The number and diversity of source populations may influence the genetic diversity of newly introduced populations and affect the likelihood of their establishment and spread. We used the cytochrome b mitochondrial gene and nuclear microsatellite loci to identify the sources of a successful invader in southern Florida, USA, Cichlasoma urophthalmus (Mayan cichlid). Our cytochrome b data supported an introduction from Guatemala, while our microsatellite data suggested movement of Mayan Cichlids from the upper Yucatán Peninsula to Guatemala and introductions from Guatemala and Belize to Florida. The mismatch between mitochondrial and nuclear genomes suggests admixture of a female lineage from Guatemala, where all individuals were fixed for the mitochondrial haplotype found in the introduced population, and a more diverse but also relatively small number of individuals from Belize. The Florida cytochrome b haplotype appears to be absent from Belize (0 out of 136 fish screened from Belize had this haplotype). Genetic structure within the Florida population was minimal, indicating a panmictic population, while Mexican and Central American samples displayed more genetic subdivision. Individuals from the Upper Yucatán Peninsula and the Petén region of Guatemala were more genetically similar to each other than to fish from nearby sites and movement of Mayan Cichlids between these regions occurred thousands of generations ago, suggestive of pre-Columbian human transportation of Mayan Cichlids through this region. Mayan Cichlids present a rare example of cytonuclear disequilibrium and reduced genetic diversity in the introduced population that persists more than 30 years (at least 7–8 generations) after introduction. We suggest that hybridization occurred in ornamental fish farms in Florida and may contribute their establishment in the novel habitat. Hybridization prior to release may contribute to other successful invasions

An experimental model of chronic severe mitral regurgitation
OBJECTIVE: To develop a minimally invasive, reproducible model of chronic severe mitral regurgitation (MR) that replicates the clinical phenotype of left atrial (LA) and left ventricular dilation and susceptibility to atrial fibrillation.
METHODS: Under transesophageal echocardiographic guidance, chordae tendinae were avulsed using endovascular forceps until the ratio of regurgitant jet area to LA area was ≥70%. Animals survived for an average of 8.6 ± 1.6 months (standard deviation) and imaged with monthly transthoracic echocardiography (TTE). Animals underwent baseline and preterminal magnetic resonance imaging. Terminal studies included TTE, transesophageal echocardiography, and rapid atrial pacing to test inducibility of atrial tachyarrhythmias.
RESULTS: Eight dogs underwent creation of severe MR and interval monitoring. Two were excluded-one died from acute heart failure, and the other had resolution of MR. Six dogs underwent the full experimental protocol; only one required medical management of clinical heart failure. MR remained severe over time, with a mean terminal regurgitant jet area to LA area of 71 ± 14% (standard deviation) and regurgitant fraction of 52 ± 11%. Mean LA volume increased over 130% (TTE: 163 ± 147%,
CONCLUSIONS: Within the 6 dogs that successfully completed the full experimental protocol, this model replicated the clinical phenotype of severe MR, which led to marked structural and electrophysiologic cardiac remodeling. This model allowed for precise measurements at repeated time points and will facilitate future studies to elucidate the mechanisms of atrial and ventricular remodeling secondary to MR and the pathophysiology of valvular atrial fibrillation
Integrated Measurement for Early Detection (MIDO) as a digital strategy for timely assessment of non-communicable disease profiles and factors associated with unawareness and control: a retrospective observational study in primary healthcare facilities in Mexico.
Objectives
The Carlos Slim Foundation implemented the Integrated Measurement for Early Detection (MIDO), a screening strategy for non-communicable diseases (NCDs) in Mexico as part of CASALUD, a portfolio of digital health services focusing on healthcare delivery and prevention/management of NCDs. We investigated the disease profile of the screened population and evaluated MIDO's contribution to the continuum of care of the main NCDs.
Design
Using data from MIDO and the chronic diseases information system, we quantified the proportion of the population screened and diagnosed with NCDs, and measured care linkage/retention and level of control achieved. We analysed comorbidity patterns and estimated prevalence of predisease stages. Finally, we estimated characteristics associated with unawareness and control of NCDs, and examined efficacy of the CASALUD model in improving NCD control.SettingPublic primary health centres in 27/32 Mexican states.
Participants
Individuals aged ≥20 years lacking healthcare access.
Results
From 2014 to 2018, 743 000 individuals were screened using MIDO. A predisease or disease condition was detected in ≥70% of the population who were unaware of their NCD status. The screening identified 38 417 new cases of type 2 diabetes, 53 133 new cases of hypertension and 208 627 individuals with obesity. Dyslipidaemia was found in 77.3% of individuals with available blood samples. Comorbidities were highly prevalent, especially in people with obesity. Only 5.47% (n=17 774) of individuals were linked with their corresponding primary health centre. Factors associated with unawareness of and uncontrolled NCDs were sex, age, and social determinants, for example, rural/urban environment, access to healthcare service, and education level. Patients with type 2 diabetes treated at clinics under the CASALUD model were more likely to achieve disease control (OR: 1.32, 95% CI: 1.09 to 1.61).
Conclusion
Patient-centred screening strategies such as MIDO are urgently needed to improve screening, access, retention and control for patients with NCDs
Estimating mortality and disability in Peru before the COVID-19 pandemic: a systematic analysis from the Global Burden of the Disease Study 2019
"Background: Estimating and analyzing trends and patterns of health loss are
essential to promote efficient resource allocation and improve Peru’s healthcare
system performance.
Methods: Using estimates from the Global Burden of Disease (GBD), Injuries, and
Risk Factors Study (2019), we assessed mortality and disability in Peru from 1990
to 2019. We report demographic and epidemiologic trends in terms of population,
life expectancy at birth (LE), mortality, incidence, prevalence, years of life lost
(YLLs), years lived with disability (YLDs), and disability-adjusted life-years (DALYs)
caused by the major diseases and risk factors in Peru. Finally, we compared Peru
with 16 countries in the Latin American (LA) region.
Results: The Peruvian population reached 33.9 million inhabitants (49.9% women)
in 2019. From 1990 to 2019, LE at birth increased from 69.2 (95% uncertainty
interval 67.8–70.3) to 80.3 (77.2–83.2) years. This increase was driven by the
decline in under-5 mortality (−80.7%) and mortality from infectious diseases in
older age groups (+60 years old). The number of DALYs in 1990 was 9.2 million (8.5–10.1) and reached 7.5 million (6.1–9.0) in 2019. The proportion of DALYs due
to non-communicable diseases (NCDs) increased from 38.2% in 1990 to 67.9% in
2019. The all-ages and age-standardized DALYs rates and YLLs rates decreased,
but YLDs rates remained constant. In 2019, the leading causes of DALYs were
neonatal disorders, lower respiratory infections (LRIs), ischemic heart disease,
road injuries, and low back pain. The leading risk factors associated with DALYs
in 2019 were undernutrition, high body mass index, high fasting plasma glucose,
and air pollution. Before the COVID-19 pandemic, Peru experienced one of the
highest LRIs-DALYs rates in the LA region.
Conclusion: In the last three decades, Peru experienced significant improvements
in LE and child survival and an increase in the burden of NCDs and associated
disability. The Peruvian healthcare system must be redesigned to respond to
this epidemiological transition. The new design should aim to reduce premature
deaths and maintain healthy longevity, focusing on effective coverage and
treatment of NCDs and reducing and managing the related disability.

Global burden of 369 diseases and injuries in 204 countries and territories, 1990–2019: a systematic analysis for the Global Burden of Disease Study 2019
Background: In an era of shifting global agendas and expanded emphasis on non-communicable diseases and injuries along with communicable diseases, sound evidence on trends by cause at the national level is essential. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) provides a systematic scientific assessment of published, publicly available, and contributed data on incidence, prevalence, and mortality for a mutually exclusive and collectively exhaustive list of diseases and injuries. Methods: GBD estimates incidence, prevalence, mortality, years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life-years (DALYs) due to 369 diseases and injuries, for two sexes, and for 204 countries and territories. Input data were extracted from censuses, household surveys, civil registration and vital statistics, disease registries, health service use, air pollution monitors, satellite imaging, disease notifications, and other sources. Cause-specific death rates and cause fractions were calculated using the Cause of Death Ensemble model and spatiotemporal Gaussian process regression. Cause-specific deaths were adjusted to match the total all-cause deaths calculated as part of the GBD population, fertility, and mortality estimates. Deaths were multiplied by standard life expectancy at each age to calculate YLLs. A Bayesian meta-regression modelling tool, DisMod-MR 2.1, was used to ensure consistency between incidence, prevalence, remission, excess mortality, and cause-specific mortality for most causes. Prevalence estimates were multiplied by disability weights for mutually exclusive sequelae of diseases and injuries to calculate YLDs. We considered results in the context of the Socio-demographic Index (SDI), a composite indicator of income per capita, years of schooling, and fertility rate in females younger than 25 years. Uncertainty intervals (UIs) were generated for every metric using the 25th and 975th ordered 1000 draw values of the posterior distribution. Findings: Global health has steadily improved over the past 30 years as measured by age-standardised DALY rates. After taking into account population growth and ageing, the absolute number of DALYs has remained stable. Since 2010, the pace of decline in global age-standardised DALY rates has accelerated in age groups younger than 50 years compared with the 1990–2010 time period, with the greatest annualised rate of decline occurring in the 0–9-year age group. Six infectious diseases were among the top ten causes of DALYs in children younger than 10 years in 2019: lower respiratory infections (ranked second), diarrhoeal diseases (third), malaria (fifth), meningitis (sixth), whooping cough (ninth), and sexually transmitted infections (which, in this age group, is fully accounted for by congenital syphilis; ranked tenth). In adolescents aged 10–24 years, three injury causes were among the top causes of DALYs: road injuries (ranked first), self-harm (third), and interpersonal violence (fifth). Five of the causes that were in the top ten for ages 10–24 years were also in the top ten in the 25–49-year age group: road injuries (ranked first), HIV/AIDS (second), low back pain (fourth), headache disorders (fifth), and depressive disorders (sixth). In 2019, ischaemic heart disease and stroke were the top-ranked causes of DALYs in both the 50–74-year and 75-years-and-older age groups. Since 1990, there has been a marked shift towards a greater proportion of burden due to YLDs from non-communicable diseases and injuries. In 2019, there were 11 countries where non-communicable disease and injury YLDs constituted more than half of all disease burden. Decreases in age-standardised DALY rates have accelerated over the past decade in countries at the lower end of the SDI range, while improvements have started to stagnate or even reverse in countries with higher SDI. Interpretation: As disability becomes an increasingly large component of disease burden and a larger component of health expenditure, greater research and developm nt investment is needed to identify new, more effective intervention strategies. With a rapidly ageing global population, the demands on health services to deal with disabling outcomes, which increase with age, will require policy makers to anticipate these changes. The mix of universal and more geographically specific influences on health reinforces the need for regular reporting on population health in detail and by underlying cause to help decision makers to identify success stories of disease control to emulate, as well as opportunities to improve.
