    Clustering transition in a system of particles self-consistently driven by a shear flow

    We introduce a simple model of active transport for an ensemble of particles driven by an external shear flow. Active refers to the fact that the flow of the particles is modified by the distribution of particles itself. The model consists in that the effective velocity of every particle is given by the average of the external flow velocities felt by the particles located at a distance less than a typical radius, RR. Numerical analysis reveals the existence of a transition to clustering depending on the parameters of the external flow and on RR. A continuum description in terms of the number density of particles is derived, and a linear stability analysis of the density equation is performed in order to characterize the transitions observed in the model of interacting particles.Comment: 11 pages, 2 figures.     Diffusive transport and self-consistent dynamics in coupled maps

    The study of diffusion in Hamiltonian systems has been a problem of interest for a number of years. In this paper we explore the influence of self-consistency on the diffusion properties of systems described by coupled symplectic maps. Self-consistency, i.e. the back-influence of the transported quantity on the velocity field of the driving flow, despite of its critical importance, is usually overlooked in the description of realistic systems, for example in plasma physics. We propose a class of self-consistent models consisting of an ensemble of maps globally coupled through a mean field. Depending on the kind of coupling, two different general types of self-consistent maps are considered: maps coupled to the field only through the phase, and fully coupled maps, i.e. through the phase and the amplitude of the external field. The analogies and differences of the diffusion properties of these two kinds of maps are discussed in detail.Comment: 13 pages, 14 figure

    Using XAI in the Clock Drawing Test to reveal the cognitive impairment pattern.

    he prevalence of dementia is currently increasing worldwide. This syndrome produces a deteriorationin cognitive function that cannot be reverted. However, an early diagnosis can be crucial for slowing itsprogress. The Clock Drawing Test (CDT) is a widely used paper-and-pencil test for cognitive assessmentin which an individual has to manually draw a clock on a paper. There are a lot of scoring systems forthis test and most of them depend on the subjective assessment of the expert. This study proposes acomputer-aided diagnosis (CAD) system based on artificial intelligence (AI) methods to analyze the CDTand obtain an automatic diagnosis of cognitive impairment (CI). This system employs a preprocessingpipeline in which the clock is detected, centered and binarized to decrease the computational burden.Then, the resulting image is fed into a Convolutional Neural Network (CNN) to identify the informativepatterns within the CDT drawings that are relevant for the assessment of the patient’s cognitive status.Performance is evaluated in a real context where patients with CI and controls have been classified byclinical experts in a balanced sample size of 3282 drawings. The proposed method provides an accuracyof 75.65% in the binary case-control classification task, with an AUC of 0.83. These results are indeedrelevant considering the use of the classic version of the CDT. The large size of the sample suggests thatthe method proposed has a high reliability to be used in clinical contexts and demonstrates the suitabilityof CAD systems in the CDT assessment process. Explainable artificial intelligence (XAI) methods areapplied to identify the most relevant regions during classification. Finding these patterns is extremelyhelpful to understand the brain damage caused by CI.     New adipokines vaspin and omentin. Circulating levels and gene expression in adipose tissue from morbidly obese women

    <p>Abstract</p> <p>Background</p> <p>Vaspin and omentin are recently described molecules that belong to the adipokine family and seem to be related to metabolic risk factors. The objectives of this study were twofold: to evaluate vaspin and omentin circulating levels and mRNA expression in subcutaneous and visceral adipose tissues in non-diabetic morbidly obese women; and to assess the relationship of vaspin and omentin with anthropometric and metabolic parameters, and other adipo/cytokines.</p> <p>Design</p> <p>We analysed vaspin and omentin circulating levels in 71 women of European descent (40 morbidly obese [BMI ≥ 40 kg/m<sup>2</sup>] and 31 lean [BMI ≤ 25]). We assessed vaspin and omentin gene expression in paired samples of visceral and subcutaneous abdominal adipose tissue from 46 women: 40 morbidly obese and 6 lean. We determined serum vaspin and plasma omentin levels with an Enzyme-Linked Immunosorbent Assay and adipose tissue mRNA expression by real time RT-PCR.</p> <p>Results</p> <p>Serum vaspin levels in the morbidly obese were not significantly different from those in controls. They correlated inversely with levels of lipocalin 2 and interleukin 6. Vaspin mRNA expression was significantly higher in the morbidly obese, in both subcutaneous and visceral adipose tissue.</p> <p>Plasma omentin levels were significantly lower in the morbidly obese and they correlated inversely with glucidic metabolism parameters. Omentin circulating levels, then, correlated inversely with the metabolic syndrome (MS). Omentin expression in visceral adipose tissue was significantly lower in morbidly obese women than in controls.</p> <p>Conclusions</p> <p>The present study indicates that vaspin may have a compensatory role in the underlying inflammation of obesity. Decreased omentin circulating levels have a close association with MS in morbidly obese women.</p

    A review of application of multi-criteria decision making methods in construction

    Construction is an area of study wherein making decisions adequately can mean the difference between success and failure. Moreover, most of the activities belonging to this sector involve taking into account a large number of conflicting aspects, which hinders their management as a whole. Multi-criteria decision making analysis arose to model complex problems like these. This paper reviews the application of 22 different methods belonging to this discipline in various areas of the construction industry clustered in 11 categories. The most significant methods are briefly discussed, pointing out their principal strengths and limitations. Furthermore, the data gathered while performing the paper are statistically analysed to identify different trends concerning the use of these techniques. The review shows their usefulness in characterizing very different decision making environments, highlighting the reliability acquired by the most pragmatic and widespread methods and the emergent tendency to use some of them in combination

    Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)

    This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes.
