    Perfil epidemiológico da hanseníase no município de Divinópolis, Minas Gerais, 2011 a 2019

    Objetivo: Analisar o perfil epidemiológico da hanseníase no município de Divinópolis, Minas Gerais, entre os anos de 2011 a 2019. Método: Trata-se de um estudo ecológico, descritivo, que utilizou dados secundários disponíveis em acesso aberto na Sala de Apoio à Gestão Estratégica e no Departamento de Informática do Sistema Único de Saúde referentes aos casos confirmados e notificados da hanseníase de residentes no município de Divinópolis, Minas Gerais, no período de 2011 a 2019. Resultados: No período do estudo foram notificados 57 casos de hanseníase, todos em pessoas acima de 20 anos, sendo a maioria em homens (50,9%), de raça/cor parda (49,1%) e com ensino fundamental incompleto (26,3%). A forma clínica predominante foi a dimorfa (38,4%) seguido pela tuberculoide (29,8%), classificação operacional multibacilar (56,2%), sendo que a maioria dos casos (93%) deram entrada no Sistema de Informação de Agravos de Notificação (SINAN) como caso novo. Quanto à avaliação do grau de incapacidade física no diagnóstico, 7% (n=4) dos casos foram notificados com grau 2. No que se refere ao tipo de saída, 87,9% dos casos tiveram alta por cura. Conclusão: Os dados epidemiológicos apontam que o diagnóstico da hanseníase em Divinópolis está sendo realizado tardiamente, o que contribui para manutenção de uma prevalência oculta.Objective: to analyze the epidemiological profile of leprosy in the municipality of Divinópolis, Minas Gerais, between the years 2011 to 2019. Method: this is an ecological, descriptive study, which used secondary data available in open access in the Strategic Management Support Room and the Department of Informatics of the Unified Health System regarding the confirmed and notified cases of leprosy of residents in the municipality of Divinópolis, Minas Gerais, in the period from 2011 to 2019. Results: during the study period, 57 cases of leprosy were reported, all in people over 20 years old, mostly in men (50.9%), of brown race/color (49.1%) and with incomplete elementary school education (26.3%). The predominant clinical form was dimorphic (38.4%) followed by tuberculoid (29.8%), multibacillary operational classification (56.2%), and most cases (93%) were entered in the Sistema de Informação de Agravos de Notificação (SINAN) as new cases. As for the evaluation of the degree of physical disability at diagnosis, 7% (n=4) of cases were reported as grade 2. Regarding the type of output, 87.9% of cases were discharged for cure. Conclusion: the epidemiological data indicate that the diagnosis of leprosy in Divinópolis is being made late, which contributes to the maintenance of a hidden prevalence

    Genomic profiling of primary and recurrent Adult Granulosa Cell Tumors of the Ovary

    Adult-type granulosa cell tumor (aGCT) is a rare malignant ovarian sex cord-stromal tumor, harboring recurrent FOXL2 c.C402G/p.C134W hotspot mutations in 97% of cases. These tumors are considered to have a favorable prognosis, however aGCTs have a tendency for local spread and late recurrences, which are associated with poor survival rates. We sought to determine the genetic alterations associated with aGCT disease progression. We subjected primary non-recurrent aGCTs (n = 7), primary aGCTs that subsequently recurred (n = 9) and their matched recurrences (n = 9), and aGCT recurrences without matched primary tumors (n = 10) to targeted massively parallel sequencing of ≥410 cancer-related genes. In addition, three primary non-recurrent aGCTs and nine aGCT recurrences were subjected to FOXL2 and TERT promoter Sanger sequencing analysis. All aGCTs harbored the FOXL2 C134W hotspot mutation. TERT promoter mutations were found to be significantly more frequent in recurrent (18/28, 64%) than primary aGCTs (5/19, 26%, p = 0.017). In addition, mutations affecting TP53, MED12, and TET2 were restricted to aGCT recurrences. Pathway annotation of altered genes demonstrated that aGCT recurrences displayed an enrichment for genetic alterations affecting cell cycle pathway-related genes. Analysis of paired primary and recurrent aGCTs revealed that TERT promoter mutations were either present in both primary tumors and matched recurrences or were restricted to the recurrence and absent in the respective primary aGCT. Clonal composition analysis of these paired samples further revealed that aGCTs display intra-tumor genetic heterogeneity and harbor multiple clones at diagnosis and relapse. We observed that in a subset of cases, recurrences acquired additional genetic alterations not present in primary aGCTs, including TERT, MED12, and TP53 mutations and CDKN2A/B homozygous deletions. Albeit harboring relatively simple genomes, our data provide evidence to suggest that aGCTs are genetically heterogeneous tumors and that TERT promoter mutations and/or genetic alterations affecting other cell cycle-related genes may be associated with disease progression and recurrences

    As complicações pós-operatórias em cirurgias cardíacas são comuns e contribuem para o aumento dos índices de morbidade e mortalidade. Objetivo: identificar em trabalhos da literatura as principais complicações no pós-operatório de cirurgias cardíacas. Material e Método: Revisão de literatura sobre as principais complicações no pós-operatório de cirurgia cardíaca. A busca foi realizada em outubro de 2022 nas fontes de dados: PubMed e Web of Science. Resultados: O processo de busca resultou em 2.744 documentos. Após primeira seleção 215 trabalhos tiveram os seus títulos e resumos analisados para uma triagem inicial. A amostra final foi de 04 estudos. As complicações da cirurgia cardíaca podem estar relacionadas a doenças pré-existentes. Forma identificadas como complicações distúrbios de sono, hepatopatia cardíaca pós-operatória, síndrome da apneia e hipopneia obstrutiva do sono (SAHOS) e arritmias. Conclusões: As complicações apresentaram prevalências diferentes nos estudos analisados e devem ser consideradas em mais estudos para melhor compreensão de fatores correlacionados auxiliando na sua prevenção e controle

    Post-intervention Status in Patients With Refractory Myasthenia Gravis Treated With Eculizumab During REGAIN and Its Open-Label Extension

    OBJECTIVE: To evaluate whether eculizumab helps patients with anti-acetylcholine receptor-positive (AChR+) refractory generalized myasthenia gravis (gMG) achieve the Myasthenia Gravis Foundation of America (MGFA) post-intervention status of minimal manifestations (MM), we assessed patients' status throughout REGAIN (Safety and Efficacy of Eculizumab in AChR+ Refractory Generalized Myasthenia Gravis) and its open-label extension. METHODS: Patients who completed the REGAIN randomized controlled trial and continued into the open-label extension were included in this tertiary endpoint analysis. Patients were assessed for the MGFA post-intervention status of improved, unchanged, worse, MM, and pharmacologic remission at defined time points during REGAIN and through week 130 of the open-label study. RESULTS: A total of 117 patients completed REGAIN and continued into the open-label study (eculizumab/eculizumab: 56; placebo/eculizumab: 61). At week 26 of REGAIN, more eculizumab-treated patients than placebo-treated patients achieved a status of improved (60.7% vs 41.7%) or MM (25.0% vs 13.3%; common OR: 2.3; 95% CI: 1.1-4.5). After 130 weeks of eculizumab treatment, 88.0% of patients achieved improved status and 57.3% of patients achieved MM status. The safety profile of eculizumab was consistent with its known profile and no new safety signals were detected. CONCLUSION: Eculizumab led to rapid and sustained achievement of MM in patients with AChR+ refractory gMG. These findings support the use of eculizumab in this previously difficult-to-treat patient population. CLINICALTRIALSGOV IDENTIFIER: REGAIN, NCT01997229; REGAIN open-label extension, NCT02301624.     Differential cross section measurements for the production of a W boson in association with jets in proton–proton collisions at √s = 7 TeV

    Measurements are reported of differential cross sections for the production of a W boson, which decays into a muon and a neutrino, in association with jets, as a function of several variables, including the transverse momenta (pT) and pseudorapidities of the four leading jets, the scalar sum of jet transverse momenta (HT), and the difference in azimuthal angle between the directions of each jet and the muon. The data sample of pp collisions at a centre-of-mass energy of 7 TeV was collected with the CMS detector at the LHC and corresponds to an integrated luminosity of 5.0 fb[superscript −1]. The measured cross sections are compared to predictions from Monte Carlo generators, MadGraph + pythia and sherpa, and to next-to-leading-order calculations from BlackHat + sherpa. The differential cross sections are found to be in agreement with the predictions, apart from the pT distributions of the leading jets at high pT values, the distributions of the HT at high-HT and low jet multiplicity, and the distribution of the difference in azimuthal angle between the leading jet and the muon at low values.United States.
