    Large Retroperitoneal Hematoma and Lumbosacral Plexopathy Following Microsurgical Decompression via the Wiltse Paraspinal Approach: A Case Report

    Paraspinal approaches are associated with fewer complications and better outcomes than midline approaches. However, most surgeons are not conversant with the surgical anatomy in these approaches, especially at the lumbosacral junction, where the anatomy is more complex. This paper reports a case of concomitant vascular and neurological complication following the use of the microscopic Wiltse approach. A 71-year-old woman presented with symptoms of spinal stenosis and right L5 radiculopathy. She underwent microsurgical decompression of a foraminal stenosis at the lumbosacral junction via the Wiltse approach, and a midline decompression from L3 to L5. The tubular retractor was docked inappropriately, being too deep and lateral. It directly punctured through the intertransverse membrane and injured the retroperitoneal segmental vessel and lumbosacral plexus. Postoperatively, she developed a large retroperitoneal hematoma and lumbosacral plexopathy, which were treated with surgery and intensive rehabilitation. This report highlights the importance of accurate retractor docking, familiarity with the surgical anatomy, and recognition of the potential complications of this technique

    Effect of Pokémon Go on self-harm using population-based interrupted time-series analysis: quasi-experimental study

    Background: Pokémon Go is a very popular location-based augmented reality game with widespread influences over the world. An emerging body of research demonstrates that playing Pokémon Go can lead to improvements in physical activity and psychosocial well-being; however, whether Pokémon Go reduces self-harm incidence at the population-level is still questionable. Objective: This study aimed to quantify how the launch of Pokémon Go in Hong Kong affected the incidence of self-harm using a quasi-experimental design. Methods: An interrupted time-series design with Poisson segmented regression adjusted for time and seasonality trends was used on data from 2012 to 2018 to detect any changes in the number of accident and emergency attendances due to self-harm, after Pokémon Go was launched. The findings were validated using a baseline control period and using other intentional injuries and minor noninjuries as control outcomes. We also assessed intervention effects by age group. Results: From January 1, 2012 to July 31, 2018, there were 13,463 accident and emergency attendances due to self-harm in Hong Kong. During the period after launching Pokémon Go, self-harm attendances dropped by 34% (adjusted incidence rate ratio: 0.66, 95% CI 0.61-0.73). When analyzed by age group, a reduction in self-harm incidence was only apparent in adults (18 to 24 years of age: adjusted incidence rate ratio: 0.78, P=.02; 25 to 39 years of age: adjusted incidence rate ratio: 0.75, P<.001; 40 years of age and older: adjusted incidence rate ratio: 0.57, P<.001). Conclusions: Self-harm incidence in the population, particularly in adults, showed a significant decline in the period after Pokémon Go was launched. Augmented reality games such as Pokémon Go show great promise as a tool to enhance psychosocial well-being and improve mental health

    A 30-Year Worldwide Research Productivity of Scientific Publication in Full-Endoscopic Decompression Spine Surgery: Quantitative and Qualitative Analysis

    Objective The ever-growing number of articles related to full-endoscopic spine surgery published in the last few decades presents a challenge which is perplexing and time-consuming in identifying the current research status. The study aims to identify and analyze the most cited works related to full-endoscopic decompression spine surgery, compare the articles published by different publishers and area, and show the current publication status of full-endoscopic research. Methods Using Bibliometrix, CiteSpace, and VOSviewer, we analyzed the bibliometric data selected from the Web of Science database between 1992 and 2022. Spine has the highest H-index with the most-cited journal in the field of full-endoscopic decompression spine surgery. China ranked as the most productive country, whereas the most cited with high H-index papers came from South Korea. For the author analysis, Yeung AT, Ruetten S, Hoogland T, Ahn Y, Choi G, and Mayer HM were the most impactful authors in the global and local citations. The most productive organization is Wooridul Spine Hospital. Conclusion The bibliometric study showed a growing trend of research on full-endoscopic decompression spine surgery over the past 30 years. It has demonstrated that there is a significant increase in the number of authors, institutions, and internationally collaborated countries. However, the quality of studies is still low, and the lack of high-quality clinical evidence and the trend of general journal submissions has somewhat affected the quality of endoscopy journals in recent years

    Search for dark matter produced in association with bottom or top quarks in √s = 13 TeV pp collisions with the ATLAS detector

    A search for weakly interacting massive particle dark matter produced in association with bottom or top quarks is presented. Final states containing third-generation quarks and miss- ing transverse momentum are considered. The analysis uses 36.1 fb−1 of proton–proton collision data recorded by the ATLAS experiment at √s = 13 TeV in 2015 and 2016. No significant excess of events above the estimated backgrounds is observed. The results are in- terpreted in the framework of simplified models of spin-0 dark-matter mediators. For colour- neutral spin-0 mediators produced in association with top quarks and decaying into a pair of dark-matter particles, mediator masses below 50 GeV are excluded assuming a dark-matter candidate mass of 1 GeV and unitary couplings. For scalar and pseudoscalar mediators produced in association with bottom quarks, the search sets limits on the production cross- section of 300 times the predicted rate for mediators with masses between 10 and 50 GeV and assuming a dark-matter mass of 1 GeV and unitary coupling. Constraints on colour- charged scalar simplified models are also presented. Assuming a dark-matter particle mass of 35 GeV, mediator particles with mass below 1.1 TeV are excluded for couplings yielding a dark-matter relic density consistent with measurements

    The Outcomes of Anterior Spinal Fusion for Cervical Compressive Myelopathy—A Retrospective Review

    AbstractBackgroundThis study examined the prognostic factors that affect the surgical outcome of anterior spinal fusion in patients with cervical compressive myelopathy (CCM).MethodsA retrospective review of the result of surgical treatment in 49 cases of CCM using anterior spinal fusion surgical techniques. The study period was from June 1990 to June 2008, with a mean follow-up of 6.57 years. Seven patients died during the follow-up period because of other medical diseases. The Japanese Orthopaedic Association score for cervical myelopathy and recovery rate were used to assess the functional results of the remaining 42 cases.ResultsA mean Japanese Orthopaedic Association score improvement from 10.21 to 14.23 points after the operation was observed. The overall recovery rate was 65.38%, with excellent or good results of 80%. After a year of follow-up, we did not find significant improvement or deterioration on the neurological status on further follow-up. Acute complications were not common. One patient underwent re-operation because of slipping of the graft during resuscitation after early post-operative acute myocardial infarction. This patient showed post-operative neurological deterioration. Two patients had subsidence of the graft, three had pseudo-arthrosis, and two had graft resorption. There were also three patients with degenerative changes of the adjacent vertebra. No re-operation was done for patients with these late complications in this review.ConclusionAnterior cervical decompression with bone fusion is a viable surgical option for patients with one level of anterior cervical cord compression, especially for patients with kyphosis or straight canal spine. For patients with two- to three-level involvement, anterior cervical decompression with bone fusion provides good functional result in proper selection of cases. We also identified some prognostic factors (male sex, symptoms less than 1 year, and age less than 70 years) in predicting a favourable outcome of anterior spinal fusion for CCM

    Searches for the ZγZ\gamma decay mode of the Higgs boson and for new high-mass resonances in pppp collisions at s=13\sqrt{s} = 13 TeV with the ATLAS detector

    International audienceThis article presents searches for the Zγ decay of the Higgs boson and for narrow high-mass resonances decaying to Zγ, exploiting Z boson decays to pairs of electrons or muons. The data analysis uses 36.1 fb1^{−1} of pp collisions at s=13 \sqrt{s}=13 recorded by the ATLAS detector at the CERN Large Hadron Collider. The data are found to be consistent with the expected Standard Model background. The observed (expected — assuming Standard Model pp → H → Zγ production and decay) upper limit on the production cross section times the branching ratio for pp → H → Zγ is 6.6. (5.2) times the Standard Model prediction at the 95% confidence level for a Higgs boson mass of 125.09 GeV. In addition, upper limits are set on the production cross section times the branching ratio as a function of the mass of a narrow resonance between 250 GeV and 2.4 TeV, assuming spin-0 resonances produced via gluon-gluon fusion, and spin-2 resonances produced via gluon-gluon or quark-antiquark initial states. For high-mass spin-0 resonances, the observed (expected) limits vary between 88 fb (61 fb) and 2.8 fb (2.7 fb) for the mass range from 250 GeV to 2.4 TeV at the 95% confidence level

    Search for direct top squark pair production in final states with two leptons in s=13\sqrt{s} = 13 TeV pppp collisions with the ATLAS detector

    International audienceThe results of a search for direct pair production of top squarks in events with two opposite-charge leptons (electrons or muons) are reported, using 36.1 fb136.1~\hbox {fb}^{-1} of integrated luminosity from proton–proton collisions at s=13\sqrt{s}=13 TeV collected by the ATLAS detector at the Large Hadron Collider. To cover a range of mass differences between the top squark t~\tilde{t} and lighter supersymmetric particles, four possible decay modes of the top squark are targeted with dedicated selections: the decay t~bχ~1±\tilde{t} \rightarrow b \tilde{\chi }_{1}^{\pm } into a b-quark and the lightest chargino with χ~1±Wχ~10\tilde{\chi }_{1}^{\pm } \rightarrow W \tilde{\chi }_{1}^{0} , the decay t~tχ~10\tilde{t} \rightarrow t \tilde{\chi }_{1}^{0} into an on-shell top quark and the lightest neutralino, the three-body decay t~bWχ~10\tilde{t} \rightarrow b W \tilde{\chi }_{1}^{0} and the four-body decay t~bνχ~10\tilde{t} \rightarrow b \ell \nu \tilde{\chi }_{1}^{0} . No significant excess of events is observed above the Standard Model background for any selection, and limits on top squarks are set as a function of the t~\tilde{t} and χ~10\tilde{\chi }_{1}^{0} masses. The results exclude at 95% confidence level t~\tilde{t} masses up to about 720 GeV, extending the exclusion region of supersymmetric parameter space covered by previous searches
