Association Between Preexisting Versus Newly Identified Atrial Fibrillation and Outcomes of Patients With Acute Pulmonary Embolism
Background Atrial fibrillation (AF) may exist before or occur early in the course of pulmonary embolism (PE). We determined the PE outcomes based on the presence and timing of AF. Methods and Results Using the data from a multicenter PE registry, we identified 3 groups: (1) those with preexisting AF, (2) patients with new AF within 2 days from acute PE (incident AF), and (3) patients without AF. We assessed the 90-day and 1-year risk of mortality and stroke in patients with AF, compared with those without AF (reference group). Among 16 497 patients with PE, 792 had preexisting AF. These patients had increased odds of 90-day all-cause (odds ratio [OR], 2.81; 95% CI, 2.33-3.38) and PE-related mortality (OR, 2.38; 95% CI, 1.37-4.14) and increased 1-year hazard for ischemic stroke (hazard ratio, 5.48; 95% CI, 3.10-9.69) compared with those without AF. After multivariable adjustment, preexisting AF was associated with significantly increased odds of all-cause mortality (OR, 1.91; 95% CI, 1.57-2.32) but not PE-related mortality (OR, 1.50; 95% CI, 0.85-2.66). Among 16 497 patients with PE, 445 developed new incident AF within 2 days of acute PE. Incident AF was associated with increased odds of 90-day all-cause (OR, 2.28; 95% CI, 1.75-2.97) and PE-related (OR, 3.64; 95% CI, 2.01-6.59) mortality but not stroke. Findings were similar in multivariable analyses. Conclusions In patients with acute symptomatic PE, both preexisting AF and incident AF predict adverse clinical outcomes. Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial
Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Elliptic flow of muons from heavy-flavour hadron decays at forward rapidity in Pb-Pb collisions at root s(NN)=2.76TeV
The elliptic flow, v(2), of muons from heavy-flavour hadron decays at forward rapidity (2.5 <y <4) is measured in Pb-Pb collisions at root s(NN)= 2.76TeVwith the ALICE detector at the LHC. The scalar product, two- and four-particle Q cumulants and Lee-Yang zeros methods are used. The dependence of the v(2) of muons from heavy-flavour hadron decays on the collision centrality, in the range 0-40%, and on transverse momentum, p(T), is studied in the interval 3 <p(T)<10 GeV/c. A positive v(2) is observed with the scalar product and two-particle Q cumulants in semi-central collisions (10-20% and 20-40% centrality classes) for the p(T) interval from 3 to about 5GeV/c with a significance larger than 3 sigma, based on the combination of statistical and systematic uncertainties. The v(2) magnitude tends to decrease towards more central collisions and with increasing pT. It becomes compatible with zero in the interval 6 <p(T)<10 GeV/c. The results are compared to models describing the interaction of heavy quarks and open heavy-flavour hadrons with the high-density medium formed in high-energy heavy-ion collisions. Percepção auditiva da fala após o reimplante coclear
RESUMO Objetivo Analisar o desempenho da percepção auditiva da fala (PF) após cirurgia de substituição do implante coclear (IC) e identificar associações com a idade, tempos de uso dos dispositivos, privação e recuperação. Método O estudo retrospectivo analisou os prontuários de 1990 a 2016 e considerou como referência o maior escore da capacidade auditiva identificada ao longo do uso do primeiro IC. Foram coletados dados epidemiológicos; etiologia; causas da substituição e marca dos dispositivos; classificação etária; idades no primeiro e segundo implante; tempos de utilização, privação e de recuperação da capacidade auditiva. Os dados foram avaliados por meio de testes estatísticos não paramétricos (IC=95%; p<0.05). Resultados Foram avaliados 68 participantes (31 adultos e 37 crianças), sendo 52,9% do sexo feminino e as principais etiologias da perda auditiva foram: idiopática (48,5%), infecciosa (33,8%) e outras causas não infecciosas (17,6%). A idade média verificada na implantação do primeiro e do segundo IC, foram: 102±143,4 e 178,9±173,4 meses. Os tempos médios de uso do primeiro IC, privação, recuperação e uso do segundo IC, foram respectivamente: (76,1±63,3); (2,8±2,4); (6,5±7,1); (75,6±48,3) meses. A substituição foi motivada principalmente pela parada abrupta de funcionamento (77,9%) e 85,3% dos participantes recuperaram a PF, que esteve significativamente associada à idade no primeiro IC, e os tempos de utilização dos dispositivos (p<0.05). Conclusão A maior parte dos indivíduos submetidos ao reimplante conseguem recuperar e/ou continuar o desenvolvimento das habilidades auditivas. A idade mais jovem e o tempo de uso dos dispositivos são fatores que influenciam na capacidade de recuperação da PF em reimplantados
Testicular Parameters and its Influence in Buffaloes (Bubalus buballis) Sexual Behaviour
Background: Brazil is one of the greatest bovine meat exporter in the world. However, the technologies applied at buffalo’s reproduction is not specific for this specie, adapted technics were established from the cattle breed. When the animal shows weight gain, gonadal and behavioral physiology fallow this tendency and the reproductive status become good and satisfactory. Thus, the aim of this study was to compare the reproduction characteristics of male buffaloes and its testicular parameters with sexual behavior.Materials, Methods & Results: Seven males Murrah breed with 67.29 ± 11.4 months of age were maintained at confinement for artificial shading. The testicular parameter was performed in restraint trunk and the sexual behavior evaluation was done with an estrus female. The sexual behavior was performed with an estrous female as a dummy. The testicular biometry was measured at the beginning and at the end of the experimental design, with the follow parameters: length, width, depth/thickness, scrotal circumference and the total volume. The data were submitted to the analysis of variance, and the means was compared with the Student-Newman-Keuls test, with significance difference P < 0.05 between the medians. The differences in the parameters were scrotal circumference (23.36 cm vs 24.86 cm), testicular length (8.71 cm vs 9.77 cm), right testicular width (8.57 cm vs 9.53 cm) and testicular volume (1.627.40 cm3 vs 2.149,68 cm3), respectively. The sexual behavior showed an increase in the Flehmen reflex, mounts (service capacity) and libido (2.00 vs 11.71; 0.57 vs 5.43; regular behavior vs very good behavior, respectively).Discussion: An increase in the testicular circumference was observed at the end of the experimental period with the values between 23.36 cm vs 24.86 cm. However, even with this increase of the testicular parameters it is lower than other male buffaloes with 36 months age. In Brazilian conditions, the testicular circumference variation is between 24.5 cm and 34 cm in animals with 60 months of age. These data shows that there are variations of the testicular parameters due to different systems of buffaloes breeding. When compared to cattle, buffaloes shows lower input of reproductive biotechnology. At the same way, the testicular growth and development depends of the nutrition and management used at the farm. In an inefficient system, buffaloes and bovine breeding could lose weight, testicular mass, and reduce testicular parameters. Besides that, the buffaloes can perform an improvement of a general condition when improvement of nutrition care occurs. The testicular parameters presents high correlation with the corporal and testicular development, which can be correlate to age and weight. Those parameters could help with the reproductive ability of the buffalo. The sexual behavior was increased and observed in the Flehmen reflex, service capacity and libido after the animal’s adaptation to the reproductive management. The buffaloes are animals extremely adaptable to different conditions, but need a period to adapt. Previous sexual experience is important during the social organization of the harem and helps in the practice of the sexual act. In these experimental conditions, we conclude that is necessary a social and behavior adaptation to the buffalo bull to present a better value of sexual characteristics to improve the fertility and production of Murrah buffaloes
