    Review of the missed opportunities for the prevention of vertical transmission of HIV in Brazil

    OBJECTIVE: The present literature review aims to highlight gaps in the treatment of preventative motherto-child HIV transmission and the risk factors in Brazil. METHODS: Among the 425 articles identified in SciELO and PubMed searches, 59 articles published between 1994 and 2016 were selected for reading and data extraction, and 33 articles were included in the present review. RESULTS: The rates of vertical HIV transmission described in the studies varied widely, from 1.8% to 27.8%, with a significant reduction over the years. However, recent rates were also found to be variable in different regions of Brazil, and despite the significant reduction in mother-to-child transmission, many gaps remain in prevention services. A failure to attend prenatal care is the main factor associated with the increased risk of vertical transmission of HIV, hindering early maternal diagnosis and the completion of preventative measures during the prenatal period and, often, the peripartum and postnatal periods. A small number of studies discussed the sociodemographic factors, including a low level of education for pregnant women and the inadequacies of health services, such as difficulties scheduling appointments and undertrained staff, associated with vertical transmission. As such, the current challenge is to better define the sociodemographic and infrastructural factors that increase the risk of mother-to-child transmission of HIV to provide the necessary investments to promote an earlier inclusion of these populations in prevention services. CONCLUSIONS: This review may serve as a guide for future programs to focus efforts on the prevention of vertical HIV transmission

    Use of Creatine Monohydrate in MDX Mice: Morphometric and Stereological Analysis of the Diaphragm

    Duchenne muscular dystrophy is a genetic disease that is clinically manifested by progressive muscle atrophy, followed by loss of strength, motor coordination and functional impairment. In the final stages of the disease, the patient has severe difficulty in breathing mechanics, due to the involvement of the muscles involved with the mechanics of breathing, including the diaphragm. The present study sought to identify the effects of creatine monohydrate supplementation in MDX mice on the morphology, morphometry and stereology of the striated muscle tissue of the diaphragm of these animals. The results indicate that, despite not influencing the increase in cell volume, supplementation acts in an anti-inflammatory way, reducing the progressive process of fibrosis that occurs in these animals in the face of muscle atrophy followed by the replacement of muscle parenchyma by connective tissue. in this way, supplementation provides a better condition for tissue maintenance, enabling more survival of MDX mice. Monohydrate creatine supplementation has been shown to be a complementary therapeutic alternative, especially for muscular dystrophies and severe myopathies

    Manejo clínico e cirúrgico da Úlcera péptica: Clinical and surgical management of peptic Ulcer

    A úlcera péptica é definida pela ruptura da barreira defensiva da mucosa epitelial do estômago e duodeno, causada pelo desequilíbrio entre fatores de proteção e de destruição desta. Normalmente, são resultadas de infecções por Helicobacter pylori ou da ação de anti-inflamatórios não esteróides. Essa afecção manifesta-se, tipicamente, por epigastralgia, plenitude pós-prandial, distensão abdominal, náuseas, vômitos, melena e hematêmese. A úlcera péptica é diagnosticada através de seus sinais, sintomas e com a esofagogastroduodenoscopia. Uma vez diagnosticada, a úlcera pode ser tratada por diversas estratégias, a depender de sua etiologia e evolução, podendo variar de mudanças no hábito de vida, como alimentação e cessação do tabagismo e etilismo, a cirurgia, como a vagotomia e gastrectomia. No geral, o tratamento medicamentoso é suficiente para a cicatrização, sendo os inibidores da bomba de prótons os medicamentos de primeira linha. Porém, nos casos em que há infecção por H. pylori, o uso de antibióticos, como claritromicina e amoxicilina, também são indicados. Raramente a cirurgia é necessária, exceto em casos de complicações, como perfuração, suspeita de malignidade e obstrução gastrointestinal, ficando o manejo cirúrgico reservado a esses casos mais complexos

    Cada vez mais as substâncias sintéticas têm ganhado espaço como antimicrobiano e consequentemente tem sido lançado na indústria farmacêutica, o que inclui a ceftriaxona. A alta utilização desses medicamentos tem constituído um problema de saúde pública global, visto que, também são considerados substâncias químicas que mais causam eventos adversos. O principal objetivo do presente estudo é abordar através da presente revisão sistematizada os principais eventos adversos decorrentes do uso de ceftriaxona na prática clínica. Trata-se de uma revisão sistemática, dos quais, foram realizadas buscas na PubMed, Periódico Capes e na Biblioteca Virtual em Saúde (BVS), com os principais Descritores e palavras-chave: “Ceftriaxone” AND “Drug-Related Side Effects and Adverse” AND Reactions, contanto com uma totalidade de 8 estudos para análise. Logo, os estudos abordam que os eventos adversos relacionados a utilização da ceftriaxona em sua maioria das vezes, foram ocasionados pelo uso não apropriado do medicamento pelos profissionais de saúde, além de condições médicas não apropriadas, tendo como menores abordagens a baixa qualidade do medicamento. Assim, é fundamental o compreendimento que mesmo a ceftriaxona apresentando eventos adversos, desde leves até mesmo raros, os riscos podem ser de alguma forma evitados, sobretudo, no que diz respeito a um protocolo e administração do medicamento adequadamente

    As the knowledge base and importance of mitochondrial physiology to evolution, health and disease expands, the necessity for harmonizing the terminology concerning mitochondrial respiratory states and rates has become increasingly apparent. The chemiosmotic theory establishes the mechanism of energy transformation and coupling in oxidative phosphorylation. The unifying concept of the protonmotive force provides the framework for developing a consistent theoretical foundation of mitochondrial physiology and bioenergetics. We follow the latest SI guidelines and those of the International Union of Pure and Applied Chemistry (IUPAC) on terminology in physical chemistry, extended by considerations of open systems and thermodynamics of irreversible processes. The concept-driven constructive terminology incorporates the meaning of each quantity and aligns concepts and symbols with the nomenclature of classical bioenergetics. We endeavour to provide a balanced view of mitochondrial respiratory control and a critical discussion on reporting data of mitochondrial respiration in terms of metabolic flows and fluxes. Uniform standards for evaluation of respiratory states and rates will ultimately contribute to reproducibility between laboratories and thus support the development of data repositories of mitochondrial respiratory function in species, tissues, and cells. Clarity of concept and consistency of nomenclature facilitate effective transdisciplinary communication, education, and ultimately further discovery

    Risk portfolio on modern finance has become increasingly technical, requiring the use of sophisticated mathematical tools in both research and practice. Since companies cannot insure themselves completely against risk, as human incompetence in predicting the future precisely that written in Al-Quran surah Luqman verse 34, they have to manage it to yield an optimal portfolio. The objective here is to minimize the variance among all portfolios, or alternatively, to maximize expected return among all portfolios that has at least a certain expected return. Furthermore, this study focuses on optimizing risk portfolio so called Markowitz MVO (Mean-Variance Optimization). Some theoretical frameworks for analysis are arithmetic mean, geometric mean, variance, covariance, linear programming, and quadratic programming. Moreover, finding a minimum variance portfolio produces a convex quadratic programming, that is minimizing the objective function ðð¥with constraintsð ð 𥠥 ðandð´ð¥ = ð. The outcome of this research is the solution of optimal risk portofolio in some investments that could be finished smoothly using MATLAB R2007b software together with its graphic analysis

    Search for heavy resonances decaying to two Higgs bosons in final states containing four b quarks

    A search is presented for narrow heavy resonances X decaying into pairs of Higgs bosons (H) in proton-proton collisions collected by the CMS experiment at the LHC at root s = 8 TeV. The data correspond to an integrated luminosity of 19.7 fb(-1). The search considers HH resonances with masses between 1 and 3 TeV, having final states of two b quark pairs. Each Higgs boson is produced with large momentum, and the hadronization products of the pair of b quarks can usually be reconstructed as single large jets. The background from multijet and t (t) over bar events is significantly reduced by applying requirements related to the flavor of the jet, its mass, and its substructure. The signal would be identified as a peak on top of the dijet invariant mass spectrum of the remaining background events. No evidence is observed for such a signal. Upper limits obtained at 95 confidence level for the product of the production cross section and branching fraction sigma(gg -> X) B(X -> HH -> b (b) over barb (b) over bar) range from 10 to 1.5 fb for the mass of X from 1.15 to 2.0 TeV, significantly extending previous searches. For a warped extra dimension theory with amass scale Lambda(R) = 1 TeV, the data exclude radion scalar masses between 1.15 and 1.55 TeV
