Cowcod (Sebastes levis) is a large (100-cm-FL), long-lived (maximum observed age 55 yr) demersal rockfish taken in multispecies commercial and recreational fisheries off southern and central California. It lives at 20–500 m depth: adults (>44 cm TL) inhabit rocky areas at 90–300 m and juveniles inhabit fine sand and clay at 40–100 m. Both sexes have similar growth and maturity. Both sexes recruit to the fishery before reaching full maturity. Based on age and growth data, the natural mortality rate is about M =0.055/yr, but the estimate is uncertain. Biomass, recruitment, and mortality during 1951–98 were estimated in a delay-difference model with catch data and abundance indices. The same model gave less precise estimates for 1916–50 based on catch data and assumptions about virgin biomass and recruitment such as used in stock reduction analysis. Abundance indices, based on rare event data, included a habitat-area–weighted index of recreational catch per unit of fishing effort (CPUE index values were 0.003–0.07 fish per angler hour), a standardized index of proportion of positive tows in CalCOFI ichthyoplankton survey data (binomial errors, 0–13% positive tows/yr), and proportion of positive tows for juveniles in bottom trawl surveys (binomial errors, 0–30% positive tows/yr). Cowcod are overfished in the southern California Bight; biomass during the 1998 season was about 7% of the virgin level and recent catches have been near 20 metric tons (t)/yr. Projections based on recent recruitment levels indicate that biomass will decline at catch levels > 5 t/yr. Trend data indicate that recruitment will be poor in the near future. Recreational fishing effort in deep water has increased and has become more effective for catching cowcod. Areas with relatively high catch rates for cowcod are fewer and are farther offshore. Cowcod die after capture and cannot be released alive. Two areas recently closed to bottom fishing will help rebuild the cowcod stock

ABSTRACT The vertical distribution of eggs of Pacific hake (Muluccius pvoductus) was determined from 27 MOCNESS samples taken on cruises off southern and central California in March 1995 and February 1996. In 1995, and 250-300 m) were sampled. In 1996, nine 25-ni depth strata were sampled down to 225 m. In 1995 Pacific hake eggs were taken in all strata down to 250-300 ni; most of the eggs were found between 50 and 150 m, with the highest densities in the 50-75-and 75-100-ni strata. Average temperatures for developing Pacific hake eggs were estimated to be 10.2"-11.3"C. Early-stage eggs were generally deeper in the water column than later-stage eggs. Most early-stage eggs were found between 75 and 150 m, with highest densities in the 125-150-111 stratum. In contrast, mid-and late-stage eggs were most dense in the 50-75-and 75-100-m strata. Early-stage eggs were taken primarily in tows between 2200 and 0600 hrs, suggesting diel periodicity in spawning. O n the 1996 cruise hake eggs had a slightly shallower distribution compared with 1995, and there was more overlap in vertical distribution between stages. Methods are given for separating Pacific hake eggs from similar eggs of other species, and criteria are described for staging Pacific hake eggs

OBJECTIVES A retrospective database was developed by the European Society of Thoracic Surgeons, collecting patients submitted to surgery for thymic tumours to analyse clinico-pathological prognostic predictors. METHODS A total of 2151 incident cases from 35 institutions were collected from 1990 to 2010. Clinical-pathological characteristics were analysed, including age, gender, associated myasthenia gravis stage (Masaoka), World Health Organization histology, type of thymic tumour [thymoma, thymic carcinoma (TC), neuroendocrine thymic tumour (NETT)], type of resection (complete/incomplete), tumour size, adjuvant therapy and recurrence. Primary outcome was overall survival (OS); secondary outcomes were the proportion of incomplete resections, disease-free survival and the cumulative incidence of recurrence (CIR). RESULTS A total of 2030 patients were analysed for OS (1798 thymomas, 191 TCs and 41 NETTs). Ten-year OS was 0.73 (95% confidence interval 0.69-0.75). Complete resection (R0) was achieved in 88% of the patients. Ten-year CIR was 0.12 (0.10-0.15). Predictors of shorter OS were increased age (P < 0-001), stage [III vs I HR 2.66, 1.80-3.92; IV vs I hazard ratio (HR) 4.41, 2.67-7.26], TC (HR 2.39, 1.68-3.40) and NETT (HR 2.59, 1.35-4.99) vs thymomas and incomplete resection (HR 1.74, 1.18-2.57). Risk of recurrence increased with tumour size (P = 0.003), stage (III vs I HR 5.67, 2.80-11.45; IV vs I HR 13.08, 5.70-30.03) and NETT (HR 7.18, 3.48-14.82). Analysis using a propensity score indicates that the administration of adjuvant therapy was beneficial in increasing OS (HR 0.69, 0.49-0.97) in R0 resections. CONCLUSIONS Masaoka stages III-IV, incomplete resection and non-thymoma histology showed a significant impact in increasing recurrence and in worsening survival. The administration of adjuvant therapy after complete resection is associated with improved surviva

Severe early onset preeclampsia: short and long term clinical, psychosocial and biochemical aspects

Preeclampsia is a pregnancy specific disorder commonly defined as de novo hypertension and proteinuria after 20 weeks gestational age. It occurs in approximately 3-5% of pregnancies and it is still a major cause of both foetal and maternal morbidity and mortality worldwide1. As extensive research has not yet elucidated the aetiology of preeclampsia, there are no rational preventive or therapeutic interventions available. The only rational treatment is delivery, which benefits the mother but is not in the interest of the foetus, if remote from term. Early onset preeclampsia (<32 weeks’ gestational age) occurs in less than 1% of pregnancies. It is, however often associated with maternal morbidity as the risk of progression to severe maternal disease is inversely related with gestational age at onset2. Resulting prematurity is therefore the main cause of neonatal mortality and morbidity in patients with severe preeclampsia3. Although the discussion is ongoing, perinatal survival is suggested to be increased in patients with preterm preeclampsia by expectant, non-interventional management. This temporising treatment option to lengthen pregnancy includes the use of antihypertensive medication to control hypertension, magnesium sulphate to prevent eclampsia and corticosteroids to enhance foetal lung maturity4. With optimal maternal haemodynamic status and reassuring foetal condition this results on average in an extension of 2 weeks. Prolongation of these pregnancies is a great challenge for clinicians to balance between potential maternal risks on one the eve hand and possible foetal benefits on the other. Clinical controversies regarding prolongation of preterm preeclamptic pregnancies still exist – also taking into account that preeclampsia is the leading cause of maternal mortality in the Netherlands5 - a debate which is even more pronounced in very preterm pregnancies with questionable foetal viability6-9. Do maternal risks of prolongation of these very early pregnancies outweigh the chances of neonatal survival? Counselling of women with very early onset preeclampsia not only comprises of knowledge of the outcome of those particular pregnancies, but also knowledge of outcomes of future pregnancies of these women is of major clinical importance. This thesis opens with a review of the literature on identifiable risk factors of preeclampsia
