    Regenerative medicine is an interdisciplinary field that combines different principles of engineering and life sciences to potentially promote the regeneration and healing of injured tissues. The use of biomaterials to replace damaged tissues or to contribute to their healing is an important component of current regenerative medicine strategies. Biomaterials are defined as any material intended to interface with biological systems to treat or replace any tissue, organ, or function in the body.     Emerging Biofabrication Techniques: A Review on Natural Polymers for Biomedical Applications

    Natural polymers have been widely used for biomedical applications in recent decades. They offer the advantages of resembling the extracellular matrix of native tissues and retaining biochemical cues and properties necessary to enhance their biocompatibility, so they usually improve the cellular attachment and behavior and avoid immunological reactions. Moreover, they offer a rapid degradability through natural enzymatic or chemical processes. However, natural polymers present poor mechanical strength, which frequently makes the manipulation processes difficult. Recent advances in biofabrication, 3D printing, microfluidics, and cell-electrospinning allow the manufacturing of complex natural polymer matrixes with biophysical and structural properties similar to those of the extracellular matrix. In addition, these techniques offer the possibility of incorporating different cell lines into the fabrication process, a revolutionary strategy broadly explored in recent years to produce cell-laden scaffolds that can better mimic the properties of functional tissues. In this review, the use of 3D printing, microfluidics, and electrospinning approaches has been extensively investigated for the biofabrication of naturally derived polymer scaffolds with encapsulated cells intended for biomedical applications (e.g., cell therapies, bone and dental grafts, cardiovascular or musculoskeletal tissue regeneration, and wound healing)

    Premio Congreso SIBB 2018El campo de la impresión y la bioimpresión 3D ha alcanzado un gran desarrollo en los últimos años. La llegada de estas técnicas de biofabricación a la ingeniería tisular han supuesto una revolución debido a las estructuras biomiméticas tan complejas que permiten sintetizar. El uso del quitosano como componente de tintas basadas en polímeros naturales es muy atractivo debido a las propiedades beneficiosas que presenta (alta biocompatibilidad, biodegradabilidad, bajo coste). Sin embargo, su aplicación se ve limitada por su baja solubilidad en condiciones fisiológicas y sus pobres propiedades mecánicas. En este artículo, revisamos el estado del arte relacionado con las estrategias actuales de impresión 3D que hacen uso de tintas y biotintas basadas en quitosano para el desarrollo de soportes biomiméticos. También analizamos las estrategias de entrecruzamiento que se aplican actualmente para mejorar su printabilidad, resaltando además los derivados de quitosano disponibles que permiten encapsular células para bioimpresión 3D, incluyendo además nuestra contribución al campo. Prevemos que el uso de quitosano en la impresión 3D aumentará significativamente en los próximos años gracias a los esfuerzos que se están desarrollando en términos de mecanismos de gelificación y derivados de quitosano que permiten la encapsulación celular.Premio Congreso SIBB 2018Award-winningPeer Reviewe

    Evaluation of Glycerylphytate Crosslinked Semi- and Interpenetrated Polymer Membranes of Hyaluronic Acid and Chitosan for Tissue Engineering

    In the present study, semi- and interpenetrated polymer network (IPN) systems based on hyaluronic acid (HA) and chitosan using ionic crosslinking of chitosan with a bioactive crosslinker, glycerylphytate (G1Phy), and UV irradiation of methacrylate were developed, characterized and evaluated as potential supports for tissue engineering. Semi- and IPN systems showed significant differences between them regarding composition, morphology, and mechanical properties after physicochemical characterization. Dual crosslinking process of IPN systems enhanced HA retention and mechanical properties, providing also flatter and denser surfaces in comparison to semi-IPN membranes. The biological performance was evaluated on primary human mesenchymal stem cells (hMSCs) and the systems revealed no cytotoxic effect. The excellent biocompatibility of the systems was demonstrated by large spreading areas of hMSCs on hydrogel membrane surfaces. Cell proliferation increased over time for all the systems, being significantly enhanced in the semi-IPN, which suggested that these polymeric membranes could be proposed as an effective promoter system of tissue repair.     A Full Vacuum Approach for the Fabrication of Hybrid White-Light-Emitting Thin Films and Wide-Range In Situ Tunable Luminescent Microcavities

    This study shows the fabrication by a dry approach at mild temperature (<150 °C) of a photoluminescence white light emitting hybrid layer. The white light emitter is obtained by evaporation of two photoluminescent small molecules, a blue (1,3,5-triphenyl-2-pyrazoline (TPP)) and an orange (Rubrene) dye within the porous of an SiO host film fabricated by glancing angle deposition. Fluorescence (Föster) resonant energy transfer between the two organic dyes allows the emission of the combined system upon excitation of the TPP molecule at wavelength of 365 nm. The distribution of the organic molecule within the host layer is analyzed as a function of the substrate temperature and vacuum conditions and the required conditions for the white emission determined by finely controlling the TPP:Rubrene ratio. The full vacuum processing of the hybrid layers provides a straightforward route for the incorporation of the white light emitters as optical defect within 1D Bragg microcavities.     Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10&nbsp;years; 78.2% included were male with a median age of 37&nbsp;years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.
