    Perspectivas de género en la educación superior: políticas y lineamientos a partir de un enfoque académico

    One of the most important challenges that face the institutions of top education is of eradicating the anequality of kind that still it demonstrates and reproduces to his interior. From his origins, the univer sities and institutes have been historically unfavorable spaces for the women, who for centuries did not even have right to accede to this educational level. By means of requirements and constant fights, from the 19th century, the women managed to enter to the top education, increasing of progressive form his presence. Nevertheless, many desigualdades still persist; they are notable in the area of the academic profession (appointments and promotions), in the evaluations and in the grant of recognitions, in the access and the permanency for areas and disciplines, as well as in some Latin - American countries, included Ecuador. It is not also possible to forget that lamentably there exist situations of harassment, harassment and violence of kind in the institutions of top education. This work has as intention think brings over of the way in which the social dimension of the kind is woven, in the area of the top education, as element that takes part in a fundamental way in the relations of power.Uno de los retos más importantes que enfrentan las instituciones de educación superior es el de erradicar la desigualdad de género que todavía se manifiesta y reproduce a su interior. Desde sus orígenes, las universidades e institutos han sido espacios históricamente desfavorables para las mujeres, quienes durante siglos ni siquiera tuvieron derecho a acceder a este nivel educativo. Por medio de exigencias y luchas continuas, a partir del siglo XIX, las mujeres lograron ingresar a la educación superior, incrementando de forma progresiva su presencia. Sin embargo, muchas desigualdades aún persisten; son notables en el ámbito de la profesión académica (nombramientos y promociones), en las evaluaciones y en el  otorgamiento de reconocimientos, en el acceso y la permanencia por áreas y disciplinas, así como en algunos países latinoamericanos, Ecuador incluido. Tampoco es posible olvidar que lamentablemente existen situaciones de acoso, hostigamiento y violencia de género en las instituciones de educación superior.     El incremento porcentual de los vehículos de propulsión eléctrica en la matriz automotora a nivel mundial, y su necesaria introducción en Cuba impone la necesidad de disponer de toda la información técnica detallada para el soporte y mantenimiento de los sistemas eléctricosde los mismos, debido a que muchos de esos servicios dejan de ser aplicados por la falta de documentación. Por ese motivo, en el presente trabajo se define como objetivo principal la realización de una descripción técnica completa del funcionamiento de los sistemas eléctricos del vehículo Minghong, modelo Aries C2, así como los principales diagramas esquemáticos. Los aspectos detallados abarcan las etapas de potencia, sus correspondientes sistemas de control, y las interconexiones entre estos. De las mediciones realizadas, se presentaron varias formas de onda obtenidas en el cargador y el controlador del motor. Con estos resultados, se dispuso de un conocimiento detallado del funcionamiento de este medio de transporte, para futuras reparaciones, no solo en el modelo mencionado, sino en otros que circulan en el país basados en una tecnología similar

    Rotura iatrogénica de electrodos de marcapasos permanente en la realización de esternotomía media: presentación de un caso

    La esternotomía media continúa siendo el gold-standart de abordaje quirúrgico en cirugía cardiaca. Aún realizada correctamente, laapertura media del esternón presenta complicaciones en el 1-5% de los casos que varían en su gravedad desde la infección de la heridaquirúrgica a la lesión de estructuras intratorácicas. En el presente estudio se describe un caso de rotura de electrodos de marcapasostras esternotomía media por retracción y fibrosis del tronco venoso braquiocefálico. Debido a la ausencia de artículos semejantes enla literatura y al extendido empleo de la esternotomía en pacientes portadores de marcapasos permanente, se pretende disminuirlas lesiones asociadas mediante la publicación de un infrecuente caso de lesión iatrogénica durante la apertura esternal, con escasosprecedentes en la literatura publicada hasta la fecha.     Waiting list management of cardiac surgical procedures is a main concern for all the Spanish autonomic health systems and for our scientific Society. The first statement for optimal timing of patients waiting for cardiac surgery was published in 2000. Since then, after significant changes in the management of some pathologies, new normative frameworks and the current healthcare situation, a review of the timing criteria to offer an adequate and updated standard of care is needed. In this document we aim to review the available literature in the field and stablish a consensus within a working group of the Spanish Society of Cardiovascular and Endovascular Surgery to optimize the priority recommendations in cardiac surgical waiting lists in our country.     Background: Mortality due to COVID-19 is high, especially in patients requiring mechanical ventilation. The purpose of the study is to investigate associations between mortality and variables measured during the first three days of mechanical ventilation in patients with COVID-19 intubated at ICU admission. Methods: Multicenter, observational, cohort study includes consecutive patients with COVID-19 admitted to 44 Spanish ICUs between February 25 and July 31, 2020, who required intubation at ICU admission and mechanical ventilation for more than three days. We collected demographic and clinical data prior to admission; information about clinical evolution at days 1 and 3 of mechanical ventilation; and outcomes. Results: Of the 2,095 patients with COVID-19 admitted to the ICU, 1,118 (53.3%) were intubated at day 1 and remained under mechanical ventilation at day three. From days 1 to 3, PaO2/FiO2 increased from 115.6 [80.0-171.2] to 180.0 [135.4-227.9] mmHg and the ventilatory ratio from 1.73 [1.33-2.25] to 1.96 [1.61-2.40]. In-hospital mortality was 38.7%. A higher increase between ICU admission and day 3 in the ventilatory ratio (OR 1.04 [CI 1.01-1.07], p = 0.030) and creatinine levels (OR 1.05 [CI 1.01-1.09], p = 0.005) and a lower increase in platelet counts (OR 0.96 [CI 0.93-1.00], p = 0.037) were independently associated with a higher risk of death. No association between mortality and the PaO2/FiO2 variation was observed (OR 0.99 [CI 0.95 to 1.02], p = 0.47). Conclusions: Higher ventilatory ratio and its increase at day 3 is associated with mortality in patients with COVID-19 receiving mechanical ventilation at ICU admission. No association was found in the PaO2/FiO2 variation

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world

    Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic. Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality. Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States. Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis. Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection
