    La hipercolesterolemia es un factor de riesgo importante en el desarrollo de enfermedades cardiovasculares. Las estatinas siguen siendo el tratamiento de elección para la dislipidemia y la disminución del riesgo cardiovascular. Constituyen un grupo terapéutico beneficioso, seguro y económico, razones por las que su consumo se ha cuadruplicado en los últimos diez años. Diversos estudios han demostrado que su mecanismo de acción da lugar a efectos adversos a nivel gastrointestinal, sistema nervioso periférico, neuromuscular y sobre el sistema endocrino entre otros. Estos efectos, pese a ser poco frecuentes, se debe tener en cuenta que su incidencia se incrementa con su consumo. Por ello, el objetivo del presente trabajo es aportar una clara visión de la situación clínica, explorar los mecanismos de acción posibles para los efectos adversos y determinar su prevalencia

    Degradation Monitoring of Photovoltaic Plants: Advanced GIS Applications

    In order to evaluate a photovoltaic (PV) plant performance, payback time, profitability and environmental impact, an analysis must be made of plant maintenance needs, module and wiring degradation, mismatches and dust effects and PV cell defects and faults. Although a wide range of studies can be found that show the theory and laboratory testing of how these circumstances may affect PV production, very few studies in the field have covered or quantified real degradation effects and faults using a systematic procedure. The authors have therefore reviewed the conditions of PV plants operating in Southern Europe, examining the most frequently found faults and types of degradation, and they look at how novel technologies, such as geographic information system (GIS) applications, can help maintainers, owners, and promoters to supervise and locate damaged PV modules and monitor their evolution and impact on plant working conditions. GIS applications in this field allow the organization of a geo-referenced database of the system, locating and supervising the thirds of each PV cell in the power plant. With this information, investors and maintainers can exert increased control on the PV plant performance and conduct better preventive maintenance measures. The examples given demonstrate that these sorts of applications can be applied both to large PV plants and to domestic installations

    68[ES] Thomas Edison fue catapultado a la fama internacional a través de su invención del fonógrafo en 1877, una máquina capaz de grabar y reproducir todo lo que “oía”. Sin embargo Edison no fue la primera persona en grabar el sonido. Ese honor le corresponde a Édouard-Léon Scott de Martinville, un inventor francés que en 1857 ideó su fonoautógrafo, una máquina que inscribía las vibraciones de los sonidos en el aire en un medio permanente. El fonoautógrafo de Scott fue un instrumento extraordinario. Desde el principio de los tiempos, el sonido había sido invisible y fugaz. El fonoautógrafo lo hizo visible y permanente al escribirlo en papel. De esta forma, las ondas de sonido pudieron estudiarse como nunca antes se había hecho. La grabación de sonido fue un logro excepcional en 1857. El fonógrafo de Edison también fue extraordinario. No solo grabó sonidos, sino que también permitió su reproducción a voluntad (fue precisamente este invento el que le proporcionó a Edison un lugar destacado como inventor)

    Neoadjuvant Intratumoral Immunotherapy with Cowpea Mosaic Virus Induces Local and Systemic Antitumor Efficacy in Canine Mammary Cancer Patients

    The lack of optimal models to evaluate novel agents is delaying the development of effective immunotherapies against human breast cancer (BC). In this prospective open label study, we applied neoadjuvant intratumoral immunotherapy with empty cowpea mosaic virus-like particles (eCPMV) to 11 companion dogs diagnosed with canine mammary cancer (CMC), a spontaneous tumor resembling human BC. We found that two neoadjuvant intratumoral eCPMV injections resulted in tumor reduction in injected tumors in all patients and in noninjected tumors located in the ipsilateral and contralateral mammary chains of injected dogs. Tumor reduction was independent of clinical stage, tumor size, histopathologic grade, and tumor molecular subtype. RNA-seq-based analysis of injected tumors indicated a decrease in DNA replication activity and an increase in activated dendritic cell infiltration in the tumor microenvironment. Immunohistochemistry analysis demonstrated significant intratumoral increases in neutrophils, T and B lymphocytes, and plasma cells. eCPMV intratumoral immunotherapy demonstrated antitumor efficacy without any adverse effects. This novel immunotherapy has the potential for improving outcomes for human BC patients

    Dynamic profiles and predictive values of some biochemical and haematological quantities in COVID-19 inpatients

    Introduction: Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection in some hospitalized patients has shown some important alterations in laboratory tests. The aim of this study was to establish the most relevant quantities associated with the worst prognosis related to COVID-19. Materials and methods: This was a descriptive, longitudinal, observational and retrospective study, in a cohort of 845 adult inpatients from Bellvitge University Hospital (L'Hospitalet de Llobregat, Barcelona, Spain). A multivariate regression analysis was carried out in demographic, clinical and laboratory data, comparing survivors (SURV) and non-survivors (no-SURV). A receiver operating characteristic analysis was also carried out to establish the cut-off point for poor prognostic with better specificity and sensibility. Dynamic changes in clinical laboratory measurements were tracked from day 1 to day 28 after the onset of symptoms. Results: During their hospital stay, 18% of the patients died. Age, kidney disease, creatinine (CREA), lactate-dehydrogenase (LD), C-reactive-protein (CRP) and lymphocyte (LYM) concentration showed the strongest independent associations with the risk of death in the multivariate regression analysis. Established cut-off values for poor prognosis for CREA, LD, CRP and LYM concentrations were 75.0 mu mol/L, 320 U/L, 80.9 mg/L and 0.69 x10(9)/L. Dynamic profile of laboratory findings, were in agreement with the consequences of organ damage and tissue destruction. Conclusions: Age, kidney disease, CREA, LD, CRP and LYM concentrations in COVID-19 patients from the southern region of Catalonia provide important information for their prognosis. Measurement of LD has demonstrated to be very good indicator of poor prognosis at initial evaluation because of its stability over time

    COVID-19 Severity and Survival over Time in Patients with Hematologic Malignancies: A Population-Based Registry Study

    Mortality rates for COVID-19 have declined over time in the general population, but data in patients with hematologic malignancies are contradictory. We identified independent prognostic factors for COVID-19 severity and survival in unvaccinated patients with hematologic malignancies, compared mortality rates over time and versus non-cancer inpatients, and investigated post COVID-19 condition. Data were analyzed from 1166 consecutive, eligible patients with hematologic malignancies from the population-based HEMATO-MADRID registry, Spain, with COVID-19 prior to vaccination roll-out, stratified into early (February–June 2020; n = 769 (66%)) and later (July 2020–February 2021; n = 397 (34%)) cohorts. Propensity-score matched non-cancer patients were identified from the SEMI-COVID registry. A lower proportion of patients were hospitalized in the later waves (54.2%) compared to the earlier (88.6%), OR 0.15, 95%CI 0.11–0.20. The proportion of hospitalized patients admitted to the ICU was higher in the later cohort (103/215, 47.9%) compared with the early cohort (170/681, 25.0%, 2.77; 2.01–3.82). The reduced 30-day mortality between early and later cohorts of non-cancer inpatients (29.6% vs. 12.6%, OR 0.34; 0.22–0.53) was not paralleled in inpatients with hematologic malignancies (32.3% vs. 34.8%, OR 1.12; 0.81–1.5). Among evaluable patients, 27.3% had post COVID-19 condition.     Pancreatic metastases from renal cell carcinoma. Postoperative outcome after surgical treatment in a Spanish multicenter study (PANMEKID)

    Background: Renal Cell Carcinoma (RCC) occasionally spreads to the pancreas. The purpose of our study is to evaluate the short and long-term results of a multicenter series in order to determine the effect of surgical treatment on the prognosis of these patients. Methods: Multicenter retrospective study of patients undergoing surgery for RCC pancreatic metastases, from January 2010 to May 2020. Variables related to the primary tumor, demographics, clinical characteristics of metastasis, location in the pancreas, type of pancreatic resection performed and data on short and long-term evolution after pancreatic resection were collected. Results: The study included 116 patients. The mean time between nephrectomy and pancreatic metastases' resection was 87.35 months (ICR: 1.51-332.55). Distal pancreatectomy was the most performed technique employed (50 %). Postoperative morbidity was observed in 60.9 % of cases (Clavien-Dindo greater than IIIa in 14 %). The median follow-up time was 43 months (13-78). Overall survival (OS) rates at 1, 3, and 5 years were 96 %, 88 %, and 83 %, respectively. The disease-free survival (DFS) rate at 1, 3, and 5 years was 73 %, 49 %, and 35 %, respectively. Significant prognostic factors of relapse were a disease free interval of less than 10 years (2.05 [1.13-3.72], p 0.02) and a history of previous extrapancreatic metastasis (2.44 [1.22-4.86], p 0.01). Conclusions: Pancreatic resection if metastatic RCC is found in the pancreas is warranted to achieve higher overall survival and disease-free survival, even if extrapancreatic metastases were previously removed. The existence of intrapancreatic multifocal compromise does not always warrant the performance of a total pancreatectomy in order to improve survival. (C) 2021 The Authors. Published by Elsevier Ltd

    Risk portfolio on modern finance has become increasingly technical, requiring the use of sophisticated mathematical tools in both research and practice. Since companies cannot insure themselves completely against risk, as human incompetence in predicting the future precisely that written in Al-Quran surah Luqman verse 34, they have to manage it to yield an optimal portfolio. The objective here is to minimize the variance among all portfolios, or alternatively, to maximize expected return among all portfolios that has at least a certain expected return. Furthermore, this study focuses on optimizing risk portfolio so called Markowitz MVO (Mean-Variance Optimization). Some theoretical frameworks for analysis are arithmetic mean, geometric mean, variance, covariance, linear programming, and quadratic programming. Moreover, finding a minimum variance portfolio produces a convex quadratic programming, that is minimizing the objective function ðð¥with constraintsð ð 𥠥 ðandð´ð¥ = ð. The outcome of this research is the solution of optimal risk portofolio in some investments that could be finished smoothly using MATLAB R2007b software together with its graphic analysis
