Since 2000 there was a downward trend in defense spending throughout the world, but in Southeast Asia, overall defense spending experienced an increasing trend. The purpose of this study is to look at the level of militarism of a country with a natural balance of power approach, in which the indicators of the economy included in the calculation. The method used is descriptive method by analyzing the index level of militarization by GMI (Global Military Index) with the main indicator is the ratio of the number of soldiers, the ratio of the military budget, population and national income (GDP). The research shows that the absolute level of militarization of the ASEAN countries are high (except for Indonesia which is very low) but it has a tendency to decrease. This should be awared, because it can be a potential threat due to an unbalanced force between countries. With this awareness, ASEAN countries can cooperate with each other and remind each other as friends, as well as defense programs can be designed to maintain the growth of the national economy and peace in the region

ANALISIS STRATEGI LOCKDOWN ATAU PEMBATASAN SOSIAL DALAM MENGHAMBAT PENYEBARAN COVID-19

At the end of 2019, a shocking event occurred for the entire world population, namely the spread of the new corona pneumonia virus (COVID-19) very quickly throughout the world. In some countries affected by COVID-19, such as China, Britain, Italy, Spain, France, Malaysia, and the Philippines have implemented a comprehensive lockdown status. This study wants to analyze the lockdown strategy or not, based on a change management strategy by the leader, using economic theory thinking developed, so that the analysis carried out by policymakers is more fundamental. This research is still very early because the new outbreak occurred at the end of 2019, so the methodology used is a qualitative method. The debate over the policy dichotomy between lockdown (strong restrictions on movement and human activity) or not lockdown (weak restrictions) is very misleading if not carefully planned. This lockdown policy is only an "intermediate" policy in preventing the COVID-19 virus from spreading further. The best strategy for Indonesia is to impose gradual, indirectly substantial restrictions

PURPOSE: As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19-free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS: This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19-free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS: Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19-free surgical pathways. Patients who underwent surgery within COVID-19-free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19-free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score-matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19-free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION: Within available resources, dedicated COVID-19-free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks

Intravenous NPA for the treatment of infarcting myocardium early: InTIME-II, a double-blind comparison on of single-bolus lanoteplase vs accelerated alteplase for the treatment of patients with acute myocardial infarction

    No full text
Aims to compare the efficacy and safety of lanoteplase, a single-bolus thrombolytic drug derived from alteplase tissue plasminogen activator, with the established accelerated alteplase regimen in patients presenting within 6 h of onset of ST elevation acute myocardial infarction. Methods and Results 15 078 patients were recruited from 855 hospitals worldwide and randomized in a 2:1 ratio to receive either lanoteplase 120 KU. kg-1 as a single intravenous bolus, or up to 100 mg accelerated alteplase given over 90 min. The primary end-point was all-cause mortality at 30 days and the hypothesis was that the two treatments would be equivalent. By 30 days, 6.61% of alteplase-treated patients and 6.75% lanoteplase-treated patients had died (relative risk 1.02). Total stroke occurred in 1.53% alteplase- and 1.87% lanoteplase-treated patients (ns); haemorrhagic stroke rates were 0.64% alteplase and 1.12% lanoteplase (P=0.004). The net clinical deficit of 30-day death or non-fatal disabling stroke was 7.0% and 7.2%, respectively. By 6 months, 8.8% of alteplase-treated patients and 8.7% of lanoteplase-treated patients had died. Conclusion Single-bolus weight-adjusted lanoteplase is an effective thrombolytic agent, equivalent to alteplase in terms of its impact on survival and with a comparable risk-benefit profile. The single-bolus regimen should shorten symptoms to treatment times and be especially convenient for emergency department or out-of-hospital administration.
