    DETERMINING SUBJECT-SPECIFIC PARAMETERS FOR A COMPUTER SIMULATION MODEL OF A ONE-HANDED TENNIS BACKHAND

    A subject-specific computer simulation model of a one-handed tennis backhand was developed to investigate the mechanisms that may cause injury to the elbow. Subject-specific parameters for the ball, racket and human were determined. Firing tennis balls from a pneumatic air cannon onto a force plate enabled parameters to be determined for a spring-damper model of a tennis ball. Data from further ball cannon tests allowed the spring constants for the stringbed and the coefficient of friction between the ball and stringbed to be optimised using a computer simulation model. The fundamental modal frequencies of the racket frame were obtained by Doppler laser vibrometry and its inertia parameters were determined from the results of oscillation and balance tests. An elite tennis player performed isovelocity tests at the wrist, elbow and shoulder to establish torque / angle / angular velocity relationships.     A subject-specific angle-driven computer model of a tennis player, combined with a forward dynamics, equipment-specific computer model of tennis ball–racket impacts, was developed to determine the effect of ball–racket impacts on loading at the elbow for one-handed backhand groundstrokes. Matching subject-specific computer simulations of a typical topspin/slice one-handed backhand groundstroke performed by an elite tennis player were done with root mean square differences between performance and matching simulations of < 0.5°over a 50 ms period starting from ball impact. Simulation results suggest that for similar ball–racket impact conditions, the difference in elbow loading for a topspin and slice one-handed backhand groundstroke is relatively small. In this study, the relatively small differences in elbow loading may be due to comparable angle–time histories at the wrist and elbow joints with the major kinematic differences occurring at the shoulder. Using a subject-specific angle-driven computer model combined with a forward dynamics, equipment-specific computer model of tennis ball–racket impacts allows peak internal loading, net impulse, and shock due to ball–racket impact to be calculated which would not otherwise be possible without impractical invasive techniques. This study provides a basis for further investigation of the factors that may increase elbow loading during tennis strokes

    Suddenly moving large classes online: Illuminating the experience of the teaching staff in one university

    [EN] In early 2020, the transition of large classes from the face-to-face to the online context occurred overnight and at scale at a time when the crisis was being faced at all levels of society, nationally and internationally. This paper is based on research which examined the impact of this sudden transition on large classes in Dublin City University with a view to illuminating the experience to inform future practice (Authors., in press). A rapid, systemised review of literature was carried out with the aim of contextualising data gathered through surveys with staff and students in relation to our experience of moving large classes online in the early stages of the Covid-19 pandemic. While the study examined the impact from the perspectives of teaching staff and students, this paper reports on the perspectives of teaching staff only. Large class teachers found this experience challenging, reporting a sense of isolation and worry.     The Nature of Boulder-Rich Deposits in the Upper Big Flat Brook Drainage, Sussex County, New Jersey

    The upper reaches of the Big Flat Brook drainage, northwest of Kittatinny Mountain, contain a variety of glacial, pro-glacial, and periglacial deposits from the Late Quaternary. The area is dominated by recessional moraines and ubiquitous ground moraine, along with meltwater deposits, drumlins, and possible postglacial periglacial features. We have identified a curious boulder-rich deposit in the vicinity of Lake Ocquittunk and Lake Wapalanne on upper Big Flat Brook. The area where these boulder deposits occur is mapped (1:24,000 surficial geology) as till. As mapped and observed, larger cobbles and boulders within the till are quartz-pebble conglomerate, quartzite, sandstone, and shale. The boulder-rich deposits differ from the typical till, however. Unlike the local till, which is more mixed in lithology, the boulder deposits are nearly exclusively Shawangunk conglomerate. The deposits are discontinuous, but appear to occur at a topographic level above the meltwater stream terraces. The boulders in the deposits lie partially embedded in soil, but are very closely spaced. The boulders range in size from ~20cm to over 100cm, and present a subrounded to subangular shape. There appears to be a fabric orientation of the boulders, NE-SW, with subsidiary orientations. As the boulder deposits differ from other mapped features in the area, we attempt to ascertain the origin for the deposits. Unique moraine dynamics, meltwater flood, periglacial slope movement, and slope-snow bank processes are possible explanations. Detailed examination of these boulder deposits will provide a finer explanation of the glacial and post-glacial geomorphic history of the area

    Most likely paths to error when estimating the mean of a reflected random walk

    It is known that simulation of the mean position of a Reflected Random Walk (RRW) {Wn}\{W_n\} exhibits non-standard behavior, even for light-tailed increment distributions with negative drift. The Large Deviation Principle (LDP) holds for deviations below the mean, but for deviations at the usual speed above the mean the rate function is null. This paper takes a deeper look at this phenomenon. Conditional on a large sample mean, a complete sample path LDP analysis is obtained. Let II denote the rate function for the one dimensional increment process. If II is coercive, then given a large simulated mean position, under general conditions our results imply that the most likely asymptotic behavior, ψ\psi^*, of the paths n1Wtnn^{-1} W_{\lfloor tn\rfloor} is to be zero apart from on an interval [T0,T1][0,1][T_0,T_1]\subset[0,1] and to satisfy the functional equation \begin{align*} \nabla I\left(\ddt\psi^*(t)\right)=\lambda^*(T_1-t) \quad \text{whenever } \psi(t)\neq 0. \end{align*} If II is non-coercive, a similar, but slightly more involved, result holds. These results prove, in broad generality, that Monte Carlo estimates of the steady-state mean position of a RRW have a high likelihood of over-estimation. This has serious implications for the performance evaluation of queueing systems by simulation techniques where steady state expected queue-length and waiting time are key performance metrics. The results show that na\"ive estimates of these quantities from simulation are highly likely to be conservative.Comment: 23 pages, 8 figure

    Association of Accelerometry-Measured Physical Activity and Cardiovascular Events in Mobility-Limited Older Adults: The LIFE (Lifestyle Interventions and Independence for Elders) Study.

    BACKGROUND:Data are sparse regarding the value of physical activity (PA) surveillance among older adults-particularly among those with mobility limitations. The objective of this study was to examine longitudinal associations between objectively measured daily PA and the incidence of cardiovascular events among older adults in the LIFE (Lifestyle Interventions and Independence for Elders) study. METHODS AND RESULTS:Cardiovascular events were adjudicated based on medical records review, and cardiovascular risk factors were controlled for in the analysis. Home-based activity data were collected by hip-worn accelerometers at baseline and at 6, 12, and 24&nbsp;months postrandomization to either a physical activity or health education intervention. LIFE study participants (n=1590; age 78.9±5.2 [SD] years; 67.2% women) at baseline had an 11% lower incidence of experiencing a subsequent cardiovascular event per 500&nbsp;steps taken per day based on activity data (hazard ratio, 0.89; 95% confidence interval, 0.84-0.96; P=0.001). At baseline, every 30&nbsp;minutes spent performing activities ≥500&nbsp;counts per minute (hazard ratio, 0.75; confidence interval, 0.65-0.89 [P=0.001]) were also associated with a lower incidence of cardiovascular events. Throughout follow-up (6, 12, and 24&nbsp;months), both the number of steps per day (per 500&nbsp;steps; hazard ratio, 0.90, confidence interval, 0.85-0.96 [P=0.001]) and duration of activity ≥500&nbsp;counts per minute (per 30&nbsp;minutes; hazard ratio, 0.76; confidence interval, 0.63-0.90 [P=0.002]) were significantly associated with lower cardiovascular event rates. CONCLUSIONS:Objective measurements of physical activity via accelerometry were associated with cardiovascular events among older adults with limited mobility (summary score &gt;10 on the Short Physical Performance Battery) both using baseline and longitudinal data.     Integrated lipidomics and proteomics point to early blood-based changes in childhood preceding later development of psychotic experiences: evidence from the Avon Longitudinal Study of Parents and Children

    Background The identification of early biomarkers of psychotic experiences (PEs) is of interest as early diagnosis and treatment of those at risk of future disorder is associated with improved outcomes. The current study investigated early lipidomic and coagulation pathway protein signatures of later PEs in subjects from the Avon Longitudinal Study of Parents and Children cohort. Methods Plasma of 115 children (age 12) who were first identified as experiencing PEs at age 18 (48 cases and 67 controls) were assessed through integrated and targeted lipidomics and semi-targeted proteomics approaches. We assessed the lipids, LPCs (n=11) and PCs (n=61), and the protein members of the coagulation pathway (n=22) and integrated this data with complement pathway protein data already available on these subjects. Results Twelve PCs, four LPCs and the coagulation protein plasminogen were altered between the control and PE group after correction for multiple comparisons. Lipidomic and proteomic datasets were integrated into a multivariate network displaying a strong relationship between most lipids that were significantly associated to PEs and plasminogen. Finally, an unsupervised clustering approach identified four different clusters with one of the clusters presenting the highest ratio cases:controls (P < 0.01) and associated with a higher concentration of smaller LDL cholesterol particles. Conclusions Our findings indicate that the lipidome and proteome of subjects who report PEs at age 18 is already altered at age 12 indicating that metabolic dysregulation may contribute to an early vulnerability to PEs and suggesting cross-talk between these LPCs, PCs and coagulation and complement proteins

    Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     Hepatitis E virus: Western Cape, South Africa

    AIM To conduct a prospective assessment of anti-hepatitis E virus (HEV) IgG seroprevalence in the Western Cape Province of South Africa in conjunction with evaluating risk factors for exposure. METHODS Consenting participants attending clinics and wards of Groote Schuur, Red Cross Children's Hospital and their affiliated teaching hospitals in Cape Town, South Africa, were sampled. Healthy adults attending blood donor clinics were also recruited. Patients with known liver disease were excluded and all major ethnic/race groups were included to broadly represent local demographics. Relevant demographic data was captured at the time of sampling using an interviewer-administered confidential questionnaire. Human immunodeficiency virus (HIV) status was self-disclosed. HEV IgG testing was performed using the Wantai assay. RESULTS HEV is endemic in the region with a seroprevalence of 27.9% (n = 324/1161) 95%CI: 25.3%-30.5% (21.9% when age-adjusted) with no significant differences between ethnic groups or HIV status. Seroprevalence in children is low but rapidly increases in early adulthood. With univariate analysis, age ? 30 years old, pork and bacon/ham consumption suggested risk. In the multivariate analysis, the highest risk factor for HEV IgG seropositivity (OR = 7.679, 95%CI: 5.38-10.96, p < 0.001) was being 30 years or older followed by pork consumption (OR = 2.052, 95%CI: 1.39-3.03, p < 0.001). A recent clinical case demonstrates that HEV genotype 3 may be currently circulating in the Western Cape.
