    An ethnobotanical research in Şanlıurfa central district and attached Villages (Turkey)

    The objective of present study is to identify the plants traditionally utilized by local communities, who reside at the rural areas attached to Central District of Şanlıurfa located in Southeast Anatolia Region, for various purposes, and to reveal the significant of such use in terms of ethnobotany. The study conducted in this respect identified 137 taxa of 87 genera from 47 families (86 species, 34 sub-species, and 17 varieties), 21 of them are cultigens. 1 taxon is from Terfeziaceae of Ascomycota division of Fungi kingdom, while the others are from Gymnospermae and Angiospermae sub-division of Spermatophyte division from the Plantae kingdom. In general, local community utilizes taxa identified in field of study as follows: 56 taxa as food, 37 as medicinal purposes, 5 for belief, 5 for pests, 4 as household goods, 3 as seasoning, 3 as ornaments, 3 as cosmetics, 2 as beverages, 2 as knick-knack, 2 as fuel, 2 as building materials, 1 as dulling, 1 as canopy, 1 as aphrodisiac, 1 as clarifying, 1 as repellent, 1 as doddle, 1 as preventive and 1 as aroma. Moreover, 11 taxa are not utilized, but 7 of them are known by their local names and 4 with their harmful effects

    Considering the total amount of energy consumed in the world, energy used in lighting is of utmost importance. For this reason, systems used in lighting should be energy efficient, and mıre efficient lighting elements should be preferred. LED, as a solid-state lighting system, is more energy efficient than the lighting systems with conventional lighting elements. Some of the LEDs used in solid-state lighting systems are named as standard 5mm LEDs and power LEDs. Because power LEDs have more light efficiency than standard 5 mm LEDs, they are produced as an alternative to conventional light sources. Power LEDs draw much more current than standard 5 mm LEDs. LEDs need LED drivers that provide them with constant current to run efficiently, and have a long life. The present research studies 10 W DC-DC converter based current limited LED driver circuits. Simulations were created for these LED driver circuits, and they are analysed through their simulations. Efficiency, input current, total circuit loss, output current and power values are measured. In this context, output current and efficiency values of the driver circuits are analysed in terms of energy efficiency, and optimised in accordance with energy efficiency

    Polyethylene glycol electrolyte (PEG-3350) solution is usually used for bowel emptying before co-lonoscopy in adults. It has also been reported to be safe in children. It is thought that bowel irrigation with this solution can be a useful treatment alternative for poisoning with slow releasing drugs, swallowed packaged substances, enteric coated drugs, drugs not binding to charcoal, and heavy metals in children. Due to high molecular weight of PEG-3350, its absorption from the intestinal mucosa is very low (0.2%). Therefore, it is less likely to have side effects. A three-year-old girl bit and ate one-third of an alkali battery and was brought to our pediatric emergency unit. Vital signs and results of physical examination and laboratory investigations were normal. Irrigation of the bowels with PEG-3350 solution given orally at a rate of 20 mL/kg/h was initiated. Upon excretion of feces of normal appearance in the sixth hour, irrigation was continued. Since rashes and itching started throughout her body in the thirtieth hour after administration of 9 L PEG-3350, the irrigation was discontinued and the patient was administered antihistamines. Rashes and itching regressed within one hour of its discontinuation. This suggested that they were due to the irrigation solution. There are five adult cases of allergic reactions to PEG-3350 reported in the literature. The case presented is the first pediatric patient developing allergic reaction to PEG-3350.Otopina polietilen glikola (PEG-3350) najčešće se primjenjuje za pražnjenje crijeva prije kolonoskopije u odraslih osoba, a opisuje se kao sigurno sredstvo i u djece. Smatra se da ispiranje crijeva ovom otopinom može biti korisna alternativa za liječenje otrovanja lijekovima sa sporim otpuštanjem, progutanim pakovanjima nekih tvari, lijekovima sa zaštitnom ovojnicom za probavni sustav, lijekovima koji se ne vežu na ugljen te teškim metalima u djece. Zbog velike molekularne težine vrlo mali dio PEG-3350 (0,2%) se apsorbira iz crijevne sluznice pa su stoga nuspojave manje vjerojatne. Djevojčica u dobi od tri godine zagrizla je i progutala jednu trećinu alkalne baterije pa je dovedena u našu hitnu pedijatrijsku službu. Vitalni znaci kao i rezultati fizikalnog pregleda i laboratorijskih pretraga bili su normalni. Započeto je ispiranje crijeva otopinom PEG-3350 peroralno, 20 mL/kg/h. Nakon što je dijete imalo stolicu normalnog izgleda u šestom satu, ispiranje je nastavljeno. Međutim, nakon što je djevojčica primila 9 L PEG-3350, u tridesetom satu davanja otopine nastupio je osip i svrbež po čitavom tijelu pa je ispiranje crijeva prekinuto i djetetu su dani antihistaminici. Osip i svrbež su se povukli unutar jednog sata od prestanka ispiranja otopinom PEG-3350. U literaturi je opisano pet slučajeva alergijskih reakcija na PEG-3350 u odraslih osoba. Ovaj prikaz opisuje prvi slučaj alergijske reakcije na PEG-3350 u djeteta

    Middle Ear Pressure and Factors Affecting It in Intubated Patients Hospitalized in Intensive Care

    Objective:To assess the probable agents affecting middle ear pressure in intubated patients hospitalized in intensive care units with various diagnoses.Methods:Middle ear pressure of 38 patients hospitalized in intensive care units within our faculty hospital was measured using portable tympanograms and acoustic reflectometry. The mode of the device to which each patient was attached and patients’ blood pressure, Glasgow Coma Score, and additional disease parameters other than admission diagnosis were recorded. All data collected were subjected to statistical analysis to determine whether or not they affected middle ear pressure.Results:Septal deviation, survey, and mode of automatic respiratory device emerged as factors affecting middle ear pressure (odds coefficient 4.796, 3.745, 2.557, respectively, with 95% CI). Although aged over 60, additional disease and nasogastric tube also compromised middle ear pressure; the levels involved were not statistically significant.Conclusion:Middle ear pressure in patients hospitalized in intensive care units may change, particularly after the seventh day. This may particularly involve septal deviation, survey, and mode of automatic respiratory device, and tympanograms and reflectometry may be added to the patient-monitoring protocol in terms of changes in middle ear pressure

    SUCCESS OF ARTIFICIAL INTELLIGENCE SYSTEM IN DETERMINING ALVEOLAR BONE LOSS FROM DENTAL PANORAMIC RADIOGRAPHY IMAGES

    Objectives: The aim of this study was to detect alveolar bone loss from dental panoramic radiographic images using artificial intelligence systems. Material and Methods: A total of 2276 panoramic radiographic images were used in this study. While 1137 of them belong to cases with bone destruction, 1139 were periodontally healthy. The dataset is divided into three parts as training (n=1856) , validation (n=210) and testing set (n= 210). All images in the data set were resized to 1472x718 pixels before training. A random sequence was created using the open-source python programming language and OpenCV, NumPy, Pandas, and Matplotlib libraries effectively. A pre-trained Google Net Inception v3 CNN network was used for preprocessing and data sets were trained using transfer learning. Diagnostic performance was evaluated with the confusion matrix using sensivitiy, specificity, precision, accuracy and F1 score. Results: Of the 105 cases with bone loss, 99 were detected by the AI system. Sensitivity was 0.94, specificity 0.88, precision 0.89, accuracy 0.91 and F1 score 0.91. Conclusion: The convolutional neural network model is successful in determining periodontal bone losses. It can be used as a system to facilitate the work of physicians in diagnosis and treatment planning in the future

    Temperature Effects Explain Continental Scale Distribution of Cyanobacterial Toxins

    Insight into how environmental change determines the production and distribution of cyanobacterial toxins is necessary for risk assessment. Management guidelines currently focus on hepatotoxins (microcystins). Increasing attention is given to other classes, such as neurotoxins (e.g., anatoxin-a) and cytotoxins (e.g., cylindrospermopsin) due to their potency. Most studies examine the relationship between individual toxin variants and environmental factors, such as nutrients, temperature and light. In summer 2015, we collected samples across Europe to investigate the effect of nutrient and temperature gradients on the variability of toxin production at a continental scale. Direct and indirect effects of temperature were the main drivers of the spatial distribution in the toxins produced by the cyanobacterial community, the toxin concentrations and toxin quota. Generalized linear models showed that a Toxin Diversity Index (TDI) increased with latitude, while it decreased with water stability. Increases in TDI were explained through a significant increase in toxin variants such as MC-YR, anatoxin and cylindrospermopsin, accompanied by a decreasing presence of MC-LR. While global warming continues, the direct and indirect effects of increased lake temperatures will drive changes in the distribution of cyanobacterial toxins in Europe, potentially promoting selection of a few highly toxic species or strains.Peer reviewe

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung’s disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung’s disease) from 264 hospitals (89 in high-income countries, 166 in middleincome countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in lowincome countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. Interpretation Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between lowincome, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030
