Neuroimaging methods for analysing connectivity in the presence of white matter lesions
White Matter Hyperintensities (WMHs) are often observed in the MRI scans of the ageing brain. Previous studies show that the WMH load correlates with cognitive decline, as well as with an increased risk of stroke and dementia. Most of the studies use a global WMH load across the whole brain as the only metric. This thesis proposes new insight into such data by introducing a methodology to analyse the WMH impact on the structural and functional brain connectivity in a localised manner, using white matter tracts. The thesis also supports other studies by exploring the potential impact of WMHs presence on tractography modelling, as well as adapts the method to Multiple Sclerosis (MS) lesion data.
In the first part of the thesis, we presented and evaluated two possible measures of the WMHs impact on the WM tracts from the structural perspective. We showed that despite the different distributions of the two measures, both show a similar relationship with the functional connectivity measure. We explored several possible options for quantifying the resting state functional connectivity between the endpoints of the WM tract in the presence of WMHs. We showed that the choice of connectivity measure (full or partial correlation), as well as gray matter parcellation, made a substantial difference in the results. We also observed variability in the results among the tracts and compared our findings to the results from the literature.
The majority of this work depends on the correct definition of the white matter tracts. Therefore, the second part of the thesis focuses on evaluating the impact of the presence of WMHs on tractography modelling. Despite changes in the microstructural parameters within the WMHs and their proximity, we found no meaningful alterations in the shape of the WM tracts in the presence of WMHs on the tract. Finally, we explored the possibility to apply some of this methodology to data from patients with Multiple Sclerosis, which is the most common cause of neurological disability in young adults. MS lesions have different aetiology to the WMHs but may have a similar appearance. We showed that directly applying the method between those two conditions may not be possible, and we proposed an alternative approach, suited to the MS data
The effectiveness of bronchial artery embolisation in patients with haemoptysis
Introduction: Bronchial artery embolisation (BAE) is one of the methods used in massive and recurring haemoptysis. The aim ofthe study is to determine the effectiveness and complications of bronchial artery embolisation in recurring haemoptysis.
Material and methods: The analysis included 47 embolisation procedures performed on 30 patients treated between 2011 and2017 in the Department of Respiratory Medicine, Allergology and Pulmonary Oncology due to haemoptysis. The patient’s age rangedbetween 18 and 71 years, while mean age at the time of BAE was 33.5 years. Patients with tuberculosis constituted 73.33% (n = 22)of the sample and underwent 31 embolisation procedures in total. The remaining part of the sample (n = 8) collectively underwent 16BAEs. The analysis was conducted by verifying the medical documentation, as well as carrying face-to-face and phone conversations.
Results: Immediate control due to the inhibition of bleeding was obtained in 95.75% of cases. Recurrence within 3 days of BAEwas reported in 5 patients (10.63%), and 4 re-embolisation procedures were conducted. In 10 patients (33.33%), recurrencewas observed during the first year post-BAE, while it was reported in 17 cases during the whole observation period (56.66% ofpatients). The subjects who underwent re-embolisation demonstrated recurrence-free periods lasting from 2 days to 63 months.In patients with recurrence but no re-embolisation, the shortest and longest haemoptysis-free time was 2 and 35 months, respectively.11 patients (36.66%) required several embolisation procedures during the whole observation period.
Conclusions: BAE is a highly successful procedure in treating haemoptysis. The risk of complications is low
Evaluation of low density lipoprotein oxidation in a course of hypothyroidism
Wstęp: Celem pracy była ocena wpływu niedoczynności
tarczycy na oksydatywną modyfikację lipoprotein małej gęstości
(LDL, low density lipoprotein).
Materiał i metody: Badaniom poddano 24 pacjentów z jawną
niedoczynnością tarczycy oraz 10 pacjentów z subkliniczną
postacią niedoczynności tarczycy. Grupę kontrolną stanowiło
24 zdrowych ochotników, u których stwierdzano prawidłowe
stężenie hormonu tyreotropowego (TSH, thyroid
stimulating hormone) w surowicy. Oksydację lipoprotein oceniano
na podstawie stężenia utlenionych LDL (oxLDL, oxidized
low density lipoprotein) w osoczu oraz przeciwciał przeciwko
utlenionym LDL (anty-oxLDL, against oxidized low
density lipoprotein) w surowicy.
Wyniki: W grupie pacjentów z jawną hipotyreozą wykazano
zwiększone stężenie utlenionych LDL w porównaniu
z grupą osób z subkliniczną hipotyreozą i grupą kontrolną.
Nie stwierdzono statystycznie istotnej różnicy w stężeniu
anty-oxLDL między grupą pacjentów z jawną lub subkliniczną
niedoczynnością tarczycy a grupą kontrolną. Stężenie
oxLDL w osoczu krwi badanych pacjentów korelowało
ujemnie ze stężeniem tyroksyny (FT4, free thyroxine), a dodatnio
ze stężeniem TSH i składników lipidowych surowicy (cholesterolu całkowitego, cholesterolu frakcji LDL, triacylogliceroli).
Wnioski: W przeprowadzonych badaniach wykazano, że
u pacjentów z niedoczynnością tarczycy występuje nasilona
oksydacja lipoprotein, zależna od stopnia hipotyreozy
i zmian w profilu lipidowym surowicy. Czynnikiem sprzyjającym
oksydacji lipoprotein u chorych z niedoczynnością
tarczycy było wysokie stężenie cholesterolu i triacylogliceroli
w surowicy krwi. Oznaczanie stężenia oxLDL krążących
w osoczu może być przydatnym testem diagnostycznym
w ocenie zagrożenia aterogenezą u pacjentów z niedoczynnością
tarczycy.Introduction: The aim of this study was to evaluate the influence
of hypothyroidism on oxidative modification of low
density lipoprotein (LDL).
Material and methods: 24 patients with overt hypothyroidism
and 10 patients with mild hypothyroidism were enrolled
to the study. The control group consisted of 24 healthy
subjects with normal serum TSH. Plasma level of oxidized
LDL (oxLDL) and serum level of antibodies against oxidized
LDL (anti-oxLDL) determined lipoprotein oxidation.
Results: Significantly increased plasma oxLDL levels were
found in patients with overt hypothyroidism in comparison
to patients with mild hypothyroidism and control group.
Anti-oxLDL levels in patients with overt or mild hypothyroidism
and in the control group showed no significant differences.
OxLDL plasma levels in patients with hypothyroidism
inversely correlated with FT4 levels and positively correlated
with TSH, total cholesterol, LDL cholesterol and triglycerides
levels. Conclusions: The presented study indicates increased lipoprotein
oxidation in patients with hypothyroidism which
depends on the degree of hypothyroidism and changes in
lipid profile. Elevated cholesterol and triglicerides levels are
the factors increasing lipoprotein oxidation. Plasma oxLDL
levels may constitute a useful marker indicating the risk for
atherosclerosis in hypothyroidism
Ocena skuteczności embolizacji tętnic oskrzelowych u pacjentów z krwiopluciem
WSTĘP: Jedną z metod postępowania w masywnym lub nawracającym krwiopluciu jest embolizacja tętnic oskrzelowych (BAE). Celem pracy było wykazanie skuteczności oraz określenie powikłań zabiegu embolizacji tętnic oskrzelowych u pacjentów z nawracającym krwiopluciem. MATERIAŁ I METODY: Analizie poddano 47 zabiegów embolizacji u 30 pacjentów przebywających w latach 2011–2017 na oddziale pulmonologii, alergologii i pulmonologii onkologicznej w Poznaniu z powodu krwioplucia. Przedział wiekowy pacjentów: 18–71 lat, średnia wieku podczas BAE: 33,5 roku. Pacjenci z mukowiscydozą stanowili 73,33% (n = 22); wykonano u nich łącznie 31 embolizacji. U pozostałych pacjentów (n = 8) wykonano łącznie 16 embolizacji. Analizy dokonano, przeprowadzając weryfikację dokumentacji medycznej oraz rozmowy bezpośredniej bądź telefonicznej. WYNIKI: Natychmiastową kontrolę wynikającą z ustąpienia krwawienia uzyskano po 95,75% embolizacjach. Nawrót krwioplucia w ciągu 3 dni od zabiegu nastąpił po 5 BAE (10,63%), wykonano 4 reembolizacje. W pierwszym roku nawrót wystąpił u 10 pacjentów (33,33%), w czasie całej obserwacji u 17 chorych (56,66%). Wśród pacjentów, którzy mieli wykonane ponowne BAE najkrótszy czas bez krwioplucia to 2 dni, a najdłuższy 63 miesiące. Wśród pacjentów z nawrotem krwawienia, ale bez ponownej embolizacji najkrótszy czas bez krwioplucia wynosi 2 miesiące, najdłuższy 35 miesięcy. Jedenastu pacjentów (36,66%) wymagało wykonania kilku embolizacji na przestrzeni omawianych lat. Neuroimaging the consciousness of self: Review, and conceptual-methodological framework
We review neuroimaging research investigating self-referential processing (SRP), that is, how we respond to stimuli that reference ourselves, prefaced by a lexical-thematic analysis of words indicative of “self-feelings”. We consider SRP as occurring verbally (V-SRP) and non-verbally (NV-SRP), both in the controlled, “top-down” form of introspective and interoceptive tasks, respectively, as well as in the “bottom-up” spontaneous or automatic form of “mind wandering” and “body wandering” that occurs during resting state. Our review leads us to outline a conceptual and methodological framework for future SRP research that we briefly apply toward understanding certain psychological and neurological disorders symptomatically associated with abnormal SRP. Association between loop diuretic dose changes and outcomes in chronic heart failure: observations from the ESC-EORP Heart Failure Long-Term Registry
[Abstract]
Aims. Guidelines recommend down-titration of loop diuretics (LD) once euvolaemia is achieved. In outpatients with heart
failure (HF), we investigated LD dose changes in daily cardiology practice, agreement with guideline recommendations,
predictors of successful LD down-titration and association between dose changes and outcomes.
Methods
and results.
We included 8130 HF patients from the ESC-EORP Heart Failure Long-Term Registry. Among patients who had dose
decreased, successful decrease was defined as the decrease not followed by death, HF hospitalization, New York Heart
Association class deterioration, or subsequent increase in LD dose. Mean age was 66±13 years, 71% men, 62% HF
with reduced ejection fraction, 19% HF with mid-range ejection fraction, 19% HF with preserved ejection fraction.
Median [interquartile range (IQR)] LD dose was 40 (25–80) mg. LD dose was increased in 16%, decreased in 8.3%
and unchanged in 76%. Median (IQR) follow-up was 372 (363–419) days. Diuretic dose increase (vs. no change) was
associated with HF death [hazard ratio (HR) 1.53, 95% confidence interval (CI) 1.12–2.08; P = 0.008] and nominally
with cardiovascular death (HR 1.25, 95% CI 0.96–1.63; P = 0.103). Decrease of diuretic dose (vs. no change) was
associated with nominally lower HF (HR 0.59, 95% CI 0.33–1.07; P = 0.083) and cardiovascular mortality (HR 0.62 95% CI 0.38–1.00; P = 0.052). Among patients who had LD dose decreased, systolic blood pressure [odds ratio
(OR) 1.11 per 10 mmHg increase, 95% CI 1.01–1.22; P = 0.032], and absence of (i) sleep apnoea (OR 0.24, 95% CI
0.09–0.69; P = 0.008), (ii) peripheral congestion (OR 0.48, 95% CI 0.29–0.80; P = 0.005), and (iii) moderate/severe
mitral regurgitation (OR 0.57, 95% CI 0.37–0.87; P = 0.008) were independently associated with successful decrease.
Conclusion. Diuretic dose was unchanged in 76% and decreased in 8.3% of outpatients with chronic HF. LD dose increase was
associated with worse outcomes, while the LD dose decrease group showed a trend for better outcomes compared
with the no-change group. Higher systolic blood pressure, and absence of (i) sleep apnoea, (ii) peripheral congestion,
and (iii) moderate/severe mitral regurgitation were independently associated with successful dose decrease
Sex- and age-related differences in the management and outcomes of chronic heart failure: an analysis of patients from the ESC HFA EORP Heart Failure Long-Term Registry
Aims: This study aimed to assess age- and sex-related differences in management and 1-year risk for all-cause mortality and hospitalization in chronic heart failure (HF) patients. Methods and results: Of 16 354 patients included in the European Society of Cardiology Heart Failure Long-Term Registry, 9428 chronic HF patients were analysed [median age: 66 years; 28.5% women; mean left ventricular ejection fraction (LVEF) 37%]. Rates of use of guideline-directed medical therapy (GDMT) were high (angiotensin-converting enzyme inhibitors/angiotensin receptor blockers, beta-blockers and mineralocorticoid receptor antagonists: 85.7%, 88.7% and 58.8%, respectively). Crude GDMT utilization rates were lower in women than in men (all differences: P\ua0 64 0.001), and GDMT use became lower with ageing in both sexes, at baseline and at 1-year follow-up. Sex was not an independent predictor of GDMT prescription; however, age >75 years was a significant predictor of GDMT underutilization. Rates of all-cause mortality were lower in women than in men (7.1% vs. 8.7%; P\ua0=\ua00.015), as were rates of all-cause hospitalization (21.9% vs. 27.3%; P\ua075 years. Conclusions: There was a decline in GDMT use with advanced age in both sexes. Sex was not an independent predictor of GDMT or adverse outcomes. However, age >75 years independently predicted lower GDMT use and higher all-cause mortality in patients with LVEF 6445%
