    Capacity of Solid Waste Management of the Municipalities in North-West Region of Bangladesh: A Far Cry with Sustainable Development

    The study addresses the overall waste management and drainage system of the Municipalities of Bangladesh. It also endeavors to compare the existing system with the requirements of making a sustainable city following the targets of SDG. It tries to highlight the shortcomings of the existing system of waste management and argues that this is never able to confine sustainability in the environment and the habitats of the Municipalities. The study is empirical in nature mainly based on primary data. Data have been collected from the officials and stakeholders of the three selected Municipalities of a North-west district of Bangladesh- Rajshahi through face to face interview and Focus Group Discussion. This paper identifies the fragile solid waste management capacity of the Municipalities of Bangladesh and argues that current practice is nothing but an inevitable setback for the country to achieve sustainable development

    Socioeconomic, livelihood and cultural profile of the Meghna River Hilsa Fishing Community in Chandpur, Bangladesh

    The goal of the study was to build a comprehensive portrait of the socioeconomic, livelihood, and cultural profile of the Meghna River fishing community in Chandpur, Bangladesh. Shatnol Malopara, an ecologically and economically suitable fishing community under the Matlab Uttor Upazila of Chandpur district, was selected for the in-depth investigation, where 410 fishermen relied solely on fishing for their livelihood. This community is made up of 185 households, supporting close to 1000 people. They are all Hindus, and fishing was their ancestral profession. A well-structured questionnaire was used to collect the data. The research revealed that the majority (35%) of fishermen were in the 18-30 age range. The community preferred nuclear families (98%), and the average family size was 5-8 individuals, which is predominant at 80%. About 60% of households lived in tiny tin shades and 40% in medium tin shades, while 60% did not have their own land. They (80%) rely on solar energy for illumination and for health facilities 50% of fisher households depend on the local pharmacy to take medication. Almost 100% of the residents in this community used potable drinking water, and 50% of fishers have ring slab latrines while the other 50% have pits. According to the survey, 60% of fishermen were very poor, 20% were poor, and 20% were moderately poor. Based on the survey, the majority (70%) of the fishermen earned between the ranges of 3000-5000 BDT (Bangladesh Taka) per month. During the ban period, the majority of fishers (50%) took out loans from various sources. Non-governmental organizations that operate microcredit businesses provided 70% of the loans to fishermen. According to the survey, 32% of fishermen had a boat and gear, while 68% worked as labor or engaged in catch-sharing with Mohajons' boats and gear. A range of crafts (Dingi nouka, Kosa nouka with mechanization) and fishing gear (Kona jal, Gulti jal, Dhon jal, Chap jal, Bada jal, Current jal, Chewa jal, etc.) was observed to be used in the study area. They have a plethora of traditional ecological knowledge as a result of their fishing ancestors. The study revealed that hilsa fishermen had a variety of issues. Extortion by local extortionists was the principal concern; other issues included inadequate credit and alternative income sources during the ban period. To assist the community in raising its standard of living, government agencies, nonprofits, and other relevant groups of organizations should adopt a number of steps. It is imperative to prioritize alternative income-generating options in this context

    Association of Serum Testosterone with Acne Vulgaris in Women

    Background: Androgens enhance the sebum production and follicular keratosis that plays the key role in the aetiology of acne. Objective: To find out the association between serum testosterone and acne vulgaris. Methods: A case control study was carried out for a period of two years in the outpatient department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. Female patients having acne vulgaris were selected as case. Healthy control (age and sex matched) were enrolled from the community. Results: The study showed that the mean age of the cases was 22.43 with standard deviation 5.2 years and the mean age of the control was 23.23 with standard deviation 5.9 years. The mean duration of disease was 62.6 months ranging from 12 months to 300 months. All the patients had presented with comedones (blackheads and whiteheads) followed by 94.3% had papules and 58.6% had pustules. Considering the site of lesion, all the patients had acne in the face. Data analysis revealed that the percentage of serum testosterone above normal was found to be high among the cases with acne (10%) whereas below normal level of serum testosterone was found among the control and the difference was statistically significant (p<0.001). Conclusion:The study found a significant association between serum testosterone and acne vulgaris. As serum testosterone is associated with acne vulgaris, testosterone levels should be measured in patients presenting with acne vulgaris especially in treatment resistant cases and anti-androgen treatment may be indicated in cases with elevated testosterone level.DOI: http://dx.doi.org/10.3329/bsmmuj.v5i1.10980 BSMMU J 2012; 5(1):1-5

    Leveraging contextual representations with BiLSTM-based regressor for lexical complexity prediction

    Lexical complexity prediction (LCP) determines the complexity level of words or phrases in a sentence. LCP has a significant impact on the enhancement of language translations, readability assessment, and text generation. However, the domain-specific technical word, the complex grammatical structure, the polysemy problem, the inter-word relationship, and dependencies make it challenging to determine the complexity of words or phrases. In this paper, we propose an integrated transformer regressor model named ITRM-LCP to estimate the lexical complexity of words and phrases where diverse contextual features are extracted from various transformer models. The transformer models are fine-tuned using the text-pair data. Then, a bidirectional LSTM-based regressor module is plugged on top of each transformer to learn the long-term dependencies and estimate the complexity scores. The predicted scores of each module are then aggregated to determine the final complexity score. We assess our proposed model using two benchmark datasets from shared tasks. Experimental findings demonstrate that our ITRM-LCP model obtains 10.2% and 8.2% improvement on the news and Wikipedia corpus of the CWI-2018 dataset, compared to the top-performing systems (DAT, CAMB, and TMU). Additionally, our ITRM-LCP model surpasses state-of-the-art LCP systems (DeepBlueAI, JUST-BLUE) by 1.5% and 1.34% for single and multi-word LCP tasks defined in the SemEval LCP-2021 task

    Fish diversity of an agriculturally influenced river in Bangladesh: current profile, threats and management perspectives

    Aim: Local and global stressors such as agricultural activities, climate change, habitat loss, eutrophication and pollution are the major causes of declining fish diversity. Therefore, for the management of fisheries and ecosystems, it is important to know the fish diversity of Little Feni River. The study was performed to know the diversity status of fishes in Little Feni River, Bangladesh during April - December, 2015. Methodology: Fishes and their related data were collected from the Little Feni River of fish landing centers, fish markets, interviewing fishermen and arotdar (or known as commission agent). Results: From this study, 47 species under 24 families were recorded in which 31 (66%) species were from freshwater and the remaining 16 (34%) species were from marine sources. Of the collected fishes, 26 species were categorized as not threatened (NT), 9 vulnerable (VU), 4 critically endangered (CR), 4 endangered (EN) and the remaining 9 could not be classified under the said list due to data deficient (DD). The results indicated the alarming decline of fish diversity in the investigated area in general and/or perhaps in the country as whole. Interpretation: Long-term investigation, appropriate evaluation and proper documentation are urgently needed so that the public, investigators and policy makers would be able to know about the current status of fish diversity, and can take initiatives for proper conservation management practices towards the gradual decline of wild fish abundance in Little Feni River

    Background: The development of digital technologies and the evolution of open innovation approaches have enabled the creation of diverse virtual organizations and enterprises coordinating their activities primarily online. The open innovation platform titled "International Natural Product Sciences Taskforce" (INPST) was established in 2018, to bring together in collaborative environment individuals and organizations interested in natural product scientific research, and to empower their interactions by using digital communication tools. Methods: In this work, we present a general overview of INPST activities and showcase the specific use of Twitter as a powerful networking tool that was used to host a one-week "2021 INPST Twitter Networking Event" (spanning from 31st May 2021 to 6th June 2021) based on the application of the Twitter hashtag #INPST. Results and Conclusion: The use of this hashtag during the networking event period was analyzed with Symplur Signals (https://www.symplur.com/), revealing a total of 6,036 tweets, shared by 686 users, which generated a total of 65,004,773 impressions (views of the respective tweets). This networking event's achieved high visibility and participation rate showcases a convincing example of how this social media platform can be used as a highly effective tool to host virtual Twitter-based international biomedical research events

    Hearing loss prevalence and years lived with disability, 1990–2019: findings from the Global Burden of Disease Study 2019

    Background Hearing loss affects access to spoken language, which can affect cognition and development, and can negatively affect social wellbeing. We present updated estimates from the Global Burden of Disease (GBD) study on the prevalence of hearing loss in 2019, as well as the condition's associated disability. Methods We did systematic reviews of population-representative surveys on hearing loss prevalence from 1990 to 2019. We fitted nested meta-regression models for severity-specific prevalence, accounting for hearing aid coverage, cause, and the presence of tinnitus. We also forecasted the prevalence of hearing loss until 2050. Findings An estimated 1·57 billion (95% uncertainty interval 1·51–1·64) people globally had hearing loss in 2019, accounting for one in five people (20·3% [19·5–21·1]). Of these, 403·3 million (357·3–449·5) people had hearing loss that was moderate or higher in severity after adjusting for hearing aid use, and 430·4 million (381·7–479·6) without adjustment. The largest number of people with moderate-to-complete hearing loss resided in the Western Pacific region (127·1 million people [112·3–142·6]). Of all people with a hearing impairment, 62·1% (60·2–63·9) were older than 50 years. The Healthcare Access and Quality (HAQ) Index explained 65·8% of the variation in national age-standardised rates of years lived with disability, because countries with a low HAQ Index had higher rates of years lived with disability. By 2050, a projected 2·45 billion (2·35–2·56) people will have hearing loss, a 56·1% (47·3–65·2) increase from 2019, despite stable age-standardised prevalence. Interpretation As populations age, the number of people with hearing loss will increase. Interventions such as childhood screening, hearing aids, effective management of otitis media and meningitis, and cochlear implants have the potential to ameliorate this burden. Because the burden of moderate-to-complete hearing loss is concentrated in countries with low health-care quality and access, stronger health-care provision mechanisms are needed to reduce the burden of unaddressed hearing loss in these settings

    Spatial, temporal, and demographic patterns in prevalence of chewing tobacco use in 204 countries and territories, 1990-2019 : a systematic analysis from the Global Burden of Disease Study 2019

    Interpretation Chewing tobacco remains a substantial public health problem in several regions of the world, and predominantly in south Asia. We found little change in the prevalence of chewing tobacco use between 1990 and 2019, and that control efforts have had much larger effects on the prevalence of smoking tobacco use than on chewing tobacco use in some countries. Mitigating the health effects of chewing tobacco requires stronger regulations and policies that specifically target use of chewing tobacco, especially in countries with high prevalence. Findings In 2019, 273 center dot 9 million (95% uncertainty interval 258 center dot 5 to 290 center dot 9) people aged 15 years and older used chewing tobacco, and the global age-standardised prevalence of chewing tobacco use was 4 center dot 72% (4 center dot 46 to 5 center dot 01). 228 center dot 2 million (213 center dot 6 to 244 center dot 7; 83 center dot 29% [82 center dot 15 to 84 center dot 42]) chewing tobacco users lived in the south Asia region. Prevalence among young people aged 15-19 years was over 10% in seven locations in 2019. Although global agestandardised prevalence of smoking tobacco use decreased significantly between 1990 and 2019 (annualised rate of change: -1 center dot 21% [-1 center dot 26 to -1 center dot 16]), similar progress was not observed for chewing tobacco (0 center dot 46% [0 center dot 13 to 0 center dot 79]). Among the 12 highest prevalence countries (Bangladesh, Bhutan, Cambodia, India, Madagascar, Marshall Islands, Myanmar, Nepal, Pakistan, Palau, Sri Lanka, and Yemen), only Yemen had a significant decrease in the prevalence of chewing tobacco use, which was among males between 1990 and 2019 (-0 center dot 94% [-1 center dot 72 to -0 center dot 14]), compared with nine of 12 countries that had significant decreases in the prevalence of smoking tobacco. Among females, none of these 12 countries had significant decreases in prevalence of chewing tobacco use, whereas seven of 12 countries had a significant decrease in the prevalence of tobacco smoking use for the period. Summary Background Chewing tobacco and other types of smokeless tobacco use have had less attention from the global health community than smoked tobacco use. However, the practice is popular in many parts of the world and has been linked to several adverse health outcomes. Understanding trends in prevalence with age, over time, and by location and sex is important for policy setting and in relation to monitoring and assessing commitment to the WHO Framework Convention on Tobacco Control. Methods We estimated prevalence of chewing tobacco use as part of the Global Burden of Diseases, Injuries, and Risk Factors Study 2019 using a modelling strategy that used information on multiple types of smokeless tobacco products. We generated a time series of prevalence of chewing tobacco use among individuals aged 15 years and older from 1990 to 2019 in 204 countries and territories, including age-sex specific estimates. We also compared these trends to those of smoked tobacco over the same time period.     Global, regional, and national burden of stroke and its risk factors, 1990–2019: a systematic analysis for the Global Burden of Disease Study 2019

    Background Regularly updated data on stroke and its pathological types, including data on their incidence, prevalence, mortality, disability, risk factors, and epidemiological trends, are important for evidence-based stroke care planning and resource allocation. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) aims to provide a standardised and comprehensive measurement of these metrics at global, regional, and national levels. Methods We applied GBD 2019 analytical tools to calculate stroke incidence, prevalence, mortality, disability-adjusted life-years (DALYs), and the population attributable fraction (PAF) of DALYs (with corresponding 95% uncertainty intervals [UIs]) associated with 19 risk factors, for 204 countries and territories from 1990 to 2019. These estimates were provided for ischaemic stroke, intracerebral haemorrhage, subarachnoid haemorrhage, and all strokes combined, and stratified by sex, age group, and World Bank country income level. Findings In 2019, there were 12·2 million (95% UI 11·0–13·6) incident cases of stroke, 101 million (93·2–111) prevalent cases of stroke, 143 million (133–153) DALYs due to stroke, and 6·55 million (6·00–7·02) deaths from stroke. Globally, stroke remained the second-leading cause of death (11·6% [10·8–12·2] of total deaths) and the third-leading cause of death and disability combined (5·7% [5·1–6·2] of total DALYs) in 2019. From 1990 to 2019, the absolute number of incident strokes increased by 70·0% (67·0–73·0), prevalent strokes increased by 85·0% (83·0–88·0), deaths from stroke increased by 43·0% (31·0–55·0), and DALYs due to stroke increased by 32·0% (22·0–42·0). During the same period, age-standardised rates of stroke incidence decreased by 17·0% (15·0–18·0), mortality decreased by 36·0% (31·0–42·0), prevalence decreased by 6·0% (5·0–7·0), and DALYs decreased by 36·0% (31·0–42·0). However, among people younger than 70 years, prevalence rates increased by 22·0% (21·0–24·0) and incidence rates increased by 15·0% (12·0–18·0). In 2019, the age-standardised stroke-related mortality rate was 3·6 (3·5–3·8) times higher in the World Bank low-income group than in the World Bank high-income group, and the age-standardised stroke-related DALY rate was 3·7 (3·5–3·9) times higher in the low-income group than the high-income group. Ischaemic stroke constituted 62·4% of all incident strokes in 2019 (7·63 million [6·57–8·96]), while intracerebral haemorrhage constituted 27·9% (3·41 million [2·97–3·91]) and subarachnoid haemorrhage constituted 9·7% (1·18 million [1·01–1·39]). In 2019, the five leading risk factors for stroke were high systolic blood pressure (contributing to 79·6 million [67·7–90·8] DALYs or 55·5% [48·2–62·0] of total stroke DALYs), high body-mass index (34·9 million [22·3–48·6] DALYs or 24·3% [15·7–33·2]), high fasting plasma glucose (28·9 million [19·8–41·5] DALYs or 20·2% [13·8–29·1]), ambient particulate matter pollution (28·7 million [23·4–33·4] DALYs or 20·1% [16·6–23·0]), and smoking (25·3 million [22·6–28·2] DALYs or 17·6% [16·4–19·0]). Interpretation The annual number of strokes and deaths due to stroke increased substantially from 1990 to 2019, despite substantial reductions in age-standardised rates, particularly among people older than 70 years. The highest age-standardised stroke-related mortality and DALY rates were in the World Bank low-income group. The fastest-growing risk factor for stroke between 1990 and 2019 was high body-mass index. Without urgent implementation of effective primary prevention strategies, the stroke burden will probably continue to grow across the world, particularly in low-income countries.publishedVersio
