    Engaging Tools for Dialogic Guidance in Higher Education

    In this article we present a toolbox for dialogic guidance that we use at the Academic Writing Centre at the University of Bergen when guiding students in various stages of the writing process. Our guidance is dialogic which means that we acknowledge that meaning and learning evolve when we interact with one another, when different and divergent voices meet; we let students themselves explore their writing, their writing processes and their texts, and find their own answers, their own solutions and their own ways. We ask open-ended questions, listen, describe and provide tools that meet different needs at different stages of the writing process, instead of judging and ‘diagnosing’ the written texts - and the students - and then proposing a ‘treatment’. Examples of our tools are spontaneous writing, the academic pentagon and the Toulmin model of argumentation. We seek to strengthen the students’ understanding and awareness of their own writing, thereby improving not just the writing at hand, but also the students’ academic writing skills and learning in general, and to develop a reflective and accepting attitude. Our students engage in dialogues with us, the tools we present, with themselves, their texts and their writing, with fellow students, with previous bachelor and master theses, and with the tradition which they are part of

    Dupilumab-induced eosinophilia in patients with diffuse type 2 chronic rhinosinusitis.

    BACKGROUND Dupilumab, a monoclonal anti-IL-4Rα antibody, is approved for several type 2 mediated inflammatory diseases like asthma, atopic dermatitis, and diffuse type 2 chronic rhinosinusitis (CRS). Clinical studies had reported a transient increase in blood eosinophils during dupilumab therapy. This study aimed to assess the impact of elevated blood eosinophils on clinical outcome and to investigate the cause of high blood eosinophil levels under dupilumab therapy. METHODS Patients suffering from diffuse type 2 CRS treated with dupilumab were examined on days 0, 28, 90, and 180 after therapy start. Sino-Nasal-Outcome-Test Score (SNOT-22), Total Nasal Polyp Score (TNPS), and blood samples were collected. Cytokine measurements and proteomics analysis were conducted. Flow cytometry analysis measured receptor expression on eosinophils. RESULTS Sixty-eighty patients were included. Baseline eosinophilia ≥0.3G/L was observed in 63.2% of patients, and in 30.9% of patients, eosinophils increased by ≥0.5G/L under dupilumab. Subjects with eosinophilia ≥0.3G/L at baseline had the best SNOT-22 mean change compared to no eosinophilia. Eosinophil elevation during dupilumab therapy had no impact on clinical scores. The eosinophil adhesion molecule VCAM-1 decreased significantly during therapy in all patients. The chemokine receptor CXCR4 was significantly down- and IL-4 upregulated in subjects with eosinophil increase. CONCLUSION Our findings suggest that increased eosinophils in type 2 CRS are associated with a good clinical response to dupilumab. Patients with elevated IL-4 at baseline developed dupilumab-induced transient eosinophilia. We identified the downregulation of VCAM-1 and surface markers CD49d and CXCR4 on eosinophils as possible explanations of dupilumab-induced eosinophilia

    The differences in production of Holstein Friesian and Holstein Friesian x Norwegian Red F1 crossbreeds

    Cilj rada bio je usporediti proizvodna, zdravstvena i svojstava plodnosti krava Holstein Friesian (HF) pasmine i njegovih F1 križanaca sa Norwegian Red pasminom krava (HF x NRF F1). Za potrebe ovoga istraživanja korišteni su samo proizvodni podaci zaključenih prvih laktacija za ukupno 57 HF x NRF F1 križanaca i 401 HF krava, te za zdravstvena i svojstva plodnosti za 120 HF x NRF F1 križanaca i 255 HF krava. Značajnost razlika u proizvodnim svojstvima testirana je analizom varijance uporabom PROC GLM procedure u SAS-u. Proizvodnja mlijeka u obje laktacije (standardna 305 dana i puna laktacija) bila je veća kod HF x NRF F1 križanaca za približno 400 kg, ali bez utvrđene značajnosti. Proizvodna svojstva (mast i protein (kg i %)) izuzevši protein u punoj laktaciji, bili su značajno veći (p<0,001; p<0,05) kod HF x NRF F1 križanaca u usporedbi sa HF kravama. Gledajući svojstva plodnosti, HF x NRF F1 križanci su imali bolju koncepciju (+1,58 %), manji broj osjemenjivanja krava po uspješnoj bređosti (-0,43), te kraće trajanje servisnog razdoblja (-22,2) u usporedbi sa HF kravama. HF x NRF F1 križanci su imala bolja zdravstvena svojstva (izuzev problema sa papcima) komparabilno sa HF kravama, odnosno imali su manje krava s mastitisom (-11,9 %), ketozom (-0,5 %), zaostalom posteljicom (-4,8 %), te dislokacijom sirišta (-1,0 %). Obzirom da su rezultati dobiveni od krava sa završenom prvom laktacijom, potrebno je analizirati i podatke od narednih laktacija, kako bi se dobio puno jasniji uvid u istraživanu problematiku.The aim of this study was to compare the production, health and fertility traits of Holstein Friesian (HF) and their F1 crosses with Norwegian Red cows (HF x NRF F1). Only the production data’s from finished first lactations of the total 57 HF x NRF F1 crosses and 401 HF cows, and for health and fertility traits of 120 HF x NRF F1 crosses and 255 HF cows, were used. The differences in production traits were tested by a least square means using the GLM procedure in SAS. The milk yield in both lactation (standard 305 days and full lactation) were higher in HF x NRF F1 crosses for approximately 400 kg, but without significance. The differences in production traits (fat and protein (kg and %)) were significant (p<0.001; p<0.05) with higher values for HF x NRF F1 crosses comparing to HF cows, with exception of protein in full lactation. Regarding the fertility traits, HF x NRF F1 crosses had a better conception rate (+1.58 %), a lower number of the insemination per pregnancy (-0.43) and a shorter duration of the service period (-22.2) comparable to HF cows. F1 crosses had better health traits (with except for hoofs problem) comparing to HF cows, they had less cows with mastitis (-11.9 %), ketosis (-0.5 %), retained placenta (-4.8 %) and dislocation of the abomasum (-1.0 %), respectively. Because of the fact that all results were obtained on the basis of data from the first lactation cows, it is necessary to do more analyses of all studied traits in subsequent lactations, in order to get a much clearer insight into the studied issues

    Differences in Misreporting of Sexual Behavior Over Time: Implications for HIV Trials

    We used data from a prospective study of 300 women attending a sexually transmitted infection clinic in Kingston, Jamaica, to compare participant self-report of recent semen exposure to actual semen exposure measured by prostate-specific antigen in vaginal swabs. Underreporting of semen exposure was significantly more frequent at follow-up than baseline, suggesting the accuracy of reports of sexual behavior may vary over time

    Vaginal recurrence of endometrial cancer: MRI characteristics and correlation with patient outcome after salvage radiation therapy

    Purpose To evaluate MRI characteristics in vaginal recurrence of endometrial cancer (EC) including tumor volume shrinkage during salvage radiotherapy, and to identify imaging features associated with survival. Methods Patients with vaginal recurrence of EC treated with external beam radiotherapy (EBRT) followed by brachytherapy (BT), and with available pelvic MRI at two time points: baseline and/or before BT were retrospectively identified from 2004 to 2017. MRI features including recurrence location and tissue characteristics on T2- and T1-weighted images were evaluated at baseline only. Tumor volumes were measured both at baseline and pre-BT. Survival rates and associations were evaluated by Cox regression and Fisher's exact test, respectively. Results Sixty-two patients with 36 baseline and 50 pre-BT pelvic MRIs were included (24/62 with both MRIs). Vaginal recurrence of EC was most commonly located in the vaginal apex (27/36, 75%). Tumors with a post-contrast enhancing peripheral rim or low T2 signal rim at baseline showed longer recurrence-free survival (RFS) (HR 0.2, 95% CI 0.1-0.9, P < 0.05 adjusted for histology; HR 0.2, 95% CI 0.1-0.8, P < 0.05, respectively). The median tumor shrinkage at pre-BT was 69% (range 1-99%). Neither absolute tumor volumes nor volume regression at pre-BT were associated with RFS. Lymphovascular space invasion (LVSI) at hysterectomy and adjuvant RT were associated with recurrence involving the distal vagina (both P < 0.05). Conclusion Vaginal recurrences with rim enhancement at baseline MRI predicted improved RFS, while tumor volume shrinkage at pre-BT did not. Distal vaginal recurrence was more common in patients with LVSI and adjuvant RT at EC diagnosis

    Elucidation of the Bovine Intramammary Bacteriome and Resistome from healthy cows of Swiss dairy farms in the Canton Tessin

    Healthy, untreated cows of nine dairy herds from the Swiss Canton Tessin were analyzed three times within one year to identify the most abundant species of the intramammary bacteriome. Aseptically collected milk samples were cultured and bacteria identified using MALDI-TOF. Of 256 cows analyzed, 96% were bacteriologically positive and 80% of the 1,024 quarters were positive for at least one bacterial species. 84.5% of the quarters were healthy with somatic cell counts (SCC) &lt; 200,000 cells/mL, whereas 15.5% of the quarters showed a subclinical mastitis (SCC ≥ 200,000 cells/mL). We could assign 1,288 isolates to 104 different bacterial species including 23 predominant species. Non-aureus staphylococci and mammaliicocci (NASM) were most prevalent (14 different species; 73.5% quarters). Staphylococcus xylosus and Mammaliicoccus sciuri accounted for 74.7% of all NASM isolates. To describe the intramammary resistome, 350 isolates of the predominant species were selected and subjected to short-read whole genome sequencing (WGS) and phenotypic antibiotic resistance profiling. While complete genomes of eight type strains were available, the remaining 15 were de novo assembled with long reads as a resource for the community. The 23 complete genomes served for reference-based assembly of the Illumina WGS data. Both chromosomes and mobile genetic elements were examined for antibiotic resistance genes (ARGs) using in-house and online software tools. ARGs were then correlated with phenotypic antibiotic resistance data from minimum inhibitory concentration (MIC). Phenotypic and genomic antimicrobial resistance was isolate-specific. Resistance to clindamycin and oxacillin was most frequently observed (65 and 30%) in Staphylococcus xylosus but could not be linked to chromosomal or plasmid-borne ARGs. However, in several cases, the observed antimicrobial resistance could be explained by the presence of mobile genetic elements like tetK carried on small plasmids. This represents a possible mechanism of transfer between non-pathogenic bacteria and pathogens of the mammary gland within and between herds. The-to our knowledge-most extensive bacteriome reported and the first attempt to link it with the resistome promise to profoundly affect veterinary bacteriology in the future and are highly relevant in a One Health context, in particular for mastitis, the treatment of which still heavily relies on antibiotics

    Global, regional, and national incidence, prevalence, and years lived with disability for 354 diseases and injuries for 195 countries and territories, 1990-2017: a systematic analysis for the Global Burden of Disease Study 2017.

    The Global Burden of Diseases, Injuries and Risk Factors 2017 includes a comprehensive assessment of incidence, prevalence, and years lived with disability (YLDs) for 354 causes in 195 countries and territories from 1990 to 2017. Previous GBD studies have shown how the decline of mortality rates from 1990 to 2016 has led to an increase in life expectancy, an ageing global population, and an expansion of the non-fatal burden of disease and injury. These studies have also shown how a substantial portion of the world's population experiences non-fatal health loss with considerable heterogeneity among different causes, locations, ages, and sexes. Ongoing objectives of the GBD study include increasing the level of estimation detail, improving analytical strategies, and increasing the amount of high-quality data. METHODS: We estimated incidence and prevalence for 354 diseases and injuries and 3484 sequelae. We used an updated and extensive body of literature studies, survey data, surveillance data, inpatient admission records, outpatient visit records, and health insurance claims, and additionally used results from cause of death models to inform estimates using a total of 68 781 data sources. Newly available clinical data from India, Iran, Japan, Jordan, Nepal, China, Brazil, Norway, and Italy were incorporated, as well as updated claims data from the USA and new claims data from Taiwan (province of China) and Singapore. We used DisMod-MR 2.1, a Bayesian meta-regression tool, as the main method of estimation, ensuring consistency between rates of incidence, prevalence, remission, and cause of death for each condition. YLDs were estimated as the product of a prevalence estimate and a disability weight for health states of each mutually exclusive sequela, adjusted for comorbidity. We updated the Socio-demographic Index (SDI), a summary development indicator of income per capita, years of schooling, and total fertility rate. Additionally, we calculated differences between male and female YLDs to identify divergent trends across sexes.
