    Out-of-school lives of physically disabled children and young people in the United Kingdom: A qualitative literature review

    Currently there appears to be few opportunities and little evidence of physically disabled children and young people (C&YP) participating in mainstream social activities. A qualitative review was undertaken to examine the factors affecting physically disabled C&YP (8–15 years) in the United Kingdom participating in out-of-school activities. Views and experiences were explored from the perspective of the service users and providers to assess current provision and to determine the need for future research into factors that may affect participation. Searches were conducted across eight databases, the references of the included studies were checked and the websites were searched. Studies that used a qualitative design that examined the views relating to out-of-school activities were included. Nine papers were identified, which included three peer-reviewed papers and six pieces of grey literature and pertinent government documents to include views and experiences of out-of-school activity provision. The main themes emerging from the review were the need for social inclusion, out-of-school activities run by volunteers and accessibility, with threads throughout, which require further research including parental influence, provision, training and attitudes. This review highlights the absence of the service user’s voice and sheds light on the limited provision and barriers affecting participation in out-of-school activities

    Revisiting the 'Missing Middle' in English Sub-National Governance

    In the light of the new Coalition Government’s proposed ‘rescaling’ of sub-national governance away from the regional level, it is an opportune time to re-consider the strength and weaknesses of the city or sub-regional approach to economic development and to search, once more, for the ‘missing middle’ in English Governance. In this context, the article initially assesses the case for city or sub regions as tiers of economic governance, before examining the lessons to be learnt from the experiences of the existing city regions in the North East of England. It argues that while contemporary plans to develop Local Enterprise Partnerships (LEPs) can be usefully considered within the context of the emerging city regional developments under the previous Labour Governments, a number of important challenges remain, particularly in relation to ensuring accountable structures of governance, a range of appropriate functions, adequate funding, and comprehensive coverage across a variety of sub-regional contexts. While the proposals of the new Government create the necessary ‘space’ to develop sub-regional bodies and offer genuine opportunities for both city and county LEPs, the scale of the sub-regional challenge should not be underestimated, particularly given the context of economic recession and major reductions in the public sector

    Musculoskeletal complaints among Italian X-ray technology students: a cross-sectional questionnaire survey

    <p>Abstract</p> <p>Background</p> <p>There is a high prevalence of musculoskeletal disorders among healthcare professional students. Although recent studies show musculoskeletal disorders are a common problem among X-ray technologists, there are no data on these disorders among students of this healthcare profession. We have therefore estimated the prevalence of musculoskeletal complaints among a group of X-ray technology students.</p> <p>Methods</p> <p>The students (n = 109) currently attending the 3-year X-ray technologist school at a large University in the Apulia region of Southern Italy were recruited for the study, with a 100% participation rate. A questionnaire collected data concerning personal characteristics, physical exposure during training activities, and the presence of musculoskeletal symptoms in the neck, shoulders, low back, hand/wrist and legs.</p> <p>Results</p> <p>The prevalence of complaints in any body site over the previous 12 months was 37%. Low back pain was the most frequently reported symptom (27%), followed by neck (16%), shoulder (11%), leg (8%) and hand/wrist (5%) pain. Poor physical activity was associated with the complaints.</p> <p>Conclusions</p> <p>Our study showed prevalence rates of musculoskeletal complaints among X-ray technology students to be somewhat high, representing about half of those found in Italian technologists. The most common musculoskeletal problem was low back pain, which had also been found in research conducted among nursing students. Our research also showed a significant association between poor physical activity and the presence of musculoskeletal disorders in young university students.</p

    Zero-Hour Contracts and Stress in UK Domiciliary Care Workers

    UK domiciliary care workers play a vital role in maintaining and improving the lives of service users who have a variety of needs. Around 60% of these employees work under zero-hours contracts but, while it is known that conditions such as temporary and shift working can influence employee health and performance, zero-hours have not been widely investigated. This project sought to firstly investigate the stress associated with working as a domiciliary care worker, as well as comparing the experiences of employees contracted to zero hours with those contracted to at least 16 hours per week. Twenty-nine semi-structured interviews (15 zero-hour, 14 contracted hours) were conducted in the West Midlands of the UK and analysed using thematic analysis. Across all participants, four predominant stressors were found. Firstly, level of pay for what is a job with high levels of responsibility were poor. Secondly, participants described struggling to maintain an adequate work-life balance due to the varied timings of visits, as well as rude and aggressive behaviour from both service users and their families. Lastly, a lack of peer support and poor care from peers was discussed. However, every respondent described the positive relationships that they develop with service users being a distinct stress-reliever. Zero-hours respondents discussed two further stressors. Power refers to the relationship between employee and management, with respondents describing the balance of power being with management. Uncertainty reflected respondents not having set hours of work or pay, and thus not being able to plan in their personal lives and sometimes not being able to pay bills. Findings suggest that domiciliary care workers are exposed to a range of stressors, with zero-hours adding to these. Further research should look into methods to improve both the job role for workers, and redress the power relationships for those with zero-hours contracts

    A small-N cross-sectional study of British unions' environmental attitudes and activism - and the prospect of a green-led renewal

    Unions understand the environmental agenda as a technocentric one but also believe it can function as a vehicle for renewal. It is developing slowly, with unions behaving cautiously—resources are scarce. Although popular with members, there is limited evidence that it is effective as a recruitment tool and whilst employers are willing to work in partnership with unions on it, this may confer only phony insider status. Overall, the agenda has limited appeal to the types of employees and employers unions must recruit in order to grow. Identifying a clear environmental premium for members may help

    Workplace wellness using online learning tools in a healthcare setting

    The aim was to develop and evaluate an online learning tool for use with UK healthcare employees, healthcare educators and healthcare students, to increase knowledge of workplace wellness as an important public health issue. A ‘Workplace Wellness’ e-learning tool was developed and peer-reviewed by 14 topic experts. This focused on six key areas relating to workplace wellness: work-related stress, musculoskeletal disorders, diet and nutrition, physical activity, smoking and alcohol consumption. Each key area provided current evidence-based information on causes and consequences, access to UK government reports and national statistics, and guidance on actions that could be taken to improve health within a workplace setting. 188 users (93.1% female, age 18–60) completed online knowledge questionnaires before (n = 188) and after (n = 88) exposure to the online learning tool. Baseline knowledge of workplace wellness was poor (n = 188; mean accuracy 47.6%, s.d. 11.94). Knowledge significantly improved from baseline to post-intervention (mean accuracy = 77.5%, s.d. 13.71) (t(75) = −14.801, p < 0.0005) with knowledge increases evident for all included topics areas. Usability evaluation showed that participants perceived the tool to be useful (96.4%), engaging (73.8%) and would recommend it to others (86.9%). Healthcare professionals, healthcare educators and pre-registered healthcare students held positive attitudes towards online learning, indicating scope for development of further online packages relating to other important health parameters

    Development of risk models for the prediction of new or worsening acute kidney injury on or during hospital admission: a cohort and nested study

    Background: Acute kidney injury (AKI) is a common clinical problem with significant morbidity and mortality. All hospitalised patients are at risk. AKI is often preventable and reversible; however, the 2009 National Confidential Enquiry into Patient Outcome and Death highlighted systematic failings of identification and management, and recommended risk assessment of all emergency admissions. Objectives: To develop three predictive models to stratify the risk of (1) AKI on arrival in hospital; (2) developing AKI during admission; and (3) worsening AKI if already present; and also to (4) develop a clinical algorithm for patients admitted to hospital and explore effective methods of delivery of this information at the point of care. Study design: Quantitative methodology (1) to formulate predictive risk models and (2) to validate the models in both our population and a second population. Qualitative methodology to plan clinical decision support system (CDSS) development and effective integration into clinical care. Data analysis: Quantitative – both traditional and Bayesian regression methods were used. Traditional methods were performed using ordinal logistic regression with univariable analyses to inform the development of multivariable analyses. Backwards selection was used to retain only statistically significant variables in the final models. The models were validated using actual and predicted probabilities, an area under the receiver operating characteristic (AUROC) curve analysis and the Hosmer–Lemeshow test.     Life after Regions? The Evolution of City-regionalism in England

Following the demise of English regional policy in 2004, city-regions have come to represent the in vogue spatial scale amongst policy elites. The result has been a proliferation of actual and proposed policies and institutions designed to operate at a, variously defined, city-regional scale in England. Nevertheless, attempts to build a city-regional tier of governance have been tentative and lacking coherence. Alongside this city-regions are to be found emerging alongside existing tiers of economic governance and spatial planning. Arguing that what we are witnessing is not ‘life after regions’ but life with (or alongside) regions, the analysis presented argues that to understand why contemporary state reorganisation results in a multiplication of the scales economic governance and spatial planning we must recognise how the state shapes policies in such a way as to protect its legitimacy for maintain regulatory control and management of the economy. The final section relates these findings to wider debates on state rescaling and speculates on the future role of transition models in sociospatial theory

    Sports mega-events as part of a nation's soft power strategy: The cases of Germany (2006) and the UK (2012)

    The potential positive impact on a nation's image has moved from being a welcome consequence to a significant justification for investing in hosting sports mega-events. Mobilising Joseph Nye's concept of 'soft power' we empirically investigate Germany's strategic use of a sports 'mega' (the 2006 FIFA World Cup) to successfully alter their image among 'foreign publics'. We then analyse the example of the London 2012 Olympic and Paralympic Games for Britain's international prestige. For both cases we draw on and analyse official government documents and newspaper sources written before and after the Games. The findings reveal the stark contrast between Germany's and Britain's sport and 'soft power' strategies: the former undertook a long-term, well-planned and resourced approach to altering its poor international image; the latter appeared far less concerned about capitalising on the Olympics to enhance Britain's (seemingly robust) international image
