    Neither hereditary periodic fever nor periodic fever, aphthae, pharingitis, adenitis: Undifferentiated periodic fever in a tertiary pediatric center

    AIM: To describe the frequency and clinical characteristics of patients with undifferentiated periodic fever (UPF) and to investigate whether a clinical classification of UPF based on the PRINTO-Eurofever score can help predicting the response to treatment and the outcome at follow-up. METHODS: Clinical and therapeutic information of patients with recurrent fever who presented at a single pediatric rheumatology center from January 2006 through April 2016 were retrospectively collected. Patients with a clinical suspicion of hereditary periodic fever (HPF) syndrome and patients with clinical picture of periodic fever, aphthae, pharingitis, adenitis (PFAPA) who were refractory to tonsillectomy underwent molecular analysis of five HPF-related genes: MEFV (NM_000243.2), MVK (NM_000431.3), TNFRSF1A (NM_001065.3), NLRP3 (NM_001079821.2), NLRP12 (NM_001277126.1). All patients who had a negative genetic result were defined as UPF and further investigated. PRINTO-Eurofever score for clinical diagnosis of HPF was calculated in all cases. RESULTS: Of the 221 patients evaluated for periodic fever, twelve subjects with a clinical picture of PFAPA who were refractory to tonsillectomy and 22 subjects with a clinical suspicion of HPF underwent genetic analysis. Twenty-three patients (10.4%) resulted negative and were classified as UPF. The median age at presentation of patients with UPF was 9.5 mo (IQR 4-24). Patients with UPF had a higher frequency of aphthae (52.2% vs 0%, P = 0.0026) and musculoskeletal pain (65.2% vs 18.2%, P = 0.0255) than patients with genetic confirmed HPF. Also, patients with UPF had a higher frequency of aphthous stomatitis (52.2% vs 10.7%, P < 0.0001), musculoskeletal pain (65.2% vs 8,0%, P < 0.0001), and abdominal pain (52.2% vs 4.8%, P < 0.0001) and a lower frequency of pharyngitis (56.6% vs 81.3%, P = 0.0127) compared with typical PFAPA in the same cohort. Twenty-one of 23 patients with UPF (91.3%) received steroids, being effective in 16; 13 (56.2%) were given colchicine, which was effective in 6. Symptoms resolution occurred in 2 patients with UPF at last follow-up. Classification according to the PRINTO-Eurofever score did not correlate with treatment response and prognosis. CONCLUSION: UPF is not a rare diagnosis among patients with periodic fever. Clinical presentation place UPF half way on a clinical spectrum between PFAPA and HPF. The PRINTO-Eurofever score is not useful to predict clinical outcome and treatment response in these patients

    Por trás dos sorrisos: sofrimento moral na oferta do cuidado oncológico

    Objective: to understand the moral experience of a nursing team that does not take care of cancer patients. Method: Qualitative study, based on a non-referential basis of Medical Anthropology, which recognizes the human being as being aware of their thoughts and practices, as well as understanding that the problems that permeate health emerge from the interaction of two cultural and social factors, environmental, political and economic contexts. Research was carried out in the Oncology Unit of a reference hospital that does not treat people with cancer in Brazil. To select two intentionally occurring, non-probabilistic participants. An analysis was an issue. Results: As a research, it is possible to affirm that you are professionals in cancer care because you have a philosophy of differentiated care, a bond with the patient and a feeling of relief related to moral growth. Conclusion: Based on two identified results, it is considered that nurses who practice cancer care live with moral and cultural issues that involve the care of cancer patients, generating moral support associated with personal, cultural and institutional values, according to the participants identified.Objetivo: comprender la experiencia moral de un equipo de enfermería que no atiende a pacientes con cáncer. Método: Estudio cualitativo, basado en una base no referencial de la Antropología Médica, que reconoce al ser humano como consciente de sus pensamientos y prácticas, así como comprender que los problemas que permean la salud surgen de la interacción de dos factores culturales y sociales. , contextos ambientales, políticos y económicos. La investigación se realizó en la Unidad de Oncología de un hospital de referencia que no atiende a personas con cáncer en Brasil. Seleccionar dos participantes no probabilísticos que ocurran intencionalmente. Un análisis fue un problema. Resultados: Como investigación, es posible afirmar que son profesionales en la atención del cáncer porque tienen una filosofía de atención diferenciada, un vínculo con el paciente y un sentimiento de alivio relacionado con el crecimiento moral. Conclusión: A partir de dos resultados identificados, se considera que los enfermeros que practican el cuidado del cáncer conviven con cuestiones morales y culturales que involucran el cuidado del paciente con cáncer, generando un apoyo moral asociado a valores personales, culturales e institucionales, según los participantes identificados.Objetivo: compreender a experiência moral de uma equipe de enfermagem no cuidado ao paciente oncológico. Método: Estudo qualitativo, com base no referencial da Antropologia Médica, que reconhece o ser humano como um ser consciente de seus pensamentos e práticas, assim compreende-se que os problemas que permeiam a saúde emergem da interação dos contextos culturais, sociais, ambientais, políticos e econômicos. A pesquisa foi realizada na Unidade Oncológica de um hospital referência no tratamento a pessoas com câncer no sul do Brasil. A seleção dos participantes ocorreu de modo intencional, não probabilístico. A análise foi a temática indutiva. Resultado: Com a pesquisa é possível afirmar que os profissionais do cuidado oncológico apresentaram uma filosofia do cuidado diferenciado, um vínculo com os pacientes e sentimento de sofrimento relacionados ao julgamento moral. Conclusão: A partir dos resultados identificados considera-se que o enfermeiro que pratica cuidados oncológicos lida com questões morais e culturais que envolvem o cuidado ao paciente oncológico, gerando um sofrimento moral associado a valores pessoais, culturais e institucionais como os identificados nas participantes

    Brain imaging of the cortex in ADHD: a coordinated analysis of large-scale clinical and population-based samples

    Objective: Neuroimaging studies show structural alterations of various brain regions in children and adults with attention deficit hyperactivity disorder (ADHD), although nonreplications are frequent. The authors sought to identify cortical characteristics related to ADHD using large-scale studies. Methods: Cortical thickness and surface area (based on the Desikan–Killiany atlas) were compared between case subjects with ADHD (N=2,246) and control subjects (N=1,934) for children, adolescents, and adults separately in ENIGMA-ADHD, a consortium of 36 centers. To assess familial effects on cortical measures, case subjects, unaffected siblings, and control subjects in the NeuroIMAGE study (N=506) were compared. Associations of the attention scale from the Child Behavior Checklist with cortical measures were determined in a pediatric population sample (Generation-R, N=2,707). Results: In the ENIGMA-ADHD sample, lower surface area values were found in children with ADHD, mainly in frontal, cingulate, and temporal regions; the largest significant effect was for total surface area (Cohen’s d=−0.21). Fusiform gyrus and temporal pole cortical thickness was also lower in children with ADHD. Neither surface area nor thickness differences were found in the adolescent or adult groups. Familial effects were seen for surface area in several regions. In an overlapping set of regions, surface area, but not thickness, was associated with attention problems in the Generation-R sample. Conclusions: Subtle differences in cortical surface area are widespread in children but not adolescents and adults with ADHD, confirming involvement of the frontal cortex and highlighting regions deserving further attention. Notably, the alterations behave like endophenotypes in families and are linked to ADHD symptoms in the population, extending evidence that ADHD behaves as a continuous trait in the population. Future longitudinal studies should clarify individual lifespan trajectories that lead to nonsignificant findings in adolescent and adult groups despite the presence of an ADHD diagnosis

    Human subcortical brain asymmetries in 15,847 people worldwide reveal effects of age and sex

    The two hemispheres of the human brain differ functionally and structurally. Despite over a century of research, the extent to which brain asymmetry is influenced by sex, handedness, age, and genetic factors is still controversial. Here we present the largest ever analysis of subcortical brain asymmetries, in a harmonized multi-site study using meta-analysis methods. Volumetric asymmetry of seven subcortical structures was assessed in 15,847 MRI scans from 52 datasets worldwide. There were sex differences in the asymmetry of the globus pallidus and putamen. Heritability estimates, derived from 1170 subjects belonging to 71 extended pedigrees, revealed that additive genetic factors influenced the asymmetry of these two structures and that of the hippocampus and thalamus. Handedness had no detectable effect on subcortical asymmetries, even in this unprecedented sample size, but the asymmetry of the putamen varied with age. Genetic drivers of asymmetry in the hippocampus, thalamus and basal ganglia may affect variability in human cognition, including susceptibility to psychiatric disorders

    Analysis of structural brain asymmetries in attention-deficit/hyperactivity disorder in 39 datasets

    Objective Some studies have suggested alterations of structural brain asymmetry in attention-deficit/hyperactivity disorder (ADHD), but findings have been contradictory and based on small samples. Here, we performed the largest ever analysis of brain left-right asymmetry in ADHD, using 39 datasets of the ENIGMA consortium. Methods We analyzed asymmetry of subcortical and cerebral cortical structures in up to 1,933 people with ADHD and 1,829 unaffected controls. Asymmetry Indexes (AIs) were calculated per participant for each bilaterally paired measure, and linear mixed effects modeling was applied separately in children, adolescents, adults, and the total sample, to test exhaustively for potential associations of ADHD with structural brain asymmetries. Results There was no evidence for altered caudate nucleus asymmetry in ADHD, in contrast to prior literature. In children, there was less rightward asymmetry of the total hemispheric surface area compared to controls (t = 2.1, p = .04). Lower rightward asymmetry of medial orbitofrontal cortex surface area in ADHD (t = 2.7, p = .01) was similar to a recent finding for autism spectrum disorder. There were also some differences in cortical thickness asymmetry across age groups. In adults with ADHD, globus pallidus asymmetry was altered compared to those without ADHD. However, all effects were small (Cohen’s d from −0.18 to 0.18) and would not survive study-wide correction for multiple testing. Conclusion Prior studies of altered structural brain asymmetry in ADHD were likely underpowered to detect the small effects reported here. Altered structural asymmetry is unlikely to provide a useful biomarker for ADHD, but may provide neurobiological insights into the trait

    <scp>ReSurveyEurope</scp>: A database of resurveyed vegetation plots in Europe

    AbstractAimsWe introduce ReSurveyEurope — a new data source of resurveyed vegetation plots in Europe, compiled by a collaborative network of vegetation scientists. We describe the scope of this initiative, provide an overview of currently available data, governance, data contribution rules, and accessibility. In addition, we outline further steps, including potential research questions.ResultsReSurveyEurope includes resurveyed vegetation plots from all habitats. Version 1.0 of ReSurveyEurope contains 283,135 observations (i.e., individual surveys of each plot) from 79,190 plots sampled in 449 independent resurvey projects. Of these, 62,139 (78%) are permanent plots, that is, marked in situ, or located with GPS, which allow for high spatial accuracy in resurvey. The remaining 17,051 (22%) plots are from studies in which plots from the initial survey could not be exactly relocated. Four data sets, which together account for 28,470 (36%) plots, provide only presence/absence information on plant species, while the remaining 50,720 (64%) plots contain abundance information (e.g., percentage cover or cover–abundance classes such as variants of the Braun‐Blanquet scale). The oldest plots were sampled in 1911 in the Swiss Alps, while most plots were sampled between 1950 and 2020.ConclusionsReSurveyEurope is a new resource to address a wide range of research questions on fine‐scale changes in European vegetation. The initiative is devoted to an inclusive and transparent governance and data usage approach, based on slightly adapted rules of the well‐established European Vegetation Archive (EVA). ReSurveyEurope data are ready for use, and proposals for analyses of the data set can be submitted at any time to the coordinators. Still, further data contributions are highly welcome.</jats:sec

En esta serie de Pastillas Sonoras, titulada Mitos que circulan como Verdades, analizaremos algunos de estos mensajes, tratando de aclararlos y de evitar su multiplicación. Pastillas sonoras es una producción colectiva entre el equipo de comunicación para la salud de las Brigadas Sanitarias de la Facultad de Ciencias Exactas de la UNLPy ADNCiencia. Con la atención puesta en la comunidad y partiendo de un concepto de salud multidimensional, un equipo integrado por profesionales del sistema de salud, de la comunicación y el diseño multimedial así como docentes y estudiantes de las distintas carreras, proponen respuestas a los interrogantes que circulan en torno a la pandemia COVID-19 y ofrecen una mirada que intenta desterrar mitos y noticias falsas en tiempos de Infodemia.
