    A review of community electrical energy systems

    This paper is aimed at new entrants into the field of electrical community energy. It reviews some of the work that is underway into community electrical energy projects. This review includes a summary of key issues and components which need consideration including some or all of the following; demand side management, energy storage (including vehicle to grid) and renewable generation. The paper looks further into the energy management schemes of these projects and summarises previously published methodology in the area

    Does a local financial incentive scheme reduce inequalities in the delivery of clinical care in a socially deprived community? A longitudinal data analysis.

    BACKGROUND: Socioeconomic deprivation is associated with inequalities in health care and outcomes. Despite concerns that the Quality and Outcomes Framework pay-for-performance scheme in the UK would exacerbate inequalities in primary care delivery, gaps closed over time. Local schemes were promoted as a means of improving clinical engagement by addressing local health priorities. We evaluated equity in achievement of target indicators and practice income for one local scheme. METHODS: We undertook a longitudinal survey over four years of routinely recorded clinical data for all 83 primary care practices. Sixteen indicators were developed that covered five local clinical and public health priorities: weight management; alcohol consumption; learning disabilities; osteoporosis; and chlamydia screening. Clinical indicators were logit transformed from a percentage achievement scale and modelled allowing for clustering of repeated measures within practices. This enabled our study of target achievements over time with respect to deprivation. Practice income was also explored. RESULTS: Higher practice deprivation was associated with poorer performance for five indicators: alcohol use registration (OR 0.97; 95 % confidence interval 0.96,0.99); recorded chlamydia test result (OR 0.97; 0.94,0.99); osteoporosis registration (OR 0.98; 0.97,0.99); registration of repeat prednisolone prescription (OR 0.98; 0.96,0.99); and prednisolone registration with record of dual energy X-ray absorptiometry (DEXA) scan/referral (OR 0.92; 0.86,0.97); practices in deprived areas performed better for one indicator (registration of osteoporotic fragility fracture (OR 1.26; 1.04,1.51). The deprivation-achievement gap widened for one indicator (registered females aged 65-74 with a fracture referred for a DEXA scan; OR 0.97; 0.95,0.99). Two other indicators indicated a similar trend over two years before being withdrawn (registration of fragility fracture and over-75 s with a fragility fracture assessed and treated for osteoporosis risk). For one indicator the deprivation-achievement gap reduced over time (repeat prednisolone prescription (OR 1.01; 1.01,1.01). Larger practices and those serving more affluent areas earned more income per patient than smaller practices and those serving more deprived areas (t = -3.99; p =0.0001). CONCLUSIONS: Any gaps in achievement between practices were modest but mostly sustained or widened over the duration of the scheme. Given that financial rewards may not reflect the amount of work undertaken by practices serving more deprived patients, future pay-for-performance schemes also need to address fairness of rewards in relation to workload

    Excited-state barrier controls E → Z photoisomerization in p-hydroxycinnamate biochromophores

    Molecules based on the deprotonated p-hydroxycinnamate moiety are widespread in nature, including serving as UV filters in the leaves of plants and as the biochromophore in photoactive yellow protein. The photophysical behavior of these chromophores is centered around a rapid E → Z photoisomerization by passage through a conical intersection seam. Here, we use photoisomerization and photodissociation action spectroscopies with deprotonated 4-hydroxybenzal acetone (pCK–) to characterize a wavelength-dependent bifurcation between electron autodetachment (spontaneous ejection of an electron from the S1 state because it is situated in the detachment continuum) and E → Z photoisomerization. While autodetachment occurs across the entire S1(ππ*) band (370–480 nm), E → Z photoisomerization occurs only over a blue portion of the band (370–430 nm). No E → Z photoisomerization is observed when the ketone functional group in pCK– is replaced with an ester or carboxylic acid. The wavelength-dependent bifurcation is consistent with potential energy surface calculations showing that a barrier separates the Franck–Condon region from the E → Z isomerizing conical intersection. The barrier height, which is substantially higher in the gas phase than in solution, depends on the functional group and governs whether E → Z photoisomerization occurs more rapidly than autodetachment

In this case the reported capital ratio of a major British building society was enhanced using a sale and leaseback transaction with a related party and a change in depreciation policy, methods which reflected limited alternatives. Analysis of the case is set in the context of the sector and addresses the questions of whether these manipulations were within then-prevailing generally accepted accounting principles and why, despite disclosure in the society's financial statements, they failed to attract public comment or concern, regulatory action or an audit qualification. In examining a major British mutual financial organization we depart from traditional analyses of managerial discretion in accounting choices in British companies

    Obturator Nerve Block with Botulinum Toxin Type B for Patient with Adductor Thigh Muscle Spasm -A Case Report-

    Obturator nerve block has been commonly used for pain management to prevent involuntary reflex of the adductor thigh muscles. One of several options for this block is chemical neurolysis. Neurolysis is done with chemical agents. Chemical agents used in the neurolysis of the obturator nerve have been alcohol, phenol, and botulinum toxin. In the current case, a patient with spasticity of the adductor thigh muscle due to cervical cord injury had obturator nerve neurolysis done with botulinum toxin type B (BoNT-B). Most of the previous studies have used BoNT-A with only a few reports that have used BoNT-B. BoNT-B has several advantages and disadvantages over BoNT-A. Thus, we report herein a patient who successfully received obturator nerve neurolysis using BoNT-B to treat adductor thigh muscle spasm

    Deindustrialization and the moral economy of the Scottish coalfields, 1947 to 1991

    The long-running deindustrialization in the Scottish coalfields, the consequence of political decisions, took place in three distinct periods analyzed here: “restructuring,” 1958–1967, when, in response to union activism, a large number of closures was offset by government and industry initiatives to provide or stimulate alternative employment; “stabilization,” 1968–1977, when closures were minimized as the broader industrial economy slowed; and then “accelerated contraction,” 1978–1987, within the larger program of economic restructuring engineered by Margaret Thatcher's Conservative UK governments. Moral economy arguments shaped the debate about deindustrialization in the first two phases: closures were legitimate only where agreed to by the workforce, who would in turn receive guaranteed economic security. These factors did not apply in the final phase, when closures were enforced and redundant miners had limited employment alternatives

    The impact of COVID-19 on young people’s mental health, wellbeing and routine from a European perspective: A co-produced qualitative systematic review

    Background The impact of the Covid-19 pandemic on young people’s (YP) mental health has been mixed. Systematic reviews to date have focused predominantly on quantitative studies and lacked involvement from YP with lived experience of mental health difficulties. Therefore, our primary aim was to conduct a qualitative systematic review to examine the perceived impact of the Covid-19 pandemic on YP’s (aged 10–24) mental health and wellbeing across Europe. Methods and findings We searched MEDLINE, PsycINFO, Embase, Web of Science, MEDRXIV, OSF preprints, Google, and voluntary sector websites for studies published from 1st January 2020 to 15th November 2022. European studies were included if they reported qualitative data that could be extracted on YP’s (aged 10–24) own perspectives of their experiences of Covid-19 and related disruptions to their mental health and wellbeing. Screening, data extraction and appraisal was conducted independently in duplicate by researchers and YP with lived experience of mental health difficulties (co-researchers). Confidence was assessed using the Confidence in the Evidence from Reviews of Qualitative Research (CERQual) approach. We co-produced an adapted narrative thematic synthesis with co-researchers. This study is registered with PROSPERO, CRD42021251578. We found 82 publications and included 77 unique studies in our narrative synthesis. Most studies were from the UK (n = 50; 65%); and generated data during the first Covid-19 wave (March-May 2020; n = 33; 43%). Across the 79,491 participants, views, and experiences of YP minoritised by ethnicity and sexual orientation, and from marginalised or vulnerable YP were limited. Five synthesised themes were identified: negative impact of pandemic information and restrictions on wellbeing; education and learning on wellbeing; social connection to prevent loneliness and disconnection; emotional, lifestyle and behavioural changes; and mental health support. YP’s mental health and wellbeing across Europe were reported to have fluctuated during the pandemic. Challenges were similar but coping strategies to manage the impact of these challenges on mental health varied across person, study, and country. Short-term impacts were related to the consequences of changing restrictions on social connection, day-to-day lifestyle, and education set-up. However, YP identified potential issues in these areas going forward, and therefore stressed the importance of ongoing long-term support in education, learning and mental health post-Covid-19. Conclusions Our findings map onto the complex picture seen from quantitative systematic reviews regarding the impact of Covid-19 on YP’s mental health. The comparatively little qualitative data found in our review means there is an urgent need for more high-quality qualitative research outside of the UK and/or about the experiences of minoritised groups to ensure all voices are heard and everyone is getting the support they need following the pandemic. YP’s voices need to be prioritised in decision-making processes on education, self-care strategies, and mental health and wellbeing, to drive impactful, meaningful policy changes in anticipation of a future systemic crisis

    Nuclear fuel cycle, with a liquid ore and fuel:toward renewable energy

    To fulfill the conditions required for a nuclear renewable energy concept, one has to explore a combination of processes going from the front end of the nuclear fuel cycle to the fuel production and the energy conversion using specific fluid fuels and reactors. Extraction of uranium from a diluted fluid ore such as seawater has been studied in various countries worldwide. This extraction should be carried out parsimoniously. An extraction rate of 103 tons of U per year over centuries would not modify significantly the equilibrium concentration of uranium in the oceans (3.3 ppb). This equilibrium results from the input of 104 tons of U per year by river waters and its scavenging on the sea floor from the 1.37 × 1018 tons of water in the oceans. For a renewable uranium extraction, the use of a specific biomass material is suggested to adsorb uranium and subsequently other transition metals. The uranium loading on the biomass would be around 100 mg per kg. After contact time, the loaded material would be dried and burned (CO2 neutral) with heat conversion into electricity. The uranium ‘burning’ in a molten salt fast reactor helps to optimize the energy conversion by burning all actinide isotopes with an excellent yield for producing a maximum amount of thermal energy from fission and converting it into electricity. This optimisation can be reached by reducing the moderation and the fission product concentration in the liquid fuel/coolant. These effects can be achieved by using a maximum amount of actinides and a minimum amount of alkaline/earth alkaline elements yielding a harder neutron spectrum. Under these optimal conditions the consumption of natural uranium would be 7 tons per year and per gigawatt (GW) of produced electricity. The coupling of uranium extraction from the sea and its optimal utilisation in a molten salt fast reactor should allow nuclear energy to gain the label renewable. In addition, the amount of seawater used by a nuclear power plant to cool the last coolant fluid and the turbine would be ∼2.1 × 109 tons per year for a fast molten salt reactor, corresponding to 7 tons of natural uranium extractable per year.     Estimating geological CO2 storage security to deliver on climate mitigation

    Carbon capture and storage (CCS) can help nations meet their Paris CO2 reduction commitments cost-effectively. However, lack of confidence in geologic CO2 storage security remains a barrier to CCS implementation. Here we present a numerical program that calculates CO2 storage security and leakage to the atmosphere over 10,000 years. This combines quantitative estimates of geological subsurface CO2 retention, and of surface CO2 leakage. We calculate that realistically well-regulated storage in regions with moderate well densities has a 50% probability that leakage remains below 0.0008% per year, with over 98% of the injected CO2 retained in the subsurface over 10,000 years. An unrealistic scenario, where CO2 storage is inadequately regulated, estimates that more than 78% will be retained over 10,000 years. Our modelling results suggest that geological storage of CO2 can be a secure climate change mitigation option, but we note that long-term behaviour of CO2 in the subsurface remains a key uncertainty
