    General educators' perceptions of African American males prior to pre-referral

    The purpose of this study was to examine general education teachers' perceptions of African American males to identify specific student characteristics and other variables that influence referral decision-making. The theoretical framework used to guide this study explored tenants of Critical Race Theory (CRT) to examine the practices and beliefs of 216 educators from a mid-sized school district in North Carolina in grades kindergarten-fifth. Twelve interviewees also were chosen, from the original pool of participants, to generate data relative to referral reasons. A mixed methods approach was used to describe the identification process. Measures for this study included an on-line survey and semi-structured interviews developed by the researcher. Areas surveyed included environmental factors, hereditary factors, certain biases, low socio-economic status, students' use of culturally different speech patterns and dress, lack of clarity in school guidelines for special education referrals, subjectivity in the county referral process, and African American males being raised by extended family. Two opened questions allowed participants to address other significant aspects considered relevant for referral. The majority of the participants were Caucasian, females in their mid-thirties, who had more than 10 years of experience. Based on quantitative analysis, four factors emerged as key points for referrals. These included African American males "Raised by extended family"; "Cultural biases" among teachers; "Ineffective trainings" for teachers; and student "Environmental factors". Qualitative findings, however, both supported and refuted these findings. The findings of this study are discussed, including the implications for future research

We aimed to develop a prediction model for COVID-19 ARDS in this population and to create a simple risk score calculator for use in clinical settings. Methods: Data were derived from the COVID-19 Global Rheumatology Alliance Registry from March 24, 2020, to May 12, 2021. Seven machine learning classifiers were trained on ARDS outcomes using 83 variables obtained at COVID-19 diagnosis. Predictive performance was assessed in a US test set and was validated in patients from four countries with independent registries using area under the curve (AUC), accuracy, sensitivity, and specificity. A simple risk score calculator was developed using a regression model incorporating the most influential predictors from the best performing classifier. Results: The study included 8633 patients from 74 countries, of whom 523 (6%) had ARDS. Gradient boosting had the highest mean AUC (0.78; 95% confidence interval [CI]: 0.67-0.88) and was considered the top performing classifier. Ten predictors were identified as key risk factors and were included in a regression model. The regression model that predicted ARDS with 71% (95% CI: 61%-83%) sensitivity in the test set, and with sensitivities ranging from 61% to 80% in countries with independent registries, was used to develop the risk score calculator. Conclusion: We were able to predict ARDS with good sensitivity using information readily available at COVID-19 diagnosis. The proposed risk score calculator has the potential to guide risk stratification for treatments, such as monoclonal antibodies, that have potential to reduce COVID-19 disease progression.publishersversionepub_ahead_of_prin

Published by BMJ.Objective To investigate baseline use of biologic or targeted synthetic (b/ts) disease-modifying antirheumatic drugs (DMARDs) and COVID-19 outcomes in rheumatoid arthritis (RA). Methods We analysed the COVID-19 Global Rheumatology Alliance physician registry (from 24 March 2020 to 12 April 2021). We investigated b/tsDMARD use for RA at the clinical onset of COVID-19 (baseline): abatacept (ABA), rituximab (RTX), Janus kinase inhibitors (JAKi), interleukin 6 inhibitors (IL-6i) or tumour necrosis factor inhibitors (TNFi, reference group). The ordinal COVID-19 severity outcome was (1) no hospitalisation, (2) hospitalisation without oxygen, (3) hospitalisation with oxygen/ventilation or (4) death. We used ordinal logistic regression to estimate the OR (odds of being one level higher on the ordinal outcome) for each drug class compared with TNFi, adjusting for potential baseline confounders. Results Of 2869 people with RA (mean age 56.7 years, 80.8% female) on b/tsDMARD at the onset of COVID-19, there were 237 on ABA, 364 on RTX, 317 on IL-6i, 563 on JAKi and 1388 on TNFi. Overall, 613 (21%) were hospitalised and 157 (5.5%) died. RTX (OR 4.15, 95% CI 3.16 to 5.44) and JAKi (OR 2.06, 95% CI 1.60 to 2.65) were each associated with worse COVID-19 severity compared with TNFi. There were no associations between ABA or IL6i and COVID-19 severity. Conclusions People with RA treated with RTX or JAKi had worse COVID-19 severity than those on TNFi. The strong association of RTX and JAKi use with poor COVID-19 outcomes highlights prioritisation of risk mitigation strategies for these people.publishersversionPeer reviewe

    Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Att upptäcka matematiksvårigheter

    Ett problem inom svenska skolor är att resultaten inom matematik sjunker enligt Pisas test. Vi ville därför i vår studie få reda på mer kring dessa orsaker kring varför resultaten sjunker samt hur man kan upptäcka de eventuella matematiksvårigheterna. Vi har därför gjort en kunskapsöversikt för att med hjälp utav tidigare forskning få syn på vad det finns för orsaker till matematiksvårigheter, samt hur de olika matematiksvårigheterna yttrar sig. Vi har i dennastudie använt oss av åtta vetenskapliga texter där målet var att få med ett så brett åldersspann som möjligt. I våra texter har vi sett intressanta resultat från förskola upp till niondeklass.Resultaten visar på våra misstankar kring att det finns matematiksvårigheter och att dessa redan kan upptäckas så tidigt som i förskolan. En orsak till matematiksvårigheter är dyskalkyli. Denna svårighet grundar sig i att man har svårigheter i de aritmetiska färdigheterna vilket kopplas till neurologiska defekter. Däremot gör det inte elever oförmögna att utveckla de aritmetiska kunskaper, men de kan däremot behöva extra stöd

    Elevers kommunikation i geometri

    Att arbeta med geometri på ett varierat sätt där kommunikation är i fokus ger eleverna möjligheter att befästa sina kunskaper på ett djupare plan. En studie som har gjorts på två skolor i Sverige där syftet var att undersöka hur elever kommunicerar och använder sig av geometriska begrepp. Syftet växte fram utifrån tidigare forskning, internationella rapporter samt egna erfarenheter kring geometri från verksamheten. Genom en planering som var utformad utifrån trollbrev gavs eleverna möjligheter att kommunicera i olika situationer som ljud- och videoinspelats och synliggörs i vårt resultat. Genom insamlad empiri analyserades materialet och därefter skrevs vårt resultat fram med utgångspunkt i rubriceringar som var förbestämda kategorier: muntlig kommunikation, skriftlig kommunikation samt estetisk kommunikation. Resultatet av vår studie visar bland annat på att elever uttrycker sina kunskaper på olika sätt i olika kommunikationsmoment. Vi vill genom denna studie belysa vikten av att varierande kommunikationssätt fördjupar elevernas begreppsförmåga
