    Central bisectionectomy – a feasible alternative for centrally located liver tumors

    Clinica de Chirurgie Generală şi Transplant Hepatic, I.C.Fundeni, Bucureşti, România, Al XII-lea Congres al Asociației Chirurgilor „Nicolae Anestiadi” din Republica Moldova cu participare internațională 23-25 septembrie 2015Introducere: Bisectionectomia centrală reprezintă rezecția hepatică a segmentelor 4a, 4b, 5 și 8, definite astfel de sistemul Brisbane. Denumită anterior în diferite feluri (mesohepatectomie, hepatectomie centrală, lobectomie centrală) bisectionectomia centrală este operația de elecție pentru tumorile hepatice situate în segmentele centrale, și este de preferat rezecțiilor hepatice extinse (trisectionectomia dreaptă/stângă) pentru că minimalizează riscul insuficienței hepatice postoperatorii. Prezentare de caz: Pacienta în vârstă de 54 de ani cu dureri în hipocondrul drept, scădere ponderală cca. 8 kg / 2 luni. Bioumoral: AFP, CA 19.9 și ACE – în limite normale / AgHbs, Ac Anti-HBc, Ac Anti-HCV – negative. EDS/EDI – de aspect normal, prezintă la examenul CT o formațiune tumorală hepatică situată central (segmentele 4a, 4b, 5 și 8) ce înglobează VHM, la distanță de bifurcația venei porte. Se practică o bisectionectomie centrală, cu ligaturarea pediculilor vasculari glissonieni drept anterior (seg. 5 și 8) și stâng medial (seg. 4a și 4b). Transecțiunea parenchimului hepatic se efectuează folosindu-se “SONOPET”. Cu evoluție postoperatorie bună, pacienta se externează la 9 zile postoperator. La 6 luni postoperator probele serice AFP – în limite normale, fără semne imagistice de recidivă. Bisectionectomia centrală poate fi realizată atunci când tumora nu invadează pediculii vasculari aferenți parenchimului restant (secțiunea dreaptă posterioara / secțiunea stângă laterală) și presupune doua planuri de secțiune. Concluzii: Bisectionectomia centrală este operația de elecție pentru tumorile hepatice centrale prin care se prezervă cantitatea maxima de parenchim hepatic, minimalizându-se riscul de IHA postoperatorie. Este posibilă în cazuri atent selecționate și necesită experiență în rezecțiile hepatice majore.Introduction: By Brisbane terminology central bisectionectomy is the resection of segments 4a, 4b, 5, and 8 of the liver. Formerly called in different ways (mesohepatectomy, central hepatectomy, central lobectomy) central bisectionectomy is the elective surgery for liver tumors located in central segments (4a, 4b, 5 and 8), with better results than extensive liver resections (right / left trisectionectmy) because it minimizes the risk of postoperative liver failure. Case report: A 54 years old female, presented for right upper quadrant pain, weight loss (8 kg / 2 months); Blood tests: normal AFP; negative Ag Hbs, Ab Anti-HBc, Ab Anti-HCV. UGE/LGE – with a normal aspect; CT scan revealed centrally located liver tumor (segments 4a, 4b, 5, and 8) that includes MHV, away from the portal vein bifurcation. We performed a regulated central bisectionectomy with ligation of vascular pedicles for right anterior section (seg. 5 and 8) and those for left medial section (seg 4a and 4b). With good postoperative course, the patient was discharged in the 9th postoperative day. Six months postoperatively, the patient is good, without tumor relapse signs. Central bisectionectomy can not be performed when the tumor invades the vascular pedicles afferent to remaining parenchyma and requires two planes of transection. Conclusions: Central bisectionectomy is the best choice surgery for centrally located liver tumors which preserves the maximum amount of liver parenchyma thus minimizing the risk of postoperative liver failure

    Bovine pericardium retail preserved in glutaraldehyde and used as a vascular patch

    <p>Abstract</p> <p>Background</p> <p>In this study we evaluated the performance of bovine pericardium preserved in glutaraldehyde used as a vascular patch.</p> <p>Methods</p> <p>Fourteen young pigs, six females and eight males, weighting 10.3 - 18.4 kg were used in our study. We implanted three remnants in each pig, two in the abdominal aorta and one was juxtaposed to the peritoneum. The smooth face (SF) and rough face (RF) of each remnant were implanted turned to the vessel inner portion and one remnant was juxtaposed to the peritoneum. The animals were sacrificed in 4.5 - 8 months after surgery (75 - 109 kg). The remnants were assessed for aorta wall, fibroses formation in inner apposition and calcification related to the face turned to the vessel inner portion.</p> <p>Results</p> <p>The rough face showed a lower dilatation level compared to the face implanted in adjacent aorta. There was no difference between intensity and/or incidence of graft calcification when the superficies were compared. The bovine pericardium preserved in glutaraldehyde did not show alterations in its structure when implanted with different faces turned to the inner portion of vessel.</p> <p>Conclusion</p> <p>When turned to the inner portion of the vessel, the rough face of the remnant presented a lower dilatation in relation to the adjacent aorta and a better quality of endothelium layer and did not show a difference between intensity and/or incidence of graft calcification.</p

    Application of Graphene within Optoelectronic Devices and Transistors

    Scientists are always yearning for new and exciting ways to unlock graphene's true potential. However, recent reports suggest this two-dimensional material may harbor some unique properties, making it a viable candidate for use in optoelectronic and semiconducting devices. Whereas on one hand, graphene is highly transparent due to its atomic thickness, the material does exhibit a strong interaction with photons. This has clear advantages over existing materials used in photonic devices such as Indium-based compounds. Moreover, the material can be used to 'trap' light and alter the incident wavelength, forming the basis of the plasmonic devices. We also highlight upon graphene's nonlinear optical response to an applied electric field, and the phenomenon of saturable absorption. Within the context of logical devices, graphene has no discernible band-gap. Therefore, generating one will be of utmost importance. Amongst many others, some existing methods to open this band-gap include chemical doping, deformation of the honeycomb structure, or the use of carbon nanotubes (CNTs). We shall also discuss various designs of transistors, including those which incorporate CNTs, and others which exploit the idea of quantum tunneling. A key advantage of the CNT transistor is that ballistic transport occurs throughout the CNT channel, with short channel effects being minimized. We shall also discuss recent developments of the graphene tunneling transistor, with emphasis being placed upon its operational mechanism. Finally, we provide perspective for incorporating graphene within high frequency devices, which do not require a pre-defined band-gap.Comment: Due to be published in "Current Topics in Applied Spectroscopy and the Science of Nanomaterials" - Springer (Fall 2014).     Degradability of cross-linked polyurethanes based on synthetic polyhydroxybutyrate and modified with polylactide

    In many areas of application of conventional non-degradable cross-linked polyurethanes (PUR), there is a need for their degradation under the influence of specific environmental factors. It is practiced by incorporation of sensitive to degradation compounds (usually of natural origin) into the polyurethane structure, or by mixing them with polyurethanes. Cross-linked polyurethanes (with 10 and 30%wt amount of synthetic poly([R,S]-3-hydroxybutyrate) (R,S-PHB) in soft segments) and their physical blends with poly([d,l]-lactide) (PDLLA) were investigated and then degraded under hydrolytic (phosphate buffer solution) and oxidative (CoCl2/H2O2) conditions. The rate of degradation was monitored by changes of samples mass, morphology of surface and their thermal properties. Despite the small weight losses of samples, the changes of thermal properties of polymers and topography of their surface indicated that they were susceptible to gradual degradation under oxidative and hydrolytic conditions. Blends of PDLLA and polyurethane with 30 wt% of R,S-PHB in soft segments and PUR/PDLLA blends absorbed more water and degraded faster than polyurethane with low amount of R,S-PHB

    Sex, age, deprivation and patterns in life expectancy in Quebec, Canada: a population-based study

    <p>Abstract</p> <p>Background</p> <p>Little research has evaluated disparities in life expectancy according to material deprivation taking into account differences across the lifespan between men and women. This study investigated age- and sex-specific life expectancy differentials related to area-level material deprivation for the province of Québec, Canada from 1989-2004.</p> <p>Methods</p> <p>Age- and sex-specific life expectancy across the lifespan was calculated for three periods (1989-1992, 1995-1998, and 2001-2004) for the entire Québec population residing in 162 community groupings ranked according to decile of material deprivation. Absolute and relative measures were calculated to summarize differences between the most and least deprived deciles.</p> <p>Results</p> <p>Life expectancy differentials between the most and least deprived deciles were greatest for men. Over time, male differentials increased for age 20 or more, with little change occurring at younger ages. For women, differentials increased across the lifespan and were comparable to men at advanced ages. Despite gains in life expectancy among men relative to women, differentials between men and women were greater for most deprived relative to least deprived deciles.</p> <p>Conclusions</p> <p>Similar to the US, differentials in life expectancy associated with area-level material deprivation increased steadily in Québec from 1989-2004 for males and females of all ages. Differentials were comparable between men and women at advanced ages. Previous research indicating that life expectancy differentials between most and least deprived areas are greater in men may be due to a focus on younger age groups.</p

    Measurement of the inclusive and dijet cross-sections of b-jets in pp collisions at sqrt(s) = 7 TeV with the ATLAS detector

    The inclusive and dijet production cross-sections have been measured for jets containing b-hadrons (b-jets) in proton-proton collisions at a centre-of-mass energy of sqrt(s) = 7 TeV, using the ATLAS detector at the LHC. The measurements use data corresponding to an integrated luminosity of 34 pb^-1. The b-jets are identified using either a lifetime-based method, where secondary decay vertices of b-hadrons in jets are reconstructed using information from the tracking detectors, or a muon-based method where the presence of a muon is used to identify semileptonic decays of b-hadrons inside jets. The inclusive b-jet cross-section is measured as a function of transverse momentum in the range 20 < pT < 400 GeV and rapidity in the range |y| < 2.1. The bbbar-dijet cross-section is measured as a function of the dijet invariant mass in the range 110 < m_jj < 760 GeV, the azimuthal angle difference between the two jets and the angular variable chi in two dijet mass regions. The results are compared with next-to-leading-order QCD predictions. Good agreement is observed between the measured cross-sections and the predictions obtained using POWHEG + Pythia. MC@NLO + Herwig shows good agreement with the measured bbbar-dijet cross-section. However, it does not reproduce the measured inclusive cross-section well, particularly for central b-jets with large transverse momenta.Comment: 10 pages plus author list (21 pages total), 8 figures, 1 table, final version published in European Physical Journal

    Influenza Virus-Like Particles Containing M2 Induce Broadly Cross Protective Immunity

    Current influenza vaccines based on the hemagglutinin protein are strain specific and do not provide good protection against drifted viruses or emergence of new pandemic strains. An influenza vaccine that can confer cross-protection against antigenically different influenza A strains is highly desirable for improving public health.To develop a cross protective vaccine, we generated influenza virus-like particles containing the highly conserved M2 protein in a membrane-anchored form (M2 VLPs), and investigated their immunogenicity and breadth of cross protection. Immunization of mice with M2 VLPs induced anti-M2 antibodies binding to virions of various strains, M2 specific T cell responses, and conferred long-lasting cross protection against heterologous and heterosubtypic influenza viruses. M2 immune sera were found to play an important role in providing cross protection against heterosubtypic virus and an antigenically distinct 2009 pandemic H1N1 virus, and depletion of dendritic and macrophage cells abolished this cross protection, providing new insight into cross-protective immune mechanisms.These results suggest that presenting M2 on VLPs in a membrane-anchored form is a promising approach for developing broadly cross protective influenza vaccines

    Selective Serotonin Reuptake Inhibitor (SSRI) Antidepressants in Pregnancy and Congenital Anomalies: Analysis of Linked Databases in Wales, Norway and Funen, Denmark

    Background: Hypothesised associations between in utero exposure to selective serotonin reuptake inhibitors (SSRIs) and congenital anomalies, particularly congenital heart defects (CHD), remain controversial. We investigated the putative teratogenicity of SSRI prescription in the 91 days either side of first day of last menstrual period (LMP). Methods and Findings: Three population-based EUROCAT congenital anomaly registries- Norway (2004–2010), Wales (2000–2010) and Funen, Denmark (2000–2010)—were linked to the electronic healthcare databases holding prospectively collected prescription information for all pregnancies in the timeframes available. We included 519,117 deliveries, including foetuses terminated for congenital anomalies, with data covering pregnancy and the preceding quarter, including 462,641 with data covering pregnancy and one year either side. For SSRI exposures 91 days either side of LMP, separately and together, odds ratios with 95% confidence intervals (ORs, 95%CI) for all major anomalies were estimated. We also explored: pausing or discontinuing SSRIs preconception, confounding, high dose regimens, and, in Wales, diagnosis of depression. Results were combined in meta-analyses. SSRI prescription 91 days either side of LMP was associated with increased prevalence of severe congenital heart defects (CHD) (as defined by EUROCAT guide 1.3, 2005) (34/12,962 [0.26%] vs. 865/506,155 [0.17%] OR 1.50, 1.06–2.11), and the composite adverse outcome of 'anomaly or stillbirth' (473/12962, 3.65% vs. 15829/506,155, 3.13%, OR 1.13, 1.03–1.24). The increased prevalence of all major anomalies combined did not reach statistical significance (3.09% [400/12,962] vs. 2.67% [13,536/506,155] OR 1.09, 0.99–1.21). Adjusting for socio-economic status left ORs largely unchanged. The prevalence of anomalies and severe CHD was reduced when SSRI prescriptions were stopped or paused preconception, and increased when >1 prescription was recorded, but differences were not statistically significant. The dose-response relationship between severe CHD and SSRI dose (meta-regression OR 1.49, 1.12–1.97) was consistent with SSRI-exposure related risk. Analyses in Wales suggested no associations between anomalies and diagnosed depression. Conclusion: The additional absolute risk of teratogenesis associated with SSRIs, if causal, is small. However, the high prevalence of SSRI use augments its public health importance, justifying modifications to preconception care
