    Postpandemic virtual tutorials: implications for the learning of occupational therapy students who are doing their professional practices

    Introduction: The effects of virtualization on the learning of higher education students are still uncertain, during the year 2022 face-to-face activities returned and others remained in the online format. The latter is the subject of this study. Objective: To identify the scope and challenges of synchronous virtual tutorials, carried out in the post-pandemic period, based on the experience and the opinion of occupational therapy professors, and students from a Chilean university. Methodology: A mixed methodology is used; information is produced through a focus group with 11 professors and through a questionnaire answered by 76 students. Analysis of the thematic content of the qualitative data and descriptive statistics of the quantitative data is carried out. For the integration of the information produced, data triangulation is used. Results: Synchronous virtual tutorials are useful for the acquisition of various types of learning such as the ability to articulate theory with practice. They are space that favors emotional support, offers mutual support to students, and allows the development of social thinking, reflection, and situated. Simultaneously, the findings show the preference by the students for individual learning over the need to produce learning and knowledge collaboratively in post-pandemic times. Conclusions: It is suggested to have a significant number and variety of methodologies in synchronous virtual tutorials and that these consider the contexts in which professional practice is carried out

    Determinacion del conocimiento y satisfaccion de las personas que asisten a un Telecentro de Maule Activa.

    141 p.En la actualidad el principal organismo encargado de promover el aprendizaje digital, es El Gobierno, el cual intenta disminuir la brecha digital existente sobre todo en las zonas rurales. Para llevar a cabo este desafío El Gobierno trabaja en conjunto con varias organizaciones entre las cuales se encuentra Maule Activa. El principal objetivo de este estudio, consiste en determinar el nivel de conocimiento, satisfacción y factores que inciden en la asistencia a un Telecentro de las personas que participaron en la campaña de Alfabetización Digital 2005 de Maule Activa. Para lograr este objetivo, el estudio se dividió en dos etapas: La primera etapa, un estudio cualitativo que consistió en un grupo foco con 10 operadores correspondientes a los Telecentros de las comunas de Talca, Maule, Hualañé, Rauco, Licantén, Colbún, Pencahue, San Javier, Curepto y Constitución, también se realizaron entrevistas a expertos para conocer las nuevas necesidades en el uso de las TIC’s. La segunda etapa, una encuesta aplicada a los participantes de la campaña de alfabetización digital que se llevó a cabo desde el 25 de Julio al 06 de Agosto del año 2005 en los 30 Telecentros de la región y en algunas otras dependencias como la Universidad de Talca en sus sedes de Talca y Curicó. El muestreo utilizado es este estudio fue del tipo estratificado, donde se dividió la séptima región en 30 comunas dando una muestra para el cuestionario 1 y 2 de 404 y 335 personas respectivamente. Como resultado general del estudio, se puede decir que la mayoría de la gente sí tenía conocimiento de Maule Activa y sus Telecentros, sin embargo, la imagen que tiene esta sobre la organización no concuerda con la realidad, ya que no tienen claro cuales son los servicios que presta. En cuanto a la satisfacción de las personas que participaron en la Campaña de Alfabetización Digital, el estudio entregó que el 97% de la muestra total quedó satisfecho con el curso y por ende lo recomendaría a sus familiares y amigos

    Immunosuppressive Minimization Strategies in Kidney Transplantation

    The long-term graft survival in renal transplantation results is still controversial, the toxicity and adverse reactions of the immunosuppressive drugs are implicated, as well as cellular and humoral antigen-specific immune mechanisms; therefore, different strategies for adapting immunosuppression are used to reduce the complications associated with the use of these drugs. Calcineurin inhibitors (CNI) require an adequate dose-dependent concentration leading to the appearance of drug-related adverse reactions. The variability in the required dose of CNI leads to minimization strategies that do not result in a higher acute rejection (AR) incidence when compared to other immunosuppressive agents. Early steroid withdrawal is another strategy, although with an increase in AR, but without an impact on the function and survival of the renal graft. The reduction of mycophenolate mofetil to 1.5 g/day seems to be a therapeutic option, decreasing the infectious, hematological and gastrointestinal adverse reactions. Finally, alemtuzumab, bortezomib, belatacept and cellular therapies are in the search for the new treatments, whose premise is the induction of donor-specific nonresponse in the context of operational tolerance or mixed chimerism. The use of adapted and adequate immunosuppression has led to variable results and some are very encouraging; however, they must be validated with experimental studies

    Este libro, que tiene por objetivo analizar la calidad de las elecciones celebradas entre 2006 y 2011 para ocupar la titularidad del Poder Ejecutivo de las 14 entidades federativas de la República Mexicana que conforman las regiones Centro y Centro-occidente de este país, ha sido elaborado por investigadores pertenecientes a la Red Nacional de Investigación sobre la Calidad de la Democracia en México (Renicadem), la cual cuenta con un equipo de investigación en cada una de las entidades federativas del país. A su vez, esta Red constituye una de las cuatro líneas temáticas que componen la red temática del Conacyt “Sociedad civil y calidad de la democracia”. Con todo, la presente obra puede considerarse, en dos sentidos, como el resultado parcial de estudios realizados por investigadores que conforman la mencionada Renicadem. Por un lado, trata sólo de una de las varias dimensiones que esta Red ha establecido como necesarias para analizar la calidad de la democracia: la calidad electoral (otras dimensiones, que se encuentran en proceso de investigación, son calidad de vida, rendición de cuentas y Estado de derecho).     As the knowledge base and importance of mitochondrial physiology to evolution, health and disease expands, the necessity for harmonizing the terminology concerning mitochondrial respiratory states and rates has become increasingly apparent. The chemiosmotic theory establishes the mechanism of energy transformation and coupling in oxidative phosphorylation. The unifying concept of the protonmotive force provides the framework for developing a consistent theoretical foundation of mitochondrial physiology and bioenergetics. We follow the latest SI guidelines and those of the International Union of Pure and Applied Chemistry (IUPAC) on terminology in physical chemistry, extended by considerations of open systems and thermodynamics of irreversible processes. The concept-driven constructive terminology incorporates the meaning of each quantity and aligns concepts and symbols with the nomenclature of classical bioenergetics. We endeavour to provide a balanced view of mitochondrial respiratory control and a critical discussion on reporting data of mitochondrial respiration in terms of metabolic flows and fluxes. Uniform standards for evaluation of respiratory states and rates will ultimately contribute to reproducibility between laboratories and thus support the development of data repositories of mitochondrial respiratory function in species, tissues, and cells. Clarity of concept and consistency of nomenclature facilitate effective transdisciplinary communication, education, and ultimately further discovery

Productivity of cooperative in Pelalawan was highly but efficiency still low. Profit and income were highly, even liquidity of cooperative very high, and solvability was good

    Severe early onset preeclampsia: short and long term clinical, psychosocial and biochemical aspects

    Preeclampsia is a pregnancy specific disorder commonly defined as de novo hypertension and proteinuria after 20 weeks gestational age. It occurs in approximately 3-5% of pregnancies and it is still a major cause of both foetal and maternal morbidity and mortality worldwide1. As extensive research has not yet elucidated the aetiology of preeclampsia, there are no rational preventive or therapeutic interventions available. The only rational treatment is delivery, which benefits the mother but is not in the interest of the foetus, if remote from term. Early onset preeclampsia (<32 weeks’ gestational age) occurs in less than 1% of pregnancies. It is, however often associated with maternal morbidity as the risk of progression to severe maternal disease is inversely related with gestational age at onset2. Resulting prematurity is therefore the main cause of neonatal mortality and morbidity in patients with severe preeclampsia3. Although the discussion is ongoing, perinatal survival is suggested to be increased in patients with preterm preeclampsia by expectant, non-interventional management. This temporising treatment option to lengthen pregnancy includes the use of antihypertensive medication to control hypertension, magnesium sulphate to prevent eclampsia and corticosteroids to enhance foetal lung maturity4. With optimal maternal haemodynamic status and reassuring foetal condition this results on average in an extension of 2 weeks. Prolongation of these pregnancies is a great challenge for clinicians to balance between potential maternal risks on one the eve hand and possible foetal benefits on the other. Clinical controversies regarding prolongation of preterm preeclamptic pregnancies still exist – also taking into account that preeclampsia is the leading cause of maternal mortality in the Netherlands5 - a debate which is even more pronounced in very preterm pregnancies with questionable foetal viability6-9. Do maternal risks of prolongation of these very early pregnancies outweigh the chances of neonatal survival? Counselling of women with very early onset preeclampsia not only comprises of knowledge of the outcome of those particular pregnancies, but also knowledge of outcomes of future pregnancies of these women is of major clinical importance.     Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial

    Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.
