As organizações procuram, por isso, adotar práticas e filosofias de gestão que as permitam evoluir e adquirir novas competências com padrões de eficiência mais elevados. Não descorando, o comportamento das suas áreas nevrálgicas e procurando, por isso, minimizar desperdícios e custos e reforçando a sua consciência e sensibilidade para as práticas verdes e para a sustentabilidade. O foco no desenho sustentável das operações, eleva a logística a um patamar crucial de desempenho para o sucesso organizacional, especialmente, porque a análise pressupõe um cuidado no mínimo detalhe do ciclo de vida de cada produto, ou seja, desde a sua conceção e desenvolvimento até ao planeamento da logística reversa associado ao processo de fim de vida dos produtos. A cadeia de abastecimento é um aglomerado significativo de múltiplas organizações, grande parte delas, geograficamente dispersa, que necessitam de desenvolver mecanismos adequados de cooperação para a adoção de práticas verdes e que promovam a criação de valor e o aumento da sustentabilidade. A implementação das premissas e princípios que balizam a sustentabilidade, no meio organizacional, passam, essencialmente, por uma mudança cultural e por envolvimento da organização voltada para propósitos mais amplos e menos centrados em retornos líquidos vorazes e predatórios. As medidas devem ser implementadas de forma gradual e, de forma, a garantir vitórias rápidas para galvanizar os intervenientes.The study of practices focused on organizational efficiency and sustainability is increasingly in vogue and is one of the central mechanisms for obtaining competitive advantages in the market. Therefore, organizations seek to adopt practices and management philosophies that enable them to evolve and acquire new competencies with higher efficiency standards. Not discoloring, the behavior of their neuralgic areas and thus seeking to minimize wastage and costs and reinforcing their awareness and sensitivity to green practices and sustainability. Focusing on the sustainable design of operations, it elevates logistics to a crucial level of performance for organizational success, especially because the analysis presupposes a care in the least detail of the life cycle of each product, that is, from its design and development to the planning of reverse logistics associated with the end-of-life process of the products. The supply chain is a significant cluster of multiple, largely geographically dispersed, organizations that need to develop appropriate cooperation mechanisms for the adoption of green practices and that promote value creation and increased sustainability. The implementation of the assumptions and principles that guide sustainability in the organizational environment essentially go through a cultural change and organizational involvement geared towards broader purposes and less focused on voracious and predatory liquid returns.     Investigating the role of symptom valorisation in tuberculosis patient delay in urban areas in Portugal

TB incidence has been decreasing in Portugal, but median patient delay (PD) has risen. Symptom valorisation may determine PD by influencing help-seeking behaviour. We aimed to analyse the association between symptom valorisation and PD, while characterising individuals who disregarded their symptoms. Methods: A cross-sectional study was conducted among TB patients in Lisbon and Oporto in 2019 - 2021. Subjects who delayed seeking care because they did not value their symptoms or thought these would go away on their own were considered to have disregarded their symptoms. PD was categorised using a 21-day cut-off, and a 30-day cut-off for sensitivity analysis. We estimated the effect of symptom valorisation on PD through a directed acyclic graph. Then, a multivariable regression analysis characterised patients that disregarded their symptoms, adjusting for relevant variables. We fitted Poisson regression models to estimate crude and adjusted prevalence ratios (PR). Results: The study included 75 patients. Median PD was 25 days (IQR 11.5-63.5), and 56.0% of participants had PD exceeding 21 days. Symptom disregard was reported by 38.7% of patients. Patients who did not value their symptoms had higher prevalence of PD exceeding 21 days compared to those who valued their symptoms [PR 1.59 (95% CI 1.05-2.42)]. The sensitivity analysis showed consistent point estimates but wider confidence intervals [PR 1.39 (95% CI 0.77-2.55)]. Being a smoker was a risk factor for symptom disregard [PR 2.35 (95% CI 1.14-4.82)], while living in Oporto [PR 0.35 (95% CI 0.16-0.75)] and having higher household incomes [PR 0.39 (95% CI 0.17-0.94)] were protective factors. Conclusions: These findings emphasise the importance of symptom valorisation in timely TB diagnosis. Patients who did not value their symptoms had longer PD, indicating a need for interventions to improve symptom recognition.     INOVPESCA - Redução de capturas acidentais de espécies marinhas protegidas em pescarias costeiras algarvias: inovação de procedimentos e técnicas de mitigação

    Relatório técnico iNOVPESCA, Programa MAR2020, MAR-16-01-03-FMP-0020, Universidade do Algarve, CCMARO presente relatório é uma iniciativa do Projecto iNOVPESCA, com o objetivo de documentar o trabalho desenvolvido ao longo do projeto (2018-2021) e incentivar a implementação voluntária de medidas que contribuam para uma melhoria da relação entre as pescas e as espécies marinhas protegidas, especialmente de cetáceos, contribuindo para a sustentabilidade ambiental com a redução de capturas acidentais e sustentabilidade económica do sector das pescarias costeiras Algarvias, podendo servir de exemplo a nível nacional. Este trabalho pretende ser um documento de referência sobre o nível de interação que existe entre pescarias costeiras ao longo da costa algarvia com espécies marinhas protegidas (cetáceos, aves marinhas e tartarugas), e por outro lado, pode servir como aconselhamento para todas as entidades que beneficiam das pescas e que necessitam desta atividade para o seu desenvolvimento económico e social. Para isso foi desenvolvida uma linha de orientação sobre boas práticas assente em dois aspetos chave: - Contribuir para a consciencialização, informação e treino de todas as partes interessadas, no que se refere a medidas de mitigação de conflitos entre pescas e espécies marinhas protegidas; - Promover o uso de práticas responsáveis de pesca e otimizar, em cooperação com o sector pesqueiro, soluções que ajudem a diminuir a mortalidade acidental de cetáceos ou outras espécies protegidas (ex. aves marinhas e tartarugas marinhas) e evitar situações que contribuam para perdas económicas para o pescador (como por exemplo, danos nas artes e perda ou danos no pescado por predação). As interações entre espécies marinhas protegidas e as pescas são um problema mundial com duas componentes importantes, a da conservação, quando os animais como captura não intencional ficam presos nas artes de pesca acabando por morrer, e a socioeconómica quando os animais causam danos nas artes de pesca e na captura alvo dos pescadores.Programa MAR2020, MAR-16-01-03-FMP-0020info:eu-repo/semantics/publishedVersio

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung’s disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung’s disease) from 264 hospitals (89 in high-income countries, 166 in middleincome countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in lowincome countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. Interpretation Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between lowincome, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030

    Characterisation of microbial attack on archaeological bone

    As part of an EU funded project to investigate the factors influencing bone preservation in the archaeological record, more than 250 bones from 41 archaeological sites in five countries spanning four climatic regions were studied for diagenetic alteration. Sites were selected to cover a range of environmental conditions and archaeological contexts. Microscopic and physical (mercury intrusion porosimetry) analyses of these bones revealed that the majority (68%) had suffered microbial attack. Furthermore, significant differences were found between animal and human bone in both the state of preservation and the type of microbial attack present. These differences in preservation might result from differences in early taphonomy of the bones. © 2003 Elsevier Science Ltd. All rights reserved

    NEOTROPICAL XENARTHRANS: a data set of occurrence of xenarthran species in the Neotropics

    Xenarthrans—anteaters, sloths, and armadillos—have essential functions for ecosystem maintenance, such as insect control and nutrient cycling, playing key roles as ecosystem engineers. Because of habitat loss and fragmentation, hunting pressure, and conflicts with domestic dogs, these species have been threatened locally, regionally, or even across their full distribution ranges. The Neotropics harbor 21 species of armadillos, 10 anteaters, and 6 sloths. Our data set includes the families Chlamyphoridae (13), Dasypodidae (7), Myrmecophagidae (3), Bradypodidae (4), and Megalonychidae (2). We have no occurrence data on Dasypus pilosus (Dasypodidae). Regarding Cyclopedidae, until recently, only one species was recognized, but new genetic studies have revealed that the group is represented by seven species. In this data paper, we compiled a total of 42,528 records of 31 species, represented by occurrence and quantitative data, totaling 24,847 unique georeferenced records. The geographic range is from the southern United States, Mexico, and Caribbean countries at the northern portion of the Neotropics, to the austral distribution in Argentina, Paraguay, Chile, and Uruguay. Regarding anteaters, Myrmecophaga tridactyla has the most records (n = 5,941), and Cyclopes sp. have the fewest (n = 240). The armadillo species with the most data is Dasypus novemcinctus (n = 11,588), and the fewest data are recorded for Calyptophractus retusus (n = 33). With regard to sloth species, Bradypus variegatus has the most records (n = 962), and Bradypus pygmaeus has the fewest (n = 12). Our main objective with Neotropical Xenarthrans is to make occurrence and quantitative data available to facilitate more ecological research, particularly if we integrate the xenarthran data with other data sets of Neotropical Series that will become available very soon (i.e., Neotropical Carnivores, Neotropical Invasive Mammals, and Neotropical Hunters and Dogs). Therefore, studies on trophic cascades, hunting pressure, habitat loss, fragmentation effects, species invasion, and climate change effects will be possible with the Neotropical Xenarthrans data set.
