A Business Case Framework for Planning Clinical Nurse Specialist-led Interventions
Purpose: The purpose of this article is to describe a business case framework that can guide clinical nurse specialists (CNS) in clinical intervention development.
Background: Increased emphasis on cost-effective interventions in healthcare requires skills in analyzing the need to make the business case, especially for resource-intensive interventions. This framework assists the CNS to anticipate resource use and then consider if the intervention makes good business sense.
Business Case Framework: We describe a business case framework that can assist the CNS to fully explore the problem and determine if developing an intervention is a good investment. We describe several analyses that facilitate making the business case to include the following: problem identification and alignment with strategic priorities, needs assessment, stakeholder analysis, market analysis, intervention implementation planning, financial analysis, and outcome evaluation. The findings from these analyses can be used to develop a formal proposal to present to hospital leaders in a position to make decisions. By aligning intervention planning with organizational priorities and engaging patients in the process, interventions will be more likely to be implemented in practice and produce robust outcomes.
Conclusion: The business case framework can be used to justify to organization decision makers the need to invest resources in new interventions that will make a difference for quality outcomes as well as the financial bottom line. This framework can be used to plan interventions that align with organizational strategic priorities, plan for associated costs and benefits, and outcome evaluation.
Implications for CNS Practice: Clinical nurse specialists are well positioned to lead clinical intervention projects that will improve the quality of patient care and be cost-effective. To do so requires skill development in making the business case
QSO Absorbing Galaxies at z<~1: Deep Imaging and Spectroscopy in the Field of 3C 336
We present very deep WFPC2 images and FOS spectroscopy from the Hubble Space
Telescope (HST) together with numerous supporting ground-based observations of
the field of the quasar 3C 336 (). The observations are designed
to investigate the nature of galaxies producing metal line absorption systems
in the spectrum of the QSO. Along a single line of sight, we find at least 6
metal line absorption systems (of which 3 are newly discovered) ranging in
redshift from 0.317 to 0.892. Through an extensive program of optical and IR
imaging, QSO spectroscopy, and faint galaxy spectroscopy, we have identified 5
of the 6 metal line absorption systems with luminous (L_K > 0.1 L*_K) galaxies.
These have morphologies ranging from very late-type spiral to S0, and exhibit a
wide range of inclination and position angles with respect to the QSO
sightline. The only unidentified absorber, despite our intensive search, is a
damped Lyman system at . Analysis of the absorption
spectrum suggests that the metal abundances ([Fe/H]) in this system are
similar to those in damped systems at , and to the two other damped
systems for which abundances have been determined at . We have found no
examples of intrinsically faint galaxies () at small impact
parameters that might have been missed as absorber candidates in our previous
ground-based imaging and spectroscopic programs on MgII absorbing galaxies.
There are no bright galaxies (L > 0.1 L_K) within 50h^{-1} kpc which do not
produce detectable metal lines (of Mg II 2796, 2803 and/or C IV 1548, 1550) in
the QSO spectrum. All of these results generally support the inferences which
we have previously reached from a larger survey for absorption-selected

During an 8-year study period, Arcobacter butzleri was the fourth most common Campylobacter-like organism isolated from 67,599 stool specimens. Our observations suggest that A. butzleri displays microbiologic and clinical features similar to those of Campylobacter jejuni; however, A. butzleri is more frequently associated with a persistent, watery diarrhea
Antimicrobial resistance surveillance in the AFHSC-GEIS network
International infectious disease surveillance has been conducted by the United States (U.S.) Department of Defense (DoD) for many years and has been consolidated within the Armed Forces Health Surveillance Center, Division of Global Emerging Infections Surveillance and Response System (AFHSC-GEIS) since 1998. This includes activities that monitor the presence of antimicrobial resistance among pathogens. AFHSC-GEIS partners work within DoD military treatment facilities and collaborate with host-nation civilian and military clinics, hospitals and university systems. The goals of these activities are to foster military force health protection and medical diplomacy. Surveillance activities include both community-acquired and health care-associated infections and have promoted the development of surveillance networks, centers of excellence and referral laboratories. Information technology applications have been utilized increasingly to aid in DoD-wide global surveillance for diseases significant to force health protection and global public health. This section documents the accomplishments and activities of the network through AFHSC-GEIS partners in 2009
First-year Sloan Digital Sky Survey-II (SDSS-II) Supernova Results: Hubble Diagram and Cosmological Parameters
We present measurements of the Hubble diagram for 103 Type Ia supernovae
(SNe) with redshifts 0.04 < z < 0.42, discovered during the first season (Fall
2005) of the Sloan Digital Sky Survey-II (SDSS-II) Supernova Survey. These data
fill in the redshift "desert" between low- and high-redshift SN Ia surveys. We
combine the SDSS-II measurements with new distance estimates for published SN
data from the ESSENCE survey, the Supernova Legacy Survey, the Hubble Space
Telescope, and a compilation of nearby SN Ia measurements. Combining the SN
Hubble diagram with measurements of Baryon Acoustic Oscillations from the SDSS
Luminous Red Galaxy sample and with CMB temperature anisotropy measurements
from WMAP, we estimate the cosmological parameters w and Omega_M, assuming a
spatially flat cosmological model (FwCDM) with constant dark energy equation of
state parameter, w. For the FwCDM model and the combined sample of 288 SNe Ia,
we find w = -0.76 +- 0.07(stat) +- 0.11(syst), Omega_M = 0.306 +- 0.019(stat)
+- 0.023(syst) using MLCS2k2 and w = -0.96 +- 0.06(stat) +- 0.12(syst), Omega_M
= 0.265 +- 0.016(stat) +- 0.025(syst) using the SALT-II fitter. We trace the
discrepancy between these results to a difference in the rest-frame UV model
combined with a different luminosity correction from color variations; these
differences mostly affect the distance estimates for the SNLS and HST
supernovae. We present detailed discussions of systematic errors for both
light-curve methods and find that they both show data-model discrepancies in
rest-frame -band. For the SALT-II approach, we also see strong evidence for
redshift-dependence of the color-luminosity parameter (beta). Restricting the
analysis to the 136 SNe Ia in the Nearby+SDSS-II samples, we find much better
agreement between the two analysis methods but with larger uncertainties.Comment: Accepted for publication by ApJ
Search for anomalous t t-bar production in the highly-boosted all-hadronic final state
A search is presented for a massive particle, generically referred to as a
Z', decaying into a t t-bar pair. The search focuses on Z' resonances that are
sufficiently massive to produce highly Lorentz-boosted top quarks, which yield
collimated decay products that are partially or fully merged into single jets.
The analysis uses new methods to analyze jet substructure, providing
suppression of the non-top multijet backgrounds. The analysis is based on a
data sample of proton-proton collisions at a center-of-mass energy of 7 TeV,
corresponding to an integrated luminosity of 5 inverse femtobarns. Upper limits
in the range of 1 pb are set on the product of the production cross section and
branching fraction for a topcolor Z' modeled for several widths, as well as for
a Randall--Sundrum Kaluza--Klein gluon. In addition, the results constrain any
enhancement in t t-bar production beyond expectations of the standard model for
t t-bar invariant masses larger than 1 TeV.Comment: Submitted to the Journal of High Energy Physics; this version

Search for new physics in events with opposite-sign leptons, jets, and missing transverse energy in pp collisions at sqrt(s) = 7 TeV
A search is presented for physics beyond the standard model (BSM) in final
states with a pair of opposite-sign isolated leptons accompanied by jets and
missing transverse energy. The search uses LHC data recorded at a
center-of-mass energy sqrt(s) = 7 TeV with the CMS detector, corresponding to
an integrated luminosity of approximately 5 inverse femtobarns. Two
complementary search strategies are employed. The first probes models with a
specific dilepton production mechanism that leads to a characteristic kinematic
edge in the dilepton mass distribution. The second strategy probes models of
dilepton production with heavy, colored objects that decay to final states
including invisible particles, leading to very large hadronic activity and
missing transverse energy. No evidence for an event yield in excess of the
standard model expectations is found. Upper limits on the BSM contributions to
the signal regions are deduced from the results, which are used to exclude a
region of the parameter space of the constrained minimal supersymmetric
extension of the standard model. Additional information related to detector
efficiencies and response is provided to allow testing specific models of BSM
