Exploring the role of mental imagery in the experience of self-injury:an interpretative phenomenological analysis
Background: Research has implicated causal, mediating and meaningful roles for mental imagery in the experience of psychological distress, including self-injury. Aims: The present study aimed to further the understanding of this relationship through exploring the lived experiences of mental imagery from the perspective of those who self-injure. Method: This study employed an inductive qualitative design using semi-structured interviews and Interpretative Phenomenological Analysis (IPA). Ten participants were recruited from universities in North West England. Results: Three main themes were identified from the analysis: (i) The origins and precipitants of self-injurious imagery; (ii) What it is like toexperience self-injurious imagery; and (iii) The meaning and interpretation of self-injurious imagery. Conclusions: The study findings indicate that mental imagery is an important experience for those who self-injure. Clinical and research implications of the findings are discussed
Atmospheric circulation of tidally locked exoplanets: II. Dual-band radiative transfer and convective adjustment
Improving upon our purely dynamical work, we present three-dimensional simulations of the atmospheric circulation on Earth-like (exo)planets and hot Jupiters using the Geophysical Fluid Dynamics Laboratory (GFDL)-Princeton Flexible Modelling System (fms). As the first steps away from the dynamical benchmarks of Heng, Menou & Phillipps, we add dual-band radiative transfer and dry convective adjustment schemes to our computational set-up. Our treatment of radiative transfer assumes stellar irradiation to peak at a wavelength shorter than and distinct from that at which the exoplanet re-emits radiation (âshortwave' versus âlongwave'), and also uses a two-stream approximation. Convection is mimicked by adjusting unstable lapse rates to the dry adiabat. The bottom of the atmosphere is bounded by a uniform slab with a finite thermal inertia. For our models of hot Jupiter, we include an analytical formalism for calculating temperature-pressure profiles, in radiative equilibrium, which accounts for the effect of collision-induced absorption via a single parameter. We discuss our results within the context of the following: the predicted temperature-pressure profiles and the absence/presence of a temperature inversion; the possible maintenance, via atmospheric circulation, of the putative high-altitude, shortwave absorber expected to produce these inversions; the angular/temporal offset of the hotspot from the substellar point, its robustness to our ignorance of hyperviscosity and hence its utility in distinguishing between different hot Jovian atmospheres; and various zonal-mean flow quantities. Our work bridges the gap between three-dimensional simulations which are purely dynamical and those which incorporate multiband radiative transfer, thus contributing to the construction of a required hierarchy of three-dimensional theoretical model
Fatality after deliberate ingestion of the pesticide rotenone: a case report
Rotenone is a pesticide derived from the roots of plants from the Leguminosae family. Poisoning following deliberate ingestion of these plant roots has commonly been reported in Papua New Guinea. However, poisoning with commercially available rotenone in humans has been reported only once previously following accidental ingestion in a 3.5-year-old child. Therefore, the optimal management of rotenone poisoning is not known. After deliberate ingestion of up to 200 ml of a commercially available 0.8% rotenone solution, a 47-year-old female on regular metformin presented with a reduced level of consciousness, metabolic acidosis and respiratory compromise. Metformin was not detected in premortem blood samples obtained. Despite intensive supportive management, admission to an intensive care unit, and empirical use of N-acetylcysteine and antioxidant therapy, she did not survive. Poisoning with rotenone is uncommon but is potentially fatal because this agent inhibits the mitochondrial respiratory chain. In vitro cell studies have shown that rotenone-induced toxicity is reduced by the use of N-acetylcysteine, antioxidants and potassium channel openers. However, no animal studies have been reported that confirm these findings, and there are no previous reports of attempted use of these agents in patients with acute rotenone-induced toxicity
Fatality after deliberate ingestion of sustained-release ibuprofen: a case report
INTRODUCTION: Ibuprofen is a nonsteroidal anti-inflammatory drug available over the counter and on prescription for the management of pain and inflammation. Severe toxicity is rare following deliberate self-poisoning with ibuprofen, and patients are usually either asymptomatic or develop only mild gastrointestinal toxicity. Although there have been nine other reported fatalities, co-existent factors have probably contributed to all of these deaths. We report here a fatality from isolated toxicity following self-poisoning with sustained-release ibuprofen. CASE REPORT: A 26-year-old female presented after deliberate ingestion of up to 105 g sustained-release ibuprofen, with a reduced level of consciousness, severe metabolic acidosis and haemodynamic compromise. Despite intensive supportive management, gut decontamination with multidose activated charcoal and correction of the metabolic acidosis with sodium bicarbonate and haemofiltration, the patient did not survive. The ibuprofen concentration ante mortem on presentation in peripheral blood was 760 mg/l and the concentrations post mortem were 518 mg/l in peripheral blood, 74 mg/kg in liver extract and 116 mg/l in the gastric contents. DISCUSSION: Most patients with ibuprofen poisoning are either asymptomatic or have mild gastrointestinal symptoms; severe poisoning with ibuprofen is rare. The impact of attaining the Welsh Baccalaureate Advanced Diploma on academic performance in bioscience higher education
Since the introduction of the Welsh Baccalaureate Advanced Diploma Qualification (WBQ) in 2003, an increasing number of students are applying to higher education institutions (HEIs) with this qualification. The advanced-level WBQ is regarded as equivalent to one General Certificate of Education A-Level (GCE A-Level). This study assesses the impact of attaining the WBQ in addition to three GCE A-Levels on overall university degree performance in comparison to attaining four GCE A-Levels, in three cohorts of undergraduate students (Year 1â=â318, Year 2â=â280, Year 3â=â236) studying Biosciences from 2005 to 2011 at a UK HEI. Binary logistic regression was used to compare the academic attainment of students who had achieved four GCE A-Levels to those who had achieved three GCE A-Levels in addition to the WBQ. Comparisons were also made between students who had achieved three GCE A-Levels and those who had achieved three GCE A-Levels in addition to the WBQ. The results suggest that students who achieved the WBQ qualification in its current form, in addition to three GCE A-Levels, performed less well academically in undergraduate studies than those who achieved four GCE A-Levels. Furthermore, this effect was still present when the balance between coursework and examination was considered, and when students who had achieved the WBQ in addition to three GCE A-Levels were compared to students who had achieved three GCE A-Level
High-throughput sequence analysis of variants of human cytomegalovirus strains Towne and AD169
The genomes of commonly used variants of human cytomegalovirus (HCMV) strains Towne and AD169 each contain a substantial mutation in which a region (UL/bâČ) at the right end of the long unique region has been replaced by an inverted duplication of a region from the left end of the genome. Using high-throughput technology, we have sequenced HCMV strain Towne (ATCC VR-977) and confirmed the presence of two variants, one exhibiting the replacement in UL/bâČ and the other intact in this region. Both variants are mutated in genes RL13, UL1, UL40, UL130, US1 and US9. We have also sequenced a novel AD169 variant (varUC) that is intact in UL/bâČ except for a small deletion that affects genes UL144, UL142, UL141 and UL140. Like other AD169 variants, varUC is mutated in genes RL5A, RL13, UL36 and UL131A. A subpopulation of varUC contains an additional deletion affecting genes IRS1, US1 and US2
HCMV pUL135 remodels the actin cytoskeleton to impair immune recognition of infected cells
Immune evasion genes help human cytomegalovirus (HCMV) establish lifelong persistence. Without immune pressure, laboratory-adapted HCMV strains have undergone genetic alterations. Among these, the deletion of the UL/bâ domain is associated with loss of virulence. In a screen of UL/bâ, we identified pUL135 as a protein responsible for the characteristic cytopathic effect of clinical HCMV strains that also protected from natural killer (NK) and TÂ cell attack. pUL135 interacted directly with abl interactor 1 (ABI1) and ABI2 to recruit the WAVE2 regulatory complex to the plasma membrane, remodel the actin cytoskeleton and dramatically reduce the efficiency of immune synapse (IS) formation. An intimate association between F-actin filaments in target cells and the IS was dispelled by pUL135 expression. Thus, F-actin in target cells plays a critical role in synaptogenesis, and this can be exploited by pathogens to protect against cytotoxic immune effector cells. The burden of mental disorders, substance use disorders and self-harm among young people in Europe, 1990â2019: Findings from the Global Burden of Disease Study 2019
Summary Background Mental health is a public health issue for European young people, with great heterogeneity in resource allocation. Representative population-based studies are needed. The Global Burden of Disease (GBD) Study 2019 provides internationally comparable information on trends in the health status of populations and changes in the leading causes of disease burden over time. Methods Prevalence, incidence, Years Lived with Disability (YLDs) and Years of Life Lost (YLLs) from mental disorders (MDs), substance use disorders (SUDs) and self-harm were estimated for young people aged 10-24 years in 31 European countries. Rates per 100,000 population, percentage changes in 1990-2019, 95% Uncertainty Intervals (UIs), and correlations with Sociodemographic Index (SDI), were estimated. Findings In 2019, rates per 100,000 population were 16,983 (95% UI 12,823 â 21,630) for MDs, 3,891 (3,020 - 4,905) for SUDs, and 89·1 (63·8 - 123·1) for self-harm. In terms of disability, anxiety contributed to 647·3 (432â912·3) YLDs, while in terms of premature death, self-harm contributed to 319·6 (248·9â412·8) YLLs, per 100,000 population. Over the 30 years studied, YLDs increased in eating disorders (14·9%;9·4-20·1) and drug use disorders (16·9%;8·9-26·3), and decreased in idiopathic developmental intellectual disability (â29·1%;23·8-38·5). YLLs decreased in self-harm (â27·9%;38·3-18·7). Variations were found by sex, age-group and country. The burden of SUDs and self-harm was higher in countries with lower SDI, MDs were associated with SUDs. Interpretation Mental health conditions represent an important burden among young people living in Europe.
