    Criação de um espaço de aprendizagem significativa no planetário do parque Ibirapuera

    Este trabalho visa apresentar uma proposta de pesquisa-intervenção desenvolvida no Planetário do Parque Ibirapuera (SP), cujo objetivo é a implantação de um espaço de aprendizagem desafiador e que promova a popularização de conteúdos relacionados à Astronomia, Astrofísica e Cosmologia por meio da exposição de objetos e experimentos relacionados à área. O artigo apresenta a concepção básica e os critérios utilizados do ponto de vista pedagógico para as indicações dos objetos e experimentos que serão expostos no Planetário. Considerando a estrutura de mapas conceituais buscou-se a melhor forma de apresentar, distribuir e expor objetos e experimentos de Astronomia visando estimular uma aprendizagem significativa dos conceitos abordados, valorizando-se a aproximação e a interação do público com o ambiente não-formal de aprendizagem

    Clinical Impact of Monoclonal Antibodies in the Treatment of High-Risk Patients with SARS-CoV-2 Breakthrough Infections:The ORCHESTRA Prospective Cohort Study

    The clinical impact of anti-spike monoclonal antibodies (mAb) in Coronavirus Disease 2019 (COVID-19) breakthrough infections is unclear. We present the results of an observational prospective cohort study assessing and comparing COVID-19 progression in high-risk outpatients receiving mAb according to primary or breakthrough infection. Clinical, serological and virological predictors associated with 28-day COVID-19-related hospitalization were identified using multivariate logistic regression and summarized with odds ratio (aOR) and 95% confidence interval (CI). A total of 847 COVID-19 outpatients were included: 414 with primary and 433 with breakthrough infection. Hospitalization was observed in 42/414 (10.1%) patients with primary and 8/433 (1.8%) patients with breakthrough infection (p &lt; 0.001). aOR for hospitalization was significantly lower for breakthrough infection (aOR 0.12, 95%CI: 0.05–0.27, p &lt; 0.001) and higher for immunocompromised status (aOR:2.35, 95%CI:1.08–5.08, p = 0.003), advanced age (aOR:1.06, 95%CI: 1.03–1.08, p &lt; 0.001), and male gender (aOR:1.97, 95%CI: 1.04–3.73, p = 0.037). Among the breakthrough infection group, the median SARS-CoV-2 anti-spike IgGs was lower (p &lt; 0.001) in immunocompromised and elderly patients &gt;75 years compared with that in the immunocompetent patients. Our findings suggest that, among mAb patients, those with breakthrough infection have significantly lower hospitalization risk compared with patients with primary infection. Prognostic algorithms combining clinical and immune-virological characteristics are needed to ensure appropriate and up-to-date clinical protocols targeting high-risk categories.</p

    ACIDENTES CAUSADOS POR SERPENTES DO GÊNERO Bothrops EM PEQUENOS ANIMAIS – REVISÃO DE LITERATURA

    As serpentes do gênero Bothrops são responsáveis por cerca de 90% dos casos de acidentes ofídicos em seres humanos na América do Sul, sendo que deste valor, 89% dos casos ocorrem no Brasil, isso ocorre devido ao fato de que o Brasil possui as características necessárias para o habitat das serpentes botrópicas. O acidente botrópico apresenta como características as marcas das presas da serpente no local da picada, edema, sangramento local e até necrose; sendo assim o veneno possui ações proteolíticas, vasculotóxicas e coagulantes. Para o diagnóstico do acidente botrópico deve-se levar em consideração o histórico do paciente, achados no exame físico e nos exames laboratoriais, que sugiram o acidente. Sempre que houver suspeita de acidente botrópico deve ser avaliado o tempo de coagulação sanguínea (TC), que geralmente encontra-se de aumentado (maior que 10 minutos) a incoagulável (maior que 30 minutos). O tratamento baseia-se na soroterapia devendo ser realizado o mais rápido possível, podendo ser utilizado o soro antibotrópico ou soro polivalente. Considerando a importância dos acidentes botrópicos no nosso país, objetivou-se com esta revisão bibliográfica abordar o acidente botrópico, tanto pelas suas propriedades tóxicas quanto farmacológicas, bem como as medidas terapêuticas na condição deste tipo de acidente

    Colorectal Cancer Stage at Diagnosis Before vs During the COVID-19 Pandemic in Italy

    IMPORTANCE Delays in screening programs and the reluctance of patients to seek medical attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced colorectal cancers at diagnosis. OBJECTIVE To evaluate whether the SARS-CoV-2 pandemic was associated with more advanced oncologic stage and change in clinical presentation for patients with colorectal cancer. DESIGN, SETTING, AND PARTICIPANTS This retrospective, multicenter cohort study included all 17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December 31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period), in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was 30 days from surgery. EXPOSURES Any type of surgical procedure for colorectal cancer, including explorative surgery, palliative procedures, and atypical or segmental resections. MAIN OUTCOMES AND MEASURES The primary outcome was advanced stage of colorectal cancer at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding, lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery, and palliative surgery. The independent association between the pandemic period and the outcomes was assessed using multivariate random-effects logistic regression, with hospital as the cluster variable. RESULTS A total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years) underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142 (56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR], 1.07; 95%CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95%CI, 1.15-1.53; P &lt; .001), and stenotic lesions (OR, 1.15; 95%CI, 1.01-1.31; P = .03). CONCLUSIONS AND RELEVANCE This cohort study suggests a significant association between the SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for these patients

    Risk portfolio on modern finance has become increasingly technical, requiring the use of sophisticated mathematical tools in both research and practice. Since companies cannot insure themselves completely against risk, as human incompetence in predicting the future precisely that written in Al-Quran surah Luqman verse 34, they have to manage it to yield an optimal portfolio. The objective here is to minimize the variance among all portfolios, or alternatively, to maximize expected return among all portfolios that has at least a certain expected return. Furthermore, this study focuses on optimizing risk portfolio so called Markowitz MVO (Mean-Variance Optimization). Some theoretical frameworks for analysis are arithmetic mean, geometric mean, variance, covariance, linear programming, and quadratic programming. Moreover, finding a minimum variance portfolio produces a convex quadratic programming, that is minimizing the objective function ðð¥with constraintsð ð 𥠥 ðandð´ð¥ = ð. The outcome of this research is the solution of optimal risk portofolio in some investments that could be finished smoothly using MATLAB R2007b software together with its graphic analysis
