    Conhecimento dos pacientes sobre a prescrição medicamentosa no serviço de emergência

    Objective: To identify which orientations were received by the patient about the medication prescription and which professional performed itto evaluate the patients' knowledge about prescription drugs and to correlate it to socioeconomic variables, comorbidities, and the frequency with which the patient seeks emergency serviceand to evaluate the knowledge about the medication prescribed after the health care. Method: This was a cross-sectional study on 304 patients that received emergency service's discharge along with medication prescription. Applied instruments: sociodemographic characterization and evaluation of the knowledge about the prescribed medication. We used a descriptive and inferential analysis. Results: Most subjects had no doubts about how or for how long to take the medicationand presented questions about adverse reactions and what to do in case of forgetting to take the medication doses. There was a significant association between ageeducational levelcomorbiditythe frequency of emergency service's useand knowledge about medications. Conclusion: a total of 48% of the patients declared to need information about adverse effects and what to do if they forget to take the medication.Objetivo: Identificar quais orientações foram recebidas pelo paciente sobre prescrição medicamentosa e qual profissional as realizou; avaliar conhecimento dos pacientes sobre medicamentos prescritos; e correlacioná-lo às variáveis socioeconômicas, comorbidades e frequência com que o paciente procura o serviço de emergência com o conhecimento sobre medicamentos prescritos após o atendimento. Método: Estudo transversal e analítico com 304 pacientes que receberam alta do serviço juntamente com prescrição medicamentosa. Instrumentos aplicados: caracterização sócio demográfica e avaliação do conhecimento sobre a medicação prescrita. Utilizou-se análise descritiva e inferencial. Resultados: A maioria não teve dúvida sobre como nem por quanto tempo tomar o medicamento; e apresentou dúvidas sobre reações adversas e o que fazer no caso de esquecimento de doses do medicamento. Associação significativa entre idade; escolaridade; comorbidade; usuário frequente do serviço de emergência e conhecimento sobre medicamentos.     Atopic dermatitis, also known as atopic eczema, is a chronic and recurrent skin disease. It is characterized by red rashes, severe itching and dry skin. It can affect people of all ages, but is more common in children. Genetic and environmental factors contribute to its development. Treatment usually involves skin care, anti-inflammatory medications, and managing triggers. Medical monitoring is essential for effective management. Atopic dermatitis can affect pregnancy in several ways. Some women may experience an improvement in symptoms during pregnancy due to hormonal changes, while others may notice a worsening. The objective of this study is to understand the relationship between pregnancy and the condition of atopic dermatitis. Results in managing atopic dermatitis vary from person to person. Effective treatment often involves a combination of skin care, doctor-prescribed medications, and identifying and reducing triggers. Treatment often involves the use of topical corticosteroids, immunomodulators and emollients to keep the skin hydrated.A dermatite atópica, também conhecida como eczema atópico, é uma doença de pele crônica e recorrente. Fatores genéticos e ambientais contribuem para o seu desenvolvimento. O tratamento geralmente envolve cuidados com a pele, medicamentos anti-inflamatórios e gerenciamento de fatores desencadeantes. O acompanhamento médico é fundamental para um manejo eficaz. A dermatite atópica pode afetar a gravidez de diversas maneiras. Algumas mulheres podem experimentar uma melhoria nos sintomas durante a gravidez devido às alterações hormonais, enquanto outras podem notar uma piora. O objetivo deste estudo é compreender a relação da gravidez com a condição da dermatite atópica. Os resultados no manejo da dermatite atópica variam de pessoa para pessoa.     Introduction: Cluster headache, a form of headache characterized by acute and intense episodes of unilateral pain, often accompanied by autonomic symptoms, has been the focus of considerable attention in the fields of neurology and genetics.Methodology: This systematic literature review was conducted with the aim of exploring the correlations between cluster headache and environmental and genetic variables, employing a rigorous and comprehensive approach to identify relevant studies in the scientific literature. The methodology adheres to recommended guidelines for systematic reviews, prioritizing the inclusion of studies published between 2013 and 2023, with access to the full text.Results: It is crucial to recognize that the genetic pattern of cluster headache does not present itself univocally, revealing a multifactorial complexity in its etiology. While genetic factors may confer predisposition, their clinical expression appears to be modulated by environmental influences. Studies exploring the interactions between genes and the environment have focused on understanding how extrinsic factors, such as exposure to certain environmental stimuli, can modulate genetic activity and play a role in triggering cluster headache episodes.Conclusion: Cluster headache represents a unique and challenging neurological condition that continues to intrigue healthcare professionals and researchers. Its distinctive feature of intense pain, accompanied by autonomic symptoms, adds to the complexity of the clinical picture.Introdução: A cefaleia em salvas, uma forma de dor de cabeça caracterizada por episódios agudos e intensos de dor unilateral, frequentemente acompanhada de sintomas autonômicos, tem sido objeto de considerável atenção no campo da neurologia e da genética. Metodologia: Esta revisão sistemática da literatura foi conduzida com o objetivo de explorar as correlações entre a cefaleia em salvas e variáveis ambientais e genéticas, utilizando uma abordagem rigorosa e abrangente para identificar estudos relevantes na literatura científica. A metodologia obedece às diretrizes recomendadas para revisões sistemáticas, priorizando a inclusão de estudos previstos entre 2013 e 2023, com acesso ao texto completo. Resultado: É crucial reconhecer que o padrão genético da cefaleia em salvas não se apresenta de maneira unívoca, revelando uma complexidade multifatorial em sua etiologia. Ainda que fatores genéticos possam conferir uma predisposição, sua expressão clínica parece ser modulada por influências ambientais. Estudos explorando as interações entre genes e ambiente têm se dedicado a compreender como fatores extrínsecos, como expor certos estímulos ambientais, podem modular a atividade genética e desempenhar um papel no desencadeamento de episódios de cefaléia em salvas. Conclusão: A cefaleia em salvas representa uma condição neurológica singular e desafiadora que continua a intrigar profissionais da saúde e pesquisadores.     Amicus curiae on the request for consultative opinion formulated by the American Commission on Human Rights about Differentiated Approaches Regarding Persons Deprived of Liberty

    These are reasons for amicus curiae presented to the Inter-American Court of Human Rights by the Center for Studies in Human Rights Systems and the Clinic for Access to Justice and Education in the Prisons of the Federal University of Paraná, on the occasion of the Request for Advisory Opinion (OC) number 29 formulated by the American Commission of Humans to the Inter-American Court, on different approaches to the matter of persons deprived of their liberty. The play is part of the context of the pandemic of COVID-19, which heightened vulnerabilities and subjected the prison population as a whole to a framework of hyper vulnerability, given the sanitary measures that imply reduced rights and weakened means of enforcement. Still, it advances on other contexts of overlapping vulnerabilities, such as concerning transgender women, the LGBTQI + public, pregnant women, breastfeeding women, and those accompanied by children in prison. Finally, the absence of the OC about other vulnerabilities not explicitly listed by the request of the IACHR, such as persons with disabilities deprived of liberty, with special emphasis on visual impairments and cases of need for use of wheelchairs with special needs, should be addressed.Trata-se de razões de amicus curiae apresentadas à Corte Interamericana de Direitos Humanos pelo Núcleo de Estudos em Sistemas de Direitos Humanos e Clínica de Acesso à Justiça e Educação nas Prisões da Universidade Federal do Paraná, por ocasião da Solicitação de Opinião Consultiva (OC) número 29 formulada pela Comissão Americana de Humanos à Corte Interamericana, sobre enfoques diferenciados em matéria de pessoas privadas de liberdade. A peça parte do contexto da pandemia da COVID-19 que aguçou vulnerabilidades e sujeitou a população carcerária como um todo a um quadro de hipervulnerabilidade, diante das medidas sanitárias que implicam em redução de direitos e em fragilização dos meios de fiscalização. Ainda, avança sobre outros contextos de vulnerabilidades superpostas como em relação às mulheres transgênero, o público LGBTQI+, mulheres gestantes, lactantes e acompanhadas de crianças no cárcere. Por fim, ataca-se as ausências da solicitação de OC em relação à outras vulnerabilidades não elencadas explicitamente pela solicitação da CIDH, como é o caso de pessoas com deficiência privadas de liberdade, com especial ênfase à deficiência visual e aos casos de necessidade de uso de cadeiras de rodas que necessitam de cuidados específicos e inexistentes no sistema penitenciário

Data from 33 centers in 16 countries on 5 continents showed that attendance at tuberculosis centers was lower during the first 4 months of the pandemic in 2020 than for the same period in 2019.     Risk portfolio on modern finance has become increasingly technical, requiring the use of sophisticated mathematical tools in both research and practice. Since companies cannot insure themselves completely against risk, as human incompetence in predicting the future precisely that written in Al-Quran surah Luqman verse 34, they have to manage it to yield an optimal portfolio. The objective here is to minimize the variance among all portfolios, or alternatively, to maximize expected return among all portfolios that has at least a certain expected return. Furthermore, this study focuses on optimizing risk portfolio so called Markowitz MVO (Mean-Variance Optimization). Some theoretical frameworks for analysis are arithmetic mean, geometric mean, variance, covariance, linear programming, and quadratic programming. Moreover, finding a minimum variance portfolio produces a convex quadratic programming, that is minimizing the objective function ðð¥with constraintsð ð 𥠥 ðandð´ð¥ = ð. The outcome of this research is the solution of optimal risk portofolio in some investments that could be finished smoothly using MATLAB R2007b software together with its graphic analysis

    Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world

    Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic. Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality. Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States. Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis. Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection

    Differential cross section measurements for the production of a W boson in association with jets in proton–proton collisions at √s = 7 TeV

    Measurements are reported of differential cross sections for the production of a W boson, which decays into a muon and a neutrino, in association with jets, as a function of several variables, including the transverse momenta (pT) and pseudorapidities of the four leading jets, the scalar sum of jet transverse momenta (HT), and the difference in azimuthal angle between the directions of each jet and the muon. The data sample of pp collisions at a centre-of-mass energy of 7 TeV was collected with the CMS detector at the LHC and corresponds to an integrated luminosity of 5.0 fb[superscript −1]. The measured cross sections are compared to predictions from Monte Carlo generators, MadGraph + pythia and sherpa, and to next-to-leading-order calculations from BlackHat + sherpa. The differential cross sections are found to be in agreement with the predictions, apart from the pT distributions of the leading jets at high pT values, the distributions of the HT at high-HT and low jet multiplicity, and the distribution of the difference in azimuthal angle between the leading jet and the muon at low values.United States.     Juxtaposing BTE and ATE – on the role of the European insurance industry in funding civil litigation

    One of the ways in which legal services are financed, and indeed shaped, is through private insurance arrangement. Two contrasting types of legal expenses insurance contracts (LEI) seem to dominate in Europe: before the event (BTE) and after the event (ATE) legal expenses insurance. Notwithstanding institutional differences between different legal systems, BTE and ATE insurance arrangements may be instrumental if government policy is geared towards strengthening a market-oriented system of financing access to justice for individuals and business. At the same time, emphasizing the role of a private industry as a keeper of the gates to justice raises issues of accountability and transparency, not readily reconcilable with demands of competition. Moreover, multiple actors (clients, lawyers, courts, insurers) are involved, causing behavioural dynamics which are not easily predicted or influenced. Against this background, this paper looks into BTE and ATE arrangements by analysing the particularities of BTE and ATE arrangements currently available in some European jurisdictions and by painting a picture of their respective markets and legal contexts. This allows for some reflection on the performance of BTE and ATE providers as both financiers and keepers. Two issues emerge from the analysis that are worthy of some further reflection. Firstly, there is the problematic long-term sustainability of some ATE products. Secondly, the challenges faced by policymakers that would like to nudge consumers into voluntarily taking out BTE LEI
