    Body Mass Index, Blood Pressure and Serum Cortisol Level as Stress Index in Symptomatic HIV/AIDS Male Subjects on Antiretroviral Therapy Negative to Malaria Parasite in Nnewi, Anambra State, Nigeria

    Background: HIV infection is a risk factor for a variety of endocrine problems. Objectives: This study investigated the body mass index (BMI), Systolic blood pressure (SBP), Diastolic blood pressure (DBP) and serum cortisol level as stress factor/index symptomatic HIV/AIDS male subjects on ART who are negative to malaria parasite in Nnewi, Anambra State, Nigeria. Methods: A total of 274 adult male participants aged between 18 and 60 (42 ±13) years were randomly recruited at the Voluntary Counseling and Testing (VCT) Centre in Nnamdi Azikiwe University Teaching Hospital and grouped based on WHO criteria for staging HIV into symptomatic HIV (stage 11) infected male participants on ART (A: n=69), Symptomatic HIV subjects not on ART (B: n= 69), Asymptomatic HIV positive subjects (C: n= 68) and HIV seronegative subjects (D: n= 68). Blood samples were collected from the participants for the determination of HIV status by immunochromatography and HIV confirmation by Western Blot. Enzyme-Linked Immunosorbent Assay (ELISA) was used to assay for cortisol level. Results: The results showed a significantly increased BMI and decreased mean serum cortisol level in HIV/AIDS seropositive participants on ART than in those, not on ART (p<0.05). Also, the BMI and mean serum cortisol level were significantly decreased and increased respectively in  HIV/AIDS seropositive participants not on ART than in Asymptomatic HIV positive subjects and control respectively (p<0.05). However, the mean SBP and DBP did not differ significantly between the groups studied (p>0.05). Conclusion: This study revealed a decreased stress index in HIV/AIDS subjects on ART with hypercortisolism and lower BMI in symptomatic HIV participants, not on ART.     The Influence of Large-Scale Structure on Halo Shapes and Alignments

    Alignments of galaxy clusters (the Binggeli effect), as well as of galaxies themselves have long been studied both observationally and theoretically. Here we test the influence of large-scales structures and tidal fields on the shapes and alignments of cluster-size and galaxy-size dark matter halos. We use a high-resolution N-body simulation of a Λ\LambdaCDM universe, together with the results of Colberg et al. (2005), who identified filaments connecting pairs of clusters. We find that cluster pairs connected by a filament are strongly aligned with the cluster-cluster axis, whereas unconnected ones are not. For smaller, galaxy-size halos, there also is an alignment signal, but its strength is independent of whether the halo is part of an obvious large-scale structure. Additionally, we find no measureable dependence of galaxy halo shape on membership of a filament. We also quantify the influence of tidal fields and find that these do correlate strongly with alignments of halos. The alignments of most halos are thus caused by tidal fields, with cluster-size halos being strongly aligned through the added mechanism of infall of matter from filaments.Comment: 8 pages, 6 figures, accepted for publication in MNRA

    The depolarisation properties of powerful extragalactic radio sources as a function of cosmic epoch

    We use the observed polarisation properties of a sample of 26 powerful radio galaxies and radio-loud quasars to constrain the conditions in the Faraday screens local to the sources. We adopt the cosmological redshift, low-frequency radio luminosity and physical size of the large-scale radio structures as our `fundamental' parameters. We find no correlation of the radio spectral index with any of the fundamental parameters. The observed rotation measure is also independent of these parameters. The difference between the rotation measures of the two lobes of an individual source as well as the dispersion of the rotation measure show significant correlations with the source redshift, but not with the radio luminosity or source size. Thus the small-scale structure observed in the rotation measure is caused by a Faraday screen local to the sources. The observed asymmetries between the lobes of our sources show no significant trends with each other or other source properties. Finally, we show that the commonly used model for the depolarisation of synchrotron radio emission by foreground Faraday screens is inconsistent with our observations. We apply alternative models to our data and show that they require a strong increase of the dispersion of the rotation measure inside the Faraday screens with cosmological redshift. Correcting our observations with these models for redshift effects, we find a strong correlation of the depolarisation measure with redshift and a significantly weaker correlation with radio luminosity. All our results are consistent with a decrease in the order of the magnetic field structure of the Faraday screen local to the sources for increasing cosmological redshift.     Mark correlations: relating physical properties to spatial distributions

    Mark correlations provide a systematic approach to look at objects both distributed in space and bearing intrinsic information, for instance on physical properties. The interplay of the objects' properties (marks) with the spatial clustering is of vivid interest for many applications; are, e.g., galaxies with high luminosities more strongly clustered than dim ones? Do neighbored pores in a sandstone have similar sizes? How does the shape of impact craters on a planet depend on the geological surface properties? In this article, we give an introduction into the appropriate mathematical framework to deal with such questions, i.e. the theory of marked point processes. After having clarified the notion of segregation effects, we define universal test quantities applicable to realizations of a marked point processes. We show their power using concrete data sets in analyzing the luminosity-dependence of the galaxy clustering, the alignment of dark matter halos in gravitational NN-body simulations, the morphology- and diameter-dependence of the Martian crater distribution and the size correlations of pores in sandstone. In order to understand our data in more detail, we discuss the Boolean depletion model, the random field model and the Cox random field model. The first model describes depletion effects in the distribution of Martian craters and pores in sandstone, whereas the last one accounts at least qualitatively for the observed luminosity-dependence of the galaxy clustering.Comment: 35 pages, 12 figures. to be published in Lecture Notes of Physics, second Wuppertal conference "Spatial statistics and statistical physics

    Accelerated expansion from structure formation

    We discuss the physics of backreaction-driven accelerated expansion. Using the exact equations for the behaviour of averages in dust universes, we explain how large-scale smoothness does not imply that the effect of inhomogeneity and anisotropy on the expansion rate is small. We demonstrate with an analytical toy model how gravitational collapse can lead to acceleration. We find that the conjecture of the accelerated expansion being due to structure formation is in agreement with the general observational picture of structures in the universe, and more quantitative work is needed to make a detailed comparison.Comment: 44 pages, 1 figure.     The XMM Cluster Survey: X-ray analysis methodology

    The XMM Cluster Survey (XCS) is a serendipitous search for galaxy clusters using all publicly available data in the XMM-Newton Science Archive. Its main aims are to measure cosmological parameters and trace the evolution of X-ray scaling relations. In this paper we describe the data processing methodology applied to the 5,776 XMM observations used to construct the current XCS source catalogue. A total of 3,675 > 4-sigma cluster candidates with > 50 background-subtracted X-ray counts are extracted from a total non-overlapping area suitable for cluster searching of 410 deg^2. Of these, 993 candidates are detected with > 300 background-subtracted X-ray photon counts, and we demonstrate that robust temperature measurements can be obtained down to this count limit. We describe in detail the automated pipelines used to perform the spectral and surface brightness fitting for these candidates, as well as to estimate redshifts from the X-ray data alone. A total of 587 (122) X-ray temperatures to a typical accuracy of < 40 (< 10) per cent have been measured to date. We also present the methodology adopted for determining the selection function of the survey, and show that the extended source detection algorithm is robust to a range of cluster morphologies by inserting mock clusters derived from hydrodynamical simulations into real XMM images. These tests show that the simple isothermal beta-profiles is sufficient to capture the essential details of the cluster population detected in the archival XMM observations. The redshift follow-up of the XCS cluster sample is presented in a companion paper, together with a first data release of 503 optically-confirmed clusters.Comment: MNRAS accepted, 45 pages, 38 figures.     Bigger than ourselves: the Southgate narrative and the search for a sense of common purpose

The paper examines two ‘turns’ in English national sporting culture, ‘Beckhamisation’ and ‘Southgatism’, and their contribution to an ‘imagined community’ through processes of ‘banal nationalism’. It examines the critiques of various academic and media commentators to demonstrate the link between the trappings of sport (in this case football), and people’s understanding of ‘us’ and ‘them’. Music/songs, flags, language, multi-cultural representation, team ethos and espoused values, are not just signifiers, but have a pivotal part to play in representing, repressing and resisting particular forms of Englishness. The focus here is on those national sporting occasions that all too often have been associated with virulent forms of nationalism. We conclude that Southgatism holds out more hope for a progressive sporting patriotism than did its Beckhamite predecessor, but that this has yet to be tested in the febrile cauldron (the ‘hostile environment’ for immigrants and Brexit) currently forging English national identity

    Global patient outcomes after elective surgery: prospective cohort study in 27 low-, middle- and high-income countries.

    BACKGROUND: As global initiatives increase patient access to surgical treatments, there remains a need to understand the adverse effects of surgery and define appropriate levels of perioperative care. METHODS: We designed a prospective international 7-day cohort study of outcomes following elective adult inpatient surgery in 27 countries. The primary outcome was in-hospital complications. Secondary outcomes were death following a complication (failure to rescue) and death in hospital. Process measures were admission to critical care immediately after surgery or to treat a complication and duration of hospital stay. A single definition of critical care was used for all countries. RESULTS: A total of 474 hospitals in 19 high-, 7 middle- and 1 low-income country were included in the primary analysis. Data included 44 814 patients with a median hospital stay of 4 (range 2-7) days. A total of 7508 patients (16.8%) developed one or more postoperative complication and 207 died (0.5%). The overall mortality among patients who developed complications was 2.8%. Mortality following complications ranged from 2.4% for pulmonary embolism to 43.9% for cardiac arrest. A total of 4360 (9.7%) patients were admitted to a critical care unit as routine immediately after surgery, of whom 2198 (50.4%) developed a complication, with 105 (2.4%) deaths. A total of 1233 patients (16.4%) were admitted to a critical care unit to treat complications, with 119 (9.7%) deaths. Despite lower baseline risk, outcomes were similar in low- and middle-income compared with high-income countries. CONCLUSIONS: Poor patient outcomes are common after inpatient surgery. Global initiatives to increase access to surgical treatments should also address the need for safe perioperative care. STUDY REGISTRATION: ISRCTN5181700
