    Legislation comparison on teaching of foreign languages in Basic Education according to the LOE and the LOMCE

    La Orden de 10 de agosto de 2007 (desarrollo normativo LOE) establece que el alumnado debía cursar la única lengua extrajera con un horario lectivo que superaba las 4 horas semanales en cada uno de los ciclos. Sin embargo, la Orden de 17 de marzo de 2015 (desarrollo normativo LOMCE) recoge hasta una segunda lengua extranjera y con una carga de docencia lectiva directa de los idiomas superior al doble de la normativa anterior. Además, el mapa de desarrollo curricular en esta última ley presenta los criterios de evaluación de cada uno de los ciclos y su relación con el resto de elementos curriculares, y no tiene como referente los contenidos para todo el desarrollo curricular, como ocurre en la LOE. En este sentido, aunque la tendencia de la enseñanza de las lenguas extranjeras en Andalucía sigue unos patrones muy ambiciosos a través del desarrollo de los currículos de las mismas desde la LOE hasta la LOMCE, prioriza el desarrollo de las destrezas en detrimento de la acumulación de contenidos. Este intento de enseñanza es un gran paso hacia otra dimensión en la forma de adquisición de las competencias clave.The Decree of August 10, 2007 (normative development LOE) establishes that students had to learn only one foreign language with a class schedule which exceeded 4 hours per week in each of the cycles. However, the Decree of March 17, 2015 (normative development LOMCE) collects up to a second foreign language and with a direct teaching load of language lessons higher than twice the previous regulations. In addition, the curricular development map in this last law presents the evaluation criteria of each of the cycle and their relation to the rest of the curricular elements. It does not have as a reference the contents for the whole curriculum development, as in the LOE. In this respect, although the trend of teaching foreign languages in Andalusia follows very ambitious patterns through the development of their curricula from the LOE to the LOMCE, it prioritizes the development of skills at the expense of the accumulation of content.     Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Robust estimation of bacterial cell count from optical density

    Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals <1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data

    A l occasion de l ouverture de la maison de santé de Cluny en janvier 2012, une nouvelle organisation des soins s est mise en place ainsi que l opportunité de réfléchir à des actions de prévention rendues possible par l exercice coordonné. Mais quelles sont les représentations et les attentes des différents professionnels de santé (intervenants ou non de la maison de santé), de quelques usagers représentatifs et de quelques personnalités qualifiées? Quel peut être l impact en termes de prévention? Ce travail est une étude qualitative effectuée à partir de 32 entretiens semi-dirigés réalisés entre le 20 janvier et le 13 avril 2012. L échantillon non représentatif comprenait 11 usagers et 26 professionnels de terrain. Les entretiens ont été retranscrits intégralement puis analysés de façon individuelle et transversale. Les interviewés considèrent unanimement que la maison de santé est un véritable atout pour le territoire. Elle représente un guichet unique, une structure rassurante pour la population. C est un gage de qualité de vie pour les professionnels, et de service rendu à la population. C est aussi un lieu d exercice coordonné pluridisciplinaire. Les intervenants attendent de la maison de santé la mise en place d actions de prévention, mais aussi une amélioration de la prise en charge globale des patients et une attractivité pour les professionnels de santé, en particulier les jeunes. Les représentations et attentes sont conformes aux priorités fixées par le Plan Régional de Santé en matière de prévention. En revanche, les usagers semblent plus demandeurs , et peu de professionnels de santé sont sensibilisés, probablement par manque de culture Santé Publique et par l existence de nombreux freins.     Resumen basado en el de la publicaciónResumen en inglésSe presenta una investigación que analiza la calidad de la formación virtual no universitaria debida al escenario de la docencia no presencial sobrevenida originada por el COVID-19, a través del instrumento SULODITOOL. Esta herramienta se desarrolló bajo los auspicios de otros instrumentos de valoración en el marco de la LomLOE, y está conformado por 10 indicadores de supervisión ponderados mediante lógica difusa. En este sentido, la agregación de las variables lingüísticas de 242 jueces expertos se realizó mediante la función OR probabilística y se desfusificaron mediante el método del centroide de área para calcular las citadas ponderaciones. Los resultados analíticos y gráficos de las ponderaciones evidencian que existen ciertos indicadores, como el diseño formativo y la metodología usada, que deberían contemplarse como factores clave en todos los escenarios estudiadosES

    Investigation of airways using MDCT for visual and quantitative assessment in COPD patients.

    International audienceMultidetector computed tomography (MDCT) acquisition during a single breath hold using thin collimation provides high resolution volumetric data set permitting multiplanar and three dimensional reconstruction of the proximal airways. In chronic obstructive pulmonary disease (COPD) patients, this technique provides an accurate assessment of bronchial wall thickening, tracheobronchial deformation, outpouchings reflecting dilatation of the submucous glands, tracheobronchomalacia, and expiratory air trapping. New software developed to segment adequately the lumen and walls of the airways on MDCT scans allows quantitative assessment of the airway dimensions which has shown to be reliable in clinical practice. This technique can become important in longitudinal studies of the pathogenesis of COPD, and in the assessment of therapeutic interventions

    Revisiting Species Identification within the Enterobacter cloacae Complex by Matrix-Assisted Laser Desorption Ionization–Time of Flight Mass Spectrometry

    International audienceMatrix-assisted laser desorption ionization–time of flight mass spectrometry (MALDI-TOF MS) is commonly used by clinical microbiology laboratories to identify pathogens, despite some limitations of the technique. The Enterobacter cloacae complex (ECC) taxonomy has recently been expanded, leading to uncertain identification of some species within the ECC when commercial MALDI-TOF MS is used. This technique is especially unsuited in the case of E. hormaechei, the main species responsible for infections and one of the most prone, within the ECC, to acquire antibiotic resistance. Hence, rapid and reliable identification at the species level could improve patient management. Here, we evaluated the performance of the Bruker Microflex MALDI-TOF MS instrument to identify ECC isolates using two databases and algorithms in comparison to the hsp60 gene sequencing reference method: the Bruker database included in the MALDI Biotyper software and an extensive online database coupled to an original Mass Spectrometric Identification (MSI) algorithm. Among a panel of 94 ECC isolates tested in triplicate, the online database coupled to MSI software allowed the highest rate of identification at the species level (92%) compared to the MALDI Biotyper database (25%), especially for the species E. hormaechei (97% versus 20%). We show that by creating a database of MALDI-TOF reference spectral profiles with a high number of representatives associated with the performant MSI software, we were able to substantially improve the identification of the E. cloacae complex members, with only 8% of isolates misidentified at the species level.     p53 dysregulation in B-cell malignancies: More than a single gene in the pathway to hell

    International audienceTP53 deletion or mutation is frequent in B-cell malignancies and is associated with a low response rate. We describe here the p53 landscape in B-cellmalignancies, from B-Acute Lymphoblastic Leukemia to Plasma Cell Leukemia, by analyzing incidence of gain or loss of function of actors both upstream and within the p53 pathway, namely MYC, RAS, ARF, MDM2, ATM and TP53. Abnormalities are not equally distributed and their incidence is highly variable among malignancies. Deletion and mutation, usually associated, of ATM or TP53 are frequent in Diffuse Large B-Cell Lymphoma and Mantle Cell Lymphoma. MYC gain, absent in post-GC malignancies, is frequent in B-Prolymphocytic-Leukemia,MultipleMyeloma and Plasma Cell Leukemias. RAS mutations are rare except in MM and PCL. Multiple Factorial Analysis notes that MYC deregulation is closely related to TP53 status. Moreover, MYC gain, TP53 deletion and RAS mutations are inversely correlated with survival. Based on this landscape, we further propose targeted therapeutic approaches for the different B-cell malignancies
