Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study
Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery. Actas de congresoLas VII Jornadas de Investigación “Encuentro y Reflexión” y I Jornadas de Investigación de becarios y doctorandos. Proyecto, investigación e innovación en Urbanismo, Arquitectura y Diseño Industrial se centraron en cuatro ejes: el proyecto; la dimensión tecnológica y la gestión; la dimensión social y cultural y la enseñanza en Arquitectura, Urbanismo y Diseño Industrial, sustentados en las líneas prioritarias de investigación definidas epistemológicamente en el Consejo Asesor de Ciencia y Tecnología de esta Universidad Nacional de Córdoba.
Con el objetivo de afianzar continuidad, formación y transferencia de métodos, metodología y recursos se incorporó becarios y doctorandos de los Institutos de investigación.
La Comisión Honoraria la integraron las tres Secretarias de Investigación de la Facultad, arquitectas Marta Polo, quien fundó y María del Carmen Franchello y Nora Gutiérrez Crespo quienes continuaron la tradición de la buena práctica del debate en la cotidianeidad de la propia Facultad.

Formation of DNA complexes in aqueous and organic solvents
From a physical-chemical point of view, it is challenging to form complexes with polyelectrolytes, consisting of only molecule of the largest component, i.e. the component with the highest number of charges. In this study, complexes are formed with DNA because of its potential applications as an artificial vector for gene delivery. The aim of this work is to prepare complexes in aqueous solutions as well as in organic solvents containing only one DNA molecule. For this purpose, the topology, equilibrium and conformation of complexes between a supercoiled DNA pUC19 (2686 base pairs) and
spermine containing hydrophilic and/or hydrophobic moieties or a polylysine with a hydrophilic block are determined by means of dynamic (DLS) and static light scattering (SLS), atomic force microscopy (AFM), and circular dichroism (CD) spectroscopy. It is demonstrated that all of these complexes consisted of only one molecule of the polyanion. Only the polylysine-b-polyethylene glycol copolymer satisfied the conditions: 1) 100% neutralization of DNA charges and with a small excess of the cation (lower than 30%) and 2) form stable complexes at every charge ratio. rnDNA complex formation is also investigated in organic solvents. Precipitation is induced by neutralizing the charge of the supercoiled DNA pUC19 with the surfactants dodecyltrimethylammonium bromide (DTAB) and tetradecyltrimethylammonium bromide (TTAB). After isolation and drying of the solids, the complexes are dissolved in organic solvents. DNA-TTA complexes are only soluble in methanol and DNA-DTA in DMF. The complexes again consisted of only
one DNA molecule. The final topology of the complexes is different in methanol than in DMF. In the former case, DNA seems to be compacted whereas in the latter case, the DNA-DTA complexes seem to have an expanded conformation. Upon complex formation with polycations in organic solvents (with polyvilylpyridine brush (b-PVP) in methanol and with a protected polylysine in DMF), DNA aggregates and precipitates. rnDNA is linearized with an enzyme (SmaI) to investigate the influence of the initial topology of the polyanion on the final conformation of the complexes in organic solvents. Two main differences are evidenced: 1. Complexes in organic solvents formed with linear DNA have in general a more expanded conformation and a higher tendency to aggregate. 2. If a polycation, i.e. the b-PVP, is added to the linear DNA-TTA complexes in methanol, complexes with the polycation are formed at a higher charge ratio. In DMF, the addition of the same b-PVP and of b-PLL did not lead to the formation of complexes.rnVom physiko-chemischen Gesichtspunkt aus ist es eine große Herausforderung Polyelektrolytkomplexe aus nur einem Molekül des größten Komponenten, z.B. aus dem mit der größten Ladungsdichte, zu formen. In dieser Arbeit wurden die Komplexe aus DNA wegen ihrer möglichen Verwendung als künstliche Vektoren in der Gentherapie verwendet. Hierbei war es das Ziel Komplexe sowohl in wässrigen als auch organischen Lösungsmittel mit nur einem DNA-Molekül zu bilden. Zu diesem Zwecke wurden Topologie, Gleichgewicht und Konformation der Komplexe zwischen supercoiled DNA pUC19 (2686 Basenpaare) und Spermin mit dessen hydrophilen und hydrophoben Resten oder Polylysin mit dessen hydrophilen Block durch
dynamische (DLS) und statischer Lichtstreuung (SLS), Rasterkraftmikroskopie oder CD-Spektroskopie untersucht. Ferner wurde nachgewiesen, dass alle Komplexe aus nur einem Molekül des Polyanions bestehen. Lediglich das Polylysin-b-Polyethylenglycol-Copolymer war in der Lage folgende Kriterien zu erfüllen: 1. 100% Neutralisation der DNA-Ladungen mit einem kleinen Überschuss an Kation (weniger 30%) und 2. Stabile Komplexe bei verschiedenen Ladungsverhältnissen rnDesweitern wurde die DNA-Komplexbildung auch in organischen Lösungsmitteln untersucht. Bei der Neutralisation der supercoiled pUC19 mit den Tensiden Dodecyltrimethylammoniumbromid (DTAB) und Tetradecyltrimethylammoniumbromid (TTAB) wurde diese ausgefällt. Nach Isolierung und Trocknung der Feststoffe wurden die Komplexe in organischen Lösemittel gelöst. DNA-TTA Komplexe sind nur in Methanol löslich, DNA-DTA hingegen nur in DMF. Es wurden unterschiedliche Topologien für die die verschiedenen Lösemittel gefunden. Im ersten Fall scheint es als würde die DNA
kompaktiert werden während im letzteren Fall scheint es hingegen so als würden sich die Komplexe mit Dodecylammonium in DMF ausbreiten. DNA-Komplexe mit Polykationen wie Polyvinylpyridin (b-PVP) zu in Methanol oder geschützten Polylysinbürsten (b-PLL) in DMF hingegen aggregieren und fallen aus. rnFerner wurde die DNA mit Hilfe eines Restriktionsenzyms (SmaI) linerisiert und dessen Einfluss von der ursprüngliche Topologie des Polyanions auf die finale Konformation der Komplexe in organischen Lösemittel zu untersuchen. Dabei wurden zwei große Unterschied deutlich: 1. Komplexe mit linearer DNA besitzen in organischen Lösemittel generell eine größere Ausdehnung und eine höhere Tendenz zu aggregieren. 2. Wenn ein Polykation wie zum Beispiel b-PVP zu den lineare DNA-TTA-Komplexen in Methanol hinzugegeben wird werden Komplexe mit einer höheren Ladungsdichte geformt. In DMF ist dies weder für das b-PVP noch für das b-PLL der Fall.r
An Analysis of the Rice-Cultivation Dynamics in the Lower Utcubamba River Basin Using SAR and Optical Imagery in Google Earth Engine (GEE)
One of the world’s major agricultural crops is rice (Oryza sativa), a staple food for more than half of the global population. In this research, synthetic aperture radar (SAR) and optical images are used to analyze the monthly dynamics of this crop in the lower Utcubamba river basin, Peru. In addition, this study addresses the need to obtain accurate and timely information on the areas under cultivation in order to calculate their agricultural production. To achieve this, SAR sensor and Sentinel-2 optical remote sensing images were integrated using computer technology, and the monthly dynamics of the rice crops were analyzed through mapping and geometric calculation of the surveyed areas. An algorithm was developed on the Google Earth Engine (GEE) virtual platform for the classification of the Sentinel-1 and Sentinel-2 images and a combination of both, the result of which was improved in ArcGIS Pro software version 3.0.1 using a spatial filter to reduce the “salt and pepper” effect. A total of 168 SAR images and 96 optical images were obtained, corrected, and classified using machine learning algorithms, achieving a monthly average accuracy of 96.4% and 0.951 with respect to the overall accuracy (OA) and Kappa Index (KI), respectively, in the year 2019. For the year 2020, the monthly averages were 94.4% for the OA and 0.922 for the KI. Thus, optical and SAR data offer excellent integration to address the information gaps between them, are of great importance to obtaining more robust products, and can be applied to improving agricultural production planning and management
Solution Properties and Potential Biological Applications of Zwitterionic Poly(ε‑<i>N</i>‑methacryloyl‑l‑lysine)
Poly(ε-<i>N</i>-methacryloyl-l-lysine)
(PMALys) was synthesized by free radical polymerization yielding a
zwitterionic polymer with <i>M</i><sub>w</sub> = 721 000
g mol<sup>–1</sup>. The polymer dissolves in pure water as
well as in aqueous salt solution up to 5 M NaClO<sub>4</sub> and over
wide range of pH values (1.3 ≤ pH ≤ 12.7) as single
chains without any sign for aggregate formation. The zwitterionic
polymer shows an expanded random coil structure at and close to isoelectric
conditions and further expands upon addition of acid and base, respectively.
The polymer fulfills four major prerequisites for a promising nano
carrier in potential biomedical applications: (1) It is biocompatible,
indicated by a low cytotoxicity. (2) It does not aggregate in concentrated
human blood serum solution. (3) The amino groups in the polyzwitterion
may be utilized for conjugations as demonstrated by labeling reactions
with AlexaFluor488. (4) Cell uptake experiments revealed little uptake
in bone marrow dendritic cells, i.e., little <i>unspecific</i> uptake, which is mandatory for a successful specific targeting of
cells. Finally, upon addition of Zn<sup>2+</sup> ions the polyzwitterions
may be converted into polycations which are demonstrated to form complexes
with DNA. Such complexes may be advantageous for application in gene
transfection studies
Pneumocystis
Introduction: Specific environmental factors may play a role in the development of Pneumocystis pneumonia (PCP) in HIV-positive patients. The aim of this study was to estimate the PCP incidence and mortality in hospitalized HIV-positive patients in Spain during the combination antiretroviral therapy (cART) era (1997 to 2011), as well as to analyze the climatological factors and air pollution levels in relation to hospital admissions and deaths. Methods: We carried out a retrospective study. Data were collected from the National Hospital Discharge Database and the State Meteorological Agency of Spain. A case-crossover analysis was applied to identify environmental risk factors related to hospitalizations and deaths. For each patient, climatic factors and pollution levels were assigned based on readings from the nearest meteorological station to his or her postal code. Results: There were 13,139 new PCP diagnoses and 1754 deaths in hospitalized HIV-positive patients from 1997 to 2011. The PCP incidence (events per 1000 person-years) dropped from 11.6 in 1997 to 2000, to 5.4 in 2004 to 2011 (p<0.001). The mortality (events per 10,000 person-years) also decreased from 14.3 in 1997 to 2000, to 7.5 in 2004 to 2011 (p<0.001). Most hospital admissions and deaths occurred in the winter season and the fewest occurred in the summer, overlapping respectively with the lowest and highest temperatures of the year in Spain. Moreover, lower temperatures prior to PCP admission, as well as higher concentrations of NO2 and particulate matter up to 10 m in size (PM10) at the time of admission were associated with higher likelihoods of hospital admission due to PCP when two weeks, one month, 1.5 months or two months were used as controls (p<0.01). Furthermore, higher concentrations of ozone at one month (p=0.007), 1.5 months (p<0.001) and two months (p=0.006) prior to admission were associated with higher likelihoods of hospital admission with PCP. For PCP-related deaths, lower temperatures prior to admission and higher concentrations of atmospheric PM10 at the time of admission were related to higher likelihood of death when two weeks, one month and 1.5 months were used as controls (p<0.05). Conclusions: PCP was a significant health problem in the cART era (1997 to 2011), and PCP epidemiology was adversely influenced by colder climatological factors and higher ambient air pollution levels

La anterior selección, motivados por la aseveración de Manuel Castells -hace casi 20 años ya- que la innovación y la difusión de la tecnología parecía ser la herramienta apropiada para el desarrollo en la era de la información. Este 2020, sin embargo, ante la situación disruptiva que aquejó y aqueja a la sociedad red como una estructura social emergente de la Era de la Información basada en redes de producción, energizadas por el poder y la experiencia; falló y debe reencontrar su rumbo. Es así que los problemas acuciantes, ahora, fueron: la atención sanitaria y la superación de la epidemia de Sars Cov 2; tomó forma la, hasta entonces, visión irrealista de Castells que … no podemos avanzar con nuestros modelos de desarrollo actual, destruyendo nuestro entorno y excluyendo a la mayor parte de la humanidad de los beneficios de la revolución tecnológica más extraordinaria de la historia, sin sufrir una devastadora reacción por parte de la sociedad y la naturaleza.
Fue así que el Cuarto Mundo, específicamente, donde la suficiencia de recurso humano, de capital, trabajo, información y mercado -vinculados todos a través de la tecnología- supuso que atendería eficazmente a través de la población que podía por su capacidad hacer uso racional y profesional del conocimiento, las necesidades de la mayoritaria población vulnerable y vulnerada.
Por lo anterior, poner en el centro a las personas, en entornos de tarea y trabajo globales hiperconectados combinando espacios físicos, corrientes de información con canales de conexión expeditos, y formando profesionales del conocimiento que asuman y afronten los retos derivados de la transformación digital de empresas, universidades, y organizaciones, pero en condiciones de equidad y sujetos de prosperidad, será el desafío en los escenarios presentes y futuros inmediatos.The previous selection, motivated by the assertion of Manuel Castells -almost 20 years ago- that innovation and diffusion of technology seemed to be the appropriate tool for development in the information age. This 2020, however, in the face of the disruptive situation that afflicted and continues to afflict the network society as an emerging social structure of the Information Age based on production networks, energized by power and experience; He failed and must find his way again. It was thus that the Fourth World, specifically, where the sufficiency of human resources, capital, work, information and market - all linked through technology - meant that it would serve effectively through the population that could, due to its capacity, make rational use. and knowledge professional, the needs of the majority vulnerable and vulnerable population.
Therefore, putting people at the center, in hyperconnected global task and work environments, combining physical spaces, information flows with expedited connection channels, and training knowledge professionals who assume and face the challenges derived from the digital transformation of companies, universities, and organizations, but in conditions of equality and subject to prosperity, will be the challenge in the present and immediate future scenarios.Bogot
