DIAGNÓSTICOS DE ENFERMAGEM EM PESSOAS SOROPOSITIVAS PELO VÍRUS LINFOTRÓPICO T HUMANO
Objetivo: identificar diagnósticos de enfermagem em pessoas sintomáticas e soropositivas para o vírus linfotrópico T humano. Método: pesquisa documental, realizada nos prontuários de pessoas soropositivas para o vírus linfotrópico T humano, cadastradas no Centro de Testagem e Aconselhamento de um município do interior do estado da Bahia. A análise dos registros foi realizada com base na Taxonomia II da NANDA-I 2018-2020. Resultados: foram identificados 13 diagnósticos de enfermagem distribuídos em 5 domínios: Conforto; Eliminação e Troca; Atividade e repouso; Autopercepção; Enfrentamento/Tolerância ao estresse. Os diagnósticos mais prevalentes foram: Dor crônica, em 21 (100%) dos soropositivos, Incontinência urinária em nove (43%), e Deambulação prejudicada em 8 (38%). Conclusão: a identificação dos diagnósticos em pessoas sintomáticas e soropositivas para o vírus linfotrópico T humano fornece subsídios para a construção de planos de cuidados de enfermagem específicos.Descritores: Vírus 1 Linfotrópico T humano. Ações de fortalecimento de vínculos interpessoais entre os agentes comunitários de saúde de um Centro de Saúde da Família
O Agente Comunitário de Saúde integra a equipe de saúde da Atenção Primária à Saúde, favorecendo a criação de vínculos, além de garantir que as ações de saúde adentrem no domicílio das famílias. Logo, são trabalhadores com carga de trabalho intensa, em condições de riscos ocupacionais e sobrecarga que podem causar adoecimento. O estudo teve como objetivo desenvolver ações para fortalecimento de vínculos interpessoais entre os agentes comunitários de saúde de um centro de saúde da família. Trata-se de pesquisa de intervenção, realizada em um Centro de Saúde da Família no interior do estado do Ceará, em setembro de 2021. Os participantes foram oito Agentes Comunitários de Saúde. A intervenção aconteceu em cinco momentos distintos, semanalmente, com duração de 40 minutos. Para registro de cada encontro e coleta de dados, utilizou-se um diário de campo e um instrumento de avaliação com cinco questões subjetivas. Para interpretação e análise das falas registradas, utilizou-se o método de análise de conteúdo de Bardin. A análise dos discursos durante os encontros de intervenção possibilitou a identificação de quatro categorias importantes, sendo elas: Família; Desafios no Ambiente de Trabalho; Valorização Profissional e Saúde no Trabalho. A categoria “Família” foi trazida quase sempre como um fator de manutenção do trabalho. Já na categoria “Desafios no Ambiente de Trabalho”, observou-se a pressão no trabalho, a falta de compreensão e sentimento de raiva/agressividade verbal. Na categoria “Valorização Profissional”, identificou-se o sentimento de desvalorização da categoria. Na categoria “Saúde no Trabalho”, pôde-se avaliar que as participantes se encontravam relativamente bem, mas foi evidente o cansaço e a sobrecarga de trabalho. Os encontros semanais de intervenção propiciaram a expressão de sentimentos e vivências únicas, comunicação e integração em equipe, contribuindo com o fortalecimento das relações interpessoais e qualificação do trabalho na unidade de saúde.
Nandrolone and resistance training induce heart remodeling: Role of fetal genes and implications for cardiac pathophysiology
Aims: This study was conducted to assess the isolated and combined effects of nandrolone and resistance training on cardiac morphology, function, and mRNA expression of pathological cardiac hypertrophy markers.Main methods: Wistar rats were randomly divided into four groups and submitted to 6 weeks of treatment with nandrolone and/or resistance training. Cardiac parameters were determined by echocardiography. Heart was analyzed for collagen infiltration. Real-time RT-PCR was used to assess the pathological cardiac hypertrophy markers.Key findings: Both resistance training and nandrolone induced cardiac hypertrophy. Nandrolone increased the cardiac collagen content, and reduced the cardiac index in non-trained and trained groups, when compared with the respective vehicle-treated groups. Nandrolone reduced the ratio of maximum early to late transmitral flow velocity in non-trained and trained groups, when compared with the respective vehicle-treated groups. Nandrolone reduced the alpha-myosin heavy chain gene expression in both non-trained and trained groups, when compared with the respective vehicle-treated groups. Training reduced the beta-myosin heavy chain gene expression in the groups treated with vehicle and nandrolone. Only the association between training and nandrolone increased the expression of the skeletal alpha-actin gene and atrial natriuretic peptide in the left ventricle.Significance: This study indicated that nandrolone, whether associated with resistance training or not, induces cardiac hypertrophy, which is associated with enhanced collagen content, re-expression of fetal genes the in left ventricle, and impaired diastolic and systolic function. Ischemic stroke caused by large-artery atherosclerosis: a red flag for subclinical coronary artery disease
BackgroundThe coronary calcium score (CAC) measured on chest computerized tomography is a risk marker of cardiac events and mortality. We compared CAC scores in two multiethnic groups without symptomatic coronary artery disease: subjects in the chronic phase after stroke or transient ischemic attack and at least one symptomatic stenosis ≥50% in the carotid or vertebrobasilar territories (Groupathero) and a control group (Groupcontrol).MethodsIn this cross-sectional study, Groupathero included two subgroups: GroupExtraorIntra, with stenoses in either cervical or intracranial arteries, and GroupExtra&Intra, with stenoses in at least one cervical and one intracranial artery. Groupcontrol had no history of prior stroke/transient ischemic attacks and no stenoses ≥50% in cervical or intracranial arteries. Age and sex were comparable in all groups. Frequencies of CAC ≥100 and CAC > 0 were compared between Groupathero and Groupcontrol, as well as between GroupExtraorIntr, GroupExtra&Intra, and Groupcontrol, with bivariate logistic regressions. Multivariate analyses were also performed.ResultsA total of 120 patients were included: 80 in Groupathero and 40 in Groupcontrol. CAC >0 was significantly more frequent in Groupathero (85%) than Groupcontrol (OR, 4.19; 1.74–10.07; p = 0.001). Rates of CAC ≥100 were not significantly different between Groupathero and Groupcontrol but were significantly greater in GroupExtra&Intra (n = 13) when compared to Groupcontrol (OR 4.67; 1.21–18.04; p = 0.025). In multivariate-adjusted analyses, “Groupathero” and “GroupExtra&Intra” were significantly associated with CAC.ConclusionThe frequency of coronary calcification was higher in subjects with stroke caused by large-artery atherosclerosis than in controls
Genomics and epidemiology for gastric adenocarcinomas (GE4GAC): a Brazilian initiative to study gastric cancer
Abstract Gastric cancer (GC) is the fifth most common type of cancer worldwide with high incidences in Asia, Central, and South American countries. This patchy distribution means that GC studies are neglected by large research centers from developed countries. The need for further understanding of this complex disease, including the local importance of epidemiological factors and the rich ancestral admixture found in Brazil, stimulated the implementation of the GE4GAC project. GE4GAC aims to embrace epidemiological, clinical, molecular and microbiological data from Brazilian controls and patients with malignant and pre-malignant gastric disease. Local hydrological conditions influence tree diversity and composition across the Amazon basin
Tree diversity and composition in Amazonia are known to be strongly determined by the water supplied by precipitation. Nevertheless, within the same climatic regime, water availability is modulated by local topography and soil characteristics (hereafter referred to as local hydrological conditions), varying from saturated and poorly drained to well-drained and potentially dry areas. While these conditions may be expected to influence species distribution, the impacts of local hydrological conditions on tree diversity and composition remain poorly understood at the whole Amazon basin scale. Using a dataset of 443 1-ha non-flooded forest plots distributed across the basin, we investigate how local hydrological conditions influence 1) tree alpha diversity, 2) the community-weighted wood density mean (CWM-wd) – a proxy for hydraulic resistance and 3) tree species composition. We find that the effect of local hydrological conditions on tree diversity depends on climate, being more evident in wetter forests, where diversity increases towards locations with well-drained soils. CWM-wd increased towards better drained soils in Southern and Western Amazonia. Tree species composition changed along local soil hydrological gradients in Central-Eastern, Western and Southern Amazonia, and those changes were correlated with changes in the mean wood density of plots. Our results suggest that local hydrological gradients filter species, influencing the diversity and composition of Amazonian forests. Overall, this study shows that the effect of local hydrological conditions is pervasive, extending over wide Amazonian regions, and reinforces the importance of accounting for local topography and hydrology to better understand the likely response and resilience of forests to increased frequency of extreme climate events and rising temperatures
Estimating the global conservation status of more than 15,000 Amazonian tree species
Estimates of extinction risk for Amazonian plant and animal species are rare and not often incorporated into land-use policy and conservation planning. We overlay spatial distribution models with historical and projected deforestation to show that at least 36% and up to 57% of all Amazonian tree species are likely to qualify as globally threatened under International Union for Conservation of Nature (IUCN) Red List criteria. If confirmed, these results would increase the number of threatened plant species on Earth by 22%. We show that the trends observed in Amazonia apply to trees throughout the tropics, and we predict thatmost of the world’s >40,000 tropical tree species now qualify as globally threatened. A gap analysis suggests that existing Amazonian protected areas and indigenous territories will protect viable populations of most threatened species if these areas suffer no further degradation, highlighting the key roles that protected areas, indigenous peoples, and improved governance can play in preventing large-scale extinctions in the tropics in this century
Pervasive gaps in Amazonian ecological research
Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4
While the increasing availability of global databases on ecological communities has advanced our knowledge
of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In
the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of
Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus
crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced
environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian
Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by
2050. This means that unless we take immediate action, we will not be able to establish their current status,
