    A MINIMAL INVASIVE TECHNIQUE FOR A LONG TRANS-SPHINCTERIC FISTULA IN ANO- A CASE STUDY

    Fistula in ano is an uncommon condition of the GIT, with a prevalence of 0.01% in the general population, with higher frequency in men than in women at 2:1 ratio. Complete fistulotomy is the current surgical procedure opted by many surgeons but it may concedes potential complications such as displacement of the anal canal, incontinence, uneven gluteal contour with ugly scar and high rate of recurrence. Presentation of case: Study was conducted on a male patient aged 28 years, with non-healing wound on left gluteal region since eight months pain and swelling in perianal region since two days, on per rectal examination internal opening was found on 6 o clock and external opening at 4 o clock approx.6 inches from the anal verge, diagnosed as trans-sphincteric fistula in ano. In this case study we applied a technique to save the tissue and low complications. We applied two Ksharsutra, one for cutting the tract and second for drainage was used. Second Ksharsutra removed within 2 week after proper drainage. After 6 Ksharsutra change cut through was done at 6 o clock position .wound was healed after six weeks. Conclusion: Fistula in ano having distant external opening such as in gluteal region is rare condition. The present study involved a minimal invasive technique which has negligible complications with a sure cure

    Envisioning the future of behavioral decision-making: A systematic literature review of behavioral reasoning theory

    Behavioral theories have been extensively referred to in consumer behavior literature to understand the factors influencing user intentions and behavior. Behavioral reasoning theory (BRT) is a relatively new theory that determines the linkage between beliefs, reasons, motives, intentions, and behavior. This study conducts a systematic literature review (SLR) to synthesize the existing body of knowledge around BRT. The present SLR critically examines the origin of this theory and its linkages with other behavioral theories, thus providing insightful knowledge on its foundations. Further, it presents the state-of-the-art research knowledge, research themes, implications, and future directions related to BRT literature. Our analysis shows that the reasons for and reasons against construct plays an important role in predicting consumer behavior. This study also finds that research related to BRT is growing rapidly and needs methodological advancements. These findings will enable scholars and practitioners to better understand how BRT works, what its strengths and potential are, the contexts in which it has been utilized, its existing limitations, and the sort of methodological advancements needed in future studies on marketing.</p

    A PROSPECTIVE STUDY ON EVALUATION OF DIAGNOSTIC PERFORMANCE OF GENEXPERT MTB IN CSF FOR EARLY DIAGNOSIS OF TUBERCULAR MENINGITIS IN A TERTIARY CARE HOSPITAL ODISHA.

    Aim The study aims to establish the diagnostic accuracy of GeneXpert MTB/RIF in CSF for early diagnosis of tubercular meningitis and to compare the efficacy of CSF GeneXpert MTB/RIF with CSF culture for mycobacterium. Methodology This was a prospective, cross-sectional study conducted in the Department of Medicine, Neurology, S.C.B Medical College, Cuttack. All patients of age &gt;18 with clinical features suggestive of tubercular meningitis were included in the study. All routine blood tests were performed along with malaria, leptospira, chest radiograph, CT scan or MRI (selected patients) and CSF study to exclude other causes of meningitis. CSF sample subjected to biochemistry, cytology, ZN stain, MGIT culture, and Xpert MTB/RIF. The positive results for each test (ZN stain, MGIT culture, and Xpert MTB/RIF) were compared using Pearson’s chi-squared test. All statistical analyses were done using the SPSS 21.0 version. Results Out of 100 suspected TBM patients 40 were finally classified as definite TBM, 6 were probable TBM, 2 were possible TBM, and 52 were not having TBM. Tubercular meningitis occurred more commonly in the population 21-40 years and in males. The overall sensitivity of CSF GeneXpert MTB/RIF, Zn stain, and MGIT culture was 62.5%, 29.16%, and 66.5% respectively, and specificity of 100% for each in diagnosing TBM. Rifampicin resistance was detected only in two cases. Conclusion GeneXpert MTB/RIF test can rapidly confirm a diagnosis of TBM with 62.5% sensitivity and 100% specificity, along with rifampicin resistance. It can be a useful diagnostic method in patients of suspected TBM either AFB smear-negative or positive due to its rapidity and simultaneous detection of rifampicin resistance. Recommendation Positive GeneXpert results are to be read cautiously and should be well correlated with the clinical and treatment history of the patient

    Dysregulated miRNAome and Proteome of PPRV Infected Goat PBMCs Reveal a Coordinated Immune Response

    In this study, the miRNAome and proteome of virulent Peste des petits ruminants virus (PPRV) infected goat peripheral blood mononuclear cells (PBMCs) were analyzed. The identified differentially expressed miRNAs (DEmiRNAs) were found to govern genes that modulate immune response based on the proteome data. The top 10 significantly enriched immune response processes were found to be governed by 98 genes. The top 10 DEmiRNAs governing these 98 genes were identified based on the number of genes governed by them. Out of these 10 DEmiRNAs, 7 were upregulated, and 3 were downregulated. These include miR-664, miR-2311, miR-2897, miR-484, miR-2440, miR-3533, miR-574, miR-210, miR-21-5p, and miR-30. miR-664 and miR-484 with proviral and antiviral activities, respectively, were upregulated in PPRV infected PBMCs. miR-210 that inhibits apoptosis was downregulated. miR-21-5p that decreases the sensitivity of cells to the antiviral activity of IFNs and miR-30b that inhibits antigen processing and presentation by primary macrophages were downregulated, indicative of a strong host response to PPRV infection. miR-21-5p was found to be inhibited on IPA upstream regulatory analysis of RNA-sequencing data. This miRNA that was also highly downregulated and was found to govern 16 immune response genes in the proteome data was selected for functional validation vis-a-vis TGFBR2 (TGF-beta receptor type-2). TGFBR2 that regulates cell differentiation and is involved in several immune response pathways was found to be governed by most of the identified immune modulating DEmiRNAs. The decreased luciferase activity in Dual Luciferase Reporter Assay indicated specific binding of miR-21-5p and miR-484 to their target thus establishing specific binding of the miRNAs to their targets.This is the first report on the miRNAome and proteome of virulent PPRV infected goat PBMCs

    Comparative and temporal transcriptome analysis of peste des petits ruminants virus infected goat peripheral blood mononuclear cells

    Peste des petits ruminanats virus (PPRV), a morbillivirus causes an acute, highly contagious disease – peste des petits ruminants (PPR), affecting goats and sheep. Sungri/96 vaccine strain is widely used for mass vaccination programs in India against PPR and is considered the most potent vaccine providing long-term immunity. However, occurrence of outbreaks due to emerging PPR viruses may be a challenge. In this study, the temporal dynamics of immune response in goat peripheral blood mononuclear cells (PBMCs) infected with Sungri/96 vaccine virus was investigated by transcriptome analysis. Infected goat PBMCs at 48 h and 120 h post infection revealed 2540 and 2000 differentially expressed genes (DEGs), respectively, on comparison with respective controls. Comparison of the infected samples revealed 1416 DEGs to be altered across time points. Functional analysis of DEGs reflected enrichment of TLR signaling pathways, innate immune response, inflammatory response, positive regulation of signal transduction and cytokine production. The upregulation of innate immune genes during early phase (between 2-5 days) viz. interferon regulatory factors (IRFs), tripartite motifs (TRIM) and several interferon stimulated genes (ISGs) in infected PBMCs and interactome analysis indicated induction of broad-spectrum anti-viral state. Several Transcription factors – IRF3, FOXO3 and SP1 that govern immune regulatory pathways were identified to co-regulate the DEGs. The results from this study, highlighted the involvement of both innate and adaptive immune systems with the enrichment of complement cascade observed at 120 h p.i., suggestive of a link between innate and adaptive immune response. Based on the transcriptome analysis and qRT-PCR validation, an in vitro mechanism for the induction of ISGs by IRFs in an interferon independent manner to trigger a robust immune response was predicted in PPRV infection

    Dynamics of Hot QCD Matter -- Current Status and Developments

    The discovery and characterization of hot and dense QCD matter, known as Quark Gluon Plasma (QGP), remains the most international collaborative effort and synergy between theorists and experimentalists in modern nuclear physics to date. The experimentalists around the world not only collect an unprecedented amount of data in heavy-ion collisions, at Relativistic Heavy Ion Collider (RHIC), at Brookhaven National Laboratory (BNL) in New York, USA, and the Large Hadron Collider (LHC), at CERN in Geneva, Switzerland but also analyze these data to unravel the mystery of this new phase of matter that filled a few microseconds old universe, just after the Big Bang. In the meantime, advancements in theoretical works and computing capability extend our wisdom about the hot-dense QCD matter and its dynamics through mathematical equations. The exchange of ideas between experimentalists and theoreticians is crucial for the progress of our knowledge. The motivation of this first conference named "HOT QCD Matter 2022" is to bring the community together to have a discourse on this topic. In this article, there are 36 sections discussing various topics in the field of relativistic heavy-ion collisions and related phenomena that cover a snapshot of the current experimental observations and theoretical progress.     Azimuthal anisotropy of charged jet production in root s(NN)=2.76 TeV Pb-Pb collisions

    We present measurements of the azimuthal dependence of charged jet production in central and semi-central root s(NN) = 2.76 TeV Pb-Pb collisions with respect to the second harmonic event plane, quantified as nu(ch)(2) (jet). Jet finding is performed employing the anti-k(T) algorithm with a resolution parameter R = 0.2 using charged tracks from the ALICE tracking system. The contribution of the azimuthal anisotropy of the underlying event is taken into account event-by-event. The remaining (statistical) region-to-region fluctuations are removed on an ensemble basis by unfolding the jet spectra for different event plane orientations independently. Significant non-zero nu(ch)(2) (jet) is observed in semi-central collisions (30-50% centrality) for 20 <p(T)(ch) (jet) <90 GeV/c. The azimuthal dependence of the charged jet production is similar to the dependence observed for jets comprising both charged and neutral fragments, and compatible with measurements of the nu(2) of single charged particles at high p(T). Good agreement between the data and predictions from JEWEL, an event generator simulating parton shower evolution in the presence of a dense QCD medium, is found in semi-central collisions.     Production of He-4 and (4) in Pb-Pb collisions at root(NN)-N-S=2.76 TeV at the LHC

    Results on the production of He-4 and (4) nuclei in Pb-Pb collisions at root(NN)-N-S = 2.76 TeV in the rapidity range vertical bar y vertical bar <1, using the ALICE detector, are presented in this paper. The rapidity densities corresponding to 0-10% central events are found to be dN/dy4(He) = (0.8 +/- 0.4 (stat) +/- 0.3 (syst)) x 10(-6) and dN/dy4 = (1.1 +/- 0.4 (stat) +/- 0.2 (syst)) x 10(-6), respectively. This is in agreement with the statistical thermal model expectation assuming the same chemical freeze-out temperature (T-chem = 156 MeV) as for light hadrons. The measured ratio of (4)/He-4 is 1.4 +/- 0.8 (stat) +/- 0.5 (syst). (C) 2018 Published by Elsevier B.V.Peer reviewe
