    A high oleic sunflower oil fatty acid esters of plant sterols mixed with dietary diacylglycerol reduces plasma insulin and body fat accumulation in Psammomys obesus

    <p>Abstract</p> <p>Background</p> <p>Metabolic syndrome is associated with subsequent development of cardiovascular diseases and type 2 diabetes. It is characterized by reduced response to insulin, central obesity, and dyslipidemia. Intake of plant sterols (PS) has been shown to confer a healthier lipid profile and ameliorate cardiovascular disease risk factors in experimental animals and humans. In this study we used an animal model of type 2 diabetes to assess the effects of a preparation of PS esterified to high oleic sunflower oil fatty acids mixed with dietary diacylglycerol (PS-HOSO) on diabetic related metabolic parameters. <it>Psammomys obesus </it>(<it>P. obesus</it>) were fed high energy (HE) diet supplemented by either PS-HOSO or control oil. Following 4.5 weeks of intervention, animals were divided into fasting and non-fasting modes prior to outcome measurements. Glucose and insulin levels as well as blood lipid profile, body weight, and fat accumulation were evaluated in fasting and non-fasting modes.</p> <p>Results</p> <p><it>P. obesus </it>fed with a HE diet displayed a characteristic heterogeneity in their blood glucose and insulin levels with a subset group displaying type 2 diabetes symptoms. PS-HOSO treatment significantly reduced total cholesterol (24%, <it>P </it>< 0.001) and non-HDL cholesterol (34%, <it>P </it>< 0.01) compared to the control diet. Among fasting animals, body weight at end point and epididymal fat-to-liver weight ratio were significantly (<it>P </it>< 0.05 each) reduced (7% and 16%, respectively) compared to controls. Interestingly, fasting blood glucose levels were similar between groups, whereas plasma insulin level at end point was 44% lower in the PS-HOSO group compared to control group (<it>P </it>< 0.0001)</p> <p>Conclusion</p> <p>PS-HOSO supplementation to diabetes-prone gerbils counteracts the increase in body weight and epididymal fat accumulation, and also results in a drop in circulating insulin levels. These effects are pointing out that PS-HOSO may serve as a functional ingredient for metabolic syndrome or diabetic sufferers, which not only influences body weight, but also prevents or reverses insulin resistance and hyperlipidemia.</p

    Use of Optical Coherence Tomography to Detect Retinal Nerve Fiber Loss in Children With Optic Pathway Glioma

    Optic pathway glioma (OPG) presents in childhood and can cause significant morbidity and visual loss. Magnetic resonance imaging (MRI) is the current imaging modality of choice for evaluation of OPG progression, but it is a relatively limited resource often requiring sedation in the pediatric age group. Additionally, OPG progression on MRI does not always correlate with clinical progression. As a result, several other modalities for evaluating OPG are being investigated, including optical coherence tomography (OCT), a readily available imaging technique in ophthalmic practice. The purpose of the present study was to examine the association between retinal nerve fiber layer (RNFL) thickness measured using OCT and optic nerve function in children with OPG with and without neurofibromatosis-1 (NF-1). A retrospective chart review was conducted to identify children diagnosed with OPG from 2001 to 2015 at a tertiary pediatric medical center. The correlation between OCT measurements and clinical visual parameters was statistically analyzed. Included were 23 children with imaging-confirmed OPG and spectral domain OCT: 10 with NF-1 (mean age at diagnosis 5.8 years) and 13 without (mean age at diagnosis 5.9 years). The glioma involved the chiasma-hypothalamus in 19 patients, optic nerve in 11, and optic tract in 7; more than one anatomic site was affected in 15. Symptoms were reported in 2 patients with NF-1 and most patients without NF-1. Visual field defects included monocular, bitemporal, nasal, and homonymous hemianopia. Initial mean RNFL was 85.4 μm in the NF-1 group and 65 μm in the non-NF-1 group. Visual acuity deteriorated in 1/10 patients and 5/13 patients, respectively. Repeated OCT showed continued RNFL thinning in 3 patients (5 eyes) in the NF-1 group and in 8 patients (11 eyes) in the non-NF-1 group, often associated with a decrease in optic nerve function. In conclusion, visual function in children with OPG is correlated with repeated OCT measurements and weakly with neuroimaging. Children without NF-1 are usually symptomatic and have a worse clinical outcome. These findings may have important implications when considering initiating, continuing or stopping chemotherapy for OPG. The application of OCT in the assessment of OPG and the correlation of the findings to clinical progression can have a significant impact on OPG patient management

    Perceived language proficiency and pain assessment by registered and student nurses in native English-speaking and EAL children aged 4-7 years

    Aims and Objectives To identify the factors that influence decisions made by health professionals when assessing the pain of native English speaking and children whose English is an Additional Language (EAL). Background Pain assessment in children is often poorly executed following acute injury. Whilst a range of pain assessment tools have been developed, little guidance is provided for assessing pain in EAL. Design Factorial survey design. Methods Twenty Minor Injuries Unit (MIU) nurses and twenty children’s nursing students participated in an electronic survey to make judgments on 12 scenarios describing a child attending a MIU following an incident, accompanied by a parent. Respondents had to decide the most important form of pain assessment, and whether they would ask a parent or an interpreter to assess the pain of the child. An open-ended question asked about the difficulties found in making a judgment. Results Observation of the child’s behaviour was the most common pain assessment reported. The Visual Analogue Scale was significantly associated with children with proficient English. Respondents were significantly more likely to involve parents in the assessment if they could speak English well compared to parents with poor English skills. Moreover, nursing students were significantly more likely than registered nurses to call for support from an interpreter. Thematic analysis identified three themes related to difficulties with pain assessment: contrasting approaches, differing perceptions of pain, and overcoming challenges. Conclusions The reduced ability to communicate between child, parent and healthcare professional highlights the need to identify forms of assessment based on individual cases. Relevance to clinical practice The number of children with EAL has seen a marked rise over the last decade. In situations where communication ability is reduced, assessment of pain should tailored to meet the needs of the child. This may require timely access to interpreter services

    A rational design for a nanoparticle is suggested, which will maximize its arrival efficiency from the plasma membrane to the nuclear surrounding. The design is based on grafting the particle surface with polymer spacers, each ending with a motor protein associating molecule, for example, nuclear localization signal peptide. It is theoretically shown that the spacer polymer molecular weight can be adjusted to significantly increase the effective particle processivity time. This should lead to appreciable enhancement of active transport of the nanocarrier, and consequently drug delivery, to the nucleus

    Geodesic Mean Shift

    In this paper we introduce a versatile and robust method for analyzing the feature space associated with a given surface. The method is based on the mean-shift operator which was shown to be successful in image and video processing. Its strength stems from the fact that it works in a single space of joint geometry and attributes called the feature-space. The feature-space attributes can be scalar or vector, and are either given (e.g. temperature, pressure) or extracted (e.g. curvature, geodesic centricity). The procedure works as a gradient descent that finds maxima of an estimated probability density function in feature-space. Using this technique on surfaces introduces several difficulties. First, meshes as opposed to images do not present a regular and uniform sampling of the feature-space. Second, on boundary embedded surfaces the shifting procedure must be constrained to stay on the surface, obeying geodesic distances. Based on local parameterizations scheme, we generalize the meanshift procedure to unstructured embedded boundary meshes. Furthermore, we also support piecewise linear attribute definitions and not only piecewise constant by employing hardware-supported area sampling using rasterization
