    Factors affecting the uptake of Exclusive Breastfeeding (EBF) in Kisumu East District, Kenya

    Exclusive breastfeeding (EBF) the best means of infant nutrition. This is because it offers great benefits to maternal and child health in general, including prevention of vertical transmission of HIV/ AIDS. This study sought to identify the factors associated with the uptake of EBF among mothers with children aged 0-<6 months. Overall, the uptake of EBF was higher than is seen in other countries in Africa and across the globe. Binary logistic regression identified having male children and ownership of certain assets as positive predictors of EBF uptake. Absence of a chronic disease and knowledge of cultural and traditional practices surrounding EBF were shown to be negatively associated with uptake of EBF.     The TARGET_TIA: A complete, flexible and sound territorial impact assessment tool

    This chapter presents TARGET_TIA as a relevant and flexible Territorial Impact Assessment (TIA) methodology. TARGET_TIA was created in a context where existing ESPON TIA tools were mainly designed for assessing ex ante territorial impacts of EU directives. Hence, in view of the need to properly assess the main ex post territorial impacts of EU Cohesion Policy in several countries in a relevant and sound way, the author decided to design, test and apply his own TIA methodology, named TARGET_TIA. When compared with other existing TIA methodologies, TARGET_TIA can be used both at ex ante and ex post policy evaluation phases. In addition, it brings to the table the possibility to use counterfactual evaluation elements to allow the production of credible and sound TIA evaluation scores. Following on from its implementation in assessing the main territorial impacts of EU policies and programmes, mostly at the ex post evaluation phase, it is possible to conclude that it is a credible, flexible, easy-to-operate, cost-effective and robust TIA methodology, which can be applied to projects, programmes and policies, at all territorial levels.info:eu-repo/semantics/acceptedVersio

    Cost Effectiveness of a Potential Vaccine for Human papillomavirus

    Human papillomavirus (HPV) infection, usually a sexually transmitted disease, is a risk factor for cervical cancer. Given the substantial disease and death associated with HPV and cervical cancer, development of a prophylactic HPV vaccine is a public health priority. We evaluated the cost-effectiveness of vaccinating adolescent girls for high-risk HPV infections relative to current practice. A vaccine with a 75% probability of immunity against high-risk HPV infection resulted in a life-expectancy gain of 2.8 days or 4.0 quality-adjusted life days at a cost of 246relativetocurrentpractice(incrementalcosteffectivenessof246 relative to current practice (incremental cost effectiveness of 22,755/quality-adjusted life year [QALY]). If all 12-year-old girls currently living in the United States were vaccinated, >1,300 deaths from cervical cancer would be averted during their lifetimes. Vaccination of girls against high-risk HPV is relatively cost effective even when vaccine efficacy is low. If the vaccine efficacy rate is 35%, the cost effectiveness increases to $52,398/QALY.     Immune checkpoint inhibitors for advanced non-small cell lung cancer: emerging sequencing for new treatment targets

    Lung cancer is the leading cause of cancer-related deaths in the world. Immune checkpoint inhibitors (ICI) stimulate cytotoxic lymphocyte activity against tumour cells. These agents are available for the treatment of nonsmall cell lung cancer (NSCLC) after failure of platinumbased therapy. One recent study has demonstrated that ICI monotherapy was superior to platinum-based chemotherapy for first-line treatment. Nevertheless, this benefit was only for a minority of the population (30%) whose tumour programmed death receptor ligand-1 (PD-L1) expression was above 50%. Therefore, several strategies are under investigation. One option for patients with PD-L1 expression lower than 50% may be the combination of ICI with platinum-based chemotherapy or with ICIs against different targets.     Atomically-precise texturing of hexagonal boron nitride nanostripes

    Monolayer hexagonal boron nitride (hBN) is attracting considerable attention because of its potential applications in areas such as nano- and opto-electronics, quantum optics and nanomagnetism. However, the implementation of such functional hBN demands precise lateral nanostructuration and integration with other two-dimensional materials, and hence, novel routes of synthesis beyond exfoliation. Here, a disruptive approach is demonstrated, namely, imprinting the lateral pattern of an atomically stepped one-dimensional template into a hBN monolayer. Specifically, hBN is epitaxially grown on vicinal Rhodium (Rh) surfaces using a Rh curved crystal for a systematic exploration, which produces a periodically textured, nanostriped hBN carpet that coats Rh(111)-oriented terraces and lattice-matched Rh(337) facets with tunable width. The electronic structure reveals a nanoscale periodic modulation of the hBN atomic potential that leads to an effective lateral semiconductor multi-stripe.     Noninvasive positive pressure ventilation for acute respiratory failure in children: a concise review

    Noninvasive positive pressure ventilation (NPPV) refers to the delivery of mechanical respiratory support without the use of endotracheal intubation (ETI). The present review focused on the effectiveness of NPPV in children > 1 month of age with acute respiratory failure (ARF) due to different conditions. ARF is the most common cause of cardiac arrest in children. Therefore, prompt recognition and treatment of pediatric patients with pending respiratory failure can be lifesaving. Mechanical respiratory support is a critical intervention in many cases of ARF. In recent years, NPPV has been proposed as a valuable alternative to invasive mechanical ventilation (IMV) in this acute setting. Recent physiological studies have demonstrated beneficial effects of NPPV in children with ARF. Several pediatric clinical studies, the majority of which were noncontrolled or case series and of small size, have suggested the effectiveness of NPPV in the treatment of ARF due to acute airway (upper or lower) obstruction or certain primary parenchymal lung disease, and in specific circumstances, such as postoperative or postextubation ARF, immunocompromised patients with ARF, or as a means to facilitate extubation. NPPV was well tolerated with rare major complications and was associated with improved gas exchange, decreased work of breathing, and ETI avoidance in 22-100% of patients. High FiO2 needs or high PaCO2 level on admission or within the first hours after starting NPPV appeared to be the best independent predictive factors for the NPPV failure in children with ARF. However, many important issues, such as the identification of the patient, the right time for NPPV application, and the appropriate setting, are still lacking. Further randomized, controlled trials that address these issues in children with ARF are recommended
